The fascia lata sling procedure for treating recurrent genuine stress incontinence of urine.

The fascia lata sling procedure has been used over the past 22 years in our unit for treating recurrent urinary stress incontinence when irreparably poor local support tissues were suspected. Sixty-nine patients had undergone one previous operation to correct urinary stress incontinence. One hundred one patients had two or more previous operations. The cure rate for the condition of genuine stress incontinence has been 100% in the last 148 cases and 98.2% overall. However, the cure rate for the symptom of urinary stress incontinence was 92.4%. There were only three sling failures in the entire series, occurring in the first 22 cases. Ten other patients had urinary incontinence with stress because of motor urge incontinence. The most troublesome postoperative problem has been delayed voiding.

[Malignant mesenchymoma in the region of Denonvillier's fascia].

A case with malignant mesenchymoma in the region of Denonvillier's fascia is presented. This localization has not been described in the literature up to now. It presented clinically by acute urinary retention. Morphologically it consists of three components: leiomyosarcoma, liposarcoma and chondrosarcoma. Tumor ablation with adenomectomy was performed by a retropubic approach.

[Hematolymphatic transport in skin, subcutaneous tissue and superficial lamina of the fascia of the lower leg in varicose veins].

The lymphatic bed of the skin and subcutaneous tissues of the lower leg from 20 operated patients was studied by light and electron microscopy and by phlebographic methods. Three stages of development of the disease were examined: without complicated form of varicose veins, complicated form, and postthrombophlebitic syndrome. Morphological features of the state of the lymphatic bed of the skin, subcutaneous connective tissue and fascia of the lower leg in the initial stage of disease show the fine structure changes of lymphatic vessels and capillary walls interpreted as a compensation phenomenon. It seems that the structure alterations of endotheliocytes of lymphatic capillaries and the connective tissue surrounding them found in this study in the complicated form of the disease and the postthrombophlebitic syndrome are the basis of the mechanism of transport-resorption insufficiency of the lymph vessels' terminal flow paths.

The use of autologous fascia lata for cardiac valve replacement: preliminary results.

Since November 1969, 53 patients have been operated on at the Toronto General Hospital for cardiac valve replacement with valves fashioned from autologous fascia lata. Sixty-three such valves have been inserted, including single aortic and mitral, double and triple valve replacements. The preliminary results indicate that operative mortality, considering the status of the patient submitted to the procedure, is comparable to that of plastic prosthetic valve replacement at this centre. Early follow-up confirms that anticoagulation is unnecessary and thromboembolism can be avoided with this form of valve substitution.

Some biological properties of bovine trypsinized fascia xenografts.

Trypsinized, lyophilized and radiation-sterilized bovine fascia grafts were used for treatment of muscle and peritoneal defects induced in mice. The survival rate, general and surgery region condition of normal and pregnant mice after transplantation were investigated. The influence of grafts on the adjacent tissues and on regeneration processes was also examined. It is suggested that the grafts prepared by this new method, owing to their good tolerance and long resorption time, could be used in a human clinic.

Implantation of a strip of fascia in the venous wall.

In 12 adult rabbits, according to the pull through technique a strip of muscular fascia was implanted in the wall of the femoral vein. Of 11 of these animals, sacrificed at intervals varying from 0 days to 3 months after the operation, the area of implantation was removed and histologically studied. Two cases showed wound infection and one case, moreover, venous thrombosis. Among the other 9 cases, in 2 specimens the end of the strip was found to lie within the venous wall, while in the remaining 7 specimens the strip protruded into the venous lumen. In the latter cases, the strip proved to be firmly attached to the venous wall within 4 days; within the same time the protruding part of the strip became completely covered by venous endothelial cells. Loose connective and fatty tissue adherent to the strip only disappeared, when the end of the strip freely floated in the streaming blood.

Superficial fascia rhytidectomy. A restoration of the SMAS with control of the cervicomental angle.

The surgical correction of an anatomic defect should re-establish the normal relationship of the tissues which contribute to the defect and restore or improve function. When the repair of the anatomic defect is not maintained, it may be necessary to alter the tissues to accomplish a more lasting result. Restoration of the SMAS cephalad to the cervicomental angle by superficial fascia rhytidectomy with a complete transverse resection of a segment of the platysma muscle at the level of the cervicomental angle fulfills the criteria of the young face by improving the function and contours of the face (changes that are characteristic of aging).

Acute necrotizing fasciitis of the endopelvic fascia.

A patient with an extensive necrotizing infection of the endopelvic fascia is presented. Cultures and tissue sections revealed the organisms responsible to be a mixed infection of Bacteroides, Staphylococcus, and Streptococcus. Hypertension and diabetic vascular disease were felt to contribute to the initiation of the disease process in this unusual site.

Chronic compartmental syndrome in the tensor fasciae latae muscle.

Chronic compartmental syndromes have been reported in various locations in the upper and lower extremities, but to the best of the authors' knowledge it has not been described in the tensor fasciae latae muscle. Two patients, men of about 60 years of age, complained of pain on exertion and sitting associated with swelling of the tensor muscle. In both cases CT scans showed muscle enlargement. Intracompartmental pressure values were high both at rest and during exercise. After fasciotomy the patients were relieved of pain, and intracompartmental pressure values were normalized.

