Work-related injuries in Athens County 1982 to 1986. A comparison of emergency department and workers' compensation data.

In 1987, the Panel on Occupational Safety and Health Statistics issued a report concluding that the existing national surveillance system for occupation injuries might result in substantial underreporting of occupational injuries. In this study, we examined two sources of data on occupational injuries, the National Electronic Injury Surveillance System (NEISS) and lost-work time claims to the Bureau of Workers' Compensation (BWC), available in one community, Athens County, Ohio. Based on comparison of the NEISS and BWC data sets, we conclude that neither data set alone gives a complete nor an accurate picture of occupational injuries in Athens County. The two may provide a more complete representation of occupational injuries when examined together. Using the NEISS and BWC data sets in combination results in a total number of injuries higher than that predicted by national norms.

Self-inflicted burns in Athens, Greece: a six-year retrospective study.

Self-inflicted burns represent a major social and medical problem. The aim of this study was to record the epidemiology, mortality and etiology of suicide attempts by burning, in Athens, Greece. Over a 6-year period from April 1997 to April 2003, all the medical charts of the patients who were admitted to the Burn Center of the General State Hospital of Athens, Greece, with self-inflicted burns were retrospectively studied. Of the 1435 burn patients, 53 (3.69%) had attempted suicide by self-inflicted burn. Their ages ranged from 18 to 90 years old (mean 53.5 years). Females (57%) outnumbered males (43%). The mean total body surface area (TBSA) burned was 41.6% (range: 15-100%). The overall mortality rate was very high (75.4%). A preexisting psychiatric disorder was present in 43.3% of the patients. In conclusion, the extent and the depth of the burn injuries could explain the high mortality rate seen in these patients, in correlation with their negativism to the treatment. Burn care professionals should be familiar with self-inflicted burn patients who constitute a considerable proportion of major burns and require constant psychiatric support in addition to burn care.

Induced abortions, contraceptive practices, and tobacco smoking as risk factors for ectopic pregnancy in Athens, Greece.

A case-control study of the role of induced abortion and other factors on the subsequent occurrence of ectopic pregnancy was undertaken in 1986-1987 in Athens, Greece, where a similar study 20 years ago found a tenfold risk of ectopic pregnancy among women with one or more illegal induced abortions. Seventy women residents of Athens, consecutively admitted to the major state maternity hospital with a diagnosis of ectopic pregnancy, were individually matched with women with a newly diagnosed pregnancy of the same order as the ectopic index pregnancy. Two control women were found for each of 63 cases, but only one control for each of the remaining seven cases. All cases and controls were interviewed by the same qualified obstetrician. Statistical analysis was undertaken with stratification of individual matched triplets and pairs, as well as through conditional multiple regression procedures. The relative risk of recurrence of an ectopic pregnancy was 6.39 with 95% confidence interval (CI) 1.96-21.04. Miscarriages did not increase the risk of ectopic pregnancy. The relative risk for subsequent ectopic pregnancy among women with one or more induced abortion, compared to women without such abortions, was 1.87 (CI 0.84-4.16) controlling only for the matching factors, and 1.71 (CI 0.69-4.27) when marital status (a possible selection factor) was also accounted for in the conditional logistic regression. There was no evidence for increasing risk with increasing number of induced abortions. Past use of an intrauterine device (IUCD) was associated with a relative risk of 3.89 (0.72-21.02); the relative risk increased with the duration of use of the IUCD.(ABSTRACT TRUNCATED AT 250 WORDS)

Changes in public attitudes towards mental illness in the Athens area (1979/1980-1994).

Opinions about mental illness were measured, in 1994, in a probability sample consisting of 360 residents of two boroughs in greater Athens. These boroughs represent the catchment area of a community mental health centre. The aim of this study was to compare the differences in attitudes towards mental illness with a matched sample of 360 respondents drawn from the sample of the 1979/1980 attitudinal study, conducted in the same area. The instrument used was the Opinion about Mental Illness scale, which yields five factors. In all five factors statistically significant differences between the two samples were observed. The recent study sample expressed more positive attitudes towards the social integration of the mentally ill, and did not favour the social discrimination against and restriction of mental patients. These results could be explained in the context of a positive and tolerant social climate in the Athens area, strengthened by the implementation of local community mental health intervention programmes.

