[Acute upper genital tract infections. II. A. bacteriological study and the therapeutic consequences].

The authors have studied a prospective series of 62 cases of acute upper genital tract infections of different severities. Aerobic and anaerobic bacteriological examination were carried out from different sites: urine, blood, vagina, endocervix, intra-uterine devices and the pouch of Douglas. It is absolutely necessary to follow a strict technique according to the clinical examination in order to interpret the results according to a methodological plan. The results show that the gonococcus was found in 9 out of the 62 cases, which had a very varied flora. The value of the different smears and swabs is then discussed. In this way it is shown that specimens taken from the endocervix are definitely of great value although the ideal is to culture directly from intra-uterine devices or from specimens taken laparoscopically from the tubes. It is possible to give different courses of antibiotics: these should include however active substances against the group of anaerobes, which justifies adding metronidazole routinely. The use of corticoids which are often prescribed to limit the sequelae has not been proven to be of value.

[Clinical and etiologic study of 90 cases of chronic diarrhea].

90 patients with chronic diarrhoea underwent this prospective study. They were seen in a private hospital of Lima during 1990 and 1991. According to a methodologic plan for determining sources and the diseases that originate chronic diarrhoea. In all patients hematologic, bioquimic, coprocultures, coproparasitologic exams were done, chest and intestinal transit X-rays. All underwent duodenal content culture. Colon X-ray in 25 cases; proctosigmoidoscopy in 14 and upper digestive endoscopy in 19 patients. Abdominal echography in 12 and TAC in 2 cases. The final results showed as determinant diseases for chronic diarrhoea, according to their frequency: enteroparasitosis (23.3%), functional digestive disorders (20.0%), intestinal bacterial overpopulation (15.5%) of unknown origin (8.8%), colon diverticulus (7.7%) proven and probably (5.5%), lactose intolerance (3.3%), diabetes mellitus (2.2%), and in one case (1.1%) the following: intestinal linfoma, pancreas malignancy, AIDS, colonic and deformation and megaloblastic anemia. The causes of chronic diarrhoea are several and multifactorals and in this study we prove the preeminence of the intestinal parasitosis, functional disorders and intestinal bacterial overpopulation and with less frequency other pathologies.

Inspection of clinical investigations by the German health authorities.

Based on the regulations of the German Drug Law of 1976 (Arzneimittelgesetz, AMG) the inspections of clinical investigations by the competent health authorities focus on the protection of the human being taking part in the trial. Standards for the planning, methodology, conduct, report and documentation of clinical trials are laid down in a guideline (Grundsätze für die ordnungsgemässe Durchführung der klinischen Prüfung, Dec. 1987), a kind of national GCP standard which has to be respected by sponsors, physicians and authorities, not only for inspection but also for drug registration.

Methodological advances and plans for improving regulatory success for confirmatory studies.

Regulatory success for new medical products requires convincing statistical results from planned analyses for relevant and reliable data from well-designed clinical studies. Statistical methodology has a critical role for such success through strengthening the robustness of study findings to sources of bias, variability and spurious events. Areas of particular importance for statistical methodology include the structure for study design, the schedule and procedures for data collection, and plans for primary data analyses. This paper discusses some challenging statistical issues for these areas and suggests potential strategies for addressing them. The objective of these strategies as well as other methodological advances is the minimization of ambiguity in the findings of confirmatory studies. In this way, their effective use can enhance regulatory success.

Development of methodologic tools for planning and managing library services. I. Project goals and approach.

In July 1966, the Institute for Advancement of Medical Communication began work on a project aimed at developing methods for collecting objective data suitable for planning and guiding local, regional, and national programs to improve biomedical libraries and the biomedical information complex. This article constitutes an introduction to a series of reports on the methodologic tools that have been developed. It describes the overall purpose and initial goals of the project, gives the general plan of the work, and presents five basic concepts that underlie the project's approach and structure the entire effort.

Development of a planning and evaluation methodology for assessing the contribution of theory to a diabetes prevention lifestyle intervention.

The benefits of utilizing and measuring application of theory for behavior change programs are numerous, including the emergence of new theories and theoretically bound strategies. Despite recent attempts to make theory use more salient, there remains a dearth of practical frameworks for the development and evaluation of theory-based programs. Without literature documenting how theories have been specifically applied to interventions and their evaluation, health educators may not be well prepared to utilize theory for the design, implementation, or as a focus of the evaluation. Using a case study example of a diabetes prevention program, this article describes how theory was used for the program design and the evaluation and provides a framework for using theory in other programs. Issues discussed include: the challenges in successfully utilizing theory for intervention development and the processes of developing theoretically based instruments.

A new methodology for the planning of single-cut corrective osteotomies of mal-aligned long bones.

BACKGROUND: Corrections of combined torsional and angular deformities of long bones may be performed creating a single osteotomy which is oriented so that rotating the two fragments on the created osteotomy plane allows to correct all deformities in one step. A practical geometrical tool is presented to facilitate the difficult preoperative planning of such osteotomies. METHODS: The geometrical tool consists of two limbs connected by a mobile disk representing the osteotomy plane. This allows the two limbs to be deliberately bent and rotated against each other. Thereby, the mobile disk will change orientation in such a way that it will indicate the osteotomy plane needed in order to anatomically align the two limbs. The geometrical principle of the tool has been confirmed mathematically and compared with data from the literature. Five deformed test bones have been used to test the effectiveness of the tool. FINDING: . The geometrical principle of the tool is equivalent with the mathematical data from the literature. The maximal osteotomy angle which can be indicated by the tool is 65 degrees , with an error of +/-3 degrees compared to mathematically calculated values. The five test bones were all aligned anatomically with appropriate accuracy. INTERPRETATION: The presented tool can be easily used and facilitates largely the preoperative planning of a single cut osteotomy for complex deformities of long bones. It allows to avoid sophisticated mathematical calculations and helps to avoid the risk for errors in planning and performing correctional osteotomies.

[Methodology of a planned clinical dietetic experiment with a program for processing the results on the Elektronika BZ-34 microcomputer].

Methods of planned clinical experiments on dietotherapy, adapted for the study of food additives in the absence of an adequate placebo, were improved due to the modification of the mathematical analysis. The schedule of the experiment involves exclusion of nonuniformity, reduction of the number of necessary studies and high accuracy of the final results. For the statistical analysis of the data obtained in the experiment a program has been proposed for the microcomputer.

Development of methodologic tools for planning and managing library services. IV. Bibliography of studies selected for methods and data useful to biomedical libraries.

This selective bibliography is intended to serve as a guide to empirical studies reporting data and methods that can be used by medical librarians to assess their own efforts objectively and to improve the effectiveness and efficiency of services they offer. The decision rules that governed selection of items for the bibliography are specified in detail. A total of 178 items published between 1915 and mid-1968 met the selection criteria. The list of items is supplemented by a keyword index derived from titles. Half the items are journal articles; a third of these articles appeared in the Bulletin, and most of the remainder in thirteen other library and information science journals. Most of the non-journal items are technical reports issued by the organization that conducted or sponsored the work. The characteristics of this literature suggest that few medical libraries, unless they are part of a university system that includes the collection of a library or information science school, are likely to have quick access to the literature base needed to support a comprehensive program of self-evaluation studies and the continuing education of their own staff. Regional medical libraries might well undertake to ensure both ready access to, and awareness of, literature on the scientific aspects of librarianship.
