On the potential of personal networks for hospitals.

OBJECTIVE: Today, there are many barriers that prevent seamless electronic voice and data communication with remote content and services that the user wants to access at a certain time and place. This is especially the case in hospitals, where the communication needs are very complex and context dependent, and where strict privacy and safety conditions apply. Personal network (PN) technology is expected to enable the various private networks of a single person to communicate seamlessly with each other, hiding underlying network complexity from the user. We have analyzed the usefulness of PNs in hospitals. METHODS: We studied the use case of a medical emergency surgery, and built a demonstrator by integrating models of the specialists' personal area networks, home networks, office networks, and a hospital network into a single virtual network. We performed user studies in various hospitals, using a combination of individual context interviews, scenario and requirements verification, guided experimenting with the demonstrator and concluding workshops. RESULTS: We show that, in case of medical emergencies, PNs can provide a means to enlist remote assistance from peers wherever they are in the world at a particular moment. It can also ease the communication between the operating theatre and the outside world, e.g. for educational purposes.

Homosexuality and adoption in Brazil.

Western societies are undergoing legal and policy changes in relation to laws governing the family, marital status, sexual orientation and the welfare of children, including in Brazil where, in the 1990s, the rights of homosexuals were incorporated into ongoing debates about what constitutes a family. This paper discusses the issue of adoption of children by homosexual men in Brazil, using information from court records from 1995-2000 in Rio de Janeiro, and from interviews with two judges, five psychologists and four social workers who evaluate those wishing to adopt. It uses the case records of one man's application to adopt, in which homosexuality became a central issue. Both the construction of masculinity in relation to parenting and concepts of the family were the parameters upon which the decision to allow him to adopt or not depended. Because the legislation does not specify what the sexual orientation of would-be adoptive parents should be, it is possible for single persons to adopt if they show they can be good parents. As more single people, alone or in couples, seek to adopt, it is important to clarify the criteria for judicial decisions on adoption applications. A dialogue is therefore needed on the meaning of family and whether and how it relates to sexual orientation. It is only on this basis that the courts can take a clear decision as to whether being homosexual is a relevant issue in regard to applications to adopt or not.

Awakening the global conscience: who will benefit from new vaccines in the 21st century?

The World Health Organization's (WHO) Global Programme for Vaccines and Immunization has three linked aims. The first is to give the so-called Expanded Programme on Immunization (EPI) vaccines to as many of the 125 million children who are born into the world each year as possible. The second is to ensure for the world an adequate supply of vaccines, and to safeguard vaccine quality. The third is in the field of vaccine research and development, not only coordinating the worldwide effort for new and improved vaccines but also preparing the way for their introduction into the EPI and continually to seek ways to simplify vaccination schedules. Enormous effort has gone into the polio eradication campaign. Following brilliant progress in both India and Africa, the goal of eradication by the year 2000 or shortly thereafter now seems achievable. Although a decision has not yet been taken, an attempt at measles eradication may well be the next huge challenge. The Global Programme for Vaccines and Immunization (GPV) is closely tied up with the Children's Vaccine Initiative (CVI) which is the name given to an umbrella organization which seeks to unite all the forces in the world promoting widespread use of vaccines. The chief functions of CVI are to be a strategic think tank, particularly as regards priorities; and to be a strong, consistent and effective advocacy group. Previously there had been some misunderstanding of the respective roles of WHO and CVI. These have now been cleared up by a dual stratagem--one single person, Dr JW Lee, as the administrative head of both Programmes; and one single Scientific Advisory Group of Experts or SAGE to guide the scientific aspects of both Programmes. The author has the honour of being Chairman of SAGE. It is apparent, however, that resource constraints are looming large in the affairs of both GPV and CVI. Clearly, the newer vaccines will be more expensive than the traditional ones, and the world must grope for new mechanisms to bring the fruits of the current upsurge of interest in vaccine development to all the countries of the world, not just the affluent ones.

Essential hypertension and social coping behavior.

Experimental studies of personality as a psychological risk factor in essential hypertension are reviewed. While no single personality trait has been found to be consistently and specifically related to hypertension, a certain behavior pattern emerges from this literature. This pattern, which includes increased anxiety, inappropriate coping behaviors in socially distressing situations and, possibly, a negative cognitive set, is compared with experimental findings from social competence research. Considerable overlap between the pattern of behavioral characteristics of hypertensives and individuals who display deficits in social skills is noted. The flight/fight concept in stress research, the differentiation of assertion from aggression and inhibition and their physiological equivalents are integrated in a three-dimensional model of social functioning in hypertensives. The clinical implications of this conceptualization are discussed in reference to the need for more comprehensive behavioral approaches to hypertension management.