Development of a Gerodontology course in Athens: a pilot study.

AIM: To describe the development of an undergraduate Gerodontology course in Athens Dental School. BACKGROUND: Because of demographic changes, undergraduate dental curricula should place appropriate emphasis on the oral care of the elderly. Therefore, the Athens Dental School Curriculum Committee authorised the development of a new Gerodontology course. METHODS: The new course was introduced in the 10th (final) semester of undergraduate studies. Teaching responsibilities were shared amongst staff from various Dental School departments and the National Health System. The course was elective and mainly didactic, consisting of seminars within the Dental School, educational visits to hospitals and geriatric day centres and elective clinical work in the comprehensive care clinic. The students evaluated the course at the end of the semester and indicated its strengths and weaknesses from their perspective. CONCLUSION: The new course was generally satisfying. Based on the experience and evaluation of the first pilot year and taking into consideration the existing barriers, we plan to improve and expand educational activities, mainly including improved methods of teaching and assessment, and more clinical assignments.

Prevalence and determinants of exclusive breastfeeding during hospital stay in the area of Athens, Greece.

AIM: To assess breastfeeding practices, focusing on the prevalence and the determinants of exclusive breastfeeding during hospital stay. METHODS: A cross-sectional study of 1603 healthy women, who delivered healthy infants weighing more than 2500 g, was conducted in the area of Athens, Greece. Participants completed a self-administered questionnaire on the day they were discharged from the maternity ward. Classification of breastfeeding and recall period from birth to discharge were in accordance with the WHO criteria. Hierarchical logistic regression analysis was used to study determinants of exclusive breastfeeding initiation. RESULTS: Breastfeeding initiation was reported by 96.1% of the participants. However, exclusive breastfeeding was initiated only in 19.1% and predominant breastfeeding in 7.2% of the cases. The univariate analysis showed that maternal younger age, low educational level, unemployment, vaginal delivery, infant birthweight >3000 g, multiparity, early initiation of breastfeeding, rooming-in and awareness on the existence of breastfeeding centres were associated with higher rates of exclusive breastfeeding. Logistic regression analysis revealed that rooming-in (OR 3.72, p<0.01), demand feeding (OR 2.18, p<0.01), type of delivery (OR 1.61, p<0.01) and the source of information received about breastfeeding are more important determinants of exclusive breastfeeding than the socio-demographic parameters. CONCLUSIONS: Exclusive breastfeeding during hospital stay in the area of Athens is low. Demand feeding and rooming-in positively influence exclusive breastfeeding initiation, while caesarean section and information by mass media have a negative impact.

Etiological characterization of hepatitis B surface antigen-negative hepatitis among adult patients in Athens, Greece.

In a 4-month period, 216 cases of acute viral hepatitis were diagnosed in adults at the Infectious Diseases Hospital, Athens, Greece. Twenty-six percent of these were hepatitis B surface antigen negative. A full set of clinical specimens was obtained from 19 of these patients, who were studied in depth for the etiology of their hepatitis. A total of 7 of the 19 patients had serological evidence of hepatitis A virus infection, and 2 had evidence of recent hepatitis B virus infection. The remaining 10 patients lacked evidence of hepatitis A virus, hepatitis B virus, cytomegalovirus, or Epstein-Barr virus infection related to their illness and were classified as having type non-A, non-B hepatitis. Types A and non-A, non-B hepatitis were clinically similar in these adults patients. However, patients with type non-a, non-B hepatitis were, on the average, older and more likely to have received parenteral inoculations during the 6 months before contracting hepatitis. Approximately 76% of the 216 consecutive cases of acute viral hepatitis were probably type B, approximately 10% were probably type A, and approximately 14% were probably type non-A, non-B, although the latter two types may be underestimated because of the possibility of superinfection with these viruses in asymptomatic hepatitis B surface antigen carriers. Thus, all three types of viral hepatitis appear to be important in the etiology of liver disease in Athens, Greece.

Waterborne diseases prevention: evaluation of inspection scoring system for water sites according to water microbiological tests during the Athens 2004 pre-Olympic and Olympic period.

