Malaria in travelers returning from short organized tours to holiday resorts in Mombassa, Kenya.

BACKGROUND: Short trips to holiday resorts in Mombassa, Kenya, have gained popularity among Israelis since the early 1990s. A cluster of cases of malaria among returned travelers raised concern that preventive measures were being neglected. OBJECTIVES: To characterize the demographic and clinical features of malaria acquired in Kenya, and to assess the adequacy of preventive measures. METHODS: Data were collected from investigation forms at the Ministry of Health. All persons who acquired malaria in Kenya during the years 1999-2001 were contacted by phone and questioned about use of chemoprophylaxis, attitudes towards malaria prevention, and disease course. Further information was extracted from hospital records. RESULTS: Kenya accounted for 30 (18%) of 169 cases of malaria imported to Israel and was the leading source of malaria in the study period. Of 30 malaria cases imported from Kenya, 29 occurred after short (1-2 weeks) travel to holiday resorts in Mombassa. Average patient age was 43 +/- 12 years, which is older than average for travelers to tropical countries. Only 10% of the patients were fully compliant with malaria chemoprophylaxis. The most common reason for non-compliance was the belief that a short trip to a holiday resort carries a negligible risk of malaria. Only 3 of 13 patients (23%) who consulted their primary physician about post-travel fever were correctly diagnosed with malaria. Twenty percent of cases were severe enough to warrant admission to an intensive care unit; one case was fatal. CONCLUSIONS: Measures aimed at preventing malaria and its severe sequelae among travelers should concentrate on increasing awareness of risks and compliance with malaria chemoprophylaxis.

[The use of cryopreserved arterial allografts as a resort material in limb salvage surgery].

UNLABELLED: We report in this paper our experience and results in the surgical management of patients with critical ischemia of the lower extremities, utilizing cryopreserved arterial allografts as a resort material. MATERIAL AND METHODS: The series includes 31 consecutive patients with critical ischemia of the lower extremities, who received a cryopreserved arterial allograft as a resort material, from January 2001 to December 2002. Average age was 72.2 +/- 8.8 years, 26 were male and among the risk factors 58 were heavy smokers, 45% had diagnosed some cardiac/coronary disease and 29% suffered of chronic bronchitis. Almost 80% of the allografts were implanted in the leg arteries, beyond the level of the popliteal trifurcation. Allografts were provided by the Tissue Bank of our Hospital. RESULTS: Mid term patency rates (36 months) was 36.5%; however, the limb salvage rates was 60% for the same period of time. CONCLUSION: Limb salvage surgery of critical limb ischemia accomplished with cryopreserved arterial allografts provides acceptable results and should be taken in consideration as a resort alternative for the surgical management of these patients.

[Sanatorium-health resort treatment in the correction of the functional disorders in children with cardiovascular pathology who live in a large industrial city].

The incidence rate of functional disorders in the cardiovascular system as well as in the vegetative nervous system was found out to be on the increase in those children with cardial pathology residing in a large industrial centre. Sanatorium-health resort treatment was noted to make for improvement of vegetative regulation of the cardiac activity, enhancement of compensatory-and-adaptive potentialities of the organism, which facts warrant a wider use of health resorts in the rehabilitative treatment of children experiencing additional environmental loads of a large industrial centre.

[State and problems of health resort construction in the Caucasian Mineral Waters Region].

Although the requirement of population in recreation and rehabilitation services is increasing, the number of Russians using spa treatment at institutions of Caucasian Mineral Water Health Resort is now 3.5 times less than at the end of the eighties. The author suggests approaches to and methods for solution of problems of health resorts and their institutions, which should serve as landmarks for managing actions at regional and local levels.

Ballistocardiography in sanatorial and health resort rehabilitation of patients after myocardial infarction.

A sanatorial and health resort rehabilitation was carried out in patients after myocardial infarction. The rehabilitation program included exercise therapy and carbonic acid baths. The ballistocardiographic studies conducted with a diagnostic purpose indicate that carbonic acid balneotherapy may be used in patients after the third month following the onset of myocardial infarction. Clinico-physiological investigations indicate that ballistocardiography may serve to control the effectiveness of sanatorial and health resort rehabilitation and to determine the mechanism of action of the physical factors involved.

[Prospects for sanatorium-health resort treatment of oncological patients].

