Colon carcinoma metastatic to the thyroid gland.

Metastatic carcinoma to the thyroid gland rarely is encountered in clinical practice; however, autopsy series have shown that it is not a rare occurrence. A case of adenocarcinoma of the colon with metastases to the thyroid is reported. A review of the literature reveals that melanoma, breast, renal, and lung carcinomas are the most frequent tumors to metastasize to the thyroid. Metastatic disease must be considered in the differential diagnosis of cold nodules on radionuclide thyroid scans, particularly in patients with a known primary.

Expression of the endothelin-1 gene in the rat thyroid gland and changes in its peptide and mRNA levels in goiter formation and iodide-induced involution.

Endothelin-1 (ET-1) is a major vasoconstrictor peptide, first found in endothelial cells, and later in many other tissues, including the thyroid gland. We analysed the expression of the ET-1 gene in the rat thyroid gland and changes in ET-1 mRNA and peptide levels in goiter development and involution, two circumstances characterised by vascular changes. Thyroid hyperplasia was induced in adult Wistar rats by feeding a low iodine diet (LID) supplemented with 0.25% thiouracil for 10 days, and LID alone for 2 further days (H.12 group). Involution was induced by injecting 100 micrograms iodide and refeeding a normal diet during 6 h, 12 h, and 24 h (I.6h, I.12h, I.24h groups). Rats fed a normal iodine diet were used as controls. A specific 488 bp cDNA corresponding to the known sequence of pre-pro ET-1 was found by RT-PCR from RNA extracts in all thyroid experimental groups, as well as in lung and kidney which were used as positive controls. RP-HPLC analysis showed that ET-1 immunoreactivity eluted similarly as mature ET-1. During hyperplasia, ET-1 mRNA and peptide levels were increased 3.5- and 5-fold respectively. The relative volume of the vascular bed was more than doubled. During iodide-induced involution, the glandular ET-1 mRNA level remained elevated. The concentration of ET-1 peptide increased and was significantly greater at 12 h involution than in the H.12 group. At this time, the capillary reticulum reverted to individual capillaries and the vascular bed was significantly reduced. These data demonstrate that the ET-1 gene is expressed in the rat thyroid gland and that the ET-1 mRNA and peptide levels are increased during thyroid hyperplasia and remain elevated during a phase of rapid iodide-induced involution. These data suggest that changes in ET-1 production may play a role in control of thyroid gland trophic regulation and vascularity.

[A nodule in the thyroid gland. Preoperative examinations and treatment--an analysis of 990 cases].

INTRODUCTION: It is very difficult to know whether a nodule in the thyroid gland is benign or malignant as judged by a clinical examination, scintigraphy, US, or FNA. MATERIAL AND METHODS: Over a 10-year period a series of 1509 patients were operated on for diseases in the thyroid gland. The indication for surgical intervention in 990 patients was a nodule in the thyroid gland, defined as a solitary nodule or a suspicious nodule in a goitre. RESULTS: The incidence of malignancy in these 990 patients with a nodule was 6.3% with no difference between men and women. Forty-four per cent were follicular adenomas and the rest were colloid nodules. No increased incidence of malignancy in nodules was found with increasing size, whereas the incidence significantly increased with advancing age. Scintigraphy was carried out in almost all the patients (98.9%). Seventy-five per cent had a cold nodule, 9% irregular uptake, and 17% a warm nodule. Only one malignancy, a follicular carcinoma, was a warm nodule. FNA was performed in 955 of the 990 patients (97%). The incidence of follicular neoplasia was 33% in the benign nodule and 70% in the follicular adenoma. DISCUSSION: When malignant cells were found by FNA, it was an almost certain sign of malignancy with very few false positive results, whereas a follicular adenoma or a malignant tumour cannot be excluded by a negative answer. The risk of complications was very low, with an 0.3% incidence of recurrent nerve palsy. If and when the risk can be kept at a reasonably low level, the advantages of thyroid surgery will out-number the disadvantages and early surgery is therefore preferable.

Hyperplasia of the thyroid gland and concurrent musculoskeletal deformities in western Canadian foals: reexamination of a previously described syndrome.

