The most important signs of vitality in burned bodies are soot deposits in the respiratory tract, the esophagus and the stomach as well as elevated CO-Hb values in the blood. But these findings show only that a person was exposed to fire fumes while alive; they are no indicator for vital heat exposure. As the external findings in burned bodies are often not very conclusive due to postmortem consumption by the fire, the internal findings are all the more important. Macroscopic signs that hot fumes were inhaled may be edematous swelling and vesicular detachment of the mucosa in the pharynx, the larynx and/or the upper section of the esophagus. As histological changes in the respiratory tract soot deposits, vesicular detachment of the epithelium, hyperemia and edema of the tracheal and bronchial mucosa as well as increased secretion of mucus have been described. These findings may partly be absent although the victim was alive during the fire, which can make diagnosis more difficult. Especially in peracute deaths vitality findings are usually sparse. The significance of the macroscopic and histological parameters of vitality was systematically investigated on the basis of our own autopsy material from the years 1996-2002 (88 cases) and compared with the statements found in the literature. In our study material, the vitality parameters were incomplete in 23% of the burned corpses. In 3% of the cases the question whether there was an antemortem heat exposure could not be answered.
There are many eye cosmetics available to enhance the beauty or improve the appearance of the face. To prevent infection, most eye cosmetics contain preservatives. Fragrance is usually absent to keep the products as safe as possible. Hypoallergenic products contain fewer ingredients and may be more appropriate for patients with sensitive skin.

A surgical draping technique is presented that decreases surgical time, maximizes surgical site exposure, and decreases the risk of infection. It was also designed to enhance patent comfort.
New insight has recently been obtained into the molecular mechanisms regulating the function of lymphatic endothelial cells. Vascular endothelial growth factors-C and -D have been shown to stimulate lymphangiogenesis, and their receptor VEGFR-3 has been linked to human hereditary lymphoedema, although there is evidence that other genes are also involved. These data suggest that it may become possible to stimulate lymphatic growth and function and to treat tissue oedema involved in many diseases.

BACKGROUND AND OBJECTIVE: To compare the conditions for insertion of the laryngeal mask airway using sevoflurane or propofol plus fentanyl. We evaluated the haemodynamic changes and cost of induction of anaesthesia in both groups. METHODS: Sixty patients were equally and randomly divided into two groups. Both groups received fentanyl 1 microg kg(-1). Patients in the sevoflurane group were induced with 8% sevoflurane and those in the propofol group with propofol 2.5 mg kg(-1). Conditions for insertion were graded on a three-point scale using six variables. Overall, conditions were assessed as excellent, satisfactory or poor based on the total score in each group. Systolic and diastolic arterial pressure and heart rate were recorded for 6 min after mask insertion. The financial cost of induction in both groups was calculated. RESULTS: The mean (+/- SD) time taken from induction to successful laryngeal mask insertion was significantly shorter with propofol (68.70 +/- 22.60 s) compared with sevoflurane (149.83 +/- 55.25 s). Excellent or satisfactory conditions were observed in 30 (100%) patients in the propofol group and in 29 (96.66%) in the sevoflurane group. Systolic and diastolic arterial pressures were significantly lower in the propofol group. The cost of sevoflurane used was 3.95 euros +/- 1.48 (Rs 216.23 +/- 64.66) (P < 0.05) compared with that of propofol, which was 3.23 euros +/- 0.65 (Rs 141.00 +/- 28.20). CONCLUSIONS: Although there was a faster induction with propofol-fentanyl, conditions for insertion of the laryngeal mask airway were similar in both groups. Haemodynamic stability was better with sevoflurane-fentanyl. The propofol-fentanyl combination was more cost-effective.
PURPOSE: The surgical procedures used until now to rebuild the conjunctiva in patients affected by ocular cicatricial pemphigoid (OCP) have not demonstrated appreciable anatomical and functional success. This study reports the postoperative clinical and cytological outcomes of a human amniotic membrane transplantation (AMT) to rebuild the conjunctiva in a case of late stage OCP. METHODS: We present a 75-year-old woman with a severe form of pemphigoid with entropion, trichiasis, symblepharon and blunting of the fornices, who underwent excision of the scarred and inflamed tissue covering the ocular surface and AMT. RESULTS: Our data show an improvement of the ocular surface condition, with reacquired fornix depth, reduced inflammation and presence of goblet cells at each follow-up. CONCLUSION: Amniotic membrane transplantation was successful as a first step procedure to reduce inflammation and to rebuild a physiological conjunctival epithelium in late stage OCP.

