Primary carcinoma of the fallopian tube: a 20-year literature review.

Case series and important case reports of primary fallopian tube carcinoma published in the English literature from 1973 to 1992 are reviewed. Meta-analysis revealed a mean age of presentation of 56.7 years, with a nulliparity rate of 27.5 per cent and a mean parity of 1.7. Abnormal vaginal bleeding and discharge, and pelvic pain are the most common presenting symptoms. Despite developments in cytology, transvaginal ultrasound, and tumor markers, most cases remain undiagnosed preoperatively. In addition to direct intraperitoneal spread, early lymphatic spread is now recognized as a frequent cause of treatment failure. FIGO has recently released a staging classification for fallopian tube malignancy incorporating detailed surgical staging, which should allow uniformity and comparison between future series. Treatment regimes are empirically based on therapy for epithelial ovarian malignancy, but none have been subjected to controlled trials. Recent case series support extensive debulking surgery and adjuvant platinum-based combination chemotherapy for optimizing prognosis, although results from radiotherapy and hormonal therapy are largely disappointing. Promise is expressed in tumor markers and "second-look" laparotomy for monitoring disease response and planning management.

[Pulmonary damage in the rabbit caused by porcine pancreatic elastase and leukocyte lysate: unusual microscopic aspects].

Intratracheal injection of rabbit whole leukocyte homogenate induced emphysema-like lesions in rabbits. The lesions were produced only by preparations having elastinolytic activity. The pathological aspects appeared similar to those induced by the administration of porcine pancreatic elastase. The pulmonary changes, resembling several of the anatomic appearances of panacinar human emphysema, may be a suitable experimental model for studying histogenesis of panacinar human emphysema. Numerous abnormal fenestrations were present in air spaces walls and were a constant feature in opposition to that has been reported in elastase-treated hamsters. For the presence of a prominent dilatation of the periarterial lymphatic network, this experimental model might be used also for studying the ultrastructural features of pulmonary lymphatic vessels.

Tumors of the uterine isthmus: clinicopathologic features and immunohistochemical characterization of p53 expression and hormone receptors.

Five (3%) of 161 endometrial cancers treated surgically between 1988 and 1992 were classified as primary isthmic tumors, and their clinicopathologic features, p53 expression, and hormone receptor status were evaluated. Three were endometrioid adenocarcinomas with squamous differentiation, one was a mixed serous and clear cell carcinoma, and one was a malignant müllerian mixed tumor; all five were high grade, invaded the entire thickness of the myometrium, and exhibited lymphatic space invasion. Four of the five patients had extrauterine metastases identified at the time of hysterectomy. All five patients died due to progressive disease with the survival time ranging from 1 to 23 months. Abnormal p53 gene expression was identified immunohistochemically in three of the five isthmic tumors. Weak positivity for estrogen and progesterone receptors was demonstrated in one case, with the remaining four being negative. Tumors arising in and confined to the uterine isthmus are unusual and, in our series, were uniformly aggressive with an unfavorable prognosis. The histopathologic features and biologic behavior of the isthmic tumors appeared similar to those of other high-grade endometrial cancers arising in the uterine corpus.

Inhibitory effect of coffee on hepatoma proliferation and invasion in culture and on tumor growth, metastasis and abnormal lipoprotein profiles in hepatoma-bearing rats.

We have already reported that instant coffee powder (ICP) and ICP-loaded rat sera could suppress proliferation and invasion of rat ascites hepatoma cell line of AH109A in vitro. In this report, we examined the mechanisms for suppression of tumor cell proliferation and invasion by ICP, and the effect of ICP on in vivo tumor growth, metastasis and abnormal lipoprotein profiles in hepatoma-bearing rats. ICP, when directly added to the culture media, induced cell cycle arrest (elongation of S phase) at a lower concentration (0.3 mg/mL) and apoptosis at a higher concentration (0.6-1.2 mg/mL). ICP and ICP-loaded rat sera showed reactive oxygen species (ROS)-scavenging property and canceled the enhancement of invasive activity of hepatoma cells induced by ROS in vitro. These results suggest that ICP suppresses the proliferation by inducing cell cycle arrest and apoptosis, and the invasion by scavenging ROS and that ICP could retain these properties after their gastrointestinal absorption. The hepatoma-bearing rats were fed with a 20% casein diet (20C) or 20C supplemented with 0.1%, ICP for 14 d. Dietary ICP significantly reduced solid tumor growth and tended to reduce hepatoma metastases to lung and lymphatic nodes, suggesting that ICP could suppress tumor cell proliferation and invasion in vivo. In addition, dietary ICP significantly increased serum high-density lipoprotein (HDL)-cholesterol and tended to reduce very low-density and low-density lipoprotein (VLDL+LDL)-cholesterol, resulting in amelioration of abnormal lipoprotein profiles occurred in hepatoma-bearing rats. In conclusion, ICP has the ability to induce cell cycle arrest and apoptosis in hepatoma cells and to suppress tumor cell invasion by reducing oxidative stresses in vitro, and it could also exhibit these effects in vivo, leading to the inhibition of tumor growth and metastases.

Richter's syndrome with identification of marker chromosomes.

A case is presented of a man with Richter's syndrome with diffuse histiocytic lymphoma following a ten-year history of untreated chronic lymphatic leukemia. He did not respond to therapy. The lymphoma cells had 61 chromosomes with aneuploidy of 13 chromosomes and 11 structurally altered chromosomes. Only chromosomes 8, 9, 12, 14, and 18 were diploid and without abnormality. Significantly, part of chromsome 1q was duplicated as in the partial trisomy of 1q reported to characterize a number of hematologic neoplasms. A large marker chromosome with subterminal centromere was a tandem duplication of chromosome 4q.

