The use of human fibrin glue and of the suture in polyglactin 910 in extraintracranial by-pass.

At the Division of Neurosurgery of Fatebenefratelli-Oftalmico Hospital in Milano, 12 of the last 22 temporosylvian anastomosis have been performed employing sutures with separate points in mixed materials, partly nylon and partly Polyglactin 910 (reabsorbable) in order to avoid as much as possible the disadvantages of permanent sutures; in the other 10 consecutive cases reducing about 50% the points of suture and employing human fibrin glue as complement of the suture, in order to respect better the arterial wall and to reduce significantly the critical times of the operation (clampage). The Authors compared the clinical and instrumental results of the patients of this series with the ones of a previous series operated with traditional technique and pointed out a better success in the last operations (mixed materials and fibrin glue).

[Surgery for severe traumas of the kidney. Value of initial computed tomography, problem of surgical indication and use of a perirenal resorbable prosthesis of polyglactin 910].

We report our recent experience in the surgical treatment of severe closed renal trauma, describing the value of the initial tomography scan, the problems encountered with indications for surgery, and use of a resorbable peri-renal prosthesis made of polyglactine 910. During the last 4 years, 41 patients were hospitalized for renal trauma. Eleven of these patients were operated for severe lesions (Chatelain stage 3 and 4). Surgical treatment was as conservative as possible: total organ repair was possible in 5 cases and partial repair in 3, the kidney was lost in 3 cases.

[Radiation enteritis after pelvic irradiation of cancer of the rectum. Prevention by omentoplasty and pelvic partitioning with polyglactin 910 mesh].

To prevent radiation enteritis during post-operative irradiation of the pelvis for rectal carcinoma, the greater omentum, fed by the left gastro-epiploic pedicle, is pulled down into the pelvis which is separated from the abdomen by an absorbable polyglactin 910 mesh. The mesh, under tension, is attached superiorly to the lips of the posterior peritoneal section and anteriorly to the upper border of the pubis. This procedure has been used in seven patients, four of whom have been irradiated post-operatively without any gastrointestinal complication.

Use of polyglactin 910 mesh (Vicryl) in pelvic oncologic surgery.

Various synthetic materials have been shown to be useful surgical adjuncts in shielding the small intestine from pelvic radiation or in creating a new pelvic floor after major radical resections. Promising preliminary results with a polyglactin 910 (Vicryl) mesh in preventing radiation enteropathy prompted the authors to evaluate its clinical usefulness in reconstruction of the pelvic floor. Use of this mesh in pelvic exenterations (five total, one posterior) for advanced or recurrent gynecologic malignancies was associated with one enteroperineal fistula but no pelvic infection. In comparison with other pelvic reconstruction devices, this absorbable mesh significantly reduced intestinal morbidities in pelvic exenterations. This procedure appears to be feasible, reproducible, and safe, especially in patients who have undergone previous irradiation or those with an unsuitable omentum.

[Polyglactin 910 implants in muscle revision surgery for the reconstruction of physiologic limits in Tenon's capsule].

Iatrogenic damage to the orbital fascial system during strabismus surgery may lead to adherences with resulting problems of motility. To prevent new adherences in revisional surgery, several materials were tried, with varying degrees of success. The authors tested Polyglactin 910, popular as a suture material in strabismus surgery, in the form of a net. The material is hydrolytically soluble. Since the tissue response is finished before dissolution of the implant, it seemed that Polyglactin would be a suitable barrier to the formation of adhesions in Tenon's capsule. The implant was applied in 11 patients, some of whom had already undergone several strabismus operations and had adhesions at the surgical site. In one case the implant was used in a primary procedure. In one patient revisional surgery was needed four weeks after surgery because of overeffect, and this provided an opportunity to investigate the implanted area histologically.

Use of polyglactin 910 knitted mesh tubing to stabilize particulate hydroxyapatite in alveolar ridge augmentation. A preliminary report.

A universal problem with the use of hydroxyapatite (HA) particles has been the tendency for these particles to migrate beyond planned boundaries. This is a preliminary report on the use of polyglactin 910 knitted mesh (Vicryl) tubing containing HA particles in ridge-augmentation procedures, assessing the technique and the containment of HA particles. The technique is described and experience with six cases is presented. This study found no clinical or radiographic evidence of particle migration after normal function had been resumed. In one case the graft failed to become firmly incorporated with the bone and the whole graft migrated anteriorly on the mandible but still provided improved function for the patient. The technique facilitates the placement of HA particles and would be particularly useful if open mucosal flap techniques were being used for ridge augmentation. The use of Vicryl tubing for HA augmentation procedures prevents the migration of particles and allows an easier and more accurate placement of the grafts.