STUDY OBJECTIVES: To evaluate the inspection grading system for water sites implemented during the Athens 2004 Olympic inspection programme. DESIGN: The relation between the standardised inspections results of 716 water supply systems and 289 public swimming pools, and microbiological test results of 2358 samples collected during inspections was examined. SETTING: Athens, Thessaloniki, Patra, Volos, and Iraklio, Greece. Inspections and sampling conducted during a two year period before the 2004 Olympics. MAIN RESULTS: Swimming pools unsatisfactory inspection grading results were significantly associated with positive water microbiological test results (relative risk = 2.5, p<0.05). One of the six violations of swimming pools and five of the seven violations of water supply systems designated as "critical" water safety hazards in the inspection reports were significantly associated with positive microbiological test results. The receiver operating characteristic analysis identified the unsatisfactory score designed in the swimming pools standardised inspection report, as the ideal score (-15), in adequately producing positive microbiological test results (sensitivity 13.2%, specificity 89%). CONCLUSIONS: This study shows the utility of standardised inspection grading systems in waterborne diseases prevention planning and implementation strategies of policy makers and regulators. Future water quality assessment should be based on the implementation of a robust standardised inspection system and reduce the need of microbiological tests.

Problems related to the organization and opening of the first day hospital in Athens.

Problems in connection with the preparation and opening of the first Day Hospital in Athens are described, as well the difficulties created by local conditions. These problems are mainly related to our out-dated legislation, bureaucracy, public prejudice about the psychiatric patient, the lack of supplementary helping services and more general state care for the rehabilitation of psychiatric patients, as well as the fact that the single Day Hospital in Athens will be called upon to cover a very large sector of our population.

An analysis of factors that influence personal exposure to toluene and xylene in residents of Athens, Greece.

BACKGROUND: Personal exposure to pollutants is influenced by various outdoor and indoor sources. The aim of this study was to evaluate the exposure of Athens citizens to toluene and xylene, excluding exposure from active smoking. METHODS: Passive air samplers were used to monitor volunteers, their homes and various urban sites for one year, resulting in 2400 measurements of toluene and xylene levels. Since both indoor and outdoor pollution contribute significantly to human exposure, volunteers were chosen from occupational groups who spend a lot of time in the streets (traffic policemen, bus drivers and postmen), and from groups who spend more time indoors (teachers and students). Data on individual and house characteristics were obtained using a questionnaire completed at the beginning of the study; a time-location-activity diary was also completed daily by the volunteers in each of the six monitoring campaigns. RESULTS: Average personal toluene exposure varied over the six monitoring campaigns from 53 to 80 microg/m3. Urban and indoor concentrations ranged from 47-84 microg/m3 and 30 - 51 microg/m3, respectively. Average personal xylene exposure varied between 56 and 85 microg/m3 while urban and indoor concentrations ranged from 53-88 microg/m3 and 27-48 microg/m3, respectively. Urban pollution, indoor residential concentrations and personal exposures exhibited the same pattern of variation during the measurement periods. This variation among monitoring campaigns might largely be explained by differences in climate parameters, namely wind speed, humidity and amount of sunlight. CONCLUSION: In Athens, Greece, the time spent outdoors in the city center during work or leisure makes a major contribution to exposure to toluene and xylene among non-smoking citizens. Indoor pollution and means of transportation contribute significantly to individual exposure levels. Other indoor residential characteristics such as recent painting and mode of heating used might also contribute significantly to individual levels. Groups who may be subject to higher exposures (e.g. those who spent more time outdoors because of occupational activities) need to be surveyed and protected against possible adverse health effects.

Atmospheric circulation types and daily mortality in Athens, Greece.

We investigated the short-term effects of synoptic and mesoscale atmospheric circulation types on mortality in Athens, Greece. The synoptic patterns in the lower troposphere were classified in 8 a priori defined categories. The mesoscale weather types were classified into 11 categories, using meteorologic parameters from the Athens area surface monitoring network; the daily number of deaths was available for 1987-1991. We applied generalized additive models (GAM), extending Poisson regression, using a LOESS smoother to control for the confounding effects of seasonal patterns. We adjusted for long-term trends, day of the week, ambient particle concentrations, and additional temperature effects. Both classifications, synoptic and mesoscale, explain the daily variation of mortality to a statistically significant degree. The highest daily mortality was observed on days characterized by southeasterly flow [increase 10%; 95% confidence interval (CI), 6.1-13.9% compared to the high-low pressure system), followed by zonal flow (5.8%; 95% CI, 1.8-10%). The high-low pressure system and the northwesterly flow are associated with the lowest mortality. The seasonal patterns are consistent with the annual pattern. For mesoscale categories, in the cold period the highest mortality is observed during days characterized by the easterly flow category (increase 9.4%; 95% CI, 1.0-18.5% compared to flow without the main component). In the warm period, the highest mortality occurs during the strong southerly flow category (8.5% increase; 95% CI, 2.0-15.4% compared again to flow without the main component). Adjusting for ambient particle levels leaves the estimated associations unchanged for the synoptic categories and slightly increases the effects of mesoscale categories. In conclusion, synoptic and mesoscale weather classification is a useful tool for studying the weather-health associations in a warm Mediterranean climate situation.