Coping styles predict change in personality disorders.

The present follow-up study is the first to investigate whether dispositional coping styles prospectively influence change in the Millon Clinical Multiaxial Inventory (MCMI-II) personality disorders. The sample consisted of 155 psychiatric outpatients who where investigated twice--namely six and seven years after their admission to the clinic. Coping styles prospectively predicted change in most of the MCMI-II personality disorders, whereas sex and age did not affect the outcome. The use of problem-focused coping and the seeking of social support improved the patient levels on the MCMI-II personality disorder scores. Dysfunctional coping styles such as selfdistraction, disengagement, use of alcohol or drugs, and venting of emotions, however, aggravated the patient MCMI-II personality disorders. Furthermore, the effects of coping patterns tended to be specific for single personality disorders.

Interface between hospital and fire authorities--a concept for management of incidents in hospitals.

INTRODUCTION: Although every hospital needs a security plan for the support of immobile patients who do not possess autonomous escape capabilities, little information exists to assist in the development of practical patient evacuation methods. HYPOTHESIS: 1) In hospitals during disasters, incident leadership of the fire authorities can be supported effectively by hospital executives experienced in the management of mass casualties; and 2) As an alternative for canvas carry sheets, rescue drag sheets can be employed for emergency, elevator-independent, patient evacuation. METHODS: A hospital evacuation exercise was planned and performed to obtain experiences in incident command and to permit calculation of elevator-independent patient transport times. Performance of incident leadership was observed by means of pre-defined checklists. The effectiveness and efficiency of carrying teams with five persons each were compared to those with a rescue drag sheet employed by a single person. RESULTS: Incident command for hospitals during a disaster is enhanced considerably by pre-defined and trained executives who are placed at the immediate disposal of the fire authorities. For elevator-independent patient transport, the rescue drag sheet was superior to conventional carrying measures because of a reduced number of transport personnel required to move each patient. With this method, patient transport times averaged 54 m/min. flat and 18 seconds for one floor descent. CONCLUSION: Experiences from a hospital during an evacuation exercise provided decision criteria for changes in the disaster preparedness plan. Hospital incident leadership was assigned to executives-in-charge in close co-operation with the fire authorities. All beds were equipped with a rescue drag sheet. Both concepts may help to cope with an emergency evacuation of a hospital.

Personality and disinhibitory psychopathology: alcoholism and antisocial personality disorder.

We discuss the relation between personality factors and two adult forms of disinhibitory psychopathology--alcohol abuse or dependence and antisocial personality disorder. First, we briefly review various methodological issues relevant to research in this area. Next, we review empirical findings relating three broad-band personality trait dimensions neuroticism/emotionality, impulsivity/disinhibition, extraversion/sociability) to both alcohol abuse and dependence and antisocial personality disorder. Finally, theoretical models of the relationship between personality and each of these two disorders are presented. We conclude that although no single personality description is likely to be both a sensitive and specific indicator of either alcoholism or antisocial personality disorder, personality variables are important components of etiological models of these disorders.

Psychological aspects of severe obesity.

Studies of several overweight persons conducted before their undergoing antiobesity surgery have shown 1) that there is no single personality type that characterizes the severely obese; 2) that this population does not report greater levels of general psychopathology than do average-weight control subjects; and 3) that the complications specific to severe obesity include body image disparagement and binge eating. Studies conducted after surgical treatment and weight loss have shown 1) that self-esteem and positive emotions increase; 2) that body image disparagement decreases; 3) that marital satisfaction increases, but only if a measure of satisfaction existed before surgery; and 4) that eating behavior is improved dramatically. The results of surgical treatment are superior to those for dietary treatment alone. Practitioners should be aware that severely obese persons are subjected to prejudice and discrimination and should be treated with an extra measure of compassion and concern to help alleviate their feelings of rejection and shame.

Dependent children and suicide of married parents.

