Incidence of catheter-associated urinary tract infection among dogs in a small animal intensive care unit.

OBJECTIVE: To determine incidence of and possible risk factors for catheter-associated urinary tract infection (UTI) among dogs hospitalized in an intensive care unit and compare results of bacterial culture of urine samples with results of bacterial culture of catheter tips. DESIGN: Prospective study. ANIMALS: 39 dogs. PROCEDURE: A standard protocol for aseptic catheter placement and maintenance was used. Urine samples were obtained daily and submitted for bacterial culture. When possible, the urinary catheter tip was collected aseptically at the time of catheter removal and submitted for bacterial culture. Bacteria that were obtained were identified and tested for antimicrobial susceptibility. RESULTS: 4 of the 39 (10.3%) dogs developed a UTI. The probability of remaining free from UTI after 1 day in the intensive care unit was 94.9%, and the probability of remaining free from UTI after 4 days was 63.3%. Bacteria isolates were generally common urinary tract pathogens and were susceptible to most antimicrobials. Specific risk factors for catheter-associated UTI, beyond a lack of antimicrobial administration, were not identified. Positive predictive value of bacterial culture of urinary catheter tips was only 25%. CONCLUSIONS AND CLINICAL RELEVANCE: Results suggest that placement of an indwelling urinary catheter in dogs is associated with a low risk of catheter-associated UTI during the first 3 days after catheter placement, provided that adequate precautions are taken for aseptic catheter placement and maintenance. Results of bacterial culture of urinary catheter tips should not be used to predict whether dogs developed catheter-associated UTI.

Silence of chromosomal amplifications in colon cancer.

Oncogene activation by gene amplification is a major pathogenetic mechanism in human cancer. Using comparative genomic hybridization, we determined that metastatic human colon cancers commonly acquire numerous extra copies of chromosome arms 7p, 8q, 13q, and 20q. We then examined the consequence of these amplifications on gene expression using DNA microarrays. Of 55,000 transcripts profiled, 2,146 were determined to map to one of the four common colon cancer amplicons and to also be expressed in normal or malignant colon tissues. Of these, only 81 transcripts (3.8%) demonstrated a 2-fold increase over normal expression among cancers bearing the corresponding chromosomal amplification. Chromosomal amplifications are common in colon cancer metastasis, but increased expression of genes within these amplicons is rare.

[Influence of the GABAergic system on eating behavior induced by the tail pinching technic in rats].

The present experiment was undertaken to investigate the possible involvement of GABAergic neurons in the regulation of the feeding behavior induced by tail pinching. Injections before the tail pinching of gamma-aminobutyric acid (GABA) agonist (Muscimol: 570 micrograms.kg-1 i.p. 20 min before testing), GABA antagonist (Picrotoxine: 1,5 mg.kg-1 i.p. 30 min before testing) and a GABA transaminase inhibitor (Acetate dipropyle: 200 mg.kg-1 i.p. 40 min before testing) have no effects on the ingestion of sweet milk. These preliminary results provide support for the hypothesis that the feeding behavior induced by TP is not mediated through GABAergic system.

Genomic analyses implicate hormonal and metabolic dysregulation in polycystic ovary syndrome.

Polycystic ovary syndrome (PCOS) and its underlying features remain poorly understood. In this genetic study (n = 544,513), we expand the number of genetic loci from 16 to 29, and additionally identify 31 associated plasma proteins. Many risk-increasing loci were associated with later age at menopause, underscoring the reproductive longevity related to an increased oocyte number and/or availability across the lifespan. Hormonal regulation in the etiology of this condition, through metabolic and reproductive features, was emphasized. The proteomic analysis highlighted metabolic biology known to be related to PCOS. A polygenic risk score (PRS) was associated with adverse cardiometabolic outcomes, with differing relevance of testosterone and body mass index in women and men. Finally, while oligo-anovulation and anovulatory infertility are features of PCOS, we observed no impact of PCOS susceptibility on childlessness. We suggest that PCOS susceptibility confers balanced pleiotropic influences on fertility in women, and life-long adverse metabolic consequences in both sexes.

Spatial disorientation in Alzheimer's disease.

Spatial disorientation was investigated in 28 ambulatory patients meeting the National Institute of Neurological Disorders and Stroke-Alzheimer's Disease and Related Disorders Association Work Group criteria for "probable" Alzheimer's disease. Based on caregivers' reports, 39% of subjects engaged in at least three of four behavioral measures of spatial disorientation three or more times a week; these patients did not significantly differ from other Alzheimer's disease subjects with regard to age, sex, education, or symptom duration. Using stepwise regression analysis, we found that neuropsychologic measures of memory and visuoconstructive functions, but not disease severity, attention, or language impairment, emerged as significant predictors of spatial disorientation. In the setting of impaired memory, the tendency of some patients with Alzheimer's disease to wander or to get lost may implicate particularly severe dysfunction of right hemisphere neocortical areas concerned with visuospatial processes.

