[On the epidemiological situation in quarantine, natural focal and other infections on the territory of the Southern Federal District].

The data on the sanitary and epidemiological situation in the Southern Federal District are presented. The analysis of morbidity in tuberculosis, measles, HIV infection, viral hepatitis A, typhoid fever, cholera and quarantine infections, Crimean hemorrhagic fever, West Nile fever, rabies, malaria has been carried out. Special attention has been given to "new and newly returning infections", and among them to the spread of SARS ("atypical pneumonia"). The role of regional epidemiological safety programs, in particular such program as "The prophylaxis of quarantine and natural focal infections and the sanitary protection of the territory of the Southern Federal District of the Russian Federation from the import and spread infectious diseases in 2003-2005", has been substantiated.

Postexposure prophylaxis, isolation, and quarantine to control an import-associated measles outbreak--Iowa, 2004.

On March 12, 2004, a college student infectious with measles returned to Iowa from India by a commercial airliner; the case was subsequently linked to two other measles cases. This report updates information about this outbreak and provides details regarding vaccination, quarantine, and other measures used by Iowa public health authorities to interrupt disease transmission in a vulnerable population. The effective uses of quarantine and isolation during the outbreak underscore the utility of these public health tools in halting communicable disease transmission.

Quarantine after an international biological weapons attack: medical and public health requirements for containment.

The world now faces the dreadful possibility of biological weapons attacks by terrorists. Healthcare systems would have to cope with such emergencies should all preemptive measures fail. Information gained from the Global Mercury exercise and the SARS outbreak has shown that containing an outbreak at the start is more effective than reacting to it once it has spread and that containment should be treated both nationally and internationally. On the national level this entails developing rapid and effective methods to detect and identify infected cases, and implementing isolation and control measures to lower the risk of further transmission of the disease while assuring the safety of medical teams and laboratory workers. Strategic contingency plans should incorporate well-defined procedures for hospitalization and isolation of patients, providing regional backup of medical personnel and equipment and maintaining close cooperation between the various bodies in the healthcare system. Quarantine is an effective containment measure, especially if voluntarily imposed. Modern communication systems can help by sending professional teams timely instructions and providing the public with information to reduce panic and stress during quarantine procedures. Informing the public poses a dilemma: finding a balance between giving advance warning of an imminent epidemic outbreak and ascertaining the likelihood of its occurrence. Containment of international bioterrorist attacks depends entirely on close international cooperation to implement national and international strategic contingency plans with free exchange of information and recognition of procedures.

[The quarantined dialysis room for Australia-antigen-positive patients. Theoretical, normative, structural, operational, organizational and management aspects].

The technical features of dialysis facilitate the transmission of hepatitis B virus and the outbreak of possibly serious epidemics among patients and hospital staffs. Statistical, epidemiological and clinical data have been collected on many occasions. They serve to emphasise the problem and the need for quarantined centres. The establishment of such a centre for HBsAg-positive patients is discussed in the light of personal experience in the design and setting up of quarantined rooms. The underlying technical and structural requirements are also examined. Reference is also made to two years' experience in the running of a room of this kind. Stress is laid on the importance of having a room for these patients, not only from the clinical standpoint (lessening of contagion), but also from teh social and exonomic standpoint (reduced costs and length of hospital stay, greater rehabilitation).

Quarantine at Grosse Ile.

In 1832 British colonial authorities established a quarantine station at Grosse Ile, an island near Quebec City, after cholera outbreaks in northern England aroused fears that growing numbers of emigrants would carry it to the New World. Later, treating those suffering from other contagious diseases, such as typhus and smallpox, Grosse Ile played a notable role as Canada's principal quarantine station.

[Quarantine and health control of the squirrel monkey (Saimiri sciureus) (author's transl)].

A total of two hundreds and seventy squirrel monkeys (Saimiri sciureus) of both sexes were imported from South America through an animal dealer of the U. S. A. in eight lots from May, 1973 to July, 1974. They were investigated for the presence of the parasites for a maximum of fifteen months including nine weeks of quarantine period from the time of import. The mortality rate before or immediately after arrival at our laboratory reached 56.7% in the first two lots. This, however, could be reduced later to 4.8% by improving some conditions of transportation. Bacteriological surveys revealed no shigella or salmonella infections during the quarantine period. No tuberculin positive reactors were also detected. Filarial infestations seemed to be common in this species; about 45.5% had adult worms in the peritoneal cavity and about 65% harbored microfilaria in the peripheral blood. Acanthocephla was also found in a high rate in the lower alimentary tracts. It has been suggested that the worms disappear in fifteen months by keeping animals in the intermediate host-free circumstances. The sarcocyst was found in the femoral muscle of 7 (18.9%) of 37 animals examined histologically.

