Economic profiling of physicians: does omission of pharmacy claims bias performance measurement?

OBJECTIVE: To investigate the degree to which the absence of prescriber identifying information and the absence of pharmacy claims might affect the validity of physicians' economic profiles. STUDY DESIGN: The study database consisted of 4 years of claims from a mixed-model health maintenance organization. Using the grouper of Episode Treatment Groups by Symmetry Health Data Systems, Inc, 2 episode databases were created, with and without pharmacy claims included. For each database, the responsibility for defined episodes was attributed to physicians within specialty (1) on the basis of combined professional and prescribing costs and (2) on the basis of professional costs alone. METHODS: Using the different databases and attribution rules, physicians were ranked within specialty on the economic profiling metric, and the various rankings were compared for consistency. Analyses were performed for cardiologists, family practitioners, general surgeons, and neurologists. RESULTS: The absence of prescriber identifying information appears to have only a small effect on physicians' economic profiles. The absence of pharmacy claims, on the other hand, may affect economic profile performance, but the effects differ by specialty and depend on pharmacy costs as a percentage of episode total costs for the specialty and the correlation of episode costs with and without pharmacy costs included. CONCLUSIONS: Physicians' economic profile rankings are not greatly affected by the presence or absence of prescriber identifying information in pharmacy claims. If pharmacy claims are missing altogether, however, valid economic profiling remains feasible for some clinical specialties but not for others.

Soft tissue profile of children with impaired nasal breathing.

UNLABELLED: The aim of the study was to evaluate soft tissue profile of the children with impaired nasal breathing. MATERIALS AND METHODS: Soft tissue points relative to the true vertical line (TVL) were measured on the lateral cephalograms in natural head position of 54 subjects with diagnosed nasal obstruction (34 males, 20 females, mean age 13.3+/-2.7). As controls served 33 patients receiving orthodontic treatment for different types of malocclusion (19 males, 14 females, mean age 13.4+/-2.7). Nasal airflow measurements were performed for all children. RESULTS: Both groups had retrognathic soft tissue profile, and there were no statistically significant difference between the groups in the linear and angular measurements of the soft tissue measurements, except for the interlabial gap measurement. Soft tissue profile projections to TVL were dependent on craniocervical and cervical inclination angles. In addition head extension was associated with flattened mentolabial sulcus and increased lower face height. Some of the soft profile measurements correlated with age. CONCLUSION: Soft tissue profile of the children with impaired nasal breathing in general is not different from the soft tissue profile of other orthodontic patients and mostly is dependent on the craniocervical posture and age.

Outer membrane protein profiles of Edwardsiella ictaluri from fish.

Outer membrane proteins (OMP) prepared with sodium N-lauroyl sarcocinate (SLS) from 33 Edwardsiella ictaluri isolates from fish were examined by electrophoresis. Twenty-eight isolates from channel catfish (Ictalurus punctatus) had similar OMP profiles. Ten bands (71 kilodaltons [kD] to 19.5 kD) were identified in all isolates from channel catfish. One major 35-kD protein comprised most of the protein content of the outer membrane of isolates from channel catfish. Differences existed among isolates in the amount of protein within minor OMP bands. Edwardsiella ictaluri ATCC 33202 contained larger quantities of the 38.5- and 37-kD proteins than did the other isolates. Outer membrane protein profiles of E ictaluri derived from Bengal danio (Danio devario) and walking catfish (Clarias batrachus) were identical to OMP profiles of isolates from channel catfish. In contrast, OMP profiles from single isolates from green knife fish (Eigemannia virescens) and white catfish (Ictalurus catus) were different. Variations in incubation time, SLS extraction time, SLS extraction number, and in vivo and in vitro passage had no effect on the OMP profile of E ictaluri ATCC 33202. An increase in duration of sample solubilization did affect the OMP profile of E ictaluri ATCC 33202 by decreasing the amount of protein in 52-, 46-, and 43.5-kD bands. Accompanying the decrease were increased staining intensity in the 31.5- and 28.5-kD bands and the appearance of 4 new bands (34, 33, 25.5, and 22.5 kD). Edwardsiella ictaluri, a gram-negative bacterium in the family Enterobacteriaceae, is the cause of enteric septicemia of catfish.(ABSTRACT TRUNCATED AT 250 WORDS)

Fluid-shift during computer-modulated profile hemodialysis (CMP-HD).