The current opinion that oncological patients subjected to radical treatment are not allowed to be directed to sanatoriums and resorts should be revised. It is necessary to plan joint investigations of oncologists and specialists or resort cure to determine exactly indications and contraindications to subsequent cure of such patients under sanatorium conditions in early terms after cessation of radical therapy. Patients, previously treated for malignant tumors and showing no signs of recurrence or metastases, may be directed to appropriate sanatoriums for treatment of concomitant lesions, but without applying mudtherapy, ultraviolet irradiation and general heat procedures.

Preserving the environment in the resort of Bankya.

The town of Bankya is a balneological resort (spa) situated in a picturesque valley 17 km west from Sofia. It is a place suitable for the treatment of cardiovascular diseases and certain functional diseases of the nervous system. As a national resort, it applies stricter environmental regulations for activities that might endanger the environment, including road transport. The aim of this study was to establish the health status of Bankya townspeople before the pending construction of a highway. The morbidity of the population of Bankya and neighbouring towns and villages was compared to that of the capital Sofia for the period between 1997 and 1999. The data were statistically processed using the non-parametric, chi-square analysis. The results showed similar morbidity in children from Bankya and neighbouring towns and villages, but it was significantly lower than in children from Sofia. The results for adults were also in favour of Bankya--lower morbidity than in the neighbouring towns and villages, and less than 50% of disease cases recorded for Sofia.

[The hemodynamics of the brain and eye in patients with presenile and senile nonexudative central chorioretinal dystrophy during health resort treatment].

Rheographic studies of the cerebral and ocular hemodynamics carried out in patients with presenile and senile nonexudative central chorioretinal dystrophies over a 26-day course of multiple-modality health resort treatment revealed left-side asymmetry of the earlier and more marked injuries of the ocular membranes and the carotid system, a low rheographic index of the carotid system as early as in the first stage of the disease. The findings evidence improvement of the brain and eye hemodynamics in the course of treatment, this being conducive to improvement of the ocular function during all stages of the disease. The most dynamic parameters were the rheo-ophthalmographic coefficient in the patients with all stages of the disease and the rheographic index of the central cerebral artery in those with Stages I and II maculodystrophy. Assessment of these parameters in the course of health resort therapy of the retinal conditions mentioned will help assess the treatment efficacy.

[Adaptation reaction to the health resort treatment in patients with diabetes mellitus].

Physical, clinical, biochemical, and functional examinations have been carried out in 70% of patients with diabetes mellitus type 2 treated in health resorts. Patients were treated with diet, drugs, and physiotherapy during a 3-5-week period. It was found, that the whole period of the treatment may be divided into 3 subperiods: period I is characterized by the subjective symptoms produced by the process of adaptation to different environment. This period lasts for 4 days, on average. Periods II and III are characterized by the set of symptoms reflecting a sum of therapeutical physical stimuli. Such symptoms are known in the literature as bath- or spa-reaction. Symptoms of spa-reaction were noted in about 50% of the examined patients. In 21% of them late reaction, i.e. after about 3 weeks, was seen. No worsening of diabetes mellitus was observed during spa-reaction. No correlation between the onset of this reaction and duration of the disease as well as the results of therapy was found. However, spa-reaction was more frequent in the elderly. Period III was followed by the decrease of the symptoms of spa-reaction with marked improvement in glycaemia. Authors' observations lead to some practical conclusions concerning the programs of physiotherapy in dependence on the course of patients' adaptation to the spa. Regulations concerning the routine 24-day treatment period of all patients in health resorts should be revised.

[Results of early rehabilitation after various methods of cholecystectomy at Ust'-Kachka resort].

Early rehabilitation after open cholecystectomy or cholecystectomy from a mini-approach was performed in 101 patients with cholelithiasis treated surgically at Ust-Kachka health resort. Changes in clinico-laboratory indices reflecting the condition of hepatopancreatobiliary system and central hemodynamics before and after rehabilitation were studied depending on the variant of cholecystectomy. Advantages of early rehabilitation of patients after different variants of cholecystectomy at Ust-Kachka health resort in normalization of general condition of the patients and in prevention of functional disorders progression early after operation are demonstrated.

[Evaluation of the effectiveness and limits of balneology therapy for arthrosis in Lacu Sărat Health Resort, Brăila, Romania, in the period of 1994-2003].