Absorbable synthetic mesh (polyglactin 910) for the formation of a pelvic "lid" after radical pelvic resection.

Isolation of the abdominal contents from the denuded, potentially infected pelvis after radical pelvic surgery has been advocated. Of the many materials evaluated, omentum is usually chosen. In six patients in whom omentum was unsuitable, we have substituted polyglactin 910 (Vicryl, Ethicon, Sommerville, New Jersey) mesh to form a pelvic "lid." In these patients, omentum was either densely adherent in the upper abdomen because of prior surgery or was removed as part of surgical treatment for recurrent cancer. Vicryl mesh formed a "lid" across the true pelvis. No gastrointestinal complications occurred in these patients. Four patients developed pelvic infections that resolved after surgical drainage and antibiotic therapy. No patients developed peritonitis above the pelvis. No chronic infections have been found in more than 12 to 48 months of follow-up. In this preliminary experience, Vicryl mesh appears to be useful for the formation of a pelvic "lid" after exenterative pelvic surgery when the omentum is unavailable.

Clinical evaluation of a bioresorbable membrane (polyglactin 910) in the treatment of Miller type II gingival recession.

Coverage of denuded roots has become one of the most challenging procedures in periodontal plastic surgery. The search for the appropriate root coverage technique has taken many different approaches. Various surgical options with predictable outcomes are available. In this clinical study, patients were treated using polyglactin 910 (Vicryl mesh) and a coronally positioned flap. Clinical parameters for 15 patients were recorded immediately prior to surgery and after a minimum of 6 months. Postoperatively, significant root coverage, reductions in probing depths, gains in clinical attachment levels, and highly significant increases in the width of keratinized gingiva were observed. The final esthetics, both color match and tissue contours, were acceptable to both the patients and the clinicians.

Evaluation of adhesion formation, mesh fixation strength, and hydroxyproline content after intraabdominal placement of polytetrafluoroethylene mesh secured using titanium spiral tacks, nitinol anchors, and polypropylene suture or polyglactin 910 suture.

BACKGROUND: The purpose of this study is to evaluate fixation methods for polytetrafluoroethylene (ePTFE) mesh with an in vivo model of laparoscopic ventral hernia repair. METHODS: In 40 New Zealand white rabbits, a 4 x 4-cm ePTFE mesh (n = 80, two per animal) was attached to an intact peritoneum with polyglactin 910 (PG 910) (n = 20) or polypropylene (PP) (n = 20) suture, titanium spiral tacks (TS) (n = 20), or nitinol anchors (NA) (n = 20). Mesh was harvested at 8 and 16 weeks for fixation strength testing, adhesion assessment, and collagen (hydroxyproline) content. Fixation strength on day 0 was determined with mesh attached to harvested abdominal wall. Statistical significance was determined as p < 0.05. RESULTS: There was no difference in fixation strength between PP (39.1 N) and PG 910 (40.0 N) sutures at time zero. At week 8, PP (25.7 N) was significantly stronger (p < 0.05) than PG 910 (11.4 N) suture, but not at week 16. The fixation strength of TS and NA (day 0, 15.4 vs 7.4 N; week 8, 17.5 vs 15.3 N; week 16, 19.1 vs 13.8 N) was not significantly different. Fixation with PP suture was significantly (p < 0.05) stronger than that with TS and NA at day 0 (39.1, 15.4, and 7.4 N, respectively) but not at weeks 8 or 16. The fixation strength of suture decreased significantly (p < 0.05) from day 0 to week 16 (PP: day 0 = 39.1 N, week 8 = 25.7 N, week 16 = 21.4 N; PG 910: day 0 = 40.0 N, week 8 = 11.4 N, week 16 = 12.8 N). The fixation strength of NA and TS did not change significantly (NA: day 0 = 7.4 N, week 8 = 15.3 N, week 16 = 13.8 N; TS: week 0 = 15.4 N, week 8 = 17.5 N, week 16 = 19.1 N). There were no differences in adhesion area based on fixation device used; however, there were more (p < 0.05) mesh samples using NA with adhesions compared to TS and adhesion tenacity was greater (p < 0.05) compared to that of TS, PP, and PG. Hydroxyproline content at weeks 8 and 16 was similar for all fixation devices. CONCLUSIONS: The initial fixation strength for nonabsorbable suture is significantly greater than that of the metallic fixation devices, but after 8 weeks there is no difference. Laparoscopic ventral hernia repair without transabdominal suture fixation may be predisposed to acute failure. The metallic devices have similar fixation strength, although the incidence of adhesions and tenacity of adhesions appear to be greater with the nitinol anchors. Since these devices have similar fixation strengths and most likely provide adequate supplementation to transabdominal sutures for mesh fixation after laparoscopic ventral hernia repair, their use should be based on other factors, such as their propensity for adhesions, ease of application, and cost.