Computer-modulated profile hemodialysis was examined for patients' comfort and fluid-shift. Fifteen patients were studied after two weeks of dialysis with each of the following profiles: A): constant ultrafiltration (UF) and dialysate sodium (138 mmol/l); B): decreasing UF; C): decreasing UF and decreasing high dialysate sodium (starting 10% above serum-sodium, with a gradual reduction to 138 mmol/l in the fourth hour). Patients with a calculated increase of intracellular volume (ICV) during dialysis had more complaints after dialysis than the others. ICV decreased in all patients with low serum-sodium (less than 136 mmol/l) during all profiles, whereas in patients with higher sodium, only profile C led to a decrease of ICV. However, interdialytic weight gain increased about 75% in patients with low serum-sodium under profile C. The sodium profile could help in preventing imbalance without side effects in patients with high sodium.

[A comparative study of cephalometric evaluation criteria of the cutaneous profile].

The objectives of this study is to challenge the soft tissue profile standards of esthetics and to compare them with the feeling and the sense of beauty of the large public. The facial profile of one hundred fifty-two cinema stars and top models (seventy-one men and eighty-one women) were analysed. Twenty three measurements were taken among the most used and widely accepted. These references come from the works of very reknown orthodontists and researchers: BURSTONE, HOLDAWAY, MERRIFIELD, PECK and PECK, RICKETTS, STEINER, WORMS (as described by MOSHIRI). The results demonstrate, after statistical analysis that: some values are very different compared with the references; those discrepancies neither concern the same values nor for the same amount, or to the same direction for the male or the female samples. The mean male soft tissue profile is more concave, prognathic type with more retruded upper and lower lips and more reduced lower face height than the standards described by the authors. The mean women soft tissue profile tends to be more convex with a relatively more protruded upper lip and a more retrognathic mandible than the "standards". The whole face height is significantly reduced with a more marked discrepancy for the nasal height and the lower lip length. The nasal prominence is also reduced compared with the norms. It appears then that we cannot trust anymore references that do not take into account differences between men and women ideal facial profiles for the analysis of soft tissue profile in orthodontics and maxillo-facial surgery. The values described by the authors, which are our daily standards, do not seem to match the large public sense of esthetics and therefore our patients feeling of beauty.

Identification of T. pallidum polypeptides: a comparative study between the protein profiles of in vitro cultivated and in vivo propagated Treponema pallidum subsp. pallidum by two-dimensional polyacrylamide gel electrophoresis.

Protein profiles of in vivo propagated and in vitro cultivated Treponema pallidum subsp. pallidum (Nichols strain) were analyzed using two-dimensional gel electrophoresis (2D-PAGE). A comparative study between the two protein profiles (as detected by silver stain, Western blotting, and autoradiography) demonstrated two in vitro synthesized polypeptides with approximate molecular masses of 29- and 35 kDa (both with pI 5.62) which were absent in the protein profile of the in vivo propagated T. pallidum. It also was demonstrated that the in vitro cultivated organisms lacked an antigenic polypeptide (as judged by Western blots) of approximate molecular weight 35 kDa (pI 5.34) which was present in the in vivo propagated organisms. To determine the cellular location of these polypeptides, the outer membrane proteins of T. pallidum was extracted with Triton X-114. The extracted proteins were subjected to phase partitioning and the 2D-PAGE protein profiles of the detergent and aqueous phases were compared. The results indicated that in vitro cultivation caused some changes in the cytoplasmic protein profiles of T. pallidum, but no changes were detected in the profiles of the membrane proteins.

Minor Symptoms Evaluation (MSE) Profile--a questionnaire for assessment of subjective CNS-related symptoms.

