Perceptions of South Dakota adults concerning selected adolescent health problems.

A telephone survey was conducted of a random sample of 1,015 South Dakota households to assess adults' perceptions of whether alcohol and drug abuse, teen pregnancy, and suicide attempts are problems in their community, and across the state, among seventh through twelfth grade adolescents. A majority of the respondents indicated that alcohol and drug abuse, and teen pregnancy are a problem for adolescents in their community and across the state. Regarding suicide attempts among adolescents, over a quarter of the respondents indicated that they are a problem in their community, and one half of the respondents indicated that they are a problem across the state. Alcohol and drug abuse, teen pregnancy, and suicide attempts are more likely to be reported as being a problem across the state than in the local community. Respondents overwhelmingly agreed that health education programs in schools can help prevent alcohol and drug abuse, teen pregnancy, and suicide attempts among seventh through twelfth graders.

Mosquito surveillance employing New Jersey light traps on Indian reservations in Iowa, Nebraska and South Dakota in 1984 and 1985.

A mosquito surveillance program was established in 1983 on Indian reservations in Iowa, Nebraska and South Dakota. Standard New Jersey light traps were operated on 12 reservations throughout the summer seasons in 1984 and 1985. Rainfall levels in general were higher than normal at all sites in 1984 but were lower than normal at most sites in 1985. Aedes vexans was the most abundant species collected on all reservations and Culex tarsalis second in abundance. Man-biting collections of mosquitoes near Brookings in 1984 reached 35 mosquitoes/5 minutes in early July and were largely composed of Ae. vexans, Ae. dorsalis and Ae. nigromaculis while numbers of Cx. tarsalis and An. quadrimaculatus were minor. During the same period in 1985, man biting collections averaged less than 5/5 minutes.

Salmonella Thompson associated with improper handling of roast beef at a restaurant in Sioux Falls, South Dakota.

In October 1996, we investigated an outbreak of Salmonella serotype Thompson infections associated with Restaurant A in Sioux Falls, South Dakota, and conducted two cohort studies among persons who ate at luncheons catered by Restaurant A. Fifty-two Salmonella Thompson infections were identified between 29 September and 14 October 1996. Infections occurred among employees and patrons at Restaurant A and among attendees at three luncheons catered by the restaurant on 7 October. Roast beef cooked at Restaurant A was the only food item significantly associated with illness. Cooking times and storage temperatures for roast beef were inadequate to prevent multiplication of Salmonella, and the chefs were unaware of proper cooking and storage temperatures. We conclude that improper handling of roast beef probably caused this outbreak of Salmonella Thompson infections. Better knowledge of food safety practices by the cooking staff at Restaurant A, through required food safety education, might have prevented the outbreak.

Swine tuberculosis in South Dakota.

Mycobacteria were isolated from 195 of 200 lesions in lymph nodes identified as granulomatous by meat inspectors at 4 abattoirs in South Dakota. Mycobacterium avium serotypes 1 and 2 accounted for 89% of the isolants. Mycobacteria were isolated more frequently from lesions than acid-fast bacilli were observed on microscopic examination (P less than 0.001). The frequency with which mycobacteria was isolated was similar to the occurrence of granulomatous lesions. The numbers of the various kinds of mycobacteria isolated at each of the 4 abattoirs and for the 3 meat inspection disposition classes were not significantly different.

Medicare physician payment policies: a cost accounting analysis of three internal medicine office practices in South Dakota.

Medicare financed visits comprise a higher percentage of these solo internal medicine practices than in solo practices of all types in South Dakota or practices of any type in the USA. Investment opportunity costs, not apparently considered in payment reform policies, in the three practices evaluated are greater than professional liability costs which are highlighted in reform systems. Medicare office visit reimbursement under the Customary, Prevailing and Reasonable (CPR) system did not cover economic costs in these three office practices. The 1991 transition year projections predict practice 1 will lose over $7,000. In the unlikely event that budget neutral provisions are implemented in 1992, the three practices could experience an office revenue increase of 49%, still not equal to the economic incentives of non-Medicare visits. However, the direction taken by the Health Care Financing Administration (HCFA) in developing a model Medicare Fee Schedule would permit an average increase of only 19% to the office charges of the three practices. Office practices 1 and 3 will not break even without increasing Medicare office visit volume by 37% and 57% respectively.

Muellerius capillaris (Mueller, 1889) (Nematoda: Protostrongylidae): an unusual finding in Rocky Mountain bighorn sheep (Ovis canadensis canadensis Shaw) in South Dakota.

Lungs and fecal samples from nine hunter-killed Rocky Mountain bighorn sheep were examined for lungworms. All samples contained adults and/or larvae of Muellerius capillaris (Mueller, 1889). Protostrongylus spp., the lungworms commonly reported from bighorn sheep, were not present in any samples. Larvae of M. capillaris bear a spine on the dorsal side of the posterior end and are shorter than dorsal-spined larvae of other lungworms recorded from North American ungulates. Larvae similar in shape but longer than those of Muellerius were found in free-ranging bighorn sheep in Alberta and British Columbia. In addition, dorsal-spined larvae have been found in bighorn sheep in Montana, North Dakota, and Washington. The identity of the dorsal-spined larvae is known only from sheep in South Dakota. Thus, caution must be taken when diagnosing lungworm infections in Rocky Mountain bighorn sheep.