We report our experience with the use of Argon laser photocoagulation for the treatment of recurrent trichiasis. A total of 28 lids (of 21 patients) with aberrant lashes were treated in our study. Each lash was treated with a beam of 50-200 micron (spot size), 0.2 s (time), and 1-1.2 Watt (power). Neither infiltration nor topical anaesthesia was used. The mean follow-up time was 6.57 months. Successful treatment with no evidence of recurrence was achieved in 67.9% of lids after one to two laser sessions. The remaining 32.1% of the lids required three to four sessions to achieve no recurrence of the condition. There was a significant correlation between the number of aberrant lashes per lid and the number of required laser sessions. Complete healing of the treated areas occurred within six weeks after treatment, with no vascularisation or distortion of the lid margin. Argon laser treatment appears to be a safe and effective alternative to the other recognised methods of therapy in selected cases.
Epiblepharon commonly occurs in Oriental infants and children, but usually corrects itself with age. Epiblepharon in adults is not uncommon and may be misdiagnosed or detected late. It causes symptoms of ocular irritation and inferior punctate corneal epithelial erosion. These can persist if surgical treatment is ineffective. A series of 15 lower lid epiblepharon in 10 adults is presented with the clinical features and surgical treatment by anterior lamellar reduction and repositioning. The surgical technique is discussed. All patients had significant symptomatic relief and correction of lash-corneal touch. Among 15 cases of epiblepharon, 3 with residual trichiasis post-operatively were further corrected by skin reduction and debulking of pretarsal orbicularis oculi to achieve total correction. Adequate skin reduction and debulking of orbicularis oculi, especially the pretarsal muscle, are essential in the anterior lamellar repositioning and effective correction of this condition.
PURPOSE: Trichiasis in ocular cicatricial pemphigoid (OCP) is usually due to cicatricial entropion and is a major cause of ocular morbidity. Unfortunately in this disease, direct surgery on the conjunctiva often results in marked inflammation and cicatrisation. This paper assessed a procedure that corrects cicatricial entropion of the upper lid while avoiding surgery to the conjunctiva. METHODS: A grey line upper lid split and a vertical anterior lamellar repositioning was performed on 16 lids of 11 patients with OCP. RESULTS: Anatomical success was achieved in 72% of lids at 1 year and 61% had complete success with no lashes touching the globe. These outcomes were maintained up to 4 years. There were no perioperative complications. Two patients post-operatively developed severe conjunctival inflammation that required systemic immunosuppression. The causes of failure were primary surgical failure (n = 2), progression of cicatrisation secondary to surgically induced inflammation (n = 1), development of misdirected lashes (n = 1) and late recurrence at 7 months (n = 1). One patient developed peaking of the eyelid. CONCLUSIONS: This procedure has a good long-term outcome with minimal complications. Activation of severe conjunctival inflammation occurred in 13% of cases and this must be considered pre- and post-operatively.
BACKGROUND: Upper lid epiblepharon refers to a developmental anomaly characterised by the presence of an exaggerated skin fold running horizontally across the upper lid below the lid margin, which in severe cases is associated with lash-cornea touch or even corneal punctate epithelial erosions. METHOD: This retrospective study looked at 14 patients who underwent bilateral upper lid pretarsal orbicularis debulking and skin excision to correct lash-globe contact from the upper lid. RESULTS: Surgery was successful in 25 of 28 lids, with one recurrence being successfully re-operated. The remaining 2 recurrences did not undergo further surgery as the patients were asymptomatic. CONCLUSION: This surgical procedure appears to be an effective treatment of lash inversion in upper lid epiblepharon with good aesthetic results.