Real-time PCR for simultaneous and quantitative detection of quarantine phytoplasmas from apple proliferation (16 SrX) group.

A real time PCR assay conjugated with the fluorescent SYBR Green I dye has been developed for rapid, sensitive and quantitative detection of 'Ca. Phytoplasma pyri', 'Ca. P. prunorum' and 'Ca. P. mali', quarantine members of apple proliferation (16 SrX) group. The selected primers amplify specifically a target of 217-bp fragment from the 16 Sr gene region of the 16 SrX group and not from any other tested phytoplasma groups. An artificial template consisting in a plasmid clone of a 1785-bp DNA fragment of the 16S rRNA gene, 16S/23S rDNA spacer region, tRNA-Ile and partial 23S rRNA gene of a 'Ca. P. prunorum' isolate, was used to establish a calibration curve to evaluate the number of amplified targets per sample. The sensitivity of the technique was similar to nested-PCR (10 copies of the amplified target per microl). The estimated concentration of phytoplasmas in infected pear, plum and apricot trees ranged from 9.7 x 10(3) to 3.0 x 10(5) phytoplasmas per gram of tissue. The method offers the possibility to detect simultaneously, in a single reaction, all quarantine phytoplasmas affecting fruit trees hosts in Europe.

Towards specific diagnosis of plant-parasitic nematodes using DNA oligonucleotide microarray technology: a case study with the quarantine species Meloidogyne chitwoodi.

In order to investigate the feasibility of a microarray-based method for diagnostics of plant-parasitic nematodes, we have developed a DNA oligonucleotide microarray to detect the nematode species Meloidogyne chitwoodi, which is listed as a quarantine organism in Europe. Oligonucleotide capture probes were designed from nematode SCAR and satellite DNA sequences and spotted onto epoxy-coated glass slides. PCR products were generated using specific primers, labeled with Cyanine 3 or Cyanine 5 fluorescent dyes, and hybridized overnight to the microarray. This methodology allowed the specific detection of M. chitwoodi DNA in pure and mixed samples (i.e. when M. chitwoodi DNA was mixed with DNA from a congeneric nematode species). Simultaneous hybridization of the microarray with two amplified targets labeled with different dyes proved to be efficient, without any competition between the targets. These results illustrate a significant step forward in the development of the DNA chip technology for nematode detection, and constitute to our knowledge the first report of production and use of oligonucleotide microarrays for the detection of plant-parasitic nematodes, using the quarantine species M. chitwoodi as a test organism.

A molecular protocol using quenched FRET probes for the quarantine surveillance of Tilletia indica, the causal agent of Karnal bunt of wheat.

The current surveillance protocol for Karnal bunt of wheat in most countries, including the USA, European Union (EU), and Australia, involves the tentative identification of the spores based on morphology followed by a molecular analysis. Germination of spores is required for confirmation which incurs a delay of about two weeks, which is highly unsatisfactory in a quarantine situation. A two-step PCR protocol using FRET probes for the direct detection and identification of Tilletia indica from a very few number of spores (< or =10) is presented. The protocol involves amplification of the ITS1 DNA segment in the highly repeated rDNA unit from any Tilletia species, followed by FRET analysis to detect and unequivocably distinguish T. indica and the closely related T. walkeri. This rapid, highly sensitive, fluorescent molecular tool is species-specific, and could supersede the conventional microscopic diagnosis used in a quarantine surveillance protocol for Karnal bunt which is often confounded by overlapping morphological characters of closely related species.

[Ensuring the safety of erythrocyte concentrates using quarantine].

