[Some concerns and semantic confusions about feminine skin].

Some characteristics of the skin are badly perceived by women living in a society where youth and idealized physical aspect are respected as a cult. Among the most frequent concerns, one should cite dry skin, wrinkles, striae distensae, cellulite, diffuse hair loss, hyperpilosity, as well as the unwanted effects of menopause and of the immoderate exposure to sun and sunbeds. Most of these concerns are subject to semantic confusions sustained by some medias. They are frequently targets of a worldwide ballyhoo conveyed to the general public without the support of sound physiopathological basis.

Aortic dissection, patent ductus arteriosus, iris hypoplasia and brachytelephalangy in a male adolescent.

We describe a 14-year-old male with dissection of the descending aorta, bilateral iris hypoplasia, striae distensae and brachytelephalangy, the latter being most marked in the thumbs. Inguinal herniae and a patent ductus arteriosus were surgically repaired in infancy. The pattern of abnormalities may constitute a previously undescribed syndrome. The proband died suddenly at the age of 17 years.

Validation of beighton score and prevalence of connective tissue signs in 773 Dutch children.

OBJECTIVE: Validation of the Beighton Score and the prevalence of connective tissue signs were investigated in Dutch children. METHODS: Hypermobility investigation according to Beighton was undertaken in 773 healthy children aged 4-12 years. An inventory of the signs that fitted with connective tissue disorders was compiled. RESULTS: The percentage of general hypermobility at a cutoff point of > or = 4 was 26.5% (range 11.4-49%) in children aged 4-9 yrs. At the age of 10-12 yrs, this percentage was 5.3% (range 0-7.1%). There was good agreement (kappa = 0.65) between the measurement on the left and the right sides at all ages. Of the investigated connective tissue signs, thin transparent skin was noted in 0.1%, blue sclerae in 0.1%, and an elevated palate in 2.3% of the children. It was observed that 8.2% of the children were able to touch their nose with their tongue (Gorlin's sign) and 23.7% were able to touch their chin. The other signs were not observed in any of the children. CONCLUSION: We validated the criteria for performing the Beighton Score in (Dutch) children. A cutoff point of > or = 5 should be chosen for Dutch children aged between 4 and 9 years. It appeared that one-sided Beighton Score could be determined during screening of healthy populations. Additional diagnostic tests are warranted if one of the following signs, such as a thin transparent skin, hyperelastic skin, large hematomas, prominent scars, molluscoid tumors, striae distensae, blue sclerae, elevated palate, and Gorlin's sign, is encountered in a child. Although it is not possible to extrapolate the data, we expect that the findings are relevant for children of Caucasian race.

[Skin diseases and obesity].

Obesity is a chronic multifactorial disease representing a major health problem. Among its consequences, diverse facets of the cutaneous physiology are altered. Some dermatoses are also more prevalent. The most typical ones are acanthosis nigricans, skin tags, signs of hyperandrogeny, striae distensae, stasis acroangiodermatitis, leg ulcers, lymphoedema and intertrigo.

ACTH- and prolactin-producing pituitary gland microadenoma with biphasic features of atypia and intermediate filament expression.

Herein, we report the case of a 28-year old woman clinically presenting with unclear weight gain over the last years. The patient displayed facial and neck edema in combination with unobtrusive striae distensae. Endocrinological examinations led to the diagnosis of Cushing's disease. Neuroradiological examination revealed an intrasellar tumor mass of 7 mm in diameter. Subsequently, transsphenoidal tumor resection was performed. Histological and immunohistochemical investigations revealed a pituitary gland adenoma showing a biphasic tumor growth pattern with two morphologically different tumor areas producing ACTH and prolactin respectively. Co-expression of ACTH and prolactin is exceedingly rare in pituitary adenoma. To our surprise, both tumor areas exhibited features of atypia consisting in elevated MIB-1 proliferation index in the ACTH-producing portion as well as p53 expression selectively in the prolactin-producing tumor parts. To our knowledge, this is the first case of an ACTH- and prolactin-producing pituitary gland adenoma exhibiting biphasic features of atypia.

White forelock in Marfan's syndrome: an unusual association, with review of the literature.

The cutaneous expression of Marfan's syndrome is generally limited to striae distensae and a number of uncommon associations with other dermatologic disorders. To date, there have been no reported cases of hair or nail abnormalities accompanying this disorder. We describe a patient with Marfan's syndrome who presented with an acquired white forelock and we review the literature on the association of poliosis with various entities.

[Blaschkitis in adults].