Unintentional childhood poisoning in athens: a mirror of consumerism?

OBJECTIVE: To estimate the incidence of unintentional childhood injuries resulting from accidental poisonings in the Greater Athens area and to ascertain what fraction of this incidence could be linked to specified conditions, amenable to preventive interventions. METHODS: Prospective study over 12 months of 670 children hospitalized 224 hours for accidental poisoning. Site: Two pediatric hospitals and three smaller units in Greater Athens admitting children < or = 15 years old. Information was recorded in a semistructured questionnaire and the data were analyzed through simple stratification by one or more variables. Results Accidental poisoning requiring hospitalization > or = 24 hours was 50% higher among boys than among girls, peaked towards the end of the second year, and declined sharply after the fourth year of life with an estimated incidence of 500 cases per 100,000 among children > or = 5 years old. Cigarettes were the most common agent among infants, whereas medicinal products dominated all other childhood periods. Detergents, petroleum products, and pesticides each contributed about 10% of all poisonings with detergents peaking during the second year of life, petroleum products during the third year, and pesticides remaining constant, in proportional terms, throughout childhood. During the working hours of the day the incidence of poisonings was 80% higher than during the late afternoon and evening hours or the weekends, the times when both parents are usually at home; the excess was statistically significant. The presence of both parents at home in the afternoon hours was associated with an almost 50% reduction of hospitalized poisoning. The accessibility of products with poisoning potential was of major importance. Some specific conditions that led to the incident included storage of potentially poisoning products in the refrigerator, storage of such products in containers of innocuous products, without proper labeling, and parental errors in medication. CONCLUSIONS: Unintentional childhood poisoning further reflects an interaction between inappropriate storage of consumer products and suboptimal supervision during the housekeeping hours of the day.

Prevalence of hepatitis B and C markers among refugees in Athens.

AIM: To assess the prevalence of hepatitis B and C serological markers in a population of refugees living in Athens. METHODS: One hundred and thirty refugees (81 males and 49 females, mean age +/-SD: 31.7+/-8 years) were included in the study. The hepatitis B virus surface antigen (HBsAg), the hepatitis B virus core antibody (anti-HBc) and the hepatitis C virus antibody (anti-HCV) were detected using a third-generation immunoassay. RESULTS: Twenty individuals (15.4 %) were HBsAg positive and 69 (53.1 %) were anti-HBc positive. The prevalence of HBsAg and anti-HBc was higher among refugees from Albania and Asia (statistical significant difference, P<0.008 and P<0.001 respectively). The prevalence of these markers was found irrelevant to age or sex. Anti-HCV was detected in the serum of 3 individuals (2.3 %). No differences among age, sex or ethnicity regarding anti-HCV prevalence were found. CONCLUSION: It can be concluded that refugees living in Athens are an immigrant population characterized by a high incidence of HBV infection. The prevalence of HBV markers is higher among refugees from Albania and Asia. It is therefore believed that the adherence to general precautions and the initiation of HBV vaccination programs will be necessary in the future, especially in these communities. Although the prevalence of HCV infection seems to be relatively low, extended epidemiological surveys are needed to provide valid results.

Effect of insecticide spraying for malaria control on the incidence of sandfly fever in Athens, Greece.

