Evaluation of salivary and vaginal electrical resistance for determination of the time of ovulation.

The usefulness of changes in salivary and vaginal electrical resistance (SER and VER) measurements for timing ovulation was evaluated in 15 cycles. A peak in mean SER was observed seven to eight days before the LH peak (five to nine days before the thermal nadir). The nadir of mean VER coincided with the day of maximum LH and was significantly correlated with the day of the thermal nadir. Use of SER peak and the rise of VER from its nadir in a protocol for timing insemination yielded correct timing in 93.3% (14/15) of cycles. These findings substantiate earlier reports indicating that there is a useful relationship between SER and VER trends and ovulation, which may be employed for accurate scheduling of inseminations and other procedures.

The resumption of ovulation and menstruation in a well-nourished population of women breastfeeding for an extended period of time.

We have studied a large group of Australian women breastfeeding for an extended period of time to determine the duration of lactational anovulation (n = 89) and amenorrhea (n = 101). Salivary progesterone assays were used to determine ovulation. These women had a mean of 322 days of anovulation and 289 days amenorrhea. Less than 20% had ovulated and less than 25% had menstruated by 6 months postpartum. The latest ovulation was at 750 days and the latest menstruation at 698 days. There was no significant correlation between any measure of maternal nutritional status and the duration of anovulation or amenorrhea. Neither the time of first supplement introduction to the baby nor the amount of supplement given was an accurate predictor of the return of ovulation or menstruation. However, our results clearly show that lactational amenorrhea can provide good protection against pregnancy in the 1st 6 months postpartum, even in well-nourished women who are giving their babies supplemental feeds.

Efficacy of three variations of periodic abstinence for family planning in Indonesia.

A prospective study to determine the efficacy of three alternative guidelines for the practice of periodic abstinence (PA) for family planning was conducted in Indonesia. The three methods studied were the Billings ovulation method, the Dorairaj modified mucus method, and a local version of the mucus method. For each method, the study encompassed a three-month learning phase and an additional 12-month effectiveness phase. Data from a total of 850 acceptors showed that, despite some variations in the sociodemographic characteristics of the acceptors, the Billings ovulation method had the lowest (10.4 per 100 women) and the local mucus method had the highest (26.5 per 100 women) overall life-table discontinuation rates in the effectiveness phase. One-year life-table unplanned pregnancy rates ranged from 2.5 per 100 women for the Billings method to a high of 11.5 per 100 women for the local method acceptors. Unplanned pregnancy was the main reason for termination.

[Qualitative determination of U-pregnanediol-3 alpha-glucuronide in 5 minutes. A clinical trial of a new matrix cushion test].

We have tested an easy and rapid "matrix cushion test" for qualitative determination of U-pregnandiol-3 alpha-glucuronide (U-PGD). The specificity of the test is high, the sensitivity is 4.0 microM, the inter-analysis variation is low as in 96% of the double determinations, agreement was present between the first and second analyses. A positive result on the 21st of the menstrual cycle indicates that ovulation has occurred while this cannot be exluded by a negative or doubtful result. If demonstration of a functioning corpus luteum is also required, the urine should be diluted 1:2 with tap water.

Contraceptive failure of the ovulation method of periodic abstinence.

Previously published estimates of probabilities of method and user failure for all contraceptive methods suffer from a serious methodological error and are biased downward, with the extent of bias unknown. Data from a World Health Organization clinical trial of the ovulation method of periodic abstinence were used to provide the first correctly calculated measures of method and user efficacy and to determine the characteristics that distinguish women who consciously take risks from those who do not. Probabilities of pregnancy during the first year are 3.1 percent during perfect use (method failure) and 86.4 percent during imperfect use (user failure). Thus, if used perfectly, the ovulation method is very effective. However, it is extremely unforgiving of imperfect use. Because perfect compliance is difficult for many couples who desire intercourse when it is forbidden by ovulation method rules, and because the risk of pregnancy during imperfect use is so great, the ovulation method cannot be considered an ideal contraceptive method for the typical couple, who are likely to be less compliant than couples who volunteer for a clinical trial. The probability of an accidental pregnancy is greatest when any of the three most serious rules--no intercourse during mucus days, within three days after the day of peak fecundity or during times of stress--are broken. Those who have a poor attitude toward the rules are more likely to take risks, including serious risks. Those who get away with taking a risk (i.e., do not get pregnant) are very likely to take risks again. Because breaking the most serious rules entails a 28 percent risk of pregnancy per cycle, those likely to take risks should be counseled about the probable consequences.

The refractometer index as a correction factor for urinary estradiol in rhesus females.