Proliferation and cornification during development of the mammalian epidermis.

The skin is the largest organ of the body and consists of the underlying dermis and outer epidermis. Proper embryonic development and continual renewal of the adult epidermis are essential to provide an impenetrable barrier against fluid loss and serve as our most important defense against insults from the external environment. During mammalian embryogenesis the epidermis develops from the surface ectoderm, which initially consists of a multipotent single-layer epithelium. Once these epithelial cells receive the appropriate developmental cues, they become committed to stratify, initiate a massive expansion program, and finally embark on a journey of terminal differentiation to produce the morphologically distinct layers of the epidermis. The culmination of this journey is the formation of an impervious cornified envelope via a highly specialized form of programmed cell death, termed "cornification" (reviewed in Candi et al.), which is distinct in many ways from the classic apoptotic pathways. The epidermal developmental program that is first seen in the fetus is recapitulated for the entire life of the organism. The basal layer of adult skin harbors stem cells, which can divide to produce daughter stem cells and transit amplifying (TA) cells that go on to differentiate and cornify (reviewed in Fuchs and Raghavan). In this review we summarize current knowledge about the molecular regulation of proliferation and cornification in the developing mammalian epidermis. We focus on events in the interfollicular epidermis, with special emphasis on transcriptional regulation by p63, Notch, NF-kappaB/IKK, Hox, AP-1, AP-2, and C/EBP factors. We end with a discussion about perturbations in epidermal proliferation and cornification as they pertain to human skin pathologies.

An analysis of interleukin-8, interleukin-6 and C-reactive protein serum concentrations to predict fever, gram-negative bacteremia and complicated infection in neutropenic cancer patients.

A prospective study was performed to assess the potential value of interleukin (IL)-8, IL-6, and C-reactive protein (CRP) serum levels to predict fever, gram-negative bacteremia and complicated infection in neutropenic patients with cancer. Serum samples were obtained three times a week during 208 neutropenic episodes following cytotoxic chemotherapy. Fever of any cause developed during 104 out of 191 evaluable episodes. Serum levels of neither cytokine nor CRP were predictive of fever within more than 24 h before its onset. Unlike CRP, both IL-6 and IL-8 serum levels were significantly different between microbiologically documented infections and unexplained fevers. The highest values of IL-6 and IL-8 were observed in episodes of gram-negative bacteremia. Using receiver-operating-characteristic curves, the analysis of cytokine levels measured around the onset of fever indicated that IL-8 is potentially useful for predicting gram-negative bacteremia, with a high negative predictive value of > 90% and a moderate positive predictive value of 50% (sensitivity, 70%; specificity, 91%). In patients with persistent fever, predictions of further clinical complications, defined as prolonged fever of more than 7 days' duration, pneumonia, shock and/or death due to infection, were best predicted by IL-6. With an IL-6 cutoff value of 250 pg/ml in samples obtained 8 to 32 h after onset of fever, the positive predictive value was 92%, the negative predictive value 91% (sensitivity, 85%; specificity, 95%). The positive predictive value of IL-6 in samples obtained another 24 h later from patients still febrile remained > 90%, but the negative predictive value dropped to 47%. In any of the analyses, the predictive values of CRP levels were poor and inferior to either cytokine. These findings may have clinical value in identifying subgroups of patients requiring different therapeutic approaches.

The impact of a self-administered behavioral intervention program on pediatric asthma.

This study evaluated the benefits produced by "Superstuff", a self-help program for asthmatic children aged 7-12. Forty-three children with a confirmed diagnosis of moderate to severe asthma were randomly assigned to either the totally self-administered Superstuff condition or to a nocontact Control condition. Self-report, parental, physician, and school data were collected at pre-intervention, and two, six, and twelve months post-intervention. Children receiving Superstuff reported increased asthma self-control skills, but no gains in general self-control abilities or self-esteem. Superstuff subjects also evidenced fewer interruptions of parents, greater improvement in the progression of asthma as reported by physicians (but not in the severity of the disease or intensity of average attack), and tended toward decreased school absenteeism. Superstuff did not reduce scheduled or emergency medical contacts. The demonstration of important, but modest, benefits from a low-cost, easily disseminated, self-administered intervention is discussed in the context of self-help treatment in general.

Naming consistency in Alzheimer's disease.