Campomelia, cervical lymphocele, polycystic dysplasia, short gut, polysplenia.

An aborted fetus, the offspring of consanguineous parents, had the unusual combination of campomelia, cervical lymphocele, polycystic kidneys, pancreas, and liver, short gut, and polysplenia. Births of earlier similarly affected fetuses suggest an autosomal recessive inheritance. Skeletal, lymphatic, and renal lesions were seen at 26 weeks' gestation by ultrasonography, but not at 16 weeks.

Papillary carcinoma of the thyroid after postpartum thyroiditis.

Postpartum thyroiditis is a benign, often self-limited cause of thyroid dysfunction. This report describes the first case of papillary carcinoma arising in a woman with postpartum thyroid disease. Both clinicians and pathologists must keep a high index of suspicion in postpartum patients with persistent thyroid abnormalities, even when a previous biopsy may have disclosed benign disease.

[Chronic pseudomalignant immuneproliferation (Canale-Smith syndrome) (author's transl)].

A case of chronic lymphadenopathy and splenomegaly existing over a period of 11 years, with a variety of acquired deficiencies of immunity and autoimmune phenomena, is described. It is almost identical with the 5 children reported in 1967 by Canale and Smith as a new benign syndrome "chronic lymphadenopathy simulating malignant lymphoma". The relationship to similar disorders with lymphadenopathy and immunological abnormalities is discussed.

Fluorescence in situ hybridization as a diagnostic tool in malignant lymphomas.

Primary and secondary chromosomal abnormalities play an important role in the characterization of biological, pathological, and clinical subgroups of malignant lymphomas. The introduction of fluorescence in situ hybridization (FISH) and the combination of immunophenotyping plus FISH to the diagnosis of lymphatic neoplasms allows the fast and sensitive detection of specific chromosomal changes and provides new insights into the genetic basis of lymphomagenesis. This article reviews the possibilities and limitations of molecular cytogenetic techniques in comparison to cytogenetic and molecular genetic methods and discusses their clinicopathological impact for non-Hodgkin's lymphoma and Hodgkin's disease.

The occurrence of adenoidal signs and symptoms in normal children.

A characteristic clinical picture has been attributed to enlarged adenoids. In order to investigate this concept the occurrence of certain signs and symptoms was compared in a series of children selected for adenoidectomy and in a series of normal children. Nasal obstruction, snoring and speech defect occurred more frequently in children having adenoidectomy. The symptoms of rhinorrhoea, cough and headache and the signs of mouth breathing and abnormality on anterior rhinoscopy occurred as frequently in normal children as in children having adenoidectomy.

An unusual case of coccidioidomycosis.

Dissemination of coccidioidomycosis is rare. The skin, musculoskeletal system, and central nervous system have been described as the most common sites of extrapulmonary disease. We present a case of an asymptomatic patient in whom the diagnosis of coccidioidomycosis was made on a lymph node biopsy. The biopsy was performed because of an abnormal mammogram and physical findings suggestive of malignancy.

Kawasaki syndrome (the mucocutaneous lymph node syndrome).

Kawasaki Syndrome is a newly recognized clinical entity characterized by multisystem involvement. It has an acute onset and a triphasic clinical course. Although essentially a self-limited disease, permanent vascular damage, especially involving the coronary arteries, may result. Pathologically the disease is characterized by widespread vasculitis. There is a monomodal age distribution with peak occurrence during the first 2 years of life; few affected over the age of 8 years. Males outnumber females 1.5:1, persons of Japanese extraction are overrepresented compared with other races, and Caucasians are underrepresented. Community-wide epidemics occur in diverse locations but there is no evidence for direct person-to-person transmission. Etiology remains unknown. Therapy remains supportive and should be directed at careful clinical evaluation for cardiovascular abnormalities and antiplatelet aggregation therapy.

Peritoneal cytology in endometrial carcinoma.

Peritoneal cytology is accepted as part of the evaluation for patients undergoing laparotomy for a suspected or proven gynecologic malignancy. The study subjects were 83 patients with endometrial carcinoma. We investigated the significance of peritoneal cytology in endometrial carcinoma. Cytological positive results were observed in 11 (23%) of the 48 T1 cases, 9 (50%) of the 18 T2 cases and 4 (50%) of the 8 T3 cases. Cytological positive rates by histological differentiations were 14 (28%) for 50 G1 cases, 4 (36%) for 11 G2 cases and 6 (46%) for 13 G3 cases. Cytological positive results were found in 11 (28%) of 39 cases of less than 1/3 of intramuscular infiltration and 13 (37%) of 35 cases of 1/3 or more infiltration. Five (56%) of 9 metastasis cases and 17 (29%) of 59 nonmetastasis cases were found positive cytologically. An analysis of the data indicated that the influence of positive peritoneal cytology on recurrence superceded that of other known risk factors, such as grade, myometrial invasion, extrauterine disease, and lymph node metastasis. The patients with normal cytological findings at laparotomy tend to have a significantly (p less than 0.01) better prognosis than similar patients with abnormal cytological findings.

Percutaneous aspiration of retroperitoneal lymph nodes in ovarian cancer.

Nine percutaneous lymph node aspirations were performed on six patients with ovarian carcinoma. Two aspirations were performed for initial staging, while seven were done as part of the restaging evaluation. Seven of the aspirations were positive for tumor. Two aspirations were negative, one of which was surgically confirmed. In five instances the percutaneous aspiration saved the patient an operative procedure. Percutaneous lymph node aspiration is valuable when tissue is required to confirm metastatic disease during staging evaluation, especially with equivocal or atypical lymphograms. The procedure is also useful in assessing abnormal lymphograms that persist after therapy.
