Nasolabial esthetics after Le Fort I osteotomy and V-Y closure: a statistical evaluation.

The Le Fort I osteotomy results in predictable long-term effects. The soft tissues, however, can be difficult to control because of considerable variation in their adaptation. Several adverse reactions can take place, including thinning and lateral retraction of the lip, accentuation of the nasolabial groove, reduced vermilion exposure, and increase of the nasolabial angle. In this study on 51 selected patients, the V-shaped wound in the vestibule of the maxilla after the Le Fort I osteotomy was closed with a "Y," with the bast of the "Y" in the midline of the upper lip (V-Y closure). Forward multiple regression analysis was calculated for each bony landmark. Regression equations were formulated if P < .05. The equation with the bony point with the highest r2 value was considered the most important independent variable. The selected independent variables were used to form 4 subgroups with identical vectors of movements: impaction, advancement, impaction and advancement, and dorsal impaction. In these 4 subgroups, forward multiple regression analysis was used to select equations with the highest r2 value (P < .05). The selected equations demonstrated that a V-Y plasty as single soft tissue procedure suffices only in advancement cases, and only if the nose does not need additional upward rotation. In the other 3 subgroups, additional procedures are necessary, such as alar cinch suture, reduction of the anterior nasal spine, or grinding of the paranasal area in order to prevent the above-mentioned adverse soft tissue changes.

Monoclonal antibodies in the treatment of ovarian cancer.

Monoclonal antibody application in ovarian cancer started eight years after the discovery of Köhler and Milstein, with the studies of Bast et al. which led to the introduction of CA 125 antigen estimation to everyday clinical routine. At present, monoclonal antibodies serve as a therapeutic tool for variable targets with the potential to be applied in multiple clinical situations. In the coming decade, the drug market is likely to be flooded with monoclonal antibodies. Most of the available monoclonal antibodies will be produced in bioreactors, using transgenic plants or transgenic animals. Apart from mouse antibodies, chimeric, humanised or human antibodies, oncology will certainly employ chimeric structures with antibody fragments built into the structures of receptors or growth factors. Irrespective of their origin, they will constitute an integral part of medical practice.

[An experimental-morphologic study of the regenerative capacity of the nematode Pontonema vulgaris (Bastian, 1865)].

In free esea nematodas Pontonema vulgaris (Bast). end portions were cut off and internal organs were damaged without disturbing the integrity of the cuticle (at the level of the oesophagus, anterior and medial parts of the gut and at the tail level). Reparative processes were registrated under conditions of "closed" traumas only. Removal of degenerating cells and a closure of the wound in the gut was brought about due to the active shift of the intestinal epithelium edges towards one another. The rest of degenerating cells found themselves in the gut lumen and were gradually adsorbed by entodermal cells. A deficiency of the intestinal epithelium cells is made up due to mitotic division of these cells in the epithelium areas adjacent to the site of the trauma. Regeneration in females was found to be more rapid than in males.

[Post-photosynthetic use of labeled assimilates in fiber flax].

The distribution of 14C in various tissues of fiber flax was assayed 1, 17, and 21 days after 30-min assimilation of 14CO@2 by the whole rapidly growing plant. Polymeric photosynthetic products were largely hydrolyzed in the 14C-donor part of the shoot and the hydrolysates were transported upward. The content of 14C in pigments and lipids of the donor leaves (that absorbed 14CO2) was significantly higher than that in the 14C-acceptor ones. An additional nitrogen feeding decreased the labeled sucrose: hexose ratio and inhibited transport of the assimilates from both 14C-donor and acceptor leaves. 14C transported to the shoot tip was largely used for synthesis of poorly soluble proteins (extractable with alkali and Triton X-100) in the acceptor tissues. In the donor part of the shoot, particularly in the bast, cellulose was mainly synthesized from the "new" assimilates.

[The preparation and clinical use of a radioimmunoassay CA125 kit for the diagnosis of epithelial ovarian cancer].

A self-made radioimmunoassay CA125 kit (using OC125 monoclonal antibody ascites offered by Dr. Bast Laboratory which was purified, solidified and labelled with 125I) was used for serum determination in 80 patients with epithelial ovarian cancer and in 40 standard antigen samples. The results demonstrated a statistically significant correlation between our self-made CA125 kit and an imported CENTOCOR CA125 kit (P < 0.001). Clinical experience before operation in 40 patients with epithelial ovarian cancer, 34 with endometriosis and 39 with other malignant ovarian tumors suggested that the self-made CA125 kit was more sensitive in diagnosing epithelial ovarian cancer than the other two diseases. Post-operative follow-up in 86 non-recurrent cases and 101 recurrent cases showed a 96% positive predictive rate and 91% negative predictive rate. It is concluded that self-made CA125 kit is very useful in monitoring epithelial ovarian cancer.

Paget's disease of bone--current thinking and management.