In an attempt to analyze the relatively neglected relationship between the presence of dependent children and suicide of married parents, the suicides of both married and single persons in the Chicago-Cook County region for 1970 and 1974 were examined. Single suicides and married suicides with dependent children were compared to similar groups in the general population. It was found that while marrieds with dependent children experienced the lowest average suicide rate, they also had a larger mean number of children than the population as a whole and exhibited a tendency for having children at a very young age or children at an older age in life. These findings contradict contemporary notions that an increasing number of children in a marriage tend to inhibit suicide potential. It appears that the relationship between marital suicide and dependent children is far more complex than previously indicated and requires further research for elaboration.

Mental illness among homeless individuals in a suburban county.

OBJECTIVE: This paper compares the prevalence of mental illnesses and alcohol and drug abuse and the residential histories of homeless individuals identified as having a mental illness and individuals who are not so identified. The cohort consisted of single persons applying for shelter over a 12-week period in Westchester County, a suburban county in New York State. METHODS: The sample of 201 persons (89 percent male, with a mean age of 37) represented 77 percent of consecutive single shelter applicants in a single-point-of-entry system over the study period. Information from an intake assessment was augmented by a semistructured interview to reconstruct subjects' residential history for the last five years, including periods of homelessness and time in institutions. RESULTS: Twenty-one percent of the cohort was classified as having mental illness. Seventy-two percent had a diagnosis of drug abuse or dependence, and 51 percent had alcohol abuse or dependence. For individuals with a mental illness, the use of cocaine and heroin was significantly lower, but alcohol use was somewhat greater, compared with other homeless persons. Persons with mental illness also experienced homelessness of some kind over a significantly longer period (a mean of seven years versus a mean of three years for other subjects), and they spent almost twice as many weeks during the previous five years literally homeless. Institutional time, most of which consisted of time in jail or prison, was equivalent for both groups. CONCLUSIONS: Not only is residential instability heightened among shelter users with mental illness, but over time public institutions play a critical role in their accommodations. For some homeless persons with mental illness, the circuit of shelters, rehabilitation programs, jails, and prisons may function as a makeshift alternative to inpatient care or supportive housing and may reinforce the marginalization of this population.

The urban homeless: estimating composition and size.

Although homelessness has been recognized as a serious and growing urban social problem, scientifically acceptable methods for estimating the composition and size of the homeless population have been lacking. A new research approach to estimating the size and composition of undomiciled urban populations is presented, and its utility is illustrated through a description of the literal homeless of Chicago. The homeless in the Chicago sample are unaffiliated persons living in extreme poverty, with high levels of physical and mental disability. Homelessness is interpreted as a manifestation of extreme poverty among persons without families in housing markets with declining stocks of inexpensive dwelling units suitable for single persons.

Haloperidol plus promethazine for psychosis induced aggression.

