Antigen challenge inhibits thymic emigration.

T cell development in the thymus involves a series of TCR-mediated control points including TCR-beta selection and positive and negative selection. Approximately half of the thymic sojourn is spent in the medulla, where thymocytes undergo final maturation before emigrating to the periphery. Although it is acknowledged that thymic emigration is an active process, relatively little is known about how this is regulated, why it takes so long, and whether TCR-mediated signaling can influence this step. Using wild-type and TCR transgenic mice, we found that Ag injected i.v. or intrathymically led to a striking reduction in the number of recent thymic emigrants (RTE) in the periphery. This was caused by inhibition of T cell export rather than peripheral deletion, because a cohort of RTE that was already released before in vivo Ag challenge was not depleted, and similar results were observed in Bim-deficient mice, which have impaired T cell deletion. Within the thymus, the loss of RTE was associated with retention of medullary thymocytes rather than increased negative selection. In addition to Ag-specific inhibition of export, some TCR-independent suppression of emigration was also observed that appeared to be partly the result of the inflammatory cytokine TNF. Thus, in addition to its accepted role in intrathymic selection events, TCR signaling can also play an important role in the regulation of thymic emigration.

[Lymphotropic therapy with antitubercular agents in the treatment of tuberculous epididymitis].

Whether endolymphatic intranodular injection (ELII) of isoniazid can be used in the combined treatment of male genital tuberculosis is considered. Clinical trials were performed and this method used in the combined treatment of tuberculous epididymitis was assessed. The bacteriostatic activity and blood and tissue concentration of the agent given as ELII were studied and compared with its intramuscular route of administration. The authors show it possible to employ scrotum ultrasound studies to evaluate the efficiency of treatment and estimate the scope of a surgical intervention. Based on the data of morphological study of postoperative specimens, the authors defined the time and worked out criteria for epididymectomy in tuberculous epididymitis.

Cytokine deviation induced by intrathymic injection of staphylococcal enterotoxin B (SEB).

We have previously shown that intrathymic (i.t.) injection of staphylococcal enterotoxin B (SEB) to mice induces both T cell clonal deletion and IL-2-dependent anergy. In the present study, we have used a quantitative RT-PCR to demonstrate that i.t. administration of SEB induced a significant decrease in the levels of the IL-2 and IFN-gamma mRNAs in total splenocytes, from day 7 to day 28 post-injection. I.t. SEB injection also induced a significant increase in the levels both of IL-10 and TGF-beta mRNAs on day 7, leading to a significant enhance in the IL-10 + TGF-beta/IL-2 + IFN-gamma mRNA ratio on days 7 and 28. By contrast, IL-10 and TGF-beta mRNAs were unchanged after intraperitoneal (i.p.) or subcutaneous (s.c.) SEB injections, although both IL-2 and IFN-gamma mRNA levels were decreased. The cytokine mRNA ratio was enhanced on days 7 and 28 after i.p. injection. Interestingly, a cytokine mRNA ratio of a least 10 in favour of IL-10 plus TGF-beta mRNAs was correlated with the hyporesponsive state observed in vitro after i.t. and i.p. injections. Our results clearly demonstrate that i.t. SEB administration induces a switch from Th1-type to Th2-type cytokine expression in the spleen. The deviation from IL-2 plus IFN-gamma towards IL-10 plus TGF-beta expression could be responsible for the immunoregulatory effect exerted upon SEB-reactive T cells, which is characterized by an IL-2-dependent, specific anergy in vitro. Moreover, it highlights the crucial role of the route of SEB injection in the pattern of cytokine expression.

[Clinical pharmacokinetics of kanamycin in endolymphatic therapy of peritonitis].

The pharmacokinetics of kanamycin in patients with peritonitis was studied after its intramuscular, endolymphatic and lymphotropic administration. Endolymphatic administration of kanamycin provided an increase in its activity in the inflamed tissues of the peritoneum and omentum and markedly prolonged its halflife as compared to those after the routine intramuscular administration of the drug. Lymphotropic administration of kanamycin failed to provide the same effect. Endolymphatic therapy of the patients during the postoperative period provided a decrease in the lethality, development of complications and the terms of the treatment in the hospital. The therapeutic effect of the endolymphatic administration of kanamycin to the patients with peritonitis proved to be high. The more efficient antibiotic therapy of the cases was likely due to favourable shifts in the pharmacokinetics of kanamycin after its endolymphatic administration.