Minor Symptoms Evaluation Profile (MSE-profile) is an instrument for the assessment of subjective CNS-related symptoms and has recently been documented. This self-applied questionnaire includes 24 items and uses a visual analogue scale to record perceived symptoms. Based on a subjective grouping followed by a principal component analysis of the results of 374 individuals to confirm the grouping, 15 out of the 24 items were formed into three dimensions: Contentment (7 items), Vitality (5 items) and Sleep (3 items). The Cronbach's alpha coefficient used to evaluate the internal consistency of these dimensions was 0.81, 0.81 and 0.77, respectively. Compared with the equivalent dimensions of other questionnaires for measurement of health-related quality of life, the dimensions of the MSE-profile were found to be relevant. Furthermore, the MSE-profile showed good discriminative ability to distinguish symptom profiles of patients with other diseases/complaints than hypertension. It can be concluded that the MSE-profile is practically useful, reliable and sensitive enough for the assessment of subjective CNS-related symptoms that might affect the patient's well-being/quality of life.

Relationship of serum estradiol and progesterone concentrations to the excretion profiles of their major urinary metabolites as measured by enzyme immunoassay and radioimmunoassay.

Paired daily blood and urine samples were collected from 10 apparently healthy premenopausal women to compare the hormone profiles of estradiol (E2) and progesterone in serum with those of estrone conjugates (E1Conj) and pregnanediol-3-glucuronide (PdG) in urine. Serum hormones were measured by radioimmunoassay (RIA) kits, whereas the urinary steroid metabolites were assessed by both RIA and enzyme immunoassay (EIA). RIA and EIA values for urinary E1Conj and PdG were not different, and both methods produced urinary profiles that paralleled the profile of the parent steroid in serum. However, the simplicity, flexibility, and economy of EIA will make this method more widely applicable. Mean E1Conj values lagged behind concentrations of serum E2 by one day or less, whereas daily urinary PdG profiles lagged behind serum progesterone by one to two days. Mean urinary profiles of E1Conj were similar whether or not creatinine was used to adjust for urine volume; however, creatinine indexing was beneficial when urinary profiles in individual cycles were compared with changes of serum E2.

Simulation of human pharmacokinetic profiles in mice, and impact on antimicrobial efficacy of netilmicin, ticarcillin and ceftazidime in the peritonitis-septicemia model.

Pharmacokinetic profiles in small animals substantially differ from those observed in man. We hence devised a man adapted animal model to critically assess the impact of such differences on antimicrobial efficacy. We approximated in mice the human pharmacokinetic profiles of netilmicin, ticarcillin and ceftazidime. The CD50 (curative dose for 50% of lethally intra-peritoneally infected animals) against Pseudomonas aeruginosa was comparatively determined for murine versus man-adapted pharmacokinetic profiles. With netilmicin the man-adapted profile was significantly less efficacious than the murine profile. In contrast, a significant superiority of the man-adapted profile was found with the beta-lactam drugs. We conclude that determinations of antimicrobial activity in small animals may yield misleading results in respect to man. Depending on the drug in question, murine pharmacokinetics may lead to overestimation or underestimation of antimicrobial activity. Our findings are of particular importance for the interpretation of studies in small animals comparing different antimicrobial compounds or different dosage regimens.

Soft-tissue profile changes concurrent with the orthodontic treatment.

Anthropologists have shown that the external covering made up of integument, adipose tissue, connective tissue, and muscle does not always distribute itself in a uniform, orderly manner. There are great variations in the amount and distribution of these soft-tissue elements. Therefore, a facial profile analysis that is limited to measurements on the hard skeletal structure would not appear to conform to the standards of accuracy if an assessment of the soft-tissue profile were required. The purpose of this investigation is to provide an understanding of the changes which occur in the soft-tissue profile during the orthodontic treatment concurrent with normal growth and development. At the present time, it is not possible to devise a set rule for differentiating a desireable from an undesireable soft-tissue facial profile. However, this should not prohibit the presentation of some reference material as an aid in the diagnosis and practice of orthodontics. In many instances evaluations of facial esthetics seem to be singularly influenced by the orthodontist's concept of a pleasing face. At present, the accomplishment of soft-tissue profile changes by dental movement is limited so it is very important to rely on proper timing of the orthodontic treatment through the pubertal growth period to achieve optimal profile changes.

A partial methylation profile for a CpG site is stably maintained in mammalian tissues and cultured cell lines.

