[Quadruplet pregnancies: management and obstetric and pediatric outcome].

Multi-fetal gestations are associated with increased frequency of maternal, fetal and neonatal complications. Data on the prognosis of multi-fetal pregnancies are of particular importance when the option of selective termination is considered. The present study details the obstetric management, neonatal outcome, and follow-up of seven quadruplet pregnancies in a french university center. The perinatal mortality was 250/1000. The neonatal mortality was 214/1000. The incidence of respiratory distress syndrome was 38%, bronchopulmonary dysplasia 19% and intraventricular hemorrhage 9.5%. Follow-up from one to 16 years shows that no child is handicapped. Our specific management for higher order multi-fetal pregnancies include early diagnosis, meticulous follow-up, early decrease of maternal activity, midwives at home, psychological care, delivery by cesarean section and a neonatalogist for each baby at the time of delivery.

Selective continuation in quadruplets pregnancy following treatment of GIFT plus IVF and ET.

In this report, we present the selective reduction of quadruplets to triplets by transabdominal ultrasound guided cardiac puncture and injection of 1% xylocaine and air. This treatment resulted in the successful continuation of pregnancy until premature labor developed at the 33th week. Alive births of a normal male and a normal female infant, and a dead female infant were delivered.

Selective first-trimester termination in octuplet and quadruplet pregnancies: clinical and ethical issues.

The induction of grand multiple gestations is a known complication of infertility treatments. The obstetric outcome in such cases has been very poor. We have evaluated, counseled, and performed first-trimester selective terminations for four patients, all treated with human menopausal gonadotropin, one with octuplets and three with quadruplets. In all cases, the gestations were reduced to twins. In two of the four cases, the pregnancy continued to near term without problem and resulted in the delivery of healthy twins. In the third case, pregnancy continued without problems for eight weeks, at which time renal agenesis was identified in one of the twins. In the fourth case, preterm labor five weeks later could not be stopped. Significant ethical issues must be addressed. We argue that selective termination in appropriate circumstances (eg, when the ability to carry the pregnancy to viability is very small) is ethically justified because it meets the criterion of least harm and most potential good.

Successful management of quadruplet pregnancy in a perinatal unit.

A case of quadruplet pregnancy is outlined and the management is described to demonstrate the application of monitoring and therapeutic procedures currently available for a multifetal pregnancy. Recording of the nonstressed fetal heart rate, ultrasound monitoring and glucocorticoid stimulation of pulmonary biochemical maturity were done. A successful outcome was achieved with a combined obstetric-neonatal approach and the experience of a perinatal unit.

Successful management of a quadruplet pregnancy. A case report.

A quadruplet pregnancy was diagnosed by ultrasound antenatally and managed successfully in a community teaching hospital. The patient was hospitalized at 27 weeks' gestation. She was treated with a high-protein, high-calorie diet. Sonographic follow-up and monitoring of fetal growth were done, glucocorticoids were given to attempt to stimulate fetal pulmonary maturity, and Ritodrine was given orally as a prophylactic. On three occasions intravenous Ritodrine was used to inhibit premature labor. A well-organized delivery plan with interdepartmental cooperation achieved the successful delivery of four babies at approximately 33 weeks.

Anesthetic management for birth of the "Canton" quadruplets: a multidisciplinary team approach.

This article presents a case study of the successful anesthesia management in the birth of quadruplets at Aultman Hospital in Canton, Ohio. A review of recent literature is also provided. With the increasing use of fertility drugs, multiple births will inevitably cease to be viewed as a rare phenomenon. Anesthesia practitioners will increasingly be presented with the challenge of planning for the management of these patients. The authors believe an anesthetic plan must be integrated into a multidisciplinary team approach to achieve successful management of parturient and neonates.

Transabdominal fetal reduction in quadruplet pregnancy during first trimester: a case report.

Multifetal pregnancy reduction was performed in a patient with quadruplet pregnancy at the 12th week of gestation. Reduction of the fetuses to two was carried out by injecting potassium chloride solution into the fetal chest under transabdominal ultrasound guidance. No complications were encountered and the woman was delivered of healthy twins at 37 weeks of gestation.

[Quadruplet pregnancy--the difficulties in its diagnosis and management with a case report].

The authors describe one case of quadruplet pregnancy create difficulties in diagnosis and conclude with unfavourable outcome. One of fetuses have hydrops foetus generalisatus, another foetus is malformative and the rest two fetuses are normal anatomy. Multifetal pregnancies require very early and exactly diagnosis and make up behaviour with goal is favour able outcome. In the end the authors recommended this cases must be turn to hospitals which are better possibilities for antenatal and intensive neonatal care.

Exchange of skin grafts among monozygotic quadruplets in armadillos.

