Reassessment of transillumination light scanning for the diagnosis of breast cancer.

Transillumination light scanning is a non-x-ray, noninvasive modality that uses low-intensity emissions of red and near infrared light to visualize the tissues of the breast. The absorption characteristics of the emitted light from the breast tissues are fed into a computer that transforms the analog signals into varying degrees of color saturation hues, ranging from blue to red. The basic premise for the use of transillumination is that breast cancer, because of its increased blood supply, absorbs more near infrared radiation than benign tissue. Thus, abnormal or malignant breast tissues appear blue on the light scan, while normal breast tissues appear red. The safety and lack of risk of radiation exposure of this technique have been demonstrated.

Diaphanography: transillumination of the breast.

Diaphanography is a new imaging modality utilizing light to examine the breast. While this technique is similar to transillumination, it has several significant differences. Unlike transillumination, diaphanography makes use of selected wavelengths of light and special imaging equipment. While diaphanography appears to be a promising method for the screening of breast disease, it is still in the experimental stages. Studies evaluating the efficacy of this new modality are necessary before diaphanography can replace current methods of breast examination.

[Endonasal dacryocystorhinostomy with transillumination of the lacrimal sac: implementation and results].

PATIENTS: Since Mai 1991 we performed a total of 17 endonasal dacryocystorhinostomies under transillumination of the lacrimal sac on 15 patients with lacrimal obstruction aged 11 to 88 years. 11 patients with 12 operations were followed up between 40 and 393 days (mean of 5.6 months). RESULTS: In all cases the connection between the lacrimal sac and the nasal cavity remained functionally open. One patient developed a secondary canalicular stenosis which made silicon tubing necessary. CONCLUSIONS: We consider the operation an elegant and as far as our experience goes reliable method to recanalize the lacrimal drainage system avoiding a cutaneous incision. It is simplified by the transillumination of the lacrimal sac under videoendoscopic conditions.

Infrared transillumination stereophotography of normal iris.

We studied the structure of the normal iris as seen in infrared transillumination photographs of variously pigmented eyes in subjects of different age and sex. To transilluminate the iris infrared light was passes via a fiber-glass optic through the lateral wall of the globe and photographs were taken with a Zeiss stereo slit-lamp camera. The photographs showed that the structural pattern of the iris was similar in both blue and brown eyes of either sex. The iris was rather dense in children, well developed in adults and somewhat atrophic in the elderly. On the posterior surface of the iris the radial contraction folds and the structural folds of Schwalbe and circular contraction folds were seen; they were delicate in children and more pronounced in adults and the elderly. The pupillary part of the iris ballooned forward in young people whereas in the elderly the whole iris had this configuration.

Characterization of SYBR Gold nucleic acid gel stain: a dye optimized for use with 300-nm ultraviolet transilluminators.

The highest sensitivity nucleic acid gel stains developed to date are optimally excited using short-wavelength ultraviolet or visible light. This is a disadvantage for laboratories equipped only with 306- or 312-nm UV transilluminators. We have developed a new unsymmetrical cyanine dye that overcomes this problem. This new dye, SYBR Gold nucleic acid gel stain, has two fluorescence excitation maxima when bound to DNA, one centered at approximately 300 nm and one at approximately 495 nm. We found that when used with 300-nm transillumination and Polaroid black-and-white photography, SYBR Gold stain is more sensitive than ethidium bromide, SYBR Green I stain, and SYBR Green II stain for detecting double-stranded DNA, single-stranded DNA, and RNA. SYBR Gold stain's superior sensitivity is due to the high fluorescence quantum yield of the dye-nucleic acid complexes ( approximately 0.7), the dye's large fluorescence enhancement upon binding to nucleic acids ( approximately 1000-fold), and its capacity to more fully penetrate gels than do the SYBR Green gel stains. We found that SYBR Gold stain is as sensitive as silver staining for detecting DNA-with a single-step staining procedure. Finally, we found that staining nucleic acids with SYBR Gold stain does not interfere with subsequent molecular biology protocols.

Identification of living spermatogenic cells of the mouse by transillumination-phase contrast microscopic technique for 'in situ' analyses of DNA polymerase activities.

