An experimental evaluation of antiseptic wound irrigation.

An experimental wound infection model was used to assess the value of four proprietary antiseptics applied topically in preventing the development of wound sepsis. Irrigation of wounds with either saline or noxytiolin 15 min after contamination with Staphylococcus aureus did not reduce either the incidence or degree of infection. Benzalkonium chloride and, to a lesser degree, povidone-iodine significantly reduced the infection rate, but were inferior to chlorhexidine gluconate which eliminated all overt signs of infection. The rate of healing of the chlorhexidine-treated, contaminated wounds was found to be no different from control non-infected wounds. When irrigation was carried out 45 min before wounds were contaminated, chlorhexidine was the only treatment which reduced the rate of infection (P less than 0.001). It is concluded that the superior activity of chlorhexidine in this model is a good indication that it should be a highly effective agent in the prevention of staphylococcal wound infection, and that this is probably due to a combination of bactericidal and persistent action together with low toxicity.

A study of the efficacy of noxytiolin instillation as adjunctive to radiotherapy in carcinoma of the bladder: a clinical trial.

A prospective study was conducted to establish efficacy or otherwise of Noxytiolin (N-hydroxymethyl-N-methylthiorea) instilled concurrently with radical radiotherapy in bladder carcinoma, a previous pilot study having indicated an improved survival rate. A total of 242 patients participated in the trial. The four-year survival figures were compared and it is concluded that there was no significant difference between the controls and those patients treated with Noxytiolin.

Activity of disinfectants against Gram-negative bacilli isolated from patients undergoing lung transplantation for cystic fibrosis.

Lung transplant recipients with cystic fibrosis are frequently colonized with antibiotic-resistant bacteria. We evaluated the in vitro activity of 5 disinfectants frequently used in cardiac surgery against strains of Burkholderia cepacia and Pseudomonas aeruginosa isolated from patients undergoing sequential single lung transplantation. Our results suggest that the activity of Taurolin and Noxyflex is superior to conventional disinfectants.

Bladder irrigation or irritation?

Exfoliation rates of urothelial cells following bladder irrigation were studied in patients with long-term indwelling catheters and chronic urinary tract infections (UTI). The irrigations were associated with an increased shedding of urothelial cells. Ultrastructural studies of these cells demonstrated increased disruption when compared with those obtained from normal subjects without catheters or chronic infection. The findings suggest that bladder irrigation further damages the already disrupted urothelium, which may in turn increase the predisposition of the bladder to the recurrent infections, commonly associated with patients who have indwelling urinary catheters. Bladder irrigation methods and the indications for their use require reassessment.

Comparison between irrigation and conventional treatment for empyema and pneumonectomy space infection.

A new method of treating pleural empyema or pneumonectomy space infection by irrigation was evaluated in 11 patients. The infected cavities were filled with an antibiotic or antiseptic solution for three hours, and allowed to drain for one hour. This cycle was repeated every four hours for seven to 10 days. When cultures of the infected cavity became sterile the irrigation tube was removed and the wound sealed. Using this method, infection was eradicated after an average of 11 days in five of six patients with pleural empyema and in all five patients treated for an infected pneumonectomy space, including one with a bronchopleural fistula. The results of treating 58 similar cases of intrapleural sepsis over a 10-year period by the standard methods of aspiration, open drainage, decortication, or thoracoplasty were compared with the results of irrigation. In general, cyclical irrigation resulted in a shorter hospital stay and a shorter period of wound drainage than other methods.

The fate of N-methyl-N'-(hydroxymethyl)thiourea in the rat.

1. The fate of N-methyl-N'-(hydroxy[14C]methyl)thiourea (MHT) has been studied in the male Sprague-Dawley rat. The compound is degraded to N-methylthiourea and formaldehyde. 2. N-Methylthiourea is excreted as a urinary metabolite whereas the formaldehyde is not excreted, either in the urine or the expired air, but is metabolized via formate to CO2. 3. At least 50% of an i.p. dose (100 mg/kg) of MHT is excreted unchanged and some of this undergoes hydrolysis within the urine to N-methylthiourea and formaldehyde. Production of formaldehyde leads to the formation of the urinary artefact N-(hydroxymethyl)urea. 4. Tissue-distribution studies with 14C-MHT have shown that radioactivity is selectively associated with the thyroid gland. A preliminary investigation has indicated that MHT has anti-thyroid hormone activity as it lowers the thyroxine in rat serum.

The management of empyema thoracis by thoracoscopy and irrigation.

A technique of irrigation for the management of empyema thoracis is described. Initial thoracoscopy using a laparoscope under general anaesthesia enabled adequate debridement and breakdown of loculi within the empyema cavity under direct vision. Following this procedure irrigation of the cavity with two tubes was started and continued until three consecutive cultures of drained irrigation fluid became sterile. The procedure was then discontinued. The results in 14 patients are presented using this method, irrigation was required for an average of 14 days. Patients remained in hospital for an average of 4.8 weeks. Tuberculous empyema was not found to be a contra-indication to the irrigation technique.

[Noxytioline in the local treatment of empyema (author's transl)].

The importance assumed by local treatment in non-tuberculous purulent pleurisy has suggested that intrapleural applications of a broad spectrum antiseptic could be as useful as in peritonitis. Fifteen adult patients with empyema secondary, in most cases, to local, usually Gram-negative, infections were treated with combined antibiotic therapy and locally administered noxytioline. The patients underwent 3 to 30 intrapleural lavages (average 9) via a pleurotomy drainage tube (11 cases) or after pleural puncture (4 cases). Results were very satisfactory in 12 out of 15 cases, with mild pleural sequelae in 8 cases. No local or systemic reactions were observed. This treatment proved particularly effective in 5 difficult cases.

[Prevention of peritoneal adhesions. An experimental study using noxytiolin (author's transl)].

Numerous procedures have been used to try to prevent peritoneal adhesions following operations. Noxytiolin has been used experimentally in the rat in a double blind trial on 3 series of 30 rats. A reliable procedure for making adhesions is said to be always reproducible. Comparing two control groups this study shows that statistically significantly this product lessens the number and seriousness of adhesions that can form, although it does not totally stop them developing.