A syndrome of neonatal foals characterized by hyperplasia of the thyroid gland and concurrent musculoskeletal deformities (TH-MSD) has been described in western Canada and may be increasing in incidence. In an attempt to improve recognition and understanding of this syndrome, 2946 records of equine abortuses, stillborns, and dead neonatal foals were examined to determine the laboratory involved, the year and month of submission, the breed and sex of the fetus or foal, the type of perinatal loss, the length of gestation, and whether or not the submission had evidence of a lesion of the thyroid gland, the musculoskeletal system, or other abnormal clinical or postmortem findings. One hundred and fifty-four (5.2%) records indicated the presence of an abnormal thyroid gland. Of these, 79 (2.7%) had additional lesions consistent with the TH-MSD syndrome described in the 1980s, while 75 (2.5%) were without these additional lesions. Comparisons among these two groups and a third group of fetuses and foals without lesions of the thyroids glands are described. The results confirm that the TH-MSD syndrome is a specific and unique disease with no breed or sex predilection. It is argued that there may be an "exposure-related" cause, and based on a review of similar disease syndromes of the horse, it is suggested that an examination of the feed is indicated in outbreaks of the TH-MSD syndrome.

Colonic adenocarcinoma metastatic to the thyroid gland: a case report with immunohistochemical investigation.

Clinically evident metastases of carcinomas to the thyroid gland are rare, particularly from a colorectal primary tumor. We present a case of colonic adenocarcinoma metastatic to the thyroid gland with histopathologic and immunohistochemical findings. A 68-year-old woman with a history of Dukes' stage B colon carcinoma presented a mass in the thyroid gland. The tumor was confirmed to be metastatic adenocarcinoma from the colon. The immunohistochemical findings demonstrated positive staining for cytokeratin 20, low-molecular-weight cytokeratin, villin and carcinoembryonic antigen, but stains were negative for cytokeratin 7 and thyroglobulin.

[Effect of contrast agents on the iodine-absorbing function of the thyroid gland].

Radiographic contrast agents (RCA) cause a profound and long-term blockade of iodine-absorption thyroid function. It was ascertained that the thyroid gland binds both iodine that splits off from a RCA and the agent itself. The changes were found in 125I-iodine binding by the thyroid gland after preadministering the hepatotropic poison CCl4 and the liver function stimulant phenobarbital. Antithyroid action of RCA is accounted for the interaction of the thyroid gland with native molecules of RCA and by their deiodination in the liver.

Distribution of calcitonin-containing cells in the normal neonatal human thyroid gland: a correlation of morphology with peptide content.

C-cells have been mapped in the thyroid glands of 6 human neonates by means of immunoperoxidase localization of calcitonin and tissue calcitonin content as measured by radioimmunoassay. The C-cells were concentrated in a zone in the upper two-thirds of the lateral lobes bilaterally, where they were identified individually and in small groups in both an intrafollicular and parafollicular distribution. In contrast to findings in the adult, C-cells were predominantly intrafollicular in the neonate. The relative numbers of C-cells counted per unit area of thyroid tissue correlated strongly with the calcitonin content of immediately adjacent tissue sections. In areas rich in C-cells, as many as 75 immunoperoxidase-stained cells per low-power field were counted, and the concentration of calcitonin was as high as 540 to 2100 mU/g fresh weight, values that were as great as 10 times those observed in the normal adult thyroid gland. The prominence of the C-cell population and increased tissue calcitonin content in the human neonatal thyroid gland may reflect an as yet undefined physiologic role for calcitonin in the newborn.

Effect of enhanced bromide intake on the concentration ratio I/Br in the rat thyroid gland.

Interaction of bromine with iodine was studied in the rat thyroid gland under the conditions of different bromide intake. Bromine and iodine in the thyroid dry weight were determined by instrumental neutron activation analysis (INAA). It was found that with increased bromide intake the bromine concentration in the thyroid gland increased with simultaneous decrease in the iodine concentration. The change in the I/Br concentration ratio depends on a number of halogen binding positions and on the bromide supply. The I/Br parameter reacts sensitively to the changes of bromide intake already in the region of low bromine concentration levels.

Measurement of thyroid hormone in the rat thyroid gland by radioimmunoassay.

