[Lung neoplasms. Cytological diagnosis based on puncture biopsy specimens].

Twelve-year experience gained in cytological diagnosis of lung tumors by examining puncture biopsy specimens in 2536 patients is analyzed. The results indicate that before cytological verification of the diagnosis, patients with squamous-cell and glandular lung cancers were examined at various institutions for up to 6 months and patients with nondifferentiated cancer for up to 3 months, the majority (57%) patients being of socially active age (45-60 years). In 1793 (70.7%) patients the cytological method was the only possibility of morphological interpretation of the pathological process in the lung before treatment. Analysis of informative puncture biopsy specimens helped identify the process in 2238 (92.4%) patients. Cyto-histological correlations showed high diagnostic sensitivity (98.7%), efficiency (97.3%), and specificity (86.0%) of examining puncture biopsy specimens by the cytological method.

[Analysis of 22 cases of CT-guided puncture biopsy for chest tumors].

OBJECTIVE: To evaluate the application of CT-guided puncture biopsy for diagnoses of tumorschest. METHODS: Twenty-two patients with thoracic tumors that were not pathologically identified received CT-guided puncture biopsy for histological and cytological examination. RESULTS: All biopsy procedures were successfully completed and 18 cases were identified as having malignant tumors by histological examinations, while 5 identified by cytological examinations, and definite pathological diagnoses were established in 19 cases. Pneumatothorax occurred in 2 cases and bloody sputum developed in 1 case. CONCLUSION: CT-guided puncture biopsy may provide better biopsy samples and pathological supplemented by cytological examinations improve the accuracy of diagnoses.

Malignant mixed müllerian tumor of the ovary and false negative punctures.

Malignant mixed müllerian tumour (MMMT) of the ovary is a rare and aggressive tumour with a poor prognosis. We present a case of a 57-year-old woman with a large pelvic mass, omental cake, ascites and pleural effusions, clinically highly suspect of an ovarian neoplasm. Paracentesis and ultrasound-guided biopsy of the ovary were negative for malignant disease. Therefore a CT-guided true cut biopsy was performed. The latter gave a histopathologic diagnosis of an endometrioid adenocarcinoma of the ovary. However after cytoreductive surgery anatomopathologic examination revealed a malignant mixed müllerian tumour of the ovary with heterologous differentiation. Apparently only one of the two components was found in the puncture. Adjuvant chemotherapy, active against the sarcomatous and the carcinomatous component, was given. At present the patient is well and disease free 35 months after the initial diagnosis. Cytological examination of ascites may be negative in the presence of malignant disease. If a tumour consists of two components, puncture can miss one, which may lead to undertreatment. Punctures should be discouraged as a diagnostic tool in patients in whom an ovarian malignancy is suspected.

Laparoscopic appendectomy using a single umbilical puncture (minilaparoscopy).

The surgical outcome of the first 25 patients in whom the Pelosi single-puncture laparoscopic appendectomy technique was employed demonstrates the new approach as a safe, inexpensive and effective alternative to the currently used multiple-puncture method. The results suggest single-puncture (minilaparoscopy) operative endoscopy as the ultimate goal in the progression of minimally invasive surgery.

[Puncture aspiration biopsy under ultrasonic control in the diagnosis of breast neoplasms].

The authors studied the results of sonography of the breast and zones of regional lymph drainage in 15,973 patients. In 154 of them puncture aspiration biopsy was conducted under ultrasonic control. Breast carcinoma was found in 23 patients, in 2 of them the tumor was not palpable. The examination was performed on the Aloka-SSD-630 apparatus. The sonographic picture was that of a structure with diminished echogenic properties, irregular notched contours, and non-uniform echo signals from the internal structures. Echography with puncture biopsy made it also possible to reveal the presence of nonpalpable axillary metastatic lymph nodes and to establish the precise differential diagnosis of benign and malignant intraductal tumors. The method of puncture aspiration biopsy under control of ultrasonic examination is highly effective in the differential diagnosis of breast carcinoma.

Plantar puncture wounds in children: analysis of 80 hospitalized patients and late sequelae.

