A method to assess the proportion of treatment effect explained by a surrogate endpoint.
Randomized clinical trials are the standard for evaluating new drugs, devices and procedures. Traditional clinical trials entail not only considerable expense, but require considerable time to complete. The use of surrogate endpoints constitutes an effort to control cost and completion time for clinical trials. We propose a method to quantify the proportion of treatment effect explained by a surrogate endpoint based on a general model setting which includes the commonly used linear, logistic and Cox regression models. The interpretation of this quantitative measure is facilitated by graphical displays. To reduce the variability associated with the estimate, a meta-analytic approach is proposed based on random effects models. An example using real clinical trial data is given to illustrate the proposed procedures.

[Biphosphonate-associated osteonecrosis of the maxilla. Case report and review of the literature].
In recent years, a growing number of reports in the literature have linked osteonecrosis of the jaw bones with intravenously administered bisphosphonates prescribed for the treatment of hypercalcemia of malignancy due to bone lesions of multiple myeloma or bone metastases in patients with breast or prostate cancer. Furthermore, an association between chronic oral bisphosphonate use in patients with osteoporosis or Paget's disease, and bone necrosis in the mandible or maxilla has been demonstrated in numerous case reports and case series in the last couple of years. Therapeutically, osteonecrosis of the jaws seems to be difficult to treat surgically, often resulting in a recurring or even progressing lesion. In the present case report of a bisphosphonate-associated osteonecrosis of the maxilla in a patient with osteoporosis, the current literature will be discussed, and open research questions and potential problems for our daily dental practice routine will be addressed.
Prevention and management of osteoporosis in women with breast cancer and men with prostate cancer.
Advances in cancer treatment have resulted in improved life expectancies for survivors of breast and prostate cancer. As the number of cancer survivors grows, the long-term side effects of treatment play an increasingly prominent role in the routine care of these patients. Due to similar management approaches, survivors of breast and prostate cancer are at increased risk for osteoporosis. This review summarizes the prevention and management of osteoporosis and osteopenia resulting from cancer treatment in survivors of breast and prostate cancer.
Anabolic effects of parathyroid hormone on cortical bone in ovariectomized rats.
The purpose of this study was to determine whether PTH has anabolic effects on cortical bone in ovariectomized (OVX) rats similar to those previously observed in cancellous bone. Groups of OVX rats were untreated for the first 4 weeks postovariectomy followed by treatment for 5-, 10-, or 15-week periods with vehicle, estrogen, the bisphosphonate rise-dronate (NE-58095), PTH, PTH+estrogen or PTH+NE-58095. A group of sham-operated control rats was treated with vehicle alone. Cross sections of the tibial diaphysis immediately proximal to the tibiofibular junction were subjected to quantitative bone histomorphometry. In comparison to vehicle-treated control rats, the following structural changes were detected in cortical bone of OVX rats after 15 weeks of vehicle treatment: increased cortical bone tissue area, increased cortical bone area, and a trend for increased bone marrow area. Indices of periosteal bone formation were increased in vehicle-treated OVX rats at 5 weeks, but declined to control levels at the later time points. In addition, vehicle-treated OVX rats exhibited a nonsignificant trend for increased endocortical bone formation relative to vehicle-treated control rats throughout most of the study. Estrogen, but not NE-58095, prevented most of the changes in cortical bone structure associated with ovariectomy. In comparison to vehicle treatment of OVX rats, estrogen decreased both periosteal and endocortical bone formation whereas NE-58095 decreased endocortical, but not periosteal, bone formation. Treatment of OVX rats with PTH alone increased cortical bone area and width as well as decreased bone marrow area compared to vehicle-treated OVX rats.(ABSTRACT TRUNCATED AT 250 WORDS)