Many dermatoses may exhibit lesions distributed in a linear fashion: the linearity may be related to anatomical structures (blood or lymph vessels, nerves), to mechanical factors (urticarial dermographism, striae distensae cutis, frictional melanosis) or to some other provocative injuries being themselves linear (for instance Köbner's phenomenon). Some congenital or acquired dermatoses, either inherited or sporadic, have a linear distribution following the embryonic lines described in 1901 by A. Blaschko. Most of them are nevoid skin lesions present at birth or having a later onset: epidermal nevi (naevus unius lateris, linear porokeratosis), adnexal nevi (linear sebaceous nevus, linear basal cell nevus), pigmented lesions (systematized linear achromic nevus) or intricated nevi of the connective tissue (angio-lipomatous nevus). More seldom, genodermatoses related to a X-chromosomal mosaicism, occurring in females only, exhibit also a linear arrangement of their skin symptoms following Blaschko's lines: incontinentia pigmenti, focal dermal hypoplasia, etc. There are also very common inflammatory skin diseases, without any genetical background, which sometimes present as linear dermatoses. Their pattern according to Blaschko's lines is obvious in some cases: linear lichen planus, linear lichen nitidus, linear scleroderma, linear (zosteriform) vitiligo, linear fixed drug eruption, linear chronic lupus erythematosus. In some other conditions, the arrangement following these lines is probable (lichen striatus, linear eczema) or dubious (unilateral nevoid telangiectasias). This case report concerns a 38-year old man exhibiting relapsing inflammatory linear lesions of Blaschko's lines on the chest, back and upper limbs. Histological examination showed a non specific spongiotic dermatitis with rare necrotic keratinocytes and exocytosis of lymphocytes in the stratum spinosum.(ABSTRACT TRUNCATED AT 250 WORDS)

Pruritic urticarial papules and plaques of pregnancy.

Pruritic urticarial papules and plaques of pregnancy (PUPPP) is a distinct dermatosis characterized by onset in the third trimester. Lesions generally begin on the abdomen, particularly in the striae distensae. Primigravidas are disproportionately affected by this dermatosis. There is no associated fetal or maternal morbidity. Etiology and pathogenesis are unknown. The natural history is one of spontaneous resolution, and most patients respond well to symptomatic treatment with topical corticosteroids.

Striae cutis atrophicae. Morphokinetic examinations in vitro.

The in vitro behaviour of fibroblasts and epidermis cells taken from recently developed striae distensae was studied in morphokinetic examinations in the cell culture and the results were discussed. The findings give rise to the assumption that the antianabolous effects of the adrenocorticotrophic hormones do not only indirectly cause the atrophia of the epidermis by decrease of the elasticity of the connective tissue, but it appears that primarily epidermis and corium are simultaneously involved in the pathomechanisms of these alterations by irreversible cellular damage.

[Visual field in the Cushing syndrome (author's transl)].

According to traditional opinion the basophile adenoma, in contrast to the eosinophile or chromophobe tumours of the hypophyse, practically never causes lesion of the chiasm. However, the case of a 16-year-old girl suffering from adiposis, hypertonia, striae distensae and a "full moon face" is used to demonstrate that through the precis examination of the visual field on the Goldmann perimeter even with normal disque of the optic nerve, normal sella turcica established by X-ray, and uncharacteristic hormon condition, a slight chiasmal lesion caused by pressure from below can be established.

[Pseudoxanthoma elasticum associated with Marfan syndrome (author's transl)].

We described a 26-year-old man whose brother has a mottled fundus. Many maternal relatives suffer from myopia and heart disease, and one cousin has arachnodactylia. The patient is very tall and shows various abnormalities such as arachnodactylia, highly arched palate, funnel chest and scoliosis. He also shows hyperextensibility of joints, inguinal hernia, dissecting aneurysm of ascending aorta, dilatation of aortic ring, aortic and mitral insufficiency, myopia and striae distensae. These findings are that of Marfan's syndrome. At the same time, we noticed yellowish papules on the neck, axillae, inguinal region and inner aspect of thighs. Brown patches were found around umbilicus. These skin lesions as well as angioid streaks of fundus are typical for pseudoxanthoma elasticum. This case should be an association by chance of these two genetic affections.

[Physiological cutaneous signs in normal pregnancy: a study of 60 pregnant women].

We made a prospective study of 60 pregnant women from February 1992 to July 1992. Women with pathologic pregnancies were excluded. Fifty-one women had pigmentary changes; the most frequent was linea nigra in 45. Melasma was observed in 3 women only, and the other local melanosis were rare. Vascular changes appeared in 50 women and vascular spiders in 32. Only one vascular spider was seen on a leg; all the others were on the upper part of the body. Eighteen women had a palmar erythema. New striae distensae appeared in 37 women, the more often on the abdomen. Among the other non classified skin changes, acne appeared in 14 women, oedemas of the legs in 22, oedemas of the eyelids in 3 and molluscum fibrosum gravidarum in 4. We emphasize that skin changes are a frequent and polymorphous feature in pregnancy.