BACKGROUND: Health services often manage agitated or violent people and for emergency psychiatric services such behaviour is particularly prevalent (10%). The drugs used in this situation should ensure that the person swiftly and safely becomes calm. OBJECTIVES: To examine whether haloperidol plus promethazine is an effective treatment for psychosis induced agitation/aggression. SEARCH STRATEGY: We searched the Cochrane Schizophrenia Group's Register (July 2004). SELECTION CRITERIA: We included all randomised clinical trials involving aggressive people with psychosis for which haloperidol plus promethazine was being used. DATA COLLECTION AND ANALYSIS: We reliably selected, quality assessed and extracted data from all relevant studies. For binary outcomes we calculated standard estimations of risk ratio (RR) and their 95% confidence intervals (CI). Where possible we estimated weighted number needed to treat or harm (NNT/H). MAIN RESULTS: We identified two relevant high quality studies. One compared the haloperidol plus promethazine mix with midazolam (n=301) and one with lorazepam (n=200). The combined results were largely heterogeneous. In Brazil, haloperidol plus promethazine was an effective means of tranquillisation with over two thirds of people being tranquil or sedated by 30 minutes, but midazolam was more swift (n=301, RR 2.9 CI 1.75 to 4.80, NNH 5 CI 3 to 12). In India, however, 95% of people were tranquil or sedated by 30 minutes if allocated to the combination treatment (vs lorazepam, n=200, RR 0.26 CI 0.10 to 0.68, NNT 8 CI 6 to 17). Over the next few hours of treatment reported differences are negligible. One person given midazolam had respiratory depression (reversed by flumazenil), one given lorazepam had respiratory difficulty. A single person given haloperidol plus promethazine had an epileptic fit. Once the initial tranquillisation was administered, few needed additional medications for continued agitation (n=501, 2 RCTs, RR needing additional tranquillising drugs by four hours 1.67 CI 0.62 to 4.54, 4% vs 2%, I squared 50%) and there were no differences in the low levels of use of restraints. About 28% of people in Brazil in both groups had another episode of aggression in the first day after the initial injection (n=301, RR 0.89 CI 0.62 to 1.29). About half of all people in the Indian study were discharged by four hours (n=200, RR 1.13 CI 0.85 to 1.50) and a similar proportion in Brazil by 15 days (n=301, RR 1.05 CI 0.84 to 1.29). Both studies attained 99% follow up for their primary outcomes. Even by two weeks only 4% of people could not be accounted for (n=501, 2 RCTs, RR 0.91 CI 0.38 to 2.17). AUTHORS' CONCLUSIONS: This review suggests that both benzodiazepines work, but that midazolam has a faster onset and thereby reduces the risk of exposure to violence. Both benzodiazepines have the potential to cause respiratory depression, probably midazolam more so than lorazepam, and we would question the use of this group of drugs outside of those services fully confident of observing for and managing the consequences of respiratory distress. Most evidence, however, exists for the haloperidol plus promethazine mix, with currently more than 400 people randomised to the combination. The onset of action is swift and faster than lorazepam. The combination also seems safe with no clear longer term consequences. We would expect policy makers recommending other drug managements to have equally compelling evidence to support their guidance and hope that this would not be founded in conjecture or consensus, which may be more difficult to defend than evidence from high quality studies.

The effect of sex, marriage and age at first admission on the hospitalization of schizophrenics during 2 years following discharge.

Two hundred and eighty-two schizophrenics discharged from St John's Hospital, Stone, were followed-up. The reliability of the diagnosis was measured by the extent of agreement among the hospital clinicians. The number of readmissions and the time spent in hospital were ascertained and correlated with the sex, civil state and age at first admission of the subjects. It was found that a higher proportion of males than of females was readmitted; that the proportion of single persons among male schizophrenics is higher than among comparable age groups in the general population; that single males are more frequently admitted than single females; that the peak age of first admission for males is 10 years earlier than for females and that men whose age at first admission is below the median are more frequently readmitted than those whose age at first admission is above it. Women are more frequently married than men at the onset of schizophrenia, giving rise to the suggestion that marriage has a protective effect in schizophrenia. This hypothesis was not supported by our findings. The bearing of these findings on the course, genetics and marital handicap of schizophrenia is discussed.

[Addiction and personality].

Within the field of substance abuse, it is now widely admitted that the addictive personality does not exist. No one personality type is predisposed to addiction. The predisposition to drug dependence involves many different factors: psychological, social, familial, biological. None of these factors can be the sole determinant of drug dependence. Keeping that in mind, it is of interest to review the recent data on the relationship between personality traits or disorders and opiate and cocaine dependence. Using DSM and ICD categorical assessment, no single personality disorder emerged, instead a range of personality disorders has been evaluated in opiate and cocaine dependent subjects. Every type of personality disorders (PD) existed but cluster BPD were the most common (especially antisocial personality disorder in opiate addicts). However, it is noteworthy that a large minority to a majority of subjects did not display any king of PD. The implication of these results is that antisocial PD is probably over-diagnosed in drug dependence clinical settings. The studies reviewed failed to demonstrate that personality disorders were strong predictors of outcome in opiate or cocaine dependence. However, opiate dependent PD subjects entering treatment had more severe problems and lower retention rate than non PD subjects. But the amount of improvement was not significantly different between PD subjects and non PD subjects. This demonstrated that substance dependent PD patients could benefit from treatment whose intensity and duration must be adjusted. There is good support for the idea that Sensation Seeking trait is a vulnerability factor to substance abuse. But after dependence develops, sensation seeking is probably irrelevant to continued use of the drugs. This break between the psychopathology of vulnerability of substance abuse and the psychopathology of dependence raises the question of the existence of dramatically different factors involved in both phases of addiction.

The Italian Twin Project: from the personal identification number to a national twin registry.

