[Diagnosis and treatment of liver echinococcosis complicated by cysto-biliary fistula].

890 patients with echinococcosis of the liver were operated during 1976-1992. Complicated forms of the disease were li detected in 222 (24.9%) patients, among them 141 (15.8%) patients with cysto-biliary fistulas. Ultrasonography examination and endoscopic retrograde cholangiography were regarded as the most informative methods of preoperative diagnosis of cysto-biliary fistulas. Three-stage management was worked out. The usage of laser, plasmatic scalpel and ultrasound of low frequency substantially contributed to improvement of the results of treatment of these patients.

Epidemiology of echinococcosis in Nepal.

Echinococcosis and hydatidosis is a parasitic zoonotic disease of human and animals. This disease has created public health and environment problems in all urban areas of Nepal. Based on the three year study (1993-1995) it has been revealed that the epidemiological cycle (indigenous) of Echinococcus granulosus parasite is dog-pig-dog cycle and human acquire infection accidentally through infected dog stool. However, this study has proved also the epidemiological cycle like dog-sheep-dog, dog-goat-dog and dog-buffalo-dog. This study was supported by International Development Research Centre (IDRC), Ottawa, Canada.

[The Nd YAG laser in the treatment of hepatic echinococcosis].

Nd YAG Laser photocoagulation for the purpose of residual cavity treatment following echinococcectomy was introduced in 1996, and practically implemented in the Department of General and Operative Surgery. Eleven patients with uncomplicated echinococcus of the hepar, 4 with purulent echinococcosis and 4 with rupture of the biliary tree, aged 26 to 80 years, are subjected to operation. The advantages and possibilities of dispersed laser application in hydatid disease treatment are discussed. The short-term results are estimated as satisfactory.

Imaging in Cardiac Echinococcosis.

Cardiac echinococcosis is a rare disease. Its diagnosis is sometimes difficult and requires medical imaging such as echocardiography. An 18-year-old man with multiple cardiac hydatid cysts diagnosed by echocardiography and nuclear magnetic resonance is presented. The results from these imaging techniques are presented.

Improvement of dissolution and bioavailability for mebendazole, an agent for human echinococcosis, by preparing solid dispersion with polyethylene glycol.

The solid dispersion of mebendazole was prepared with polyethylene glycol (PEG) to enhance the dissolution rate of mebendazole, an agent for the chemotherapy of human echinococcosis. The dissolution rate of the solid dispersion increased compared with the physical mixture, and also increased with the incorporation of an increasing amount of PEG-6000. An extensive improvement of the dissolution rate was observed when the ratio of the solid dispersion of mebendazole to PEG-6000 was more than 1: 2. Furthermore, greater bioavailability in rabbits was obtained after oral administration of the solid dispersion compared with the physical mixture.

Recombinant antigens for immunodiagnosis of cystic echinococcosis.

Three cDNAs, termed EpC1, TPxEg and EgG5, were isolated by immunoscreening from an Echinococcus granulosus cDNA library. The recombinant phages exhibited strong reactivity with sera from humans with confirmed cystic echinococcosis (CE) and with sera from mice infected with E. granulosus oncospheres. The cDNAs were subcloned into a pET vector, expressed as fusion proteins tagged with GST and affinity purified against the GST tag. Of the three recombinant proteins, EpC1 achieved the highest performance for serodiagnosis of CE in Western blot analysis using a panel of clinically defined human sera to initially address the sensitivity and specificity of the molecules. The protein yielded an overall sensitivity of 92.2% and specificity of 95.6%, levels unprecedented taking into account the large panel of 896 human sera that were tested. The strategy used may also prove suitable for improved immunodiagnosis of other parasitic infections.

[Cystic echinococcosis in Kayseri during the last six years.].

Cystic echinococcosis (CE) caused by the larval form of Echinococcus granulosus is a major public health problem especially in sheep-raising regions of the world. In the present study, CE cases detected during 1994-1998 by various hospitals and health directorships were investigated retrospectively. Our results show that there were a total of 699 cases of CE including 330 (47.2%) males and 369 (52.8%) females. Finally, it was found that the 699 patients had been hospitalized for a total of 9.246 days.

Th1 and Th2 inducing cytokines in Cystic echinococcosis.

This study included 62 subjects who presented at the Turgut Ozal Medical Centre in Malatya. Of these, 32 were infected with the larval stage of Echinococcus granulosus and the remaining 30 were healthy controls. The ratio of males to females was 0.45. Cystic echinococcosis (CE) occurred in all age groups; the youngest case was 12 years and the oldest case was 74 years old. The frequency of hydatid cysts located in liver was (78.1%) followed by lungs (15.7 %) and one in the brain. All patients recovered completely except for one woman (35 years old) in whom multiple cysts (21cysts) were detected in liver and omentum two years after the first operation. In 68.7 % of the cases, the cytokine that was most frequently increased was IL-4. The IL-2 cytokine increased to 46.8% followed by IL-10 (40.6 %). These results suggest that there are different immunoregulatory events and the cytokine response during CE may be in part related to the slight monocytosis observed in CE patients. In conclusion, concurrent Th1 and Th2 cytokine-type profiles were expressed in CE with the predominance of type Th2 cytokines.