BACKGROUND: Puncture wounds in the feet of children present a clinical dilemma. OBJECTIVES: To evaluate our approach, we reviewed the charts and all available images of 80 children admitted to our institution because of plantar punctures from 1988 to 1999. METHODS: The charts of 80 children were reviewed retrospectively. RESULTS: Three groups of patients were found: 59 with superficial cellulitis, 11 with retained foreign bodies, and 10 with osteoinyelitis and/or septic arthritis. There was a significant presentation delay in patients from the second and third groups. Most common organisms were Staphylococcus aureus or Group A Streptococcus. Of the 80 children, 34 were treated surgically and 46 were treated with antibiotic therapy alone. All patients with osteomyelitis and septic arthritis were re-examined; at follow-up, all but one were asymptomatic apart from residual radiologic sequelae in four. CONCLUSIONS: Patients with an established infection 24-36 hours after a plantar puncture should be admitted to hospital for parenteral antibiotic therapy. Delayed presentation is a significant marker for deep-seated infection. Further infection or relapse after initial improvement suggests the presence of osteomyelitis or a retained foreign body. A bone scan is advisable in all patients with suspected osteomyelitis: a positive bone scan necessitates aggressive early debridement combined with appropriate antibiotics; while negative bone scan, X-ray and exploration suggest that the infection is due to a foreign body, which can be detected by computed tomography.

[Treatment of open angle glaucoma with deep sclerectomy combined with laser trabecular puncture].

OBJECTIVE: To study the effect of deep sclerectomy combined with laser trabecular puncture for the treatment of open angle glaucoma (OAG). METHODS: Deep sclerectomy was performed in 32 eyes of 32 patients with moderate to advanced stages of OAG. After the exposure of external wall of Schlemm's canal and the remaining trabecula, the intraocular optic fiber of diode laser photocoagulator was directed to the trabecula-limbus near the anterior edge of the scleral spur. Two or three punctures, approximately 0.5 mm in diameter, were performed with laser beam under direct visualization until aqueous humor was extravasated slowly. The scleral flap was closed with one or two sutures. Postoperative observation included intraocular pressure (IOP), intraocular tissue reaction, the appearance of filtering bleb, gonioscopy and ultrasound biomicroscopy. The follow-up period was 4 to 24 months (7.94 +/- 6.20) months. RESULTS: The anterior chamber was recovered shortly after the operation and the visual acuity remained the same in all eyes. At the end of follow-up period, the postoperative IOP (14.86 +/- 4.15) mm Hg was lower than the preoperative IOP (29.68 +/- 5.76) mm Hg, the difference was statistically significant (t = 7.415, P < 0.001). Diffuse and obvious filtering blebs were found in all eyes after the operation, including 27 functional blebs and 5 blebs that disappeared 2 to 3 months later. Iris was burned slightly in 2 cases, pigment was released and appeared in the anterior chamber temporarily. No laser injury occurred in the cornea and the lens. High IOP was found in 2 cases after long-term follow-up and could be controlled by antiglaucoma medication. CONCLUSIONS: Deep sclerectomy with laser trabecular puncture can effectively reduce the IOP, without the occurrence of serious complications that commonly seen after the trabeculectomy. It is an effective surgical method for the treatment of OAG.

PET imaging of brain function while puncturing the acupoint ST36.

OBJECTIVE: To explore the experimental method of obtaining position emission tonogiaphy (PET) imaging evidence of changes in cerebral function by puncturing the Stomach 36 (ST36, Zusanli) acupoint. METHODS: Data on changes of cerebral glycometabolism were obtained from six healthy male volunteers with positron emission tomography. Visual experimental evidence, as well as statistical parametric mapping (SPM), was gathered while puncturing the ST36 (Zusanli, right leg) acupoint. RESULTS: There was increased glycometabolism in the hypothalamus, head of the caudate nucleus, temporal lobe, the sinistral cerebellum, postcentral gyrus, and brain stem while the acupoint ST36 was being punctured. CONCLUSIONS: Acupuncture on ST36 can lead to increase in glycometabolism in the vegetative nerve centers, which is correlated with gastric function. Visual experimental evidence of ST36 acupuncturing on functional gastrointestinal disorder was obtained in our study.

[Value of myotomy of the lower constrictor in surgery of tracheo-esophageal puncture].