We have decided that it is essential to establish a process to protect patients receiving concentrated red blood cells (CGR) from a residual risk of contamination by the HIV virus or by the hepatitis C virus. Viral inactivation techniques of labile blood products are not presently in use, only safety measures obtained by quarantine allow a reduction of this risk to a value approaching 0. We have developed, from known deep-freeze methods of red blood corpuscles, a process to place the samples of concentrated red blood cells in quarantine by means of deep-freezing. We have modified and automated the deep-freezing, thawing and washing manipulations of the red blood corpuscles in order to obtain a final product which is standardized in quantity and quality. The result of our work will show: That all the means employed are not a source of contamination of the end product. That the procedures employed may be easily adapted to all Blood Transfusion Centers. That the end-product may be used after 21 days of conservation. That the extra cost entailed by this procedure is realistic.

The quarantine of philosophy in medical education: why teaching the humanities may not produce humane physicians.

Patients increasingly see physicians not as humane caregivers but as unfeeling technicians. The study of philosophy in medical school has been proposed to foster critical thinking about one's assumptions, perspectives and biases, encourage greater tolerance toward the ideas of others, and cultivate empathy. I suggest that the study of ethics and philosophy by medical students has failed to produce the humane physicians we seek because of the way the subject matter is quarantined in American medical education. First, the liberal arts are seen as the province of undergraduate education, and not medical school. Second, philosophy, when taught in medical school, is seen by students as just one subject to be mastered along with many other more important ones, and not as a way to foster critical thinking and empathy. What is needed is a new pedagogy that combines both cognitive and affective elements to implant and nourish the liberal arts in students. Removing the quarantine of philosophy from other facets of medical education is an important first step.

In vitro characterization of solvent/detergent-treated human plasma and of quarantine fresh frozen plasma.

BACKGROUND AND OBJECTIVES: Plasma pools, solvent/detergent(S/D)-treated plasma produced from plasma pools, and single donor fresh frozen plasma that had been quarantined for at least 6 months (QFFP) differ in their composition regarding clotting factors, inhibitors and other important plasma proteins. There are poor data concerning stability of important clotting factors after thawing of frozen plasma units. MATERIALS AND METHODS: 12 plasma pools, 12 batches of S/D plasma produced from these respective plasma pools, and 12 units of QFFP were extensively analysed. The stability of fibrinogen and factors V, VII, and VIII after thawing, storage at room temperature and at +4 degrees C was also examined. RESULTS: We extensively analysed plasma pools before and after solvent/detergent treatment as well as quarantined single donor plasma units for parameters of coagulation and fibrinolysis. After the S/D step, all clotting factor activities and the activities of most inhibitors and other plasma proteins were in the normal range in all batches. Protein S and plasmin inhibitor activities decreased by 35% and 76%, respectively. S/D treatment partly activated factor VII (FVII). However, there were no marked increases of other markers of activated hemostasis. The interindividual variations of all proteins analysed were significantly lower in the S/D plasmas than in the single donor plasma units. An 8-hour storage lead to a marked decrease of FVIII activity, whereas there was no significant influence on fibrinogen and factors V and VII. CONCLUSIONS: There are no critical reductions of the activities of clotting factors, inhibitors, and other important plasma proteins due to S/D treatment. Efficacy and safety of S/D plasma is not hampered by reduced activities of protein S and plasmin inhibitor. Dosage calculation and the evaluation of clinical response is simplified by usage of the more standardized S/D plasma compared to QFFP.

Alternatives to methyl bromide treatments for stored-product and quarantine insects.

Methyl bromide is used to control insects as a space fumigant in flour and feed mills and ship holds, as a product fumigant for some fruit and cereals, and for general quarantine purposes. Methyl bromide acts rapidly, controlling insects in less than 48 h in space fumigations, and it has a wide spectrum of activity, controlling not only insects but also nematodes and plant-pathogenic microbes. This chemical will be banned in 2005 in developed countries, except for exceptional quarantine purposes, because it depletes ozone in the atmosphere. Many alternatives have been tested as replacements for methyl bromide, from physical control methods such as heat, cold, and sanitation to fumigant replacements such as phosphine, sulfuryl fluoride, and carbonyl sulfide, among others. Individual situations will require their own type of pest control techniques, but the most promising include integrated pest management tactics and combinations of treatments such as phosphine, carbon dioxide, and heat.

Ionizing irradiation quarantine treatment against oriental fruit moth (Lepidoptera: Tortricidae) in ambient and hypoxic atmospheres.