Influence of age, sex, and obesity on blood pressure of Hutterites in South Dakota.

Anthropometric and blood pressure data were evaluated in 724 male and female Hutterites and 273 randomly sampled male and female subjects in southeastern South Dakota who served as the control group. Hutterite males and females showed significant age-dependent increases of both weight and obesity (p less than 0.001) not seen in the control group. Both systolic and diastolic blood pressures (SP and DP) of the Hutterites were higher than those of the control subjects (p less than 0.001). There were highly significant correlations between age, weight, obesity (all p less than 0.001), and SP and DP of the Hutterites. Correlations between the weight and SP and DP were significant (p less than 0.001) in the control groups. The prevalence of systolic blood pressures greater than or equal to 140 mm Hg among Hutterite males ranged from 34.6 to 79.3 percent in age groups by decade vs 8.7 to 34.2 percent in age-matched control males. The prevalence of diastolic blood pressure levels greater than or equal to 90 mm Hg ranged from 0 percent in the youngest (less than or equal to 29 years) to 7.9 percent oldest age group (60+ years) of control males. The prevalence of elevated DP among male Hutterites ranged from 5.9 to 37.9 percent in corresponding age groups. A similar age-dependent trend of prevalence rates of elevated DP and SP were noted in female Hutterites but to a lesser extent than in control subjects. Elevated blood pressure levels among Hutterites may be an important contributing risk factor to the younger mean age of death of Hutterites compared to the control population.

Analysis of HLA haplotype segregation in the Schmiedeleut Hutterites of South Dakota.

HLA data from 1,085 South Dakotan Schmiedeleut Hutterites were examined for evidence of nonrandom transmission of haplotypes. The inheritance of haplotypes was viewed as a series of genetic contests between competing pairs of parental haplotypes using a maximum likelihood approach first put forward by Carlisle and Woodbury. Haplotype transmission probabilities were expressed in terms of weights, and the null hypothesis that the inheritance pattern was a random stochastic process, equivalent to the equality of the weights, was tested via the likelihood ratio. A total of 1,517 competitions was subjected to analysis, first globally using all data, and then for particular haplotypes of interest. Transmission of haplotype observed to compete with only a single other haplotype was also assessed using an exact procedure. No evidence of preferential transmission of HLA haplotypes was found. These results do not rule out transmission biases that may arise because of selection against homozygotes, mechanisms specifically affecting particular alleles or haplotypes not considered in the present study, or biases arising from maternal-fetal interactions.

Evaluation of a total dissolved solids model in comparison to actual field data measurements in the Cheyenne River, South Dakota, U.S.A.

During the summers of 2002 and 2004, in-stream integrated flow and concentration measurements for the total dissolved solids in the Cheyenne River, South Dakota, USA was conducted in order to compare the obtained actual field measurements with the predictions values made by the Bureau of Reclamation in the Environmental Impact Statement. In comparison to the actual field measurements conducted in this study, The Bureau of Reclamation extension of a small database used in the analysis for the impact of operations at the Angostura Unit over the past 50 years and into the future to predict the annual total dissolved solid loadings doesn't represent the actual loading values and various conditions in the study area. Additional integrated flow and concentration sampling is required to characterize the impact of the current Angostura Dam operations and Angostura Irrigation District return flows on the Cheyenne River in different seasons of the year.

Rural health care networks in South Dakota.

To address health care access concerns, a Regional Coordinated Care Network (RCCN) program, through Robert Wood Johnson Foundation Practice Sights Initiative funding, is being developed by the South Dakota Office of Rural Health. The intent of the initiative is to expand primary and preventive care and, hopefully, permanently reduce the number of Health Professional Shortage Areas (HPSAs) in the state by linking and enhancing providers and services. In rural areas such networking will be crucial for the delivery of health care. Surely, the impact on family physicians will be significant and salutary.

Perspectives on the diagnosis, epizootiology, and control of the 1973 duck plague epizootic in wild waterfowl at Lake Andes, South Dakota.

An epizootic of duck plague occurred in early 1973 in a population of 163,500 wild waterfowl, primarily mallards (Anas platyrhynchos), wintering on Lake Andes and the nearby Missouri River in southeastern South Dakota (USA). The diagnosis was based on pathologic lesions and confirmed by virus isolation. Control measures included quarantine, attempts to reduce virus contamination of the area, dispersal of waterfowl, and monitoring of wild waterfowl populations for mortality. The epizootic resulted in documented mortality of 18% and estimated mortality of 26% of the waterfowl at risk. Prompt implementation of control measures might have limited mortality to approximately 8%. Losses during the epizootic were equivalent to 0.12% of the annual mortality in the North American 1996 fall population of 80,000,000 wild ducks. The most likely sources of the infection were free-flying wild mallard or American black duck (Anas rubripes) carriers from the upper midwestern or northeastern United States. Duck plague serum neutralization antibodies were demonstrated in 31% of 395 apparently healthy mallards sampled prior to dispersal of the flock at Lake Andes, suggesting that tens of thousands of potential duck plague carriers entered the wild waterfowl populations of all four major flyways. Consequently, the absence of major epizootics of duck plague in wild waterfowl in the subsequent two decades is evidence that substantial numbers of duck plague carriers can occur in wild waterfowl populations without resulting in epizootic mortalities. The failure to isolate duck plague virus from apparently healthy mallards sampled during the epizootic raises questions concerning the validity of conclusions regarding the status of duck plague in wild waterfowl based upon negative results of random surveys conducted in the absence of epizootics.