The importance of the inferior myotomy for speech restoration in total laryngectomized patients following not only primary techniques (speech restoration by tracheo-esophageal puncture during total laryngectomy) but also secondary punctures, is well documented. In this paper, the results obtained by manometric studies applied to 25 patients divided into two groups are shown (the first group) (15 patients) following primary techniques, and the second one (10 patients) following secondary punctures. We observe a sphincter pressure decreasing after secondary myotomy in those patients which previously presented dysphagia, prostheses breaking or intermittent speech, therefore considering the inferior constrictor myotomy as of great value in speech rehabilitation.

[Transperineal puncture drug injection in the treatment of chronic prostatitis].

OBJECTIVE: To investigate the effect of transperineal puncture drug injection in the treatment of chronic prostatitis. METHODS: Forty cases of chronic prostatitis underwent transperineal puncture drug injection. Bacterial culture and antimicrobial agent sensitivity tests were conducted on the prostatic fluid of the patients. A sensitive antimicrobial agent was selected, mixed with lidocaine and dexamethasone, and injected transperineally into the prostate at intervals of 7 days and 4 times as one course. After one month, the effect was evaluated and the patients were followed up for 6 months. RESULTS: Among the 40 cases, the cure rate was 52.5% (21/ 40), the alleviation rate 40% (16/40), the nullify rate 7.5% (3/40) and the total effectivity rate 92.5%. No relapse was found in the 28 followed-up cases. CONCLUSION: Transperineal puncture drug injection is a safe and effective way in the treatment of chronic prostatitis.

[Cranial subdural hematoma after inadvertent dural puncture at epidural anesthesia].

Intracranial subdural haematoma has been reported to be an exceptionally rare complication of accidental dural puncture. An accidental lumbar dural puncture occurred in a 36-yr-old male undergoing orthopedic knee surgery. On the morning after the operation, the patient complained of severe occipital headache, although this was relieved with loxoprofen and rest. This was assumed to be a postdural puncture headache (PDPH) because it had a postural component (it was worse on sitting up). On the third day after the operation, the patient developed a severe diffuse headache together with nausea, which did not subside with analgesia and bed rest. Magnetic resonance imaging of the head showed a small acute subdural hematoma in the bilateral temporooccipital region with no mass effect. The patient was conscious and oriented. There was no focal neurological deficit. The patient was managed conservatively with bed rest and intravenous fluids. His condition improved without surgical decompression and was discharged on the 40 th day after the operation. Severe and prolonged PDPH shoud be considered as a warning sign of an intracranial complication.

[Observation on therapeutic effect of blood-letting puncture combined with electroacupuncture on muscular pulling injury].

OBJECTIVE: To observe the role of blood-letting puncture in the treatment of muscular injury. Methods One hundred and six cases with pulling injury of hamstring muscles were randomly divided into a treatment group and a control group. The treatment group (n=55) were treated by electroacupuncture combined with blood-letting puncture at Ashi points on the injury part. The control group (n=51) were treated with simple electroacupuncture. RESULTS: Both the 2 groups were effective. There was no significant difference between the 2 groups in the therapeutic effect for the patient with a duration of illness of 1-14 days; but for the patient with the duration of illness of over 14 days, the treatment group was better than the control group (P < 0.05). For the patient with simple injury of the belly of muscle, the treatment group was better than the control group (P < 0.05). CONCLUSION: Blood-letting puncture combined with electroacupuncture in the therapeutic effect on muscular pulling injury is superior to the control group, with treatment advantages for the patient with longer duration of illness and simple injury of belly of muscle.

[Pleural puncture biopsy--a diagnostic method in tuberculous serofibrinous pleurisy in children].

The biopsy of the parietal pleura with a special needle has become an essential diagnosis method in the adult patients with pleural disease of unknown etiology, and might become a complementary paraclinical method for children also. The certitude diagnosis in tuberculous serous fibrinous pleurisy of the child is laid on evidencing Koch's bacillus at the direct examination or in culture, and on fragments of pleural biopsy puncture, in evidencing the lesions. The study is based on the results obtained in the pleural biopsy puncture made on 6 children. The histologic examination of the pleural fragment showed, in 4 cases of 6, the presence of tuberculous lymphoepithelioid nodules, with central caseous necrosis thus granting certitude to the diagnosis. The pleural biopsy puncture permits an early and certain diagnosis in more than 2/3 of the pleurisies of tuberculous etiology.