OBJECTIVES: The objective of this article was to critically review the current knowledge on Paget's disease of bone, focusing primarily on possible etiologies and trends in management. Current courses of treatment are described, including calcitonin, diphosphonate and mithramycin therapy and the efficacy of each drug regime is analyzed. Finally, newer, more experimental pharmaceutical agents, such as gallium nitrate, are described. DATA SOURCES: Information was obtained from English language medical and scientific journals and medical/physiology text books. Index Medicus (1983-1989) and MEDLINE computerized data base were used. Paget's disease of bone, paramyxovirus, osteoclast, calcitonin and diphosphonate were the terms indexed directly. Key authors indexed included Rebel, Baste and Paget. Further information was obtained from the National Association for the Relief of Paget's Disease (England). CONCLUSIONS: Current research focuses primarily on histological and hybridization studies over the past 15 yr in favor of a slow virus as the causative agent in the disease, and the evidence supporting this is analyzed in some detail. In particular, the discovery of nuclear and cytoplasmic inclusion bodies within the osteoclasts of patients with Paget's disease and their similarity to respiratory syncytial virus and measles virus are discussed. Some form of environmental or physiological trigger may be involved, but the exact mechanism remains obscure. The current treatment of Paget's disease is based around the prevention of bone resorption using various diphosphonates and calcitonins, but many of these are associated with severe side effects. Newer pharmaceutical agents to control bone turnover, including gallium nitrate, have given promising results in preliminary medical trials.

[Diagnostic value of suspect epilepsy by means of 24 hour of ambulatory electroencephalogram].

106 cases of suspect epilepsy random observed by means of 24 hr tape recording of AEEG were given in this paper. Among them 57 cases. (accounting for 53.77%) are lightly abnormal including 42 cases (accounting for 39.62%) recorded as epilepsy burst. Comparing with EEG: Among 93 cases 25 cases as lightly abnormal by EEG (accounting for 26.88%), 52 cases recorded as lightly abnormal by AEEG (accounting for 55.91%) (P less than 0.005); arising rate of epilepsy burst 11 cases were recorded by EEG (accounting for 11.82%), 41 cases by AEEG (accounting for 44.01%) (P less than 0.005). The record of epilepsy burst patients monitored showed that positive rates of transient consciousness loss (8/15, 53.33%) and convulsions (9/19, 47.39%) are the highest. Abnormal burst time by AEEG is during sleeping (54 cases observed accounting for 77.14%) with positive rate highest at the same time, most accorded in the front temporal area (29/92, 31.52%). The author therefore, suppose that AEEG is more accurate to find abnormal bast wave or epilepsy burst wave than EEG because of superior condition in recording time to EEG and could offer more objective diagnosis reference for suspect epilepsy patients who are difficult to be checked clinically because of low positive rate of EEG.

Neonatal hypernatraemic dehydration and malnutrition associated with inadequate breastfeeding and elevated breast milk sodium.

The advantages of breastfeedng to both the mother and infant are well recognised. In rare instances if breasteding is inadequate or if the sodium content bast milk is high, malnutrition and hypernatraemia can result. A 15 days old exclusively breastfed baby presented with inadequate weight gain. On evaluation, he was found to have hypernatraemia and mother's breast milk showed high sodium concentrations. The infant needed parenteral fluid for correction of dehydration and hypernatraemia. His serum sodium and breast milk sodium of the other of the mother returned to normal gradually. He started gaining weight on exclusive breastfeeds. The present paper describes the case report and brief review of the literature.

[Structural characterization of tissue-specific galactan from flax fibers by 1H NMR and MALDI TOF mass spectrometry].

A high-molecular-mass polysaccharide galactan (M 2000 kDa) was isolated from flax at the stage of cell wall thickening of the bast fiber development. The polymer structure was studied by 1H NMR spectroscopy and MALDI TOF mass spectrometry. It is built up of Gal (59%), Rha (15%), GalA (23%), and Ara (3%) residues. The galactan backbone consists of successively alternating monomer disaccharide units (--> 4GalA1 --> 2Rha1 -->)n and is similar in its structure to the backbone of rhamnogalacturonan-1 (RG-I). Rhamnose residues bear in position 4 beta-(1 --> 4)-galactose side chains of various lengths with a polymerization degree of up to 28 or higher. A part of the side chains have branchings.

Effect of ovariectomy on the level of plasma sex hormones in albino rats.

The study was carried out to evaluate the levels of sex hormones (testosterone, progesterone and estradiol), six weeks post normal ovariectomy as against estimating the levels immediately or less 48 hours after operation. 28 adult female albino rats of Wistar strain were used. They were fed twice a day with Guinea pellets and tap water was offered ad libitum. The animals were grouped into three. Group one was sham operated and served as the control, group two was unilaterally ovariectomised (right removed), group three was bilaterally ovariectomised. The animals were sacrificed at the end of the study by decapitation and the blood collected and assayed for sex hormones. The levels of the sex hormones were measured using radioimmunoassay technique. The results showed significant increase in testosterone level in group 3 with decrease in group 2 which was not significant when compared to group 1. On the progesterone levels, there were significant decrease in group 3 and not 2 when compared to group 1. Estradiol level showed significant decrease in only group 3. The decrease in group 2 was not significant when compared with the control. This work therefore does not propose a longer period of hormonal estimation beyond 48 hours following ovariectomy as the results obtained 6 weeks post ovariectomy did not differ or alter significantly beyond 48 hours when compared with the work of Bast and Greenwald 1977, Bulcher 1977, De Jong et al 1978. It is therefore concluded that sex hormone values obtained 6 weeks after normal ovariectomy did not differ significantly as was observed by various workers and confirmed by the present study.