Assisted reproductive technologies in South Dakota: the first ten years.

One in six couples experience infertility. New assisted reproductive technologies such as in vitro fertilization (IVF) have helped thousands of couples worldwide to have a family. IVF has been available in South Dakota for the past ten years. Improvements in the clinic and laboratory have led to better live birth rates and lower incidences of multiple pregnancies. Advances in technology will help even more people overcome fertility problems in the near future.

Walking forward: the South Dakota Native American project.

BACKGROUND: The "Walking Forward" program is a scientific collaborative program between Rapid City Regional Hospital, the University of Wisconsin, the Mayo Clinic, and partnerships with the American Indian community in western South Dakota-3 reservations and 1 urban population. The purpose is to increase participation of health disparities populations on National Cancer Institute clinical trials as part of the Cancer Disparities Research Partnership program. Clinical practice suggests that Native American cancer patients present with more advanced stages of cancer and hence have lower cure rates and higher treatment-related morbidities. It is hypothesized that a conventional course of cancer treatment lasting 6 to 8 weeks may be a barrier. METHODS: Innovative clinical trials have been developed to shorten the course of treatment. A molecular predisposition to treatment side effects is also explored. These clinical endeavors will be performed in conjunction with a patient navigator research program. RESULTS AND CONCLUSIONS: Research metrics include analysis of process, clinical trials participation, treatment outcome, and assessment of access to cancer care at an early stage of disease.

Ischemic nephropathy in South Dakota: case reports and review.

Ischemic nephropathy, a relatively new term coined to describe renal insufficiency due to renal artery disease, is neither a new subject for medicine throughout the country, nor a new problem here in South Dakota. It is an important and overlooked cause of renal insufficiency and is almost certainly under-diagnosed. Five case reports and a review of the entity are described to illustrate the diversity of this clinical presentation. Intervention in such cases was thought to preserve, or even improve renal function, delaying the onset of end stage renal disease (ERSD).

"Video lottery" and treatment for pathological gambling. A natural experiment in South Dakota.

Four agencies which offer specialized treatment for pathological gambling provided data on the number of inquiries about gambling treatment and the actual number of gamblers treated before, during, and after the shutdown of video lottery in the state of South Dakota. A marked decrease in the number of inquiries and number of gamblers treated was seen during the time the machines were turned off as compared to the time periods when video lottery gambling was available. The results suggest that the accessibility and availability of video lottery machines is an important factor in the number of people being adversely impacted by gambling.

South Dakota statewide Nursing Minimum Data Set Project.

The elements of the Nursing Minimum Data Set (NMDS) were collected manually from 188 medical records in eight acute care facilities. These eight facilities represent 54 per cent of the beds in South Dakota. The purpose of the study was to describe discharge destination, nursing diagnoses, nursing interventions, and nursing resource utilization for patients with fractured femur with pinning. The sample was primarily female (69.1 per cent), with a mean age of 78.5 years. Most (84.0 per cent) patients were transferred to another facility, with 46.2 per cent going to extended care facilities. The most frequent nursing diagnoses were comfort (89.9 per cent) and physical mobility (59.6 per cent). Interventions were classified using the 16-category classification scheme developed by Werley and Lang. The most frequently recorded types of interventions were in the category of monitoring and/or surveillance (16.7 per cent of 7,555 interventions), whereas emotional support and/or counseling was much less frequent (3.0 per cent of 7,555). Discharge planning was the most frequent nursing intervention in the category of coordination and collaboration of care (54.8 per cent of 188 patients). Documentation systems have been structured to accommodate technical tasks on flow sheets, for example. Nursing resource utilization was the most difficult, and also presently the least meaningful, NMDS element to collect because each facility has different staffing, different patient classification systems, and no prescribed method for collecting these data. Manual data collection is time-consuming and expensive and therefore not recommended.

DEEP, the South Dakota Medicaid drug use review program. Is the hassle justified?

Governmentally-mandated programs are a fact of life in health care. Such programs are often considered "hassles" by health care providers, who sometimes question whether such programs can justify themselves. The South Dakota Drug Evaluation and Education Program (DEEP) is a federally-mandated program of drug use review for Medicaid patients. While the program does not mandate changes in therapy, review of the program's first year of data indicates it caused change in 68.1% of the cases resulting in benefits which include a reduction in unnecessary/unneeded medication.