Although lexical semantic deficits are postulated to play a prominent role in the anomia of Alzheimer's disease, it is unclear whether the primary disturbance is one of lexical access or one of lexical semantic loss. Response consistency on a naming task is one means of evaluating the underlying source of naming impairment. Access dysfunction usually implies variable word-finding difficulty, while a theory of lexical loss predicts that many word names would be consistently unavailable. Nineteen Alzheimer's disease patients were administered a visual confrontation naming task (the Boston Naming Test) on two occasions 6 months apart. Eighty percent of errors occurred consistently at both times; only 20% of errors occurred on only one occasion. Response consistency occurred significantly more often than expected under the assumption of no response consistency. Findings support the hypothesis that anomia in Alzheimer's disease is in part due to a loss of lexical semantic information.

Effects of 2,4-dinitrophenol on trichomonads and Entamoeba invadens.

1. 2,4-Dinitrophenol (2,4-DNP) in substrate level concentrations (200 microM-1 mM) temporarily inhibits H2 production by Tritrichomonas foetus and Trichomonas vaginalis as well as the accumulation of metronidazole, dependent on its reduction by the two trichomonad species and by Entamoeba invadens. 2. 2,4-DNP competes for the reducing equivalents which are necessary for H2 production or for the reduction of metronidazole, thereby inhibiting these processes. 2,4-DNP is reduced to 2-amino, 4-nitrophenol. 3. 2,4-DNP in concentrations up to 800 microM has no effect on the uptake of O2 by these organisms. 4. 2,4-DNP has some toxicity for T. foetus.

Ovarian hormones can organize the rat corpus callosum in adulthood.

The rat corpus callosum (CC) is larger in males than females, and is responsive to hormone manipulations during development. Previous data suggest that CC sensitivity to testosterone ends by postnatal day 8 (P8). In contrast, responsivity to ovarian hormones extends as late as P25. The current series of experiments investigates whether ovarian hormone effects on the callosum are permanent and whether CC sensitivity to ovarian hormones extends beyond P25. We found that P70 ovariectomy (Ovx) did not affect callosal size, suggesting that ovarian hormone exposure sometime prior to P70 is sufficient to feminize the CC, and that once the callosum is feminized, the effects can not be reversed. We also found that P25 ovariectomy enlarged, or defeminized, adult female CC, whereas ovary transfer starting on P55 or P70 counteracted this enlarging effect, resulting in feminized adult CC. Thus, although a previously feminized callosum is not affected by P70 ovarian hormone removal, a not-yet feminized callosum can still be feminized after P70. These findings indicate that there is flexibility in the developmental window within which the female brain is responsive to the active feminization process initiated by ovarian hormones.

Effect of a short course of neoadjuvant hormonal therapy on the response to subsequent androgen suppression in prostate cancer patients with relapse after radiotherapy: a secondary analysis of the randomized protocol RTOG 86-10.

PURPOSE: To compare, by a secondary analysis, the therapeutic benefits of androgen suppression in protocol prostate cancer patients with relapse after radiotherapy (RT) for locally advanced disease who, in the Phase III trial beginning in 1987, were assigned to receive or not receive a short course of neoadjuvant maximal androgen suppression before definitive RT. METHODS AND MATERIALS: Between 1987 and 1991, 456 patients were entered in the Radiation Therapy Oncology Group trail 86-10 and randomized to receive (Arm I) or not to receive (Arm II) neoadjuvant hormonal therapy (HT), which was 4 months of goserelin (3.6 mg every 4 weeks) and flutamide (250 mg t.i.d.) before and during RT for bulky T2-T4 tumors. The overall and disease-specific survival after both randomization and salvage HT for patients with relapse was evaluated, as well as the duration of response in those patients undergoing salvage HT. The outcomes in patients who had received neoadjuvant HT vs. those who had not were compared. The median follow-up after randomization for all alive patients was 9.0 years and was 5.5 years for alive patients after beginning salvage HT. RESULTS: Fewer patients received salvage HT on Arm I than on Arm II (45% vs. 63%, p <0.001). The outcomes by randomized treatment arm (I vs. II) from the time of beginning salvage HT were similar. At 5 years after salvage HT, the overall survival rates were 41% and 41% and the disease-specific survival rates were 50% and 50%. At 8 years after randomization, the overall survival rates were 47% and 44% and the disease-specific survival rates were 55% and 56%. CONCLUSION: Although a 4-month course of neoadjuvant and concurrent maximum androgen suppression and RT (compared with RT alone) significantly increases the freedom from relapse rate and freedom from receiving salvage HT, it does not compromise the long-term beneficial effect of subsequent salvage HT, if needed for relapse. These findings with long follow-up in patients treated for locally advanced disease diagnosed 9-14 years previously should help allay concerns of the possible development of "resistance" to androgen suppression when 4-month courses of neoadjuvant HT are used before primary treatment.

