Nurse case managers and the Internet.

The Internet (Net) and World Wide Web (Web) are pervasive in today's information-driven work settings. Print media are limited in their capability to store and obtain new data. Nurse case managers (NCMs) who are able to use the computer for obtaining information, teaching patients, managing data sets, and conducting research and scientific work are better prepared for the dynamic healthcare arena of today and the future. Understanding the structure and workings of the Net//Web can be very helpful in the formulation of searches, design of teaching modules, and storage of new information sources. Critical computer skills for NCMs and the attendant implications for nursing case management practice are discussed.

Internet case management: finding information on the World Wide Web.

In conclusion, the World Wide Web offers an incredible variety of resources, many of which are available free of charge. To begin accessing these resources, one must first become comfortable with using browser software. Once this is done, master the basics of searching. Keep in mind that for a search to be efficient, you should have a clear understanding of why you are doing the search, the basis of a Purpose-Focus-Assessment analysis. With this information in hand, you can dive in, using methods that range from broad and general--such as brute force--to highly focused and specific literature searching. Remember that although the most effective searches are those that are tailored to the situation at hand, all searching on the Internet requires persistence, tenacity, and a good dose of creative thinking!

Case management information systems: how to put the pieces together now and beyond year 2000.

The case management process is a critical management and operational component in the delivery of customer services across the patient care continuum. Case management has transcended time and will continue to be a viable infrastructure process for successful organizations in the future. A key component of the case management infrastructure is information systems and technology support. Case management challenges include effective deployment and use of systems and technology. As more sophisticated, integrated systems are made available, case managers can use these tools to continue to expand effectively beyond the patient's episodic event to provide greater levels of cradle-to-grave management of healthcare. This article explores methods for defining case management system needs and identifying automation options available to the case manager.

Workflow technology: the new frontier. How to overcome the barriers and join the future.

Hospitals are catching up to the business world in the introduction of technology systems that support professional practice and workflow. The field of case management is highly complex and interrelates with diverse groups in diverse locations. The last few years have seen the introduction of Workflow Technology Tools, which can improve the quality and efficiency of discharge planning by the case manager. Despite the availability of these wonderful new programs, many case managers are hesitant to adopt the new technology and workflow. For a myriad of reasons, a computer-based workflow system can seem like a brick wall. This article discusses, from a practitioner's point of view, how professionals can gain confidence and skill to get around the brick wall and join the future.

Word recognition software use in a busy orthopaedic practice.

For the past 3 years, our orthopaedic office has used word recognition programs to expedite medical record reporting, operative dictation, and letter writing. The use of these programs has resulted in cost savings, increased efficiency, and an improvement in the quality and the thoroughness of our medical records. The methods whereby we were able to incorporate a word recognition program into our operative dictations and our office notes are discussed and explained. Advantages and disadvantages of different programs were evaluated and discussed. We have been able to use word recognition programs in the daily function of our orthopaedic office although not without some difficulties. These programs have resulted in cost savings and time savings and provided medicolegal protection because of the thoroughness of the documentation. The use of templates allows efficiency comparable with conventional dictation. Word recognition programs can be used effectively in busy orthopaedic offices when combined with note templates.

Effects of a nursing literature reading course on promoting critical thinking in two-year nursing program students.

Critical thinking is essential for professional nursing practice. Promoting critical thinking with teaching strategies has been a focus of nursing education and nursing research. Reading and writing assignments are teaching strategies for fostering critical thinking ability. We developed a course with the emphasis on critical reading and writing. The purposes of this study were to explore the effect of the course and teaching strategies on promoting critical thinking and to understand the learning experience and perception of nursing students as part of the course assessment plan. Quasi-experimental design and purposive sampling were used. 170 students of a two-year nursing program were grouped into an experimental and a control group depending on whether or not they took the course. The change in critical thinking ability was evaluated and determined based on performance in an article critique test before and after the course. Data was collected with an assessment tool developed for the purposes of the research and was analyzed with descriptive statistics, paired t-test, and two-factor analysis of variance with repeated measures and content analysis. The findings were: (1) The scores on the critique test at the end of the course in both groups were significantly higher than before the course (p<.001). The improvement in score of the experimental group, however, was significantly higher than that of the control group (p<.001). (2) Changes in thinking pattern and learning attitude, as well as a feeling of growth and achievement, were found in content analysis of students' self-evaluation of their learning experience and perception. Critical thinking skills were applied while learning the course. The results not only provide evidence to support the positive effect of reading and writing assignments on critical thinking ability but also confirm the teaching and learning activity design for the course.

Important computer competencies for the nursing profession.

Nursing requires computer competencies. This study aimed at identifying those competencies required for the nursing profession in Taiwan. The Delphi technique was deployed in this study. In the Delphi questionnaires, computer competencies were sorted into seven domains: concepts of hardware, software, and networks; principles of computer applications; skills of computer usage; program design; limitations of the computer; personal and social issues; attitudes toward the computer. In three Delphi questionnaires, nursing informatics experts gave us their opinions on the importance of each computer competency for the nursing profession. The experts also designated when the competency should be cultivated. This study provides a comprehensive list for nursing professionals to check on their computer competence. The results of this study should also serve as good references for teachers and schools in designing related curriculums.

An exploratory study of Taiwanese consumers' experiences of using health-related websites.

It is manifest that the rapid growth of Internet use and improvement of information technology have changed our lifestyles. In recent years, Internet use in Taiwan has increased dramatically, from 3 million users in 1998 to approximately 8.6 million by the end of 2002. The statistics imply that not only health care professionals but also laypersons rely on the Internet for health information. The purpose of this study was to explore Taiwan consumers' preferences and information needs, and the problems they encountered when getting information from medical websites. Using simple random sampling and systematic random sampling, a survey was conducted in Taipei from August 26, 2002 to October 30, 2002. Using simple random sampling and systematic random sampling, 28 boroughs (Li) were selected; the total sample number was 1043. Over one-quarter (26.8 %) of the respondents reported having never accessed the Internet, while 763 (73.2%) reported having accessed the Internet. Of the Internet users, only 396 (51.9%) had accessed health-related websites, and 367 (48.1%) reported having never accessed health-related websites. The most popular topics were disease information (46.5%), followed by diet consultation (34.8%), medical news (28.5%), and cosmetology (28.5%). The results of the survey show that a large percentage of people in Taiwan have never made good use of health information available on the websites. The reasons for not using the websites included a lack of time or Internet access skills, no motivation, dissatisfaction with the information, unreliable information be provided, and inability to locate the information needed. The author recommends to enhance health information access skills, understand the needs and preferences of consumers, promote the quality of medical websites, and improve the functions of medical websites.