[Accidental dural puncture during epidural injection of corticosteroids: a different approach?].

We report 6 cases diagnosed with accidental dural puncture after epidural injection of corticosteroids for low back pain. All the patients reported postdural puncture headache during their stay in the postanesthetic recovery unit. For 3 patients, pain resolved with treatment given in the recovery unit. Two other patients also required mild analgesics for 1 week. In the last patient, a blood patch was used to treat incapacitating headache 22 days after the epidural procedure and mild analgesics were needed for 4 more weeks. It is important to establish a protocol for treating postdural puncture headache in pain clinics to facilitate decision making. Good physician-patient communication is necessary to avoid refusals for permission for other epidural techniques and to facilitate management of symptoms.

Aortic stenosis catheterization revisited: a long sheath single-puncture technique.

OBJECTIVES: To evaluate the accuracy of a new long sheath single-puncture technique in obtaining accurate transvalvular gradients in aortic stenosis. BACKGROUND: Despite advances in echo Doppler, the evaluation of aortic stenosis continues to be a common procedure in the cardiac catheterization laboratory. Experts agree that simultaneous evaluation of the proximal aortic and left ventricular pressures yields the most accurate data; however, this is difficult to achieve unless two arterial punctures are performed. METHODS: We postulated that using a 4 Fr pigtail catheter inside a 55-cm long 6 Fr sheath would provide accurate simultaneous pressure data, yet avoid the complications of two arterial punctures. We performed this technique in 13 male patients, and placed a second arterial catheter in the aortic root as a control aortic pressure. We then performed this technique in 55 other male patients without placing an additional control arterial catheter. RESULTS: In the test population, correlation of aortic valve areas and transvalvular gradients was excellent. In the larger population, adequate hemodynamic data was obtained in 52 patients, with no difficulty engaging coronary arteries or grafts via the long sheath, and with an acceptable major complication rate of 1.5%. CONCLUSIONS: Using a 4 Fr pigtail catheter with a 55-cm long 6 Fr sheath is a safe, efficient way to obtain excellent hemodynamic data in an aortic stenosis catheterization procedure.

The single puncture approach for advanced pelviscopy surgery.

1. The single puncture laparoscopy technique uses the umbilicus as the sole puncture site for the performance of surgical procedures. 2. The single puncture technique is simple, reduces complications, allows for faster recuperation and decreased operating time, is more cost-effective, and offers improved cosmetic results. 3. Each member of the surgical team must have all of the knowledge and skill related to each respective role. Major complications can only be avoided by being especially attentive to detail.

[Laser puncture in the combined treatment of a weak degree of myopia in schoolchildren].

To increase effectiveness of treatment, the influence of laser puncture on some functional indices of myopic eyes has been studied. Comparative analysis of results after laser puncture used as an independent method in 45 children (85 eyes)--group I, and in a complex accommodation-convergence system, of the eye in 66 children (130 eyes)--group II, has shown that laser puncture has a positive influence on visual acuity and accommodation-convergence system. As compared with group I, in group II normalization of functional indices (reserves of accommodation, convergence and divergence) begins earlier and stability of the results obtained is higher.

Direct puncture of the proximally occluded internal carotid artery for treatment of carotid cavernous fistulas.

Three patients with symptomatic carotid cavernous fistulas (CCFs) characterized by complete occlusion of the proximal internal carotid artery were treated by percutaneous puncture and embolization. Two patients had CCFs associated with traumatic dissections of the internal carotid artery and were treated initially with trapping procedures. Both patients had persistent symptoms related to the CCF and underwent additional surgical procedures (ophthalmic artery ligation and intraoperative embolization) without improvement. The third patient had traumatic occlusion of the internal carotid artery. After direct percutaneous puncture of the carotid artery above the occlusion, a catheter was advanced into the petrous internal carotid artery. Balloons (one case) or coil emboli (two cases) were placed into the cavernous sinus to produce CCF closure. There were no complications from this procedure. Direct puncture of the carotid artery is an alternative treatment for patients lacking safe access for CCF embolization.