We wished to determine if a partial methylation profile for a specific CpG site was stably maintained in both mammalian tissues and cultured cell lines. To accomplish this, we identified a CpG site with a partial methylation profile located upstream of the mouse adenine phosphoribosyltransferase promoter region. This site was found to be methylated at a level of approximately 25% in mouse brain, kidney, lung, and skeletal muscle tissues, at a level close to 50% in liver, and at level close to 0% in testis. These tissue-specific methylation profiles were not altered during aging. A methylation profile of approximately 25% at this CpG site was also observed in five mouse teratocarcinoma stem cell lines and one additional cultured cell line. This profile, however, was altered upon cellular differentiation, adenine phosphoribosyltransferase hemizygosity, and a loss of adenine phosphoribosyltransferase activity in some of the cultured cell lines. We conclude that partial methylation of a specific CpG site can be stably maintained both in vivo and in vitro and that a mechanism exists for its maintenance. The functional significance of a partial methylation profile remains to be determined.

Dense cored vesicles in presynaptic profiles of the rabbit dorsal lateral geniculate nucleus.

We are carrying out a study about the synaptic relations between identified synaptic profiles in the dorsal lateral geniculate nucleus (dLGN) of the rabbit. Here, the types of synaptic vesicle containing profiles of the dLGN are described. There are presynaptic large profiles containing round vesicles and pale mitochondria (RLP terminals) and small profiles that contain round vesicles and dark mitochondria (RSD terminals) which respectively arise from the retina and the visual cortex. Another type of presynaptic profile contains elliptical vesicles (F-boutons) which can be subdivided according to their cytoplasmic content. These F-boutons arise from dLGN interneurons. We have found different sized vesicles that have a dense core within RLP, and F terminals and a possible RSD terminal. The significance of the coexistance of pale and dense cored vesicles in the presynaptic profiles of the rabbit dLGN is discussed.

Plasmid profile as a useful marker of a Shigella dysenteriae type 1 strain causing an epidemic of shigellosis.

Plasmid profiles of 60 Shigella dysenteriae type 1 isolates obtained during an epidemic period (April - October 1983) were compared with that of 74 other Shigella species isolated during the same period of time and also with that of 20 S. dysenteriae type 1 isolates obtained before the epidemic period (1969 - 1982). All samples were collected during an epidemic of shigellosis from diarrhoeal patients being treated at the Dhaka Treatment Centre of the International Centre for Diarrhoeal Disease Research, Bangladesh (ICDDR,B) between April and October 1983. Approximately 4% of the patients were sampled. The isolated showed varied patterns of resistance to several antimicrobial agents. Ninety-two per cent of the 60 S. dysenteriae type 1 strains showed a typical profile of four plasmids with masses of 140, 6, 4 and 2 megadaltons (Mdal). The finding is in contrast with that of a previous study at the ICDDR,B, in which 14 different plasmid profiles were observed in 23 strains of S. dysenteriae type 1 isolated during a 10 - month period in 1982. The profile typical for this study was different from those found in other species of Shigella isolated at the same time. There was a strong association between the possession of this plasmid profile and resistance to chloramphenicol, streptomycin, tetracycline and trimethoprim - sulphamethoxazole. Twenty S. dysenterae type 1 strains isolated from the faeces of patients at the ICDDR,B during 1969 - 1982 were also analysed for their plasmid profiles. The finding is in contrast with that of the epidemic strains. The typical epidemic plasmid pattern, i.e. the 140, 6, 4 and 2 Mdal, was absent in isolates obtained before 1983 epidemic.(ABSTRACT TRUNCATED AT 250 WORDS)

[Real-time observation of blood flow velocity profiles in the right ventricular inflow tract by a newly-developed instantaneous B-mode and multi-channel Doppler echocardiography].

We observed the blood flow profile in the right ventricular inflow tract through the tricuspid valve using the newly-developed equipment which images by instantaneous B-mode and multi-channel Doppler echocardiography. The Doppler system had 64 sampling gates in an ultrasonic beam within a depth of 13 cm. A cursor line was set at an angle of 45 degrees to the tricuspid annulus on two-dimensional echocardiography in a parasternal long-axis view of the right atrium and right ventricle. The blood flow velocity was displayed on the vertical line on the left-side of the CRT image. All Doppler-shifted frequencies of the 64 channels were analyzed using a fast Fourier transform formula by a built-in processor. The Doppler-shifted frequency was displayed at 30 frames per sec. The study subjects consisted of 20 children without cardiac anomalies. Their ages ranged from 2 to 18 years. A typical blood velocity profile at the tricuspid valve ring during the rapid filling phase had an "M" shape, i.e., the velocity was greater at both margins than in the central portion, followed by a flat profile. A small retrograde flow was observed behind the posterior tricuspid leaflet at this time. The flow velocity decreased in mid-diastole, then increased again during the atrial contraction period, with either flat or parabolic profile. During inspiration, the velocity was greater and the shape of the flow profile throughout diastole tended to be flat. In systole, a slow antegrade flow was observed in the tricuspid valve ring area, and its flow profile was parabolic.(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of errors in reangulation on planar and tomographic thallium-201 washout profile curves.