The stages of spermatogenesis can be identified in freshly isolated, unstained adult mouse seminiferous tubules using a transillumination method. Late acrosome- and maturation phase spermatids, arranged in bundles at stages XII-VI give rise to a spotty transillumination pattern. Before spermiation, these cells form a continuous layer on the top of the seminiferous epithelium, recognized by a strong homogeneous central light absorption in the freshly isolated seminiferous tubules at stages VII and VIII. Other stages have a pale light absorption pattern. The accurate determination of the developmental stages of the germ cells was based on the morphology of the developing acrosomic system and of the nuclei of the spermatids, as revealed by phase contrast microscopy. Using this procedure, the activity levels of DNA polymerases alpha and beta have been studied by autoradiography of squash preparations. Using endogenous templates, assay conditions that differentiate between the solubilized DNA polymerases alpha and beta in vitro, were used to distinguish between these activities in situ in different stages of mouse spermatogenesis. Except in very late spermatids shortly before spermiation, DNA polymerases alpha and beta were detectable in all cell types examined. Coinciding with the nuclear protein transitions, elongating spermatids at steps 10-12 and maturation phase spermatids at steps 13-14 showed high DNA polymerase activities. As no replication occurs in these cells, the observations support the view that both DNA polymerases alpha and beta could be involved in repair DNA synthesis.

Optical transillumination tomography for imaging of tissue-engineered blood vessels.

Recent progress in tissue engineering led to the development of completely biological human vessels grown from the patient's own cells. Those tissue-engineered blood vessels (TEBV) are grown on an individual basis at high costs per item, and therefore require close growth monitoring and quality control. We designed and tested an optical transillumination tomography system using red laser light to image weakly scattering specimens, such as TEBV. Fixated TEBV were imaged and the results compared to optical coherence tomography. This preliminary scanner prototype had an in-plane resolution of 50 microm and allowedto see small inhomogeneities and defects in the samples. Tissue attenuation was found to be 70 cm(-1). Main advantages of the transillumination tomography scanner over optical coherence tomography were the inexpensive instrumentation and the potential to rapidly acquire complete 3D sections with a CCD camera. The prototype presented in this study provides a basis to further improve image quality and acquisition speed.

Testicular teratoma presenting as a transilluminating scrotal mass.

Prepubertal testicular neoplasms are uncommon and differ from adult neoplasms in histopathologic features and presentation. The presented case describes a 3-year-old boy with an enlarged, transilluminating scrotal mass. Ultrasonography revealed a heterogeneous mass and absence of a hydrocele. After right radical orchiectomy, pathologic examination confirmed the mass as a mature teratoma. Although tumor registries cite yolk sac tumor as the most common prepubertal testis neoplasm, more recent studies have demonstrated benign tumors such as teratomas to be more common. Although rare, some prepubertal tumors such as teratomas may transilluminate. Ultrasonography should be used to evaluate hydroceles when the testis is impalpable.

Four-year experience with intranasal transilluminating dacryocystorhinostomy using ultrasound.

BACKGROUND: The aim of this study was to investigate the results obtained by intranasal dacryocystorhinostomy. METHODS: This was a prospective, non-randomised, non-comparative case series study of 150 patients with epiphora due to nasolacrimal duct obstruction. The same team of ophthalmic and otorhinologic surgeons performed the transilluminating intranasal dacryocystorhinostomy using ultrasound. The main outcome measures were subjective improvement of epiphora, dye testing, lacrimal probing, lacrimal irrigation, and endoscopic nasal examination. RESULTS: Symptomatic relief and patency of the lacrimal apparatus was achieved in 92% (138) of the patients at the 3-month follow-up and 89% (134) at the 6-month follow-up. At 1 year, only 2 patients of the 134 initial successes showed obstruction of the nasolacrimal duct. All 16 successful patients that we have followed for 4 years have sustained their improvement. INTERPRETATION: Transilluminating intranasal dacryocystorhinostomy is a quick and safe option for the treatment of nasolacrimal duct obstruction. The main advantages over external dacryocystorhinostomy are avoidance of skin incision, reduced haemorrhage, shorter duration of the operation, and quicker recovery of the patient.

Transillumination increases oocyte recovery from ovaries collected at slaughter. A new technique report.