Oriental fruit moth, Grapholita molesta (Busck), is a pest of many rosaceous temperate fruits, including pomes, Malus spp., and stone fruits, Prunus spp., in much of the world. However, some areas are free of the pest, and shipments of fruit hosts from infested to noninfested areas may be regulated. Current quarantine treatments for oriental fruit moth include methyl bromide fumigation and cold storage for several weeks. Methyl bromide use is being restricted because it is a stratospheric ozone-depleting substance, and alternatives are sought. Cold is not tolerated by many hosts of oriental fruit moth. The objective of this research was to develop irradiation quarantine treatments against the pest under ambient and hypoxic storage conditions because some hosts of oriental fruit moth are stored in hypoxic atmospheres, and hypoxia is known to lessen the effects of irradiation. In ambient atmospheres, no adults emerged from 58,779 fifth instars (the most radiotolerant stage present in fruit) irradiated with a target dose of 200 Gy (195-232 Gy measured). In atmospheres flushed with nitrogen, 5.3% of adults emerged from 44,050 fifth instars irradiated with a target dose of 200 Gy (194-230 Gy measured), but they died at a faster rate than control adults and without laying eggs. A dose of 232 Gy (the maximum recorded when 200 Gy was targeted) is recommended to disinfest any fruit of oriental fruit moth under ambient and hypoxic atmospheres.

Hot water immersion to ensure quarantine security for Cryptophlebia spp. (Lepidoptera: Tortricidae) in lychee and longan exported from Hawaii.

We determined whether immersion in 49 degrees C water for 20 min, a quarantine treatment developed for disinfestation of fruit flies in lychee, Litchi chinensis Sonn., and longan, Dimocarpus longan (Lourd.) Steud., exported from Hawaii, would also disinfest fruit of two species of Cryptophlebia. The pattern of tolerance to heat in Cryptophlebia illepida (Butler) was generally eggs < neonates < early instars = late instars < pupae. No C. illepida survived immersion for 16 or 20 min. Late fourth and fifth instars were determined to be the most tolerant stage that occurs in harvested fruit. Late instars of Cryptophlebia ombrodelta (Lower) were more tolerant of hot-water immersion than those of C. illepida, but no C. ombrodelta late instars survived immersion for 16 or 20 min. The hot water immersion quarantine treatment for fruit flies should effectively disinfest lychees and longans of any Cryptophlebia.

"Guidance for industry: current good manufacturing practice for blood and blood components: (1). Quarantine and disposition of units from prior collections from donors with repeatedly reactive screening test for antibody to hepatitis C virus (anti-HCV); (2). Supplemental testing, and the notification of consignees and blood recipients of donor test results for anti-HCV;" availability--FDA. Notice.

The Food and Drug Administration (FDA) is announcing the availability of a guidance document (dated September 1998) entitled "Guidance for Industry: Current Good Manufacturing Practice for Blood and Blood Components: (1) Quarantine and Disposition of Units From Prior Collections From Donors With Repeatedly Reactive Screening Test for Antibody to Hepatitis C Virus (Anti-HCV); (2) Supplemental Testing, and the Notification of Consignees and Blood Recipients of Donor Test Results for Anti-HCV." The guidance document provides recommendations for donor screening and supplemental testing for antibody to hepatitis C virus (HCV), notification of consignees and quarantine of prior collections from a donor who later tests repeatedly reactive for antibody to HCV, notification of recipients of blood and blood components at increased risk for transmitting HCV.

A time-series test of the quarantine theory of involuntary commitment.

Persons believed to be mentally ill and imminently violent are often subjected to coerced treatment. Among the justifications for this practice is the argument that removing such people from the community is analogous to quarantine. We test the quarantine theory using daily data from San Francisco. Results suggest that the incidence of coerced treatment of males is inversely related to assaults and batteries by males one day later. The finding implies that persons who are involuntarily committed are at elevated risk of violent behavior. The implications of the findings for the prevention of assaultive behavior are briefly described.

Quarantine in a multi-species epidemic model with spatial dynamics.

Motivation is provided for the development of infectious disease models that incorporate the movement of individuals over a range of spatial scales. A general model is formulated for a disease that can be transmitted between different species and multiple patches, and the behavior of the system is investigated in the case in which the spatial component consists of a ring of patches. The influence of various parameters on the spatial and temporal spread of the disease is studied numerically, with particular focus on the role of quarantine in the form of travel restriction.
