Continent urinary reservoir using sigmoid colon and appendix after pelvic exenteration for bulky leiomyosarcoma: a case report.

We report on a man with bulky leiomyosarcoma occupying the entire pelvic cavity that was treated by pelvic exenteration. A urinary reservoir was constructed using a detubularized segment of sigmoid colon. The appendix was tunneled into the taenia to form a continent catheterizable stoma using the Mitrofanoff principle. Descending colostomy was also created. There is no evidence of recurrence 24 months after treatment and the patient has returned to full physical activity. Continence is complete with a capacity of up to 500 ml. and regular bowel movements. Because of the lack of intestinal anastomosis, this procedure may be recommended as an option for urinary diversion after pelvic exenteration.

[An adenocarcinoid tumor of the vermiform appendix].

Goblet cell carcinoids of the vermiform appendix are uncommon. A comprehensive review of the literature revealed about 100 reported cases. Clinical presentation included: asymptomatic patients, acute appendicitis, and/or chronic intermittent lower abdominal pain with or without a palpable mass. We report the case of a 57 year-old woman with lower abdominal pain, nausea and vomiting. Normal blood tests. X-ray of the abdomen showed dilated small bowel loops with fluid levels. Initially, conservative treatment was started. After three days with no clinical improvement, the patient was operated on. An appendicocecal inflammation was found and a terminal ileum plus cecal resection with an end-to-side anastomosis was performed. The pathological diagnosis was goblet cell carcinoid. The patient shows no evidence of recurrence after 1 year follow-up.

Mucocele of the appendix.

We describe a case of mucocele of the appendix, observed incidentally in a 42-year-old man. The plain film, ultrasound and CT features are reviewed. Adequate diagnosis and resection are important because of the risk of rupture and development of pseudomyxoma peritonei.

Rabbit Peyer's patches, appendix, and popliteal lymph node B lymphocytes: a comparative analysis of their membrane immunoglobulin components and plasma cell precursor potential.

We have characterized and compared the differentiative potential of B lymphocytes obtained from rabbit appendix (APP), Peyer's patches (PP), and popliteal lymph nodes (PLN) by immunofluorescence analysis of membrane immunoglobulin (Ig) components, by radioiodination of membrane components followed by isolation and identification of cell surface Ig, and by cell transfer studies. As assessed by cell transfer lymphoid cells from PP and APP appear to be highly enriched sources of IgA plasma cell precursors. In addition, these populations of lymphocytes have a small but significant potential to give rise to IgM and IgG plasma cells. In contrast, as shown previously (1), lymphocytes from PLN have a comparatively small potential to generate IgA plasma cells. The ability of cells from PLN and PP to give rise to plasma cells is correlated with the presence of 35 to 38% of lymphocytes from both sources having membrane light chain determinants detectable by immunofluorescent staining. When the cells were stained directly for mu chain determinants, about 33% of PLN cells and 28% of PP cells were positive. These latter percentages were shown to reflect cells bearing both endogenously synthesized b4 light chain and mu heavy chain. By indirect immunofluorescence, alpha and gamma chain determinants were also detected on PP and PLN cells; however, these Ig did not appear to be products of the cells which bore them as they were not regenerated after removal of Pronase. It was further demonstrated that purified rabbit copro-sIgA and colostral sIgA are cytophilic for PP and PLN cells. The results are discussed with respect to the origin of IgA plasma cells and the postulated bursal role of mammalian gut-associated lymphoid tissue.

[Appendix in a left crural herniated position: apropos of a case. Explanation by human ontogenesis].

The authors report about one case of left crural strangulation containing the appendix. The right flexure of the colon is fixed in its usual infrahepatic position. The ascending colon, on the other hand, floats, and the cecum is in a left iliac position. An ontogenic explanation is proposed, on the basis of recent anatomical work, and compared with traditional hypotheses about the evolution of the gastrointestinal tract in relation to the development of the peritoneum.

Intussusception of mucocele of the appendix: a case report.

A 63-year-old man had had repeated episodes of ileo-caecal intussusception, and a pre-operative barium enema revealed appendiceal mucocele as a leading cause. Resection of appendix and part of the caecum were undertaken through exploration. The post-operative course was smooth. This unusual etiology is reported and discussed.

[Bacterial flora in the appendix in rabbits with artificially induced acute appendicitis].

The authors present the results of bacteriological analysis of scrapings from the appendical mucosa of 24 rabbits with appendicitis induced by obstruction of the appendix lumen and of 5 rabbits with healthy appendices (controls). In dependence on the duration of inflammation, the recovery of aerobic bacteria was decreasing and that of anaerobic nonsporulating bacteria was increasing. In the group of anaerobic bacteria, gram-negative rods were predominant (number of isolated colonies: 2973 = 79.9%, variation range of occurrence: 10(5)-10(8) per 1 gram of material) with a remarkable prevalence of the genus Bacteroides and particularly of the species Bacteroides fragilis. Compared to the occurrence in appendices with inflammation, the recovery of bacteria in controls was minimal. The work served as model in studying pathogenesis of acute appendicitis in human medicine. (Tab. 3, Ref. 12.)

Use of the appendix in urologic reconstructive operation.