An inexpensive and convenient method of collecting large number of oocytes for in vitro procedures is by aspiration of follicles visible on the surface of isolated ovary. This method yielded only moderate numbers of oocytes per ovary, and it was found that the yield could be improved by slicing the tissue to reach deep, cortical follicles. However, slicing was time consuming and increased chances for sepsis. We developed a new technique that allows direct viewing of cortical follicles for aspiration of oocytes by transillumination of the ovarian medulla and cortex with a Plexiglas rod inserted through a small incision at the hilus. The technique, called "Transillumination-Aspiration Ovary" (TAO), increased the oocyte yield by 50% per ovary. The oocytes are probably recovered from deeper follicles which are difficult to identify during regular oocyte aspiration. The oocytes had a normal grading and exhibited normal in vitro development efficiency. Using the "TAO" technique we recovered 777 oocytes from 2160 follicles in 106 ovaries, a recovery rate of 36% from follicles and a mean of 7.3 oocytes/ovary. When we aspirated only surface follicles, we obtained 523 oocytes in 1384 visible follicles in 107 ovaries, for a recovery rates of 37% but a mean yield of 4.9 oocytes per ovary. Mean number of follicles were 20.5% with TAO and 12.8% without, thus recovery rates of oocytes per follicle were similar with both methods, but yield of oocytes per ovary was higher with TAO, thus showing that the difference between the two methods lies in higher numbers of visible follicles with TAO. Moreover, with the TAO technique 71% of the total oocytes we recovered (n=551) were grade I or II oocytes, in which 52% cleaved to the 2 to 4-cell stage and 26% had reached the blastocyst stage. We conclude that the method is effective for accurately locating cortical and peripheral follicles that contain oocytes suitable for IVF and in vitro embryo production (IVP).

Strobophotoglottographic transillumination as a method for the analysis of vocal fold vibration patterns.

The purpose of this exploratory study was to determine if laryngeal transillumination in combination with stroboscopy (strobophotoglottography; SPGG) is useful for (1) the visualization of vocal fold vibration (VFV) opening patterns, (2) the localization of initial vocal fold opening in horizontal glottal thirds (anterior, midmembranous, and posterior), (3) determination of the temporal correspondence of the so-called electroglottography (EGG)-knee and initial vocal fold separation, and, finally, (4) automatized quantitative measurements of glottal area function within endoscopic images. With stroboscopic transillumination, initial inferior vocal fold separation was detectable during the "closed" phase, where the vocal folds were still closed in the upper portion and therefore initial inferior vocal fold separation could not be visualized with usual laryngoscopy techniques. In the horizontal plane within similar fundamental frequencies in modal voice registers in two male subjects, localization of initial glottal opening depended on the voice types used (soft, normal, or pressed phonation). We found zipperlike posterior-to-anterior openings, initial midmembranous openings, initial anterior openings, as well as simultaneous initial opening of all three portions in the two healthy male adults examined. This technique proved to add temporal and spatial information to vocal fold opening patterns and extends our examination techniques to the very beginning of vocal fold opening at the inferior portion. Simultaneous electroglottogram tracking and comparison with bidirectionally illuminated stroboscopic images revealed a time-locked correspondence of the EGG-knee with the aforementioned initial inferior vocal fold separation. Bidirectional illumination combined with digital color extraction techniques allowed for image separation of subglottally and supraglottally illuminated structures. This facilitated vocal fold contour detection and automatized image processing, for example, for determination of glottal area function, and is considered to be a further step to objective automatized quantitative measurements within endoscopic images.

Transilluminated powered phlebectomy: advantages and disadvantages of a new technique.

BACKGROUND: Transilluminated powered phlebectomy is a new procedure for minimal invasive varicose vein surgery. OBJECTIVE: To evaluate this technique for its benefit and the technique-related risks and complications. METHODS: Thirty patients were prospectively operated with this new technique by the same surgeon (11 of them bilaterally [41 legs in all]). According to the sonography, sapheno-femoral-junction ligation and stripping of the long saphenous vein were done if necessary. The phlebectomy of the side branches was done with the new system (Trivex System/Smith and Nephew). The postoperative follow-up was at 10 days and 6 weeks. RESULTS: There was no intraoperative complication. The mean operation time per leg was 40 minutes. Twenty-five patients had an uneventful postoperative course. Twenty two have been very satisfied with the cosmetically result. Two patients required reoperation because of postoperative hematoma. One patient developed a seroma, which could be managed via puncture. One patient developed persistent brown scar. The overall morbidity was 12.2%. CONCLUSION: Using transilluminated powered phlebectomy, multiple and large incisions could be reduced. A perfect cosmetic outcome might be reached if the surgeon is aware of technique-related complications. To evaluate the real value of this technique, further randomized trials are necessary.