Sera from 637 Athens residents of various age groups were examined by plaque reduction neutralization test for antibodies against Naples and Sicilian Phlebotomus fever viruses. A marked change in the prevalence of antibodies to both agents was observed in persons born after 1946, when residual insecticide spraying for malaria control was initiated in Greece. The prevalence of Naples and Sicilian neutralizing antibodies among residents greater than or equal to 30 years of age was 36% and 13%, respectively. In contrast, only 4% of persons less than or equal to 29 years of age had Naples antibodies and all were negative to Sicilian. These serologic data confirm previous clinical observations that sandfly fever becam uncommon in Athens after initiation of the insecticide spraying program. Presumedly the spraying program was effective in reducing the Phlebotomus population to levels where virus transmission was minimal. New information on the specificity and duration of Phlebotomus fever neutralizing antibodies is also presented.

The history of the walls of the Acropolis of Athens and the natural history of secondary fracture healing process.

During its long and adventurous history, the Acropolis of Athens has been a site of many dramatic events. It suffered its most disastrous destruction during the Persian wars. Under the command of King Xerxes, the Persians invaded Athens and ruined the Temple of the Parthenon and the walls of the Acropolis. After their victorious sea battle at Salamis, the Athenians, led by Themistocles, returned home and tried to repair the damage. Their priority still was to defend their city by restoring the walls of the Acropolis. Materials of all kinds were salvaged from the ruins of the Acropolis and used for an immediate reconstruction of the walls. Later, when the Athenians became the leaders of the Greek world, it was decided that the walls should be rebuilt in a proper artistic way. Themistocles suggested that a small section of the walls, which had formerly been a part of the urgent restoration, should remain in place so as to remind the citizens of this historical event. This is a characteristic example of the biological and mechanical adaptation of fracture callus to musculoskeletal function. After a period of urgency with the fixation of a fracture by means of a primitive secondary callus formation, the broken limb gradually returns to its usual function. Increased mechanical loading enhances the remodelling of the callus and the replacement of woven bone with lamellar bone.

Mass gathering preparedness: the experience of the Athens 2004 Olympic and Para-Olympic Games.

Planning for environmental health management of the Athens 2004 Olympic Games included assessment of the capacity and deficiencies of relevant public health agencies in Athens and the other four Olympic cities with 17 detailed questionnaires. Different estimates of the expected number of visitors were made on the basis of experience from previous Olympic Games. Potential public health risks were identified and prioritized. A number of deficiencies in personnel, resources, training, and coordination were identified in most agencies. One hundred and ninety-six environmental health inspectors were expected to work during the Olympic period. Around 18,000 athletes and escorts from 201 countries were expected to participate in the Olympic Games and 7,000 to participate in the Para-Olympic Games. Three different scenarios regarding the number of expected visitors were explored; the most probable estimate being 1,950,000 visitors. Foodborne diseases, waterborne diseases, and heat-related illnesses were considered of high priority during the Olympic period, as were traffic accidents. The Olympic Games are a difficult assignment that demands extensive planning, appropriate resources, and timely reporting and coordination among the various public health agencies.

Prevalence of sexually transmitted infections in female sex workers in Athens, Greece - 2005.

There is little data on the prevalence of STIs in female sex workers, Greek and immigrants, working in Athens, Greece, since most of them work without any form of official license. Our aim was to establish the prevalence of STIs in asymptomatic legal Greek and immigrant female sex workers in Athens, Greece. The study involved an evaluation of gonococcal and chlamydial infection, early infectious syphilis, HIV infection, HSV-2 infection, Hepatitis B and C in 299 female sex workers who applied for an official work permit between May 2005 and October 2005. HSV-2 infection was more common in the Greek sex workers. No difference was found for the other STIs. Prevalence was related to age. A significant association was found between HSV-2 and syphilis. No HIV infection was detected. We concluded that asymptomatic sex workers can be a source of STIs which points out the need for a better health system control in Greece.

Comparison of the dental health of 12-year-old schoolchildren living in Athens and Glasgow.

In 1987 studies of the dental health of 12-year-old children in Athens and Glasgow were linked to allow comparison. Standardized examiners used the same methodology and criteria in the two studies and the data were also analysed by socio-economic background. Mean DMFT in Glasgow was 2.91, compared with a mean of 4.71 in a 1984 study. Mean DMFT for Athens 12-year olds was 2.41, in 1987, showing that irrespective of social background, they had reached the WHO Global Goal of 3.0. In Glasgow, 12-year olds from the deprived areas still fell far short of this target. As Greece's new National Health Service begins, a plea is made for the commissioning of National Dental Surveys, to assist monitoring and planning.