Long-term follow-up of fascia lata transfer for the paralytic hip in myelodysplasia.

We evaluated the results of fifty-eight fascia lata transfers and anterior hip releases in thirty-three myelodysplastic patients with a minimum follow-up of 10.9 years. The procedure does not appear to achieve hip stability or prevent recurrent flexion deformity. Pelvic obliquity occurs secondary to scoliosis and results in increased instability of the hip on the high side and ischial decubitus ulcers on the low side. The degree of scoliosis and pelvic obliquity is related to the level of neural involvement. Similarly, ambulatory status is dependent on neural function and is not affected by instability of the hip.

[Closure of recurrent perforations of the tympanic membrane by means of cialit-conserved fascia and human fibrin tissue adhesive (author's transl)].

In 20 patients with recurrent perforations of the tympanic membrane after tympanoplasties the membrane defect was closed by using cialit-conserved temporal fascia and human fibrin tissue adhesive. The defects differed in size from 2 mm up to a subtotal defect. In 19 cases there was increasing vascularisation from the perforation margins towards the transplant centre after 4 weeks. After 8 weeks the connection between the transplant and the surrounding tissue was so tight that tympanometry could be performed using pressures up to 300 mmWS without any damage. Only in 1 case there was an infection. We had to remove the transplant. In comparison with the method of using only human fibrin tissue adhesive to close recurrent perforations, this method has the advantage that even bigger perforations of the tympanic membrane can be closed. The number of recurrencies of perforation also is lessened.

Myringoplasty: the lateral graft technique with fascia.

Grafting the tympanic membrane can be successfully accomplished by a number of techniques. The otologic literature is replete with descriptions of methods, materials, and results in myringoplasty. It is generally agreed that whatever method and materials are used to graft the tympanic membrane, the graft take-rate should be at least 90%. The key to obtaining a 90% take rate is not the technique employed, but how well the surgeon performs the technique. This article describes the lateral graft technique with fascia. This is the author's preferred myringoplasty procedure. When properly implicated, the lateral graft technique provides reproducible results in any pathologic condition. Excellent hearing and morphologic results can be obtained with few complications.

[Cover of trochanter pressure sores by V-Y musculocutaneous flap of the fascia lata].

Authors report their experience with the V-Y advancement flap in the coverage of trochanteric pressure sores. Nine flaps have been performed concerning 7 patients. Trochanteric sores in paraplegia or tetraplegia may lead to suppurative arthritis of the hip, with a risk of anaerobic infection and acute septicaemia. The treatment has to be as complete as possible. The trochanteric sore is debrided, with special attention given to removing all necrotic material, the surrounding bursa, and the bony prominence of the greater trochanter. The debrided trochanteric region is then covered by a well-vascularized and thick tissue. We observed only one complication at the donor site which can be closed primarily by this technique. The V-Y advancement technique is a new, simple and safe design of tensor fasciae latae flap. The defect is covered by the thicker and the best vascularized portion of the musculo-cutaneous flap. It allows preservation of other donor sites of the gluteal region to cover other sores, which are often associated.

[A case of muscle metastasis of renal cell carcinoma treated by local resection and tensor fascia lata myocutaneous flaps].

A 44-year-old male with a little finger tip-sized tumor palpable on the left epigastric wall and a left renal tumor detected on computerized tomography (CT) was referred to this hospital in June 1991. Nephrectomy was performed in July 1991 under the diagnosis of a left renal tumor. Histologically, the tumor was a renal cell carcinoma, grade 2, clear and granular cell type. The tumor in the left abdominal wall was not extracted. As a tumor also developed in the scapular tunica muscularis, tumors in the left musculus rectus abdominis and the scapular tunica muscularis were extracted to the extent possible in July of 1992. Histologically, these tumors were metastatic clear cell carcinomas. The tumor in the left musculus rectus abdominis recurred and another was palpable in the left lateral femur. In August of 1993, the tumors in the left musculus vastus lateralis and the left musculus abdominis were extracted together with the tunica muscularis, and the defective region on the abdominal wall was closed with a left musculus tensor fascia lata flap. No evidence of recurrence has been observed to date.

Physiology of the lower eyelid retractors: tight linkage of the anterior capsulopalpebral fascia demonstrated using dynamic ultrafine surface coil MRI.

Although the histologic anatomy of the lower eyelid retractors is well defined, the physiology of the lower retractors has been determined only by interference based on anatomic and clinical findings. In this study, in five normal subjects we investigated the physiology of the lower eyelid retractors utilizing dynamic high resolution magnetic resonance imaging (MRI) with a custom designed surface coil. Measurements of the excursion of the cornea, lower eyelid margin, and anterior edge of the inferior oblique muscle were made from scans taken in upgaze and downgaze. We found that the corneal movement was substantially greater than the movement of the eyelid margin, a finding that can readily be made clinically. A more important result was that the movement of the eyelid margin and the movement of the inferior oblique margin were similar in all cases. Thus, the length of the anterior capsulopalpebral fascia between the tarsus and inferior oblique muscle remains constant in downgaze, and the source of the stretch in the lower eyelid retractors lies in the posterior capsulopalpebral system, at the capsulopalpebral head. High resolution eyelid MR has great potential to allow investigation of essential aspects of normal and pathologic eyelid anatomy and physiology.
