Acoustic reflex testing in otoneurology.

Three parameters of the acoustic reflex have been used in otoneurologic diagnosis: threshold (ART), decay (ARD), and latency (ARL). Each test is defined, put in historical perspective, and discussed in relation to diagnostic implications. We recommend that these three tests - ART, ARD, and ARL - be used as a group and describe how they are implemented in the audiologic diagnosis of acoustic tumors and other lesions affecting the auditory system in the cerebellopontine angle and brain stem.

[Monitoring of the auditory stem evoked potentials in removal of tumors of the posterior cranial fossa].

The material of intraoperative monitoring of the acoustic stem evoked potentials (ASEP) in 9 patients with tumors of the stem and in 28 patients with the parastem localization was analyzed. It was shown that changes in the amplitude of ASEP rather than latency were the decisive factor in the assessment of the functional state of the brain stem. It was found that even the pronounced changes of the bioelectrical activity recorded during the excision of the tumor could be reversible after a change of the strategy of the operative intervention and/or after performing the necessary therapeutic measures. An important test of favorable prognosis of the postoperative course is considered to be the normalization of brain activity, ASEP included, at the final stages of the operation.

Facial myokymia.

Facial myokymia is an uncommon form of dyskinesia, usually resulting from a lesion in the brain stem. The electromyogram is typical. A case is reported. The condition was due to an astrocytoma of the brain stem.

Prognostic factors for postsurgical hearing and facial nerve function in cases of cerebellopontine angle-tumours. The meaning of brain stem evoked response audiometry (BERA).

Presurgical and postsurgical brain stem evoked response audiometry (BERA) examinations were done in patients with tumours of the posterior cranial fossa. A correlation exists between the extent of presurgical BERA-alterations and postsurgical hearing-power and facial nerve function. Small tumours (less than 1 cm) correlate generally with small BERA-alterations and show a good prognosis. Conservation of hearing in cases of larger tumours can only be expected when BERA-alterations are small. Prognosis is generally bad in cases of big tumours (greater than 3 cm). BERA-alterations on the contralateral side point to an additional brain stem compression which can normalize after surgery.

Brain tumors in children. Results of treatment in 138 patients.

Tumors of the nervous system are the third most common malignant disease in Cuban children, after leukemia and lymphoma. In 138 patients with the diagnosis of brain tumor results of treatment by surgery and radiation were studied. The survival rate at 1, 3 and 5 years was 50, 40 and 37.6%. The best results were achieved in astrocytomas, mid grain and brain stem tumors and oligodendrogliomas.