Cardiac phantom studies were performed with and without a defect present to test the hypothesis that myocardial 201TI quantitative circumferential washout profile curves calculated from planar and rotating slant hole (RSH) collimator tomographic images are equally affected by errors in axial repositioning. Simulated stress images were acquired with the long axis of the phantom perpendicular to the camera surface and redistribution images were acquired to represent 50% 201TI washout with axial repositioning errors relative to the stress position ranging from 0 to 20 degrees in 5 degrees increments. There was a decrease in the 201TI washout profile curves compared to that expected (50%) in the wall tilted away from the camera surface, and a reciprocal increase in the 201TI washout profile curves in the wall tilted towards the camera surface for both imaging techniques whether a lesion was present or not. This effect became more pronounced as the error in axial repositioning was increased for both the planar (p less than 0.001) and the RSH tomographic (p less than 0.001) techniques. However, the deviation of the 201TI washout profile curves from that expected (50%) was greater for the planar imaging technique with or without a lesion (p less than 0.05 to 0.001). Thus, we conclude that 201TI quantitative circumferential washout profile curves calculated using this tomographic imaging technique are less affected by errors in axial repositioning than those calculated using an equivalent projection by standard planar imaging methods. These data emphasize the importance which must be placed on the repositioning of patients to obtain valid 201TI washout profile curves for the detection and localization of coronary artery disease.

Practice profiles in evaluating the clinical experience of family medicine trainees.

A practice profile is a compilation of information allowing definition and evaluation of any of several parameters of health-care delivery. Prerequisites for development of profiles presented in this report include: suitable classification of health problems; patient demographic data; identification of individual and groups of health-care providers; and definition of the practice or study population. Details are given for two profiles: the assigned patient and the diagnostic workload profiles. Single profiles are purely descriptive but, when evaluated by appropriate peer comparison, may form the basis for a more dynamic process--that of improvement and change. Possible applications of practice profiles are discussed.

[Fourier harmonic analysis of the sagittal profile of the Plesianthropus transvaalensis (STS5) skull].

An analytical morphometrical investigation of fronto facial profile of Plesianthropus transvaalensis was carried out by means of Fourier spectral series. Amplitude of contributors was evaluated in comparison with harmonic sets of a modern skull and two "mosaics" obtained by a convenient merging of segments of modern and hominid skull profiles. Patterns are distinctly different in Plesianthropus and modern profile because amplitude of second harmonic is greater than first in the former and viceversa in the latter; moreover while in Plesianthropus first and second harmonic give the same phase, it does not occur for modern skull. Fourier series of "mosaic" obtained by frontal profile of Plesianthropus and facial profile of modern resembles the set of modern skull while "mosaic" of modern frontal profile and hominid facial one resembles closely pattern of Plesianthropus.

Developmental and application of insulin infusion profiles for therapy of type-I diabetics with portable insulin infusion systems.

We studied the insulin requirements of seven insulin-dependent diabetics applying a glucose controlled insulin infusion system. The data were transformed into individually programmed and rectangular profiles. The MAGE, a measure of blood sugar fluctuations, was significantly lower when individually programmed step profiles were used (P less than 0.005) than it was when rectangular profiles were applied: 57.7 +/- 24.8 mg/dl vs 89.0 +/- 42.9 mg/dl. The average of measured blood glucose levels was significantly lower in individually programmed infusion profiles (P less than 0.025). The combination of individually programmed profiles and preprandial insulin bolus significantly reduced the postprandial blood glucose level and increase (P less than 0.001). Our investigations suggest that individually programmed insulin infusion profiles are able to smooth blood glucose fluctuations. When combined with an initial insulin bolus they may lead to a reduced insulin consumption after meals.