This research tried to estimate the importance of the precocious diagnosis in the arthrosis. It also analyzed the efficiency and limits of the balneology therapy based on the facilities offered by Lacu Sărat Health Resort, Brăila. The approaching method consisted of a retrospective analysis of 10565 standard records of patients suffering from arthrosis, hospitalized in the health resort, between 1994-2003. The cases fell into various categories, according to the illnesses, as follows: 2854 (27.01%) gonarthrosis, 1247 (11.82%) coxarthrosis, 2001 (18.91%) spondylosis, 2003 (18.87%) peripheral arthrosis. The balneology treatment was applied individually, taking into consideration the location, clinical stage and associated illnesses. The treatment also included physiotherapy and kinetic therapy, adjusted to the stage of disease. The treatment results were good in 73.30% of cases. The patients' recovery aims at stabilizing the development of the illness, removing the objective as well as subjective suffering and maintaining the mobility and ability of the affected segment. The conclusions of the research underline the particular contribution of the clinical evaluation in diagnosis and the efficiency of external balneology in removing the pain and inflammation, maintaining the articular mobility and ability. Thus, the cure can ensure an appropriate articular comfort for most of the patients, taking into consideration the recurrent character of the illness, with repeated periods of suffering and pain.

[Health resort treatment of children with progressive muscular dystrophy].

The results of a complex treatment at health resorts of 126 children with Duchenne myopathy and 84 children with spinal amyotrophies at the age of 5-15 years are given. The complexes included carbon-dioxide sulfurated hydrogen baths, mud application (with different localization depending on a form of the disease), fresh baths, oxygenotherapy and therapeutic physical training. The authors revealed good tolerance to procedures and the positive dynamics of the main clinical manifestations of the disease, confirmed by the electrophysiological and biochemical studies. As a result of the treatment it was possible to achieve progressiveness of the disease in all patients at different periods (from 1 month to 1 year). The authors give recommendations for the use of treatment at health resorts as a necessary link in the general chain of rehabilitative measures for children with various forms of progressive muscular dystrophies.

[Efficacy of natural therapeutic factors of Belokurikha health resort in rehabilitation of patients with chronic prostatitis].

A total of 135 patients with chronic prostatitis have undergone rehabilitation in Belokurikha health resort. Of them, 35 patients of group 1 received basic therapy (nitrosiliceous baths, therapeutic exercise, massage of the prostate, psychotherapy); 40 patients of group 2 received basic therapy plus rectal tampons with sulphide mud from the lake Gorkoye of the Altai Administrative Area; 60 patients of group 3 were given basic therapy plus rectal lavage of the prostate location with nitrosiliceous thermal water of Belokurikha health resort. Treatment efficacy was assessed by scales of the symptoms and sexual disorders, ejaculate tests, testosterone levels, immune status, results of Doppler vascular mapping of the prostate. The highest effect was achieved in group 3 (87.3% response). This therapy relieves inflammation, stimulates a sexual function, improves hemodynamics in the prostate, raises the number and motility of spermatozoa, corrects immune disorders, normalizes testosterone concentration in the blood. In group 2 the response was obtained in 81.6% patients. This treatment improves hemodynamics, raises the number of spermatozoa. In group 1 the effect was observed in 60.4% patients, it manifested with an insignificant improvement in prostatic hemodynamics, spermatogenesis, hormonal and immune systems.

[The early rehabilitation of patients, operated on for peptic ulcer and cholelithiasis, at Mirgorod health resort].

The authors analyzed the condition of 51 patients in the early period of operation for ulcer and cholelithiasis. It was found that most patients showed asthenic, dyspeptic and pam syndromes, anemia, hypoacidic or achylous gastritis, inflammation at the operative site. As a result of treatment at the Mirgorod health resort (physiotherapy, mineral water) there occurred a significant improvement in the patients' condition. It is concluded that rehabilitation of patients following surgical intervention on the 20th day of operation for ulcer and cholelithiasis at the Mirgorod health resort has good perspectives.

[Effectiveness of climatic health resort complex treatment of patients with pneumoconiosis and dust-induced bronchitis using underwater massage shower].

The article provides the results of studies of 167 cases affected with occupationally-related pneumoconiosis and dust bronchitis. 117 patients were treated with complex resort therapy including under-water massage, and 50 patients were given the same complex therapy without shower massage. The effectiveness of the therapeutic techniques were evaluated basing on both objective and subjective criteria, including supplementary laboratory and functional techniques. The results were positive in both groups, but in the major group, the favourable shifts were more expressed but statistically were not reliable. Thus, the underwater shower massage may be regarded as a positive but not decisive factor in the complex resort treatment of occupational diseases of lungs.

[10-year experience with the health resort treatment of eye diseases].

Presented are the kinds of ocular diseases which can be qualified for a health-resort therapy and the kinds of measures the most beneficial in particular cases. After the cure an improvement in the condition of the eyes was found in 68 p.c. of cases. On the basis of results one may conclude that the rehabilitating health-resort treatment is worth to be suggested.