Advances in genitourinary operation have allowed use of an appendicovesicostomy in patients requiring chronic catheterization for bladder emptying. Recent discussions of the appropriateness of incidental appendectomy have ignored the potential usefulness of the appendix as a catheterizable stoma. We present our experience with the Mitrofanoff principle to illustrate the advantages of an appendicovesicostomy and make a plea against incidental appendectomy in certain patient groups.

[Diverticulitis of the appendix: a rare cause of acute abdomen].

Diverticulosis of the vermiform appendix, either single or multiple, congenital or acquired, is rather infrequent and usually asymptomatic. However, it may be complicated by flogosis configuring an acute abdomen hardly recognizable from an acute appendicitis not related to the diverticular disease. The Authors report a case of acute appendicular diverticulitis surgically treated. A brief review of the literature is also reported.

[Mucocele of the appendix. Some diagnostic features].

The description of a case of mucocele of the appendix shows the difficulties of arriving at a preoperative diagnosis. Following an analysis of the diagnostic features it's possible to affirm that only surgery can confirm the diagnosis with the histological result. Careful surgical dissection is very important when a malignant tumor is suspected.

[Pathology of the cecal appendix in our country. An analysis of 936 surgical specimens of appendectomy].

Analysis was made of 436 specimens of appendectomy in the Caracas University Hospital during the year 1.990. Ages of the patients ranged between 9 months and 79 years (average age: 21 years). 236 were males and 200 females. The pathology found most frequently was the acute inflammatory processes, followed by secondary reactive changes to other entities, many of which were unspecified (e.g. pelvic inflammatory disease) and other specific (such as infestation by parasites) less frequently were found tumoral processes such as mucocele, carcinoid tumor and those which involved another portion of the digestive apparatus and which secondarily affect the appendix.

Endometriosis of the appendix with decidual polyp formation: a rare cause of acute appendicitis during pregnancy.

We report a case of a 28-year-old female, Grava 2, Para I, Ab. 0, who presented acute abdominal pain on the 21st week of her gestation. A diagnosis of acute appendicitis was entertained and an appendectomy performed. The pathologic specimen revealed an inflamed appendix with endometriosis and a marked decidual reaction. A decidual polyp, which occluded most of the appendiceal lumen, is proposed as a rare cause of acute appendicitis during pregnancy.

[Acute diverticulitis of the appendix. A clinical case].

The authors describe a case of acute diverticulitis of the appendix presenting as an inflammatory mass in the right lower quadrant of the abdomen. The histologic and clinical findings are discussed. This uncommon condition may mimic acute appendicitis, but in most instances it departs from typical appendicitis for later age of appearance, more indolent clinical course and increased tendency to perforation. In these cases the disease may progress with subacute interstitial inflammation, with or without abscess formation, and present as a tumor-like mass of the cecum.

[Necrotizing vasculitis of the appendix and cecum in systemic lupus erythematosus].

A 20-year old female patient with systemic lupus erythematosus with butterfly-erythema, arthritis, high positive ANA and anti-ds-DNA titres has to have laparatomy because of an acute abdomen. Histological diagnosis is a necrotizing vasculitis of the appendix. In systemic lupus erythematosus an intestinal vasculitis can be found which might be as severe as in polyarteritis nodosa.

Intussusception of vermiform appendix. A case report and review of the literature.

Intussusception of the appendix into the caecum is an uncommon condition and the diagnosis is rarely made preoperatively. Specific anatomic requirements as well as pathological conditions causing irritation or increasing peristalsis should be present for this event to occur. A case is discussed in which the diagnosis was made of an ileocolic invagination, which appeared peroperatively to be an appendicular invagination.

[Hyperplasic polyp of the appendix].

Two cases of hyperplastic polyp of the appendix are reported. Its association with other appendiceal and extraappendiceal lesions and its relationship with appendiceal mucocele are briefly considered.

Appendiceal intussusception as a manifestation of mucinous cystadenoma of the appendix: an interesting clinical entity.

In children, intussusception is a common cause of intestinal obstruction. In contrast, intussusception in the adult is an infrequent cause of intestinal obstruction and in the majority of instances has some obvious cause such as a tumor forming the lead point of the intussusceptum. Diagnosis is suggested by chronic, intermittent symptoms of intestinal obstruction, the presence of an abdominal mass, and the passage of blood per rectum. In contrast to children, treatment is invariably surgical resection without reduction. We have encountered an appendiceal intussusception as a manifestation of a mucinous cystadenoma of the appendix. There was no evidence of pelvic visceral involvement. This unique presentation is rarely reported. The patient underwent uneventful laparoscopy-assisted segmental ileocolonic resection with stapled anastomosis. An analysis of our diagnostic and therapeutic approach, as well as a review of appendiceal pathology as a cause of intussusception are presented to better understand this rare clinical entity.

Gastric lipoma and mucocele of appendix. Case history.

An interesting coexistence of two rare diseases in the same patient is presented. A 51 year old female was operated for a benign tumor of the stomach, a lipoma of about 5 cm of the gastric antrum. After the tumor removal, a mucocele of the appendix was surprisingly found, for which an appendectomy was necessary. The clinical course was uneventful and one year after the operation the patient is in a good state of health. A short review of the literature accompanies the description of the case.
