Taste alterations in liver cirrhosis: are they related to zinc deficiency?

Patients with chronic liver disease may have taste impairment and altered zinc metabolism. We evaluated Taste Detection Thresholds (TDTs) in 60 patients with liver cirrhosis and correlated the findings with disease severity and alcoholic etiology. Plasma zinc levels and urinary output were also measured. A placebo-controlled treatment trial with zinc sulphate was made in 15 patients with compensated cirrhosis in order to ascertain whether zinc deficiency caused taste alterations. Taste detection of salty, sweet and acid tastants was significantly impaired in all cirrhotic patients in comparison with normal subjects. TDTs were not influenced either by the etiology or the severity of the disease. All groups of patients had low plasma zinc levels and decompensated cirrhosis had a significantly increased urinary output of zinc. No correlation was found between taste acuity and plasma zinc levels when only cirrhotic patients were considered. The effect of zinc supplementation on TDTs did not appear to be inferior to that of the placebo. Our results indicate that taste impairment in cirrhotics is due to the disease process per se and not to zinc deficiency.

A case report of congenital aglossia.

A case of congenital aglossia was orthodontically treated and long-term retention was observed from the age of 15 to 27. The patient had congenital absence of three lower incisors, abnormally small mandible (micrognathia), severe overbite and a telescopic bite in the premolar region accompanied by an extremely narrow lower arch. Orthodontic treatment was begun when the patient was 15 years and 6 months old. The active treatment period was 2 years and 9 months. At the present time, nine years have passed since the end of the active treatment, and intercuspation between the upper and lower arches is still good. It is suggested that the maintenance of the lower arch width is essential for long-term retention in cases of congenital aglossia.

[Ageusia as the aftereffect of oxyfedrine use].

Out of oxyfedrine++ side effects known up to the present (mild agitation, stupor, heat sensation, pains in the epigastrium++, skin allergy) 24 cases of ageusia appearing usually after 4 weeks of treatment with oxyfedrine++ were presented. Disturbances of taste are found to be unpleasant for patients, and result in remarkable exacerbation of their general feeling. The complaints subside completely after finishing treatment, and the time of taste disorder withdrawal is prolonged according to the time of oxyfedrine++ treatment.

Long-term multicentric study with azelastine in patients with intrinsic asthma.

Efficacy and tolerance of azelastine (A 5610; CAS 58581-89-8), a new antiallergic compound with a unique structure, were investigated over up to one year of treatment in 225 male and female out-patients (age mean +/- s = 48 +/- 13 years) suffering from chronified intrinsic asthma. The trial was conducted by 24 German and Austrian consulting physicians of pneumology. Plasma levels indicated sufficient compliance in almost 90% of the patients. Tablets containing 4.4 mg azelastine HCl were administered b.i.d. in addition to the pre-existing antiasthmatic medication. Only other antiallergic agents were excluded. Adverse events, vital parameters and laboratory tests on safety were tightly assessed. Eighteen patients dropped out because of adverse events, 78 reported adverse experiences under therapy, normally mild and "vegetative" symptoms. No indicative findings regarding blood pressure, heart rate and laboratory tests were seen. The most frequent observations were taste disorders, asthenia and weight gain. No serious side effects at all were reported. The results on efficacy obtained from this non-controlled study may be predicative due to a baseline validation by an initial single blind placebo controlled run-in phase. Statistically significant improvements were seen for FEV1 and for the physicians' and the patients' rating on efficacy. To a lesser extent peak flow and airway resistance were improved, too. Thus, azelastine was safely tolerated over one year and might be effective in intrinsic asthma.

[Clinical study of taste abnormalities in advanced lung cancer cases].

Taste recognition thresholds for four basic tastes, sweet, salty, and bitter, have been examined in 36 advanced lung cancer patients to identify the presence or absence of taste abnormalities in such patients. Twenty-six untreated lung cancer patients, or 65.3%, demonstrated an abnormal taste threshold for at least one of the 4 taste, a result varying significantly from suitable controls (p less than 0.05). Among the total 36 lung cancer patients, the number of those showing a threshold abnormality was no different between the smoker and non-smoker groups, though abnormalities were higher in the low serum zinc level group than in the normal serum zinc level group (p less than 0.05). We also observed transient threshold elevations after chemotherapy including Cisplatin. Thus, it is thought that taste abnormalities are more prevalent in advanced lung cancer patients.

Symptoms such as tinnitus, hearing and taste disorders and facial palsy seldom occur in multiple sclerosis. Although ENT symptoms are few, vestibular and acoustic stapedius reflex tests combined with brainstem auditory evoked potentials (BAEPs) are available, and may give very useful evidence of the existence of MS. 52 patients with definite MS, and 13 patients with probable MS were examined (classified according to the McAlpine criteria). The vestibular and acoustic stapedius reflex tests were abnormal in 51%/61% and 61%/53% respectively of both groups. The corresponding figures for BAEP were 69% and 63%. The combination of the three tests on all patients showed at least one pathological finding.

Electrogustometry in hemodialysis patients.

Serous zinc levels and electrogustometry were studied on thirty patients undergoing periodic hemodialysis. After dialysis a better electrogustometric threshold (p less than 0,05) and also higher serous zinc levels were found. The two parameters showed no significant correlation. These results are suggestive of a toxical damage of taste receptors in the chronic renal failure.