The unique opportunity given by the "fiscal code", an alphanumeric identification with demographic information on any single person residing in Italy, introduced in 1976 by the Ministry of Finance, allowed a database of all potential Italian twins to be created. This database contains up to now name, surname, date and place of birth and home address of about 1,300,000 "possible twins". Even though we estimated an excess of 40% of pseudo-twins, this still is the world's largest twin population ever collected. The database of possible twins is currently used in population-based studies on multiple sclerosis, Alzheimer's disease, celiac disease, and type 1 diabetes. A system is currently being developed for linking the database with data from mortality and cancer registries. In 2001, the Italian Government, through the Ministry of Health, financed a broad national research program on twin studies, including the establishment of a national twin registry. Among all the possible twins, a sample of 500,000 individuals are going to be contacted and we expect to enrol around 120,000 real twin pairs in a formal Twin Registry. According to available financial resources, a sub sample of the enrolled population will be asked to donate DNA. A biological bank from twins will be then implemented, guaranteeing information on future etiological questions regarding genetic and modifiable factors for physical impairment and disability, cancers, cardiovascular diseases and other age related chronic illnesses.

Comparison of a diagnostic checklist with a structured interview for the assessment of DSM-III-R and ICD-10 personality disorders.

The International Diagnostic Checklists for the assessment of the DSM-III-R and ICD-10 personality disorders (IDCL-P) were compared with a structured interview, the International Personality Disorder Examination (IPDE), using a balanced test-retest design with forty psychiatric inpatients. The results, using pairwise kappa for the calculation of agreement, were as follows: any personality disorder versus no personality disorder 0.52 for DSM-III-R diagnoses and 0.75 for ICD-10 diagnoses. The range for the single personality disorders diagnosed at least five times was from -0.07 to 0.71 for DSM-III-R diagnoses and from 0.38 to 0.68 for ICD-10 diagnoses. Only for DSM-III-R diagnoses do figures exist from three other studies comparing two structured interviews with each other. The results of all four studies suggest that 60% of the variance in personality disorder diagnoses represents variance not attributable to the patients, which is scientifically unacceptable.

[Claude Bernard: a personal scientific fame that he probably would have not appreciated].

Claude Bernard has been a great medical physiologist of the ninenteenth century, who was able to convince biologists and physicians that the "experimental method" can and must be applied to the phenomena of living matter, as it is applied in physics and chemistry. With the concept of the "universality of the experimental method", the Modern Experimental Medicine begun to exist. However, C. Bernard deserves of further honor, having identified and promulgated a law of the living matter, i.e., the law concerning the "constancy of the interior biological environment". Such a law was later consacrated in 1929 by G.W. Cannon with the term "homeostasis". The homeostatic conception of biological functions had a great epistemological impact in the history of biology and medicine. As a matter of fact, all the modern biologists and physicians are today educated at the light of the homeostatic systems. Accordingly, C. Bernard is celebrated in his fame as the tutor of scientific medicine. Reading, however, what C. Bernard has written on the personalism and the eponimy of scientific ideas, one is prone to think that he would have not appreciated the dogmatic exaltation, the cult of his ideas on the biological constancy. He was aware that "... it is a fundamental rule in science to modify and change the ideas going on". And again "... But, since these ideas are not immutable verities, one must be ready to abandon and modify them when they cease to represent the reality". And again "The systems and doctrines are personal and pretend to be immortal and to preserve their personality. The experimental method, however, is impersonal; it zeroes the individuality and sacrifices the particular ideas of the a single person in favor of a general verity, which is established by means of the experimental criterion". And again "The great persons have never been promoters of absolute ed eternal verities in experimental sciences". And again "Any person who made the science to proceed, never pretended to define the limits; this person is destined to be surpassed and left apart by the progress of the future generations". And again "One of the most important obstacle that is encountered by humans along the course of knowledge is represented by the tendency of human acquisitions to be transformed into systems". With the discovery of chaos and biological systems in living matter, the law of homeostasis must be now revised.

When client means more than one: extending the foundational concept of person.

A second generation of nursing theory is emerging that clarifies and extends the original defining concepts. Definitions of person/client in specialties where practice focuses on more than single persons often include the term "aggregate." Analysis of this term reveals it to mean statistical summing of entities. Because this meaning does not incorporate interaction, the term is incompatible with domain assumptions. To retain the interaction condition, "client" for these specialties is extended to refer to both pluralities of persons and interactional units, such as families, groups, organizations, and communities.