Refractometer indexes were standardized and found to be highly correlated (r = 0.831) to actual creatinine levels. Urinary estradiol corrected by creatinine levels and refractometer indexes were found to be highly correlated (r = 0.857). Predicted ovulation was the same day in 86% of the ovulatory cycles predicted by creatinine and refractometer corrected estradiol levels. Refractometer indexes may be used in place of creatinine levels to correct for urine concentration fluctuations when predicting ovulation in the rhesus female.

The reliability of menses to indicate the return of ovulation in breastfeeding women in Manila, The Philippines.

Demographic studies generally use menses to indicate the return of fertility during breastfeeding. In a prospective study of 40 breastfeeding women in Manila, the resumption of ovarian activity was monitored by urinary hormone assays, and menstrual status was determined by weekly interview. For the women who menstruated before six months postpartum, first menses was not a good indicator of ovulation because there was a high proportion of anovular menses (67 percent), and the lag between anovular first menses and subsequent ovulation was 15.7 (+/- 4.4) weeks. After six months postpartum the proportion of anovular first menses declined to 22 percent, and the lag between anovular first menses and ovulation was 7.3 (+/- 4.6) weeks. If all anovular and ovulatory menstrual episodes are considered, the mean interval between first observed menses and first ovulation was 8.4 weeks during the first six months postpartum and only 0.1 week after six months. Thus, in breastfeeding women, menses is an inaccurate proxy measure for the timing of fertility return before six months postpartum, but a good indicator of the resumption of ovulation after six months.

[Study on the accuracy of uterine endometrial dating].

In order to examine the accuracy of endometrial dating, endometrial dating from 52 infertile women with normal luteal function was investigated. Dating from the histological findings was evaluated from three time points, the onset of the next menstrual period (NMP), the last day of the low phase of basal body temperature (BBT) and the urinary LH peak determined with the "Organon LH Color" kit (LH). Beside microscopic observation, morphometric analysis of endometrial findings was done by means of a high speed color image analysis system "Olympus SP-500". On the day of LH Color (+2), the serum LH level showed a maximum of 35.5 +/- 12.6 mIU/ml (Mean +/- S.D.) (n = 27). The correlation coefficients for histological dating and chronological dating based on NMP, BBT and LH were 0.685, 0.728 and 0.879 in gland and 0.620, 0.737 and 0.833 in stroma, respectively. The multiple correlation coefficient for histological dating evaluated by morphometric analysis and chronological dating based on LH was 0.959. So we conclude that endometrial dating evaluated by morphometric analysis seems to be the most objective and useful.

A biphasic basal body temperature record during pregnancy.

A case of biphasic temperature record is reported during the course of an ectopic pregnancy. Possible implications of this phenomenon may improve our understanding of the role of ovum transmigration and the endocrinological interaction of pregnancy with hormonal control of ovulation.

Experience with ultrasound.

Treatment with human menopausal gonadotropins is safe and effective. However, in order to maintain those properties, careful monitoring of the ovarian response to the exogenous gonadotropins is mandatory. Safety is afforded by the use of rapid assays of the estrogen level in serum or urine. The use of real-time ultrasonography to monitor follicular growth has improved the efficacy. The administration of human chorionic gonadotropin (hCG) can be delayed until follicular maturation has been achieved; hCG can be withheld if too many mature follicles are present. Monitoring therapy with both modalities as well as with clinical methods ensure a high rate of successful gestations.

[Peri-ovulatory hormone determination in females with sterility of unknown origin].

In 86 patients with Clomiphen-therapy a daily LH-, E2-monitoring and a sonographic follicle-measurement was performed. In 27 patients a normal endocrine profile, in 12 women a double estradiol arise must be registered. A missing LH peak was noted in 9 cases. In 11 women a double LH peak occurred. 7 patients showed a LH arise two hours after the estradiol peak. In 15 women the ultrasound examination showed an insufficient follicular growth. In 5 cases the typical luf-syndrome was found. Though this variations in cycle-monitoring could be noted their effects in infertility is not really explained.

[Echographic diagnosis of ovulation in healthy women].

Dynamic echographic changes in measures of the uterus, uterine mucosa, ovaries and dominant follicle during normal ovulatory menstrual cycle were established in 16 multiparas and 17 nulliparas. All measures undergo characteristic changes as they increase during follicular phase of the cycle and reach their maximum paraovulatorily. For establishment of ovulation the changes in anterior-posterior size of the uterus, uterine mucosa, diameter of the dominant follicle and qualitative changes, occurring in it, are of the greatest significance. Mathematical models are made, which allow a comparison and prognosis of the changes in the internal genital organs of a woman in respect to the ovulation.