The quality of life of the dentist as perceived by society.

Research dealing with the public's perception of the quality of life of the dentist is not well developed. Generally, the approach of researchers to this topic has been to examine the public's image of the dental profession. Specifically, the degree of prestige attributed to the dental profession by the public has attracted major attention. A review of studies reveals that while the profession enjoys relatively high prestige, it is extrinsic factors rather than the intrinsic value of the service that determine the public's image. The dental health care system and dental education as presently structured do not appear to be serving the bast interest of the public. The paper concludes with a discussion of what are considered to be major issues that the dental profession both in Canada and elsewhere will have to deal with in the coming years, in order to bring about change both in its image and the generally low utilization of the services it provides.

[Innerearhydrops a result of disturbed endolymphcirculation (author's transl)].

By means of human temporal bone slides we want to demonstrate that interruption of the so-called longitudinal endolymphcirculation causes a hydrops ahead of the block. This fact, proven by animal experiments, we observed in human specimens. Predominant locations are the anatomic narrows of the endolymphatic system: Isthmus of the endolymphatic duct, utriculoendolymphatic valve Bast, ductus reuniens. The different possible pathohistological data are: bony obliteration of the ductus endolymphaticus, embolic closure within the isthmic portion of the endolymphatic duct, blockage of the utriculoendolymphatic valve by the distended utricle or perilymphatic connective tissue strands, angled ductus reuniens in perilymphatic fibrosis.

[Alcohol impaired drivers after completion of a long-term model IVT-Hö rehabilitation].

The "Individualpsychologische Verkehrstherapie (IVT-Hö)" is a psychotherapeutic long-term rehabilitation for alcohol impaired drivers. The therapy is mainly for the benefit of people for whom re-training (BASt model) is unsuitable because of their extreme alcohol problems. 3 years after having completed the treatment and after having been re-granted their driving licences, 3.6% of the drivers had a relapse.

Relative cytotoxic activity of immunotoxins reactive with different epitopes on the extracellular domain of the c-erbB-2 (HER-2/neu) gene product p185.

Different epitopes on the extracellular domain of the HER-2 receptor can serve as distinct targets for immunotoxins. To determine the optimal epitope target for immunotoxin therapy, 7 anti-HER-2 ricin A chain murine monoclonal immunotoxins, each reactive with different epitopes of HER-2 receptor, were tested for cytotoxic activity. The immunotoxins produced 1.2-4.6 logs of cytotoxicity in limiting dilution clonogenic assays with 2 breast cancer cell lines that overexpressed HER-2. Cytotoxicity did not correlate with immunoglobulin isotype, binding affinity, relative position of epitopes or internalization of the anti-HER-2 immunotoxins. Interestingly, the most and least effective immunotoxins bound to epitopes in very close proximity. Competitive binding assays with unconjugated antibodies have previously indicated that our antibodies recognized epitopes that are arranged in a linear array. To orient this relative epitope map, deletions were prepared in the HER-2/neu gene and these mutant constructs were expressed in NIH3T3 cells. Epitope expression was determined by antibody binding and radioimmunoassay. Epitopes targeted by the PB3, 454C11 and NB3 antibodies are localized N-terminal to the epitopes recognized by ID5, BD5, 741F8 and 520C9 antibodies. The 2 non-conformational epitopes PB3 and NB3 were localized to regions corresponding to amino acides 78-242 of the p185(HER-2) protein.

Serum levels of HER-2 neu (C-erbB-2) correlate with overexpression of p185neu in human ovarian cancer.

BACKGROUND: The HER-2 neu (c-erbB-2) oncogene product p185neu is expressed by most ovarian cancers and overexpressed in approximately 30%. METHODS: Sera from patients with ovarian cancer were evaluated for neu antigen using an enzyme-linked immunoassay and for CA 125 antigen by radioimmunoassay. Tissue levels of neu from the same patients were determined by immunohistochemical staining with anti-neu monoclonal antibody. RESULTS: Elevated levels (> 2050 human neu unit [HNU]/ml) of circulating neu determinants have been detected in sera from 15% of 48 patients. Of 45 patients for whom tumor tissue had been cryopreserved, overexpression of neu was found in 17 by immunohistochemical analysis; of these 17, serum neu levels were elevated in 5 (29%). Among the 28 patients with normal to moderate tissue expression of neu, only 2 (7%) had elevated serum neu levels. Thus, elevated serum neu levels predicted tissue overexpression with a specificity of 93%. Serum neu levels were not related to serum levels of CA 125. CONCLUSION: Serum and tissue levels of neu correlate in patients with epithelial ovarian cancer.