The present paper describes (i) a hydrolysis technique with Pronase and leucine aminopeptidase using one rat thyroid gland, resulting in maximum release of thyroid hormones and minimum deiodination, and (ii) a simple and rapid procedure for thyroid hormone radioimmunoassays in thyroid hydrolysates using commercial kits intended for serum thyroid hormone determinations. The procedure is used to determine T4, T3, and rT3 concentrations and hormonal molar ratios in a thyroid gland from a male Wistar rat. The reliability of the method is also studied.

Primary leiomyosarcoma of the thyroid gland.

Primary leiomyosarcoma of the thyroid gland is extremely rare, and to the best of our knowledge only five well-documented cases have been reported in the world literature. We herein report a 58-year-old female patient with primary leiomyosarcoma of the thyroid who was successfully treated by total thyroidectomy with a modified neck dissection. Immunohistochemically, the tumor cells showed positive reactivity to alpha-smooth muscle actin and vimentin. Radical surgery was thus considered to be essential in the treatment of this rare but rather aggressive malignancy.

Progressive recruitment of follicular cells with graded secretory responsiveness during stimulation of the thyroid gland by thyrotropin.

One of the earliest responses of the thyroid cells to TSH is macropinocytosis with formation of intracellular colloid droplets. We demonstrate here that increasing stimulation with TSH not only elicits a highly individual macropinocytotic response among different follicular cells but that the fraction of TSH-responsive cells is also a function of the TSH dose. After pretreatment with T4, mice and rats were injected ip with bovine TSH and killed 2 h later. The macropinocytotic response to TSH was evaluated on periodic acid-Schiff-stained 3-microns sections of the thyroids in terms of droplet number per 25 follicles and, in addition, by assessing recruitment, i.e. percentage of droplet-containing cells. Both variables increased with increasing TSH stimulation until they reached a plateau at about 9 mU TSH in mice and at about 300 mU TSH in rats: the percentage of droplet-containing cells gradually increased in mice from 2% (no TSH) to 67% (9 mU TSH) and in rats from 11% (no TSH) to 54% (300 mU TSH). Overall pinocytotic response as well as thyrocyte recruitment could be modified by extra- and intrathyroidal factors: for example, pretreatment of the mice with an iodine-deficient diet increased the maximal percentage of droplet containing cells to nearly 90%. Obviously, two separate components of the macropinocytotic response of the thyroid gland to TSH can be distinguished: the first is the gradually increasing fraction of droplet-containing cells, the second is the well known increase of the number of colloid droplets in each TSH-responsive cell with progressive TSH stimulation. Recruitment of thyrocytes with a gradually increasing natural threshold to a hormonal stimulus appears to be a fundamental mechanism in the thyroid gland and possibly in other organs.

Metastatic liposarcoma of the thyroid gland.

Metastatic liposarcoma of the thyroid gland is exceptionally rare. A case of pleomorphic liposarcoma, which manifested as a soft tissue lump in the neck, is described in a 30-year-old woman. Fine needle aspiration cytology permitted prompt diagnosis. The ultrasound and CT appearance of this type of thyroid metastasis is described which has not been previously reported in the literature.

Gross, histomorphological and histochemical changes in thyroid gland of goat with age. II. Occurrence of ultimobranchial follicles.

Occurrence and histomorphology of ultimobranchial follicles in the thyroid glands of 26 goats of six different age groups (ranging from 20 days to above five years) was studied. The occurrence of ultimobranchial follicles was 38.46%. In 15.38% of the cases, it was found with and in 23.08% without any association of the thymic tissue. Its association with parathyroid tissue has also been recorded in one of the goats aged 7 months 11 days. Their size and shape varied greatly and were narrow and wide tubular, epithelial pear like, long follicle with colloid, sheet of stratified squamous cells attached to follicle, bilobed, evaginated and a group of follicles. The latter three forms of follicles were due to the branching of main stem UB follicle, present at different levels. They were located in perivascular connective tissue in the deeper zone of the thyroid gland but their presence in periphery of thyroid gland within the capsule was not uncommon. The epithelial lining of these follicles was highly variable from simple squamous to columnar with or without cilia which may be stratified. Light cells were seen among epithelial lining. The lumen of the follicles was filled with nucleated, non-nucleated and colloid debris which was PAS-positive. In older animal colloid contained acid mucopolysaccharides.