Conservative treatment of renal allograft rupture with polyglactin 910 mesh and gelatin resorcin formaldehyde glue.

Renal-sparing treatment of spontaneous renal allograft rupture remains a surgical challenge, since profuse hemorrhage may result from these friable kidneys during surgical repair. A technique is proposed to achieve control of local bleeding with a synthetic glue (gelatin, resorcin and formaldehyde) and external compression with a polyglactin 910 absorbable mesh. We report 4 cases of spontaneous allograft rupture associated with rejection and bleeding was controlled in all 4. Three grafts were preserved with more than 1 year of followup. The other graft had to be removed for uncontrolled vascular rejection despite satisfactory control of renal fractures. Renal wrapping with external compression is proposed to improve results of conservative management of renal allograft rupture.

Vicryl (polyglactin 910) mesh as a dural substitute.

Many substances have been used as dural substitutes. Although the various materials are effective in a variety of settings, a material that is absorbed over time, elicits minimal inflammatory reaction, is relatively inexpensive, and is easily manipulated would be beneficial. An experimental project was conducted involving closure of dural defects in dogs with a Vicryl (polyglactin 910) mesh graft. Macroscopic and histological examination performed at various times after placement revealed resorption of the graft material, little cerebromembranous adhesion formation, and complete lack of injury to subjacent cortical zones. A substantial neomembrane forms over the zone of the mesh graft. The authors suggest that Vicryl mesh has significant potential as an absorbable, minimally reactive dural substitute.

Performance comparison of nylon and an absorbable suture material (Polyglactin 910) in the closure of punch biopsy sites.

The ability to close biopsy sites primarily without the need for removing sutures is obvious. However, it was unclear whether absorbable sutures could be used for this purpose. We prospectively studied 10 healthy volunteers on whom one 3 mm punch biopsy was performed on each arm. In each subject two sutures were compared for closure of the biopsy sites, polyglactin 910 and nylon. Our primary goal was to compare absorbable and nonabsorbable sutures in the closure of punch biopsy sites. Each site was closed with one simple percutaneous suture. The sites were evaluated at 2 weeks and 6 months for redness, infection, dehiscence, scar hypertrophy, and patient satisfaction. We found no statistically significant difference between the two suture materials in any of the above parameters. Therefore we conclude that absorbable sutures are a good alternative in the primary closure of skin biopsy sites.

[Regeneration of the trachea following partial or total replacement by synthetic resorbable material (polyglactin 910)].

Though longer distances of the trachea and segments of the bronchi can be resected and the blunts be joined by end-to-end anastomosis, the replacement of parts of the wall of the tracheobronchial tree may become necessary. Alloplastic not absorbable material like silicone was not successful because of its disposition to infections and development of stenosis. Absorbable suture material proved to be suitable in tracheal surgery and it was shown that texture of Polyglactin 910 (Vicryl) could even be used for replacement of the aortic wall. We used weaved tubes of 2.5 mm diameter as prostheses of this material. We partly resected the trachea in the throat of 30 rats and in a microsurgical procedure we replaced in 17 animals the whole circumference by the tube shaped prosthesis and in 13 animals a window in the trachea by a patch, cut from such a prosthesis. By pneumonia or granulation in the anastomosis region 11 rats died or had to be sacrificed untimely. The others survived in several periods up to 10 weeks. At the time of autopsy starting after 3 weeks, a newly formed trachea could be demonstrated, which is covered more and more with respiratory epithelium. By light microscopic and scanning microscopic examinations, the development of a new wall, similar to the normal one, was seen. This wall lacks only mucus glands, but contains structures similar to hyaline cartilage. Our absorbable prosthesis cannot prevent the development of stenosis by overshooting granulation in the early postoperative period, but very soon the material is absorbed and replaced by a structure similar to the normal wall. After biodegradation, problems appear by instability of the new tissue, because of collapsing during inspiration. This problem could be solved by covering the prosthesis by an additional supporting scaffold.

[Prevention of radiation injuries of the small intestine with pelvic exclusion by polyglactin 910 mesh. Contribution to gynecologic and digestive cancer surgery].

Used in 13 cases of advanced gynecological and gastrointestinal carcinoma, pelvic exclusion using a suspension of the small intestine above the pelvis using absorbable material. The low-level of morbidity, value in dosimetric estimation, perfect feasibility and low cost of this technique should lead to its more widespread use whilst strictly respecting its indications. Nuclear magnetic resonance has been found to be a particularity novel and valuable method of post-operative investigation in visualisation and assessment of resorption of the polyglactine 910 trellis. Results in this series are analyzed at the end of a 5 to 14 month follow-up period and compared with data from experimental and clinical studies in the recent world literature.