[Transillumination technique in difficult intubations in heart surgery].

OBJECTIVE: The avoidance of myocardial depression still remains the goal of the management for cardiosurgical patients, also when unexpected difficulties in intubation face the anaesthetist. Therefore the difficult intubation should be managed by a short and easy procedure which provides safe results. To analyse the use of transillumination technique with a lightwand device (Trachlight(R), Laerdale, USA) in case of unexpected difficult intubation is the aim of our report. Methods All cardiosurgical patients (NYHA II - IV) were included from Jan 1998 - Dec 2001. After failure of the first intubation attempt by means of direct laryngoscopy (with non-adjustable vocal cord level) this intubation was qualified as an difficult intubation. In all these cases a lightwand device (Trachlight(R), Laerdale, USA) was applicated. Success, duration of the procedure, blood pressure, heart rate were recorded. Results 195 patients (out of total 7406) who could not be directly intubated by laryngoscopy (vocal cord level and arytenoid cartilage not visible), were classified as a difficult intubation. During the first year 1998 the light guided intubation (LGI) was successful as secondary procedure in 94 %, 3 cases, in which LGI failed the intubation was performed by fiberoptic method or McCoy blade. From 1999 to 2001 all difficult intubation could be managed by light guided intubation. In all cases of unexpected difficult intubation the procedure of the light guided intubation took as less than 3 min. The directly measured arterial blood pressure elevated by 14 % in comparison with the pressure prior to the passage through the larynx. Discussion and Conclusions 1998 after sufficient familiarisation of staff with the light wand device the transillumination technique was introduced as an alternative of using the McCoy blade or of using the fiberoptic method in the case of difficult intubation. Short neck or obesity, which occur as main reasons for intubation problems are surprisingly easy to control by light wand device. Therefore the light guided intubation could be an alternative procedure for unexpected difficult intubation in the setting of adult cardiac anesthesia.

Transillumination to evaluate spermatogenesis: effect of testosterone enanthate in adult male rats.

The present study has been designed to determine which stages of seminiferous cycle were modified by in vivo intramuscular administration of one or two doses of 25 mg of testosterone enanthate. Testicular weight was significantly reduced after treatment with two doses of testosterone enanthate (p < .05). Assessment of total lengths of stages per 100 mm of seminiferous tubules scored by transillumination showed that testosterone enanthate increased significantly the length of stages VI-VII, but it reduced total lengths of stages I, XII, and XIII-XIV. A dose-dependent reduction in the absolute and relative frequency of stage I was observed with testosterone enanthate treatment, whereas stages VI-VII were significantly increased. Analysis of relative frequencies of stages showed that stages XIII-XIV and stages I-V were significantly below the control value (1), whereas stages VI-VIII were over 1. In summary, results of the present study showed that transillumination is an adequate technique to assess the effect of drugs on spermatogenesis, and that testosterone enanthate modifies the pattern of the stages of the seminiferous epithelium cycle, before testicular weight is affected.

Non-ionizing near-infrared radiation transillumination spectroscopy for breast tissue density and assessment of breast cancer risk.

There is increasing attention to prevention as a means to reduce cancer incidence. Prevention interventions or therapies in turn rely on risk assessment programs to identify those women most likely to benefit from education and lifestyle changes. These programs are usually based either on interviews to identify ethnic, genetic, and lifestyle factors contributing to risk or on physical examination of the breast. For the latter it has been shown that the parenchymal density pattern observed in X-ray mammography can be used to assess an individual's risk. Extensive areas of dense, glandular tissue that are relatively radio-opaque are associated with higher breast cancer risk, with an odds ratio of 4 to 6 compared with women in whom the breast density is low owing to an abundance of adipose tissue. Near-infrared optical transillumination spectroscopy has been used previously to investigate the physiological properties of breast tissue. In this study, women were recruited who underwent recently X-ray mammography. The tissue density was assessed by a radiologist. The women then underwent optical transillumination spectroscopy, for which an instrument was developed that delivered visible and near-infrared light to the breast. After being transmitted through the breast craniocaudally in one of four quadrants, the spectrum from 625 to 1050 nm was measured. The spectra were used as input to a Principal Component Analysis (PCA) that used the corresponding mammographic density as the reference standard. The study group consisted of 92 women aged 39 to 72 years. Without further stratification for age, menopausal status, or measurement position, the PCA numerical model predicted the radiological assessment of tissue density in the mid 80% to low 90%.