Inhibition by phenytoin of in vitro secretion of calcitonin from rat thyroid glands and cultured rat C cells.

Baby rat thyroid glands and cultured rat medullary carcinoma C cells were incubated acutely with phenytoin (38-100 microM), and the calcitonin (CT) secreted into the serum-free medium was measured by radioimmunoassay (RIA). Phenytoin did not alter CT release from glands or C cells incubated in 1 mM Ca, but, when Ca was raised to 1.75 or 2.5 mM, a marked inhibitory effect of phenytoin was apparent. The inhibitory effect could be negated by including 10 microM BAY-K-8644 in the medium. Inhibitory effects on CT release also were obtained with 100 microM trifluoperazine or 100 microM nitrendipine, and these inhibitory effects also were counteracted by 10 microM BAY-K-8644. The results show that clinically relevant amounts of phenytoin can inhibit CT release, perhaps by interfering with C-cell Ca channels or by inhibiting calmodulin-dependent processes.

[Dipeptidyl peptidase IV (DPP IV, CD 26): a tumor marker in cytologic and histopathologic diagnosis of lesions of the thyroid gland].

BACKGROUND: Morphological diagnostics of thyroid gland tumours faces certain differential diagnostic problems. Extensive histological examination of the entire tumour is required for the final diagnosis of follicular and oncocytic tumours. Thus, assessment of reliable definitive cytological and/or intraoperative histological diagnosis is not possible. No marker of malignancy has been so far generally accepted in the thyroid tumour diagnosis. The aim of the study was to evaluate membrane protease dipeptidyl peptidase IV (DPP IV) in the differential diagnosis of thyroid tumours. METHODS AND RESULTS: DPP IV was assessed cytochemically in 254 smears, histochemically in 314 cryostat sections, and immunohistochemically in 309 paraffin-embedded sections obtained from the group of 336 patients. There were 283 females and 53 males with the mean age of 48 years (range 15-80 years) in this series. Sensitivity of cytochemical detection was 71%, specificity was 96%, and diagnostic accuracy was 93% using the 50% threshold. Histochemically, sensitivity was 71%, specificity was 93%, and diagnostic accuracy was 90% using the 5% threshold. Using the immunohistochemical assessment, sensitivity was 68%, specificity was 94%, and diagnostic accuracy was 91% using the 5% threshold. CONCLUSIONS: According to our results, DPP IV can be used as a marker of malignancy in well-differentiated carcinomas of follicular cell origin, namely in papillary carcinoma. However, it is less reliable in follicular and oncocytic carcinomas.

[Nitrate contaminated drinking water from private wells has an impact on the size and functional state of the thyroid gland in schoolchildren].

UNLABELLED: The authors compared the size of the thyroid gland, assessed by ultrasonographic examination of its volume and the functional state of the thyroid by examination of the serum level of TSH and anti-TPO antibodies in 492 pupils aged 10 and 13 years in two close agricultural areas. The areas differed above all as to the nitrate content of drinking water. In communities where the source of drinking water were private wells (nitrate area), as much as 68.1% of the water samples had a nitrate content > 50 mg/l and 46.4% samples > 100 mg/l. In communities where the source of drinking water was a water main with a known source (control area) the water samples did not exceed 50 mg/l (73.7% > 15 mg/l and 26.3% > 50 mg/l. RESULTS: Pupils from the nitrate area had a larger thyroid gland similarly as older pupils from the control area. A volume above 7 ml/m2 in the nitrate area was recorded in the group of 10-year olds in 25/99 (27.7%) and in the group of 13-year-old ones in 33/154 (21.4%), while in the control area it was in the 10-year-old ones 11/92 (11.9%) and in the 13-year-old ones 16/156 (10.2%, P < or = 0.01). In the nitrate area TSH > 5 mIU/l was recorded in 5.6% and higher anti-TPO antibodies in 2.6% pupils. In the control area the corresponding figure was 1.1% pupils, P < or = 0.01.