Skin grafting experiments, with long-term follow-ups, on three litters of armadillo quadruplets born in our laboratory have confirmed the familiar rule that grafts exchanged between monozygotic littermates are not rejected as a consequence of alloimmune reactions. A possible explanation of the striking discrepancy between the present findings and those of a previous study is uncertainty about the relationships of the previous animals, which were obtained from a commercial dealer at 10 days of age.

Conjoined twins in a trichorionic quadruplet pregnancy after intracytoplasmic sperm injection and quarter laser-assisted zona thinning.

OBJECTIVE: To describe the occurrence and management of conjoined twins (CTs) in a trichorionic quadruplet pregnancy after the transfer of three embryos obtained by intracytoplasmic sperm injection (ICSI) that had undergone assisted zona hatching (AZH) using a noncontact laser. DESIGN: Case report. SETTING: Private and public IVF centers. PATIENT(S): A 38-year-old nulliparous woman. INTERVENTION(S): IVF, diagnosis by two-dimensional, three-dimensional, and four-dimensional ultrasound, selective termination at 12 weeks gestation. MAIN OUTCOME MEASURE(S): Clinical follow-up and delivery. RESULT(S): After diagnosis, counselling, and selective termination, the subsequent prenatal course was uneventful, and two healthy boys were delivered by elective cesarean section at 38 weeks gestation. CONCLUSION(S): Monozygotic twinning (MZT) and CTs could complicate multifetal pregnancies obtained by assisted reproduction techniques. The relationship between CTs, IVF, and AZH should be better studied to fully understand the mechanisms that lead to MZT (with the possible role of AZH) and CTs in MZT, in particular. Three-dimensional and four-dimensional ultrasound improve prenatal assessment of the condition. Selective embryo reduction is a safe procedure for the management of these pregnancies.

An expanded genetic code with a functional quadruplet codon.

With few exceptions the genetic codes of all known organisms encode the same 20 amino acids, yet all that is required to add a new building block are a unique tRNA/aminoacyl-tRNA synthetase pair, a source of the amino acid, and a unique codon that specifies the amino acid. For example, the amber nonsense codon, TAG, together with orthogonal Methanococcus jannaschii or Escherichia coli tRNA/synthetase pairs have been used to genetically encode a variety of unnatural amino acids in E. coli and yeast, respectively. However, the availability of noncoding triplet codons ultimately limits the number of amino acids encoded by any organism. Here, we report the design and generation of an orthogonal synthetase/tRNA pair derived from archaeal tRNA(Lys) sequences that efficiently and selectively incorporates an unnatural amino acid into proteins in response to the quadruplet codon, AGGA. Frameshift suppression with L-homoglutamine (hGln) does not significantly affect protein yields or cell growth rates and is mutually orthogonal with amber suppression, permitting the simultaneous incorporation of two unnatural amino acids, hGln and O-methyl-L-tyrosine, at distinct positions within myoglobin. This work suggests that neither the number of available triplet codons nor the translational machinery itself represents a significant barrier to further expansion of the genetic code.

Trizygotic quadruplet pregnancy following in-vitro fertilization: an additional factor against replacement of three embryos in young patients?

A study of a 25 year old woman with a previous history of tubal damage, ectopic pregnancy and unsuccessful tubal surgery is reported. Following in-vitro fertilization (IVF) treatment, four viable fetuses were revealed by ultrasound scan at 10 weeks gestation. Two shared the same gestational sac, thus establishing a trizygotic quadruplet pregnancy. The implications of monozygotic twinning in humans are discussed. It is concluded that embryo splitting and its consequences should be seriously considered when multiple embryo transfer in young patients is contemplated.

Electrostatic Russian quadruplet with high demagnification and minimum influence of geometric aberrations

Based on a combined analytical and numerical modelling, a high-quality probe-forming system is proposed and developed. The Matrizant method is applied to minimize the beam spot size for a fixed beam current. For quadrupoles consisting of four metallic rods, the electric field inside of the quadruplet is computed using an accurate version of the boundary element method. The influence of the rod diameter on the beam spot size is studied for different emittances. The sensitivity of the system to energy spread and mechanical defects is also investigated.

Attempts to produce monozygotic quadruplets in cattle by blastomere separation.

The blastomeres of eight-cell cow embryos were separated by micromanipulation into four pairs, inserted in foreign zonae pellucidae, embedded in agar and cultured for approximately four days in ligated sheep oviducts. Of 44 "quarter" embryos (11 monozygotic groups) transferred to sheep, 91 per cent had continued to develop at a normal rate and 77 per cent had formed small blastocysts with a single inner cell mass. Twenty-six blastocysts freed from the agar were transferred to heifers, each heifer receiving two monozygotic embryos, one to the tip of each uterine horn. Nine recipients were diagnosed pregnant by rectal palpation on day 50, six carrying twins. Thus 15 of the embryos had continued to develop including two sets of monozygotic quadruplets and one set of monozygotic triplets. Eight fetuses developed to full term, one set of monozygotic triplets, two sets of monozygotic twins (one set born dead) and one single.
