Laparoscopy treatment of adnexal sterility.

A total of 113 patients who were examined and treated for matrimonial infertility underwent surgical laparoscopy at the Sterility and Infertility Department, Clinic of Gynecology and Obstetrics, Clinical Center of Nis. Age distribution in the group of patients who were subjected to surgical laparoscopy after diagnosis was 31.39 +/- 6.1 years, out of whom 74.3% were treated for primary sterility. Out of corrective interventions in laparoscopy, the following procedures were performed: adnexiolysis, salpingo-ovariolysis, fimbrioplasty, terminal salpingoneostomy, cyst puncture and ovarian incision. During any surgical laparoscopy, ovarian biopsy was performed for evaluation of ovarian potency and verification of probable histopathological findings. The most commonly used technique was salpingo-ovariolysis--in 22.88% of cases, followed by fimbrioplasty--18.81%. A total of 42 pregnancies were verified: 36 intrauterine (29 completed at full term) and 6 extrauterine pregnancies. Average conception rate in our patients was 37.17%, while rate of parturition was 25.66%. The rate of pregnancy in different corrective laparoscopic procedures ranged from 18.18%-61.54%. The rate of intrauterine pregnancy was the highest after bilateral fimbrioplasty (47.83%), and the lowest after bilateral neosalpingostomy--9.09%. The rate of delivery after adhesiolysis was 30.8%, after salpingo-ovariolysis--28.8%, after fimbrioplasty--18.3%, and after neostomy--12.5% of the time.

Adnexal Diseases↗

[Bilateral adnexal masses in a pregnant woman--laparoscopic management].

A case of a 30 years old pregnant woman suffering from bilateral ovary tumour was presented. When she was sixteen weeks pregnant she had both tumours enucleated using the method of laparoscopy. Diagnostic and therapeutic process have been described. Authors are convinced that laparoscopy as a method of treatment can be safely applied to pregnant women provided, that specific rules are obeyed and elaborate precautions during the surgery are taken.

Adnexal mass: an unusual presentation of small-bowel adenocarcinoma.

Malignancy of the small bowel presents unique diagnostic and therapeutic challenges resulting in a delayed diagnosis in many cases. Small-bowel tumors respond poorly to most forms of treatment. Metastatic lesions to the ovaries comprise a small percentage of all ovarian malignant neoplasms. Ovarian metastases from primary small-bowel tumors are often difficult to differentiate from primary ovarian tumors. Only few reports have described ovarian metastases from small-bowel sources. A high index of suspicion can lead to an earlier diagnosis and can have an impact on the therapeutic options as well as the survival of the patients. The current report also underscores the importance of careful review of symptoms, investigative studies and pathology.

Abdominal Pain↗

[Adnexal torsion in childhood].

Five cases of torsion of normal uterine adnexae are reported in children aged 9 to 13 years. From this series, the authors review the clinical and ultrasonographic features of the torsion of normal uterine adnexa. Ultrasonography should be performed in emergency as the conservation of the ovary depends on the precocity of diagnosis and treatment.

Adnexa Uteri↗

[Uterine, adnexal and cul-de-sac masses. Comparative study by intravaginal and trans-abdominal echography].

42 female patients were selected for Trans-Vaginal Sonography (TVS) in an ultrasonographic study, after clinical examination, computed tomography or a previous study had suggested the existence of one or more lumps in the pelvic cavity. The TVS method gave higher quality diagnostic information in 55% of the cases studied, while the Trans-Abdominal method was better in 12.5% of the cases. In 79% of the cases the TVS was considered better in image quality while 11% of the cases showed the same image quality in all of the methods. The results agree with those obtained by other authors and confirm that Trans-Vaginal Sonography is of considerable value in the study of female pelvis, as an isolated exploration, complementary or otherwise with subsequent abdominal examination with or without a full bladder.

Calcium channel blockade prevents postsurgical reformation of adnexal adhesions in rabbits.

Prevention of postoperative adhesion formation is a subject of concern to the reproductive surgeon. We developed a model to test the efficacy of calcium channel blockade-mediated adhesion prophylaxis after lysis of established pelvic adhesive disease. New Zealand White rabbits received a standardized primary adhesiogenic traumatic lesion to the left uterine horn. One week later, a laparotomy was performed for evaluation (prescore scale: 0 = no adhesions to 4+ = severe) and subsequent lysis of adhesions. Before operative closure, animals were allocated randomly to intraperitoneal treatment with either verapamil 2.5 micrograms/kg/hour or vehicle, delivered by mini-osmotic infusion pumps over the course of 200 hours. Seven days later, the animals were sacrificed and evaluated blindly for extent of adhesion reformation (postscore). Prescores for verapamil and control rabbits were not significantly different (3.3 versus 2.9). Verapamil-treated animals formed considerably fewer adhesions than did controls after adhesiolysis (postscore 0.5 versus 3.5; P less than .001). These data demonstrate a marked inhibition of adhesion reformation after lysis of pelvic adhesions under the influence of verapamil.