[Lymphotropic antibiotic therapy of obstetrical suppurative-septic diseases].

The pharmacokinetics and efficacy of gentamicin after its direct and indirect endolymphatic administration were comparatively studied. It was shown that the time course of the blood pharmacokinetics of the antibiotic was almost the same as under the both conditions. Therefore, in many cases the less complicated and more available indirect endolymphatic therapy may replace the direct labor-consuming and traumatic endolymphatic administration of drugs.

[Use of lymphotropic therapy in the multimodality treatment of patients with pulmonary tuberculosis and comorbidity].

Regional lymphotropic therapy involving 10% isoniazid administration was included into multimodality treatment in 250 patients with pulmonary tuberculosis and comorbidity. The findings have indicated that the lymphotropic therapy during the use of rifampicin, streptomycin, ethambutol enhances the efficiency of chemotherapy in different forms of pulmonary tuberculosis, including the latter concurrent with hepatic lesion or diabetes mellitus. Lymphotropic therapy is particularly beneficial to patients with poor drug tolerability. The use of lymphotropic isoniazid in the multimodality treatment of pulmonary tuberculosis leads to the earlier disappearance of the symptoms of tuberculous intoxication (by 1-1.5 months) in 80-92% of the patients, to abacillation in the same periods, to the increased frequency of decay cavity closure by an average of 15% as compared to the conventional treatment, and, in most cases, to the normalization of the biochemical parameters of hepatic function.

[The 5-fluorouracil content of the pancreas and parapancreatic tissues when it is administered regionally into the tissue of the round ligament of the liver].

In the experiment on 31 dogs there was shown a significant increase of 5-fluorouracil concentrations in the pancreas and parapancreatic tissues with regional methods of its administration in the fat of the round ligament of the liver. A more prolonged preservation of the drug concentrations at the therapeutic level in the blood was observed. The most effective method of the cytostatic therapy is the regional lymphotropic method.

[An experimental validation of the endolymphatic administration of pharmacological preparations for stimulating intestinal peristalsis in emergency surgery on the abdominal cavity].

Experiments were conducted on 75 rabbits to study the efficacy of endolymphatic administration of aceclidine, obsidan, and cerucal for stimulation of the intestine in its experimental paresis. The stimulating effect of the pharmacological agents was found to be of a longer duration in endolymphatic that in intravenous administration. This allowed endolymphatic pharmacological stimulation of the intestine to be suggested for the use in abdominal surgery.

[A lymphotropic method of therapy for patients with fresh forms of syphilis (clinical and experimental research)].

Experimental study of benzylpenicillin procain salt kinetics in lymphotropic and intramuscular administration of 600,000 U has revealed a more intensive saturation of the lymph system and a longer persistence of high concentrations of the drug in the blood serum, organs and tissues of the body after lymphotropic administration, as against intramuscular one. Comparison of the efficacies of 16-day courses of therapy of patients with fresh syphilis with benzylpenicillin procain salt administered lymphotropically (once daily, 600,000 U) or intramuscularly (twice daily, the same dose) has shown the advantages of the former method as regards the therapeutic, pharmacokinetic, and economic aspects.

[Effect of obsidan on lymph flow and lymphatic microvessels].

Life-time microscopy showed possible direct effects of obsidan on lymph vessels and flow in the experiment. With local application of the drug there was a short-term constriction, higher frequencies and lower amplitudes of lymphangionic pulsation; during its intramuscular injection, sustained dilator effects, two-phase shifts in the frequency and limitation of pulse amplitude were seen. With the two routes of obsidan administration heterogenous, unpersistent changes in the linear velocity of lymph flow were observed.

[Accumulation of gentamicin in biological fluids, organs and tissues during regional lymphotropic therapy with antibiotics].

Gentamicin distribution in biological fluids, organs and tissues, lymph nodes was studied on 70 dogs. Three administration routes were compared: lymphotropic, intramuscular and regional subcutaneous. The lymphotropic route for the drug administration was recommended not long ago. It was shown that the lymphotropic route provided the antibiotic accumulation mainly in some of the abdominal organs as compared to intramuscular administration. Regional lymphotropic administration of the antibiotic to the experimental animals resulted in higher gentamicin levels in the regional lymph nodes and regional organs as compared to the levels observed after the antibiotic regional subcutaneous administration in the same doses.