[Uterine adenomyosis in infertile patients: MR imaging findings and clinical conclusions].

PURPOSE: To evaluate the clinical value of magnetic resonance imaging (MRI) of the uterus and the pelvis in infertile female with symptomatic endometriosis for the assessment of uterine morphology and function. MATERIALS AND METHODS: Forty-one infertile women (mean age: 33.2 years) with symptomatic endometriosis documented by laparoscopy were evaluated with high-field MRI (Sonata, Siemens) using T2- and T1-weighted sequences. In addition, the patients underwent hysterosalpingoscintigraphy (HSSG) for evaluation of the uterotubal transport capacity. Pathologic findings of the uterine junction zone were correlated with the laparoscopic results, clinical symptoms and uterotubal transport capacity. RESULTS: Adenomyosis was diagnosed on T2-weigthed MR-images in 35 (85.4 %) patients (focal adenomatosis in 26 patients and diffuse adenomatosis in 9). Patients showing signs of adenomyosis tended to be older than patients without adenomyosis (mean age 34.1 years vs. 30.1 years) and showed a longer history of symptomatic endometriosis. A positive transport capacity in HSSG was observed in 73.1 % (19/26) of the patients with focal adenomyosis and in only 22.2 % (2/9) of patients with diffuse adenomyosis. In 83.3 % (5/6) of the patients without signs of adenomyosis, a positive uterotubal transport was documented. CONCLUSION: MRI of the uterus is a helpful diagnostic tool for the diagnosis of adenomyosis and for planning further therapies. Patients with the diagnosis of adenomyosis showed a reduced uterotubal transport capacity that depended on the severity of adenomyotic changes and might be a possible cause of infertility.

[Variation in X-ray dose quantity using an amorphous selenium based flat-panel detector -- a study on the dose reduction rate up to the limit of diagnostical utilization].

PURPOSE: To evaluate the diagnostic quality and minimum required dose to obtain acceptable images for diagnostic purposes in the field of musculoskeletal radiology. MATERIALS AND METHODS: A critical comparison of the image quality produced by a novel flat panel detector and the conventional screen/film system using a contrast-detail phantom was performed in phase I. Images from both systems were obtained with the same dose and displayed with similar contrast and density. In phase II images of significant anatomical structures in cadaver extremities obtained using the digital detector system and the standard film/screen system were critically evaluated. After a successive reduction in the X-ray dose for 84 patients in phase III, eight independent radiologists compared the image quality of the screen/film system to that of the novel flat panel detector. RESULTS: Phases I and II revealed a difference in the image quality achieved by the standard screen/film system and the digital detector system to the advantage of the digital detector system. In 77 of 84 patients (91.7 %), phase III showed equal image quality after a 50 % reduction in the X-ray dose. In 3 cases (3.6 %) the image quality and the level of contrast were better. No unified statement could be made for 4 patients (4.7 %). CONCLUSION: Digital imaging of skeletal disorders using the novel flat panel detector makes it possible to reduce the X-ray dose by 50 % with equal or even better image quality.

Reserpine and breast cancer in a retirement community.

In an attempt to confirm three reports suggesting a causal link between rauwolfia antihypertensive drugs and female breast carcinoma, all cases of such cancer appearing among the residents of an affluent retirement community were compared with controls chosen from a roster of all women in the same community. Most residents use a single care facility, and patterns of hypertension diagnosis, rauwolfia and other drug use, and medical-care-system patronage were abstracted from medical records. The prevalence of recorded rauwolfia use among the controls were 20 per cent, and that of other drug use was correspondingly high. The risk ratio for rauwolfia use was estimated to be 1.2 (95 per cent confidence interval, 0.7 to 2.2). Risk ratios between 1.0 and 2.0 were also found for other drug use and for measures of care-system patronage. These results do not support the hypothesis that reserpine causes breast cancer.

High resolution sonography of the menisci of the knee.

High-resolution real-time ultrasonography (5, 7.5, and 10 MHz) was used to examine the menisci of the knee in ten normal volunteers and in ten patients with knee injuries. The posterior horns of the medial and lateral menisci were easily demonstrated. Normal anatomy and pathologic changes could be imaged in the menisci, capsular ligaments, and articular cartilage. While the menisci were seen in all subjects, the weight-bearing portion of the femoral articular cartilage could not be seen in several of the symptomatic patients who could not flex their knees. Ultrasound promises to be a useful, noninvasive adjunct to conventional techniques in evaluating the injured knee--especially in assessing the posterior horns of the menisci, an area difficult to assess with arthroscopy.
