Discordant pregnancy tests in detection of alpha-subunit--producing tumors.

Ectopic production of isolated alpha-subunit of glycoprotein hormones was detected in a 32-year-old Japanese woman with metastatic lung tumor by a discordance in the immunologic pregnancy tests for native human chorionic gonadotropin (hCG) assays. Quantitative discrepancy of titrations among different types of pregnancy tests, ie, competitive (hemagglutination or latexagglutination inhibition) and noncompetitive (hemagglutination or latexagglutination) reactions, was also confirmed experimentally using a highly purified ectopic alpha-subunit from the patient's urine. Thus the discrepancy between the 2 types of assay systems was due, at least partly, to a cross-reaction of high concentrations (20,000 ng/ml) of ectopically secreted free alpha-subunit of glycoprotein hormones.

[On the alpha-fetoprotein in the diagnosis of primary liver carcinoma (author's transl)].

The existence of Alpha-Fetoprotein was investigated in sera of 544 patients with laparoscopically and/or histologically proved chronic liver diseases and cancer of the liver. The applied methods were the double-diffusion technique (micro-Ouchterlony), the counter immunelectrophoresis and the latex agglutination test. In 15 patients with primary cancer of the liver there was a positive result for Alpha-Fetoprotein with the first two methods, 73,3 and 80,67%, respectively. In other liver diseases (liver cirrhosis, chronic hepatitis, malignant tumors with liver metastases) there were only a few positive results (4,17% in patients with liver metastases of unknown origin and less than 1% for the other disease groups). In this group the latex test showed a higher frequency of positive tests for Alpha-Fetoprotein, however with a concomitant rise of positive Waaler-Rose test in 80-93%. In the case of a positive rheumatic factor the latex test will therefore demand a cautious judgement because the latex particle coated with rabbit-anti-Alpha-Fetoprotein-globulin could be agglutinated by anti-rabbit-globulin existing in human serum. A correlation between the existence of Alpha-Fetoprotein and clinical parameters or laboratory datas does not exist. Increases of Alpha-Fetoprotein shown by counter immunelectrophoresis exist in primary cancers of the liver and embryonic tumors, sometimes and only temporarely in acute viral hepatitis, in some sorts of liver cirrhosis of indian children and in the case of pathologic gestation. If these last-named diseases are excluded a positive test for Alpha-Fetoprotein by precipitation methods is highly suggestive for primary cancer of the liver. Finally, the determination of this substance has a certain value for the therapeutic control of Alpha-Fetoprotein positive malignant tumors.

[Comparative evaluation of several methods of diagnosing hepatitis A].

Immunofluorescence (IF), indirect hemagglutination test (IHA), latex-agglutination test (LA), and immunofluorescence of activated sepharose particles (IFS) were used for early specific diagnosis of viral hepatitis A (VHA). The antisera used in the tests were those from convalescents after VHA collected at 25-30 days which had no HBs antigen or antibody to it. The IF and IHA tests were found to be most sensitive for examination of VHA patients. In VHA foci, positive IF tests were found in 22%-72% contacts (in relation to the number of manifest forms in a focus of infection) which in 84.6% cases correlated with enlarged liver. In parallel examination of contacts in one of VHA foci by IF and IHA tests similar results were obtained and the infection-rate in the group was 64.3% and 64% respectively.

[Detection of immune complexes in idiopathic thrombopenia].

In 206 sera from the patients with different haematological disorders including 45 sera from patients with idiopathic thrombocytopenia, immune complexes were searched for, using the method of inhibition of latex-IgG agglutination by rheumatoid factor (RF) and C1q complement component. In the group of patients in 27 cases (13%) presence of endogenous RF was found. The sera of 149 patients (72.5%) inhibited RF and C1q. With the exception of sera from patients with thrombocytopenia, all the remaining ones either inhibited RF and C1q or contained endogenous RF. In the sera of patients with idiopathic thrombocytopenia the inhibitory factors were found in 46.5% and endogenous RF in 22.5%. In 12 patients in this group selected on clinical grounds the levels of immune complexes were investigated before and during 2 weeks after splenectomy. Apart from some changes immediately after this operation no significant differences were observed in the levels of immune complexes during this brief follow-up.

Serological characterization of Actinobacillus pleuropneumoniae biotype 1 strains antigenically related to both serotypes 2 and 7.

Nine Danish Actinobacillus pleuropneumoniae biotype 1 isolates were shown by latex agglutination and indirect haemagglutination to possess capsular polysaccharide epitopes identical to those of serotype 2 strain 1536 (reference strain of serotype 2) and strain 4226 (Danish serotype 2 strain). Immunodiffusion confirmed the antigenic relationship with serotype 2 and further demonstrated an antigenic relationship with strain WF83 (reference strain of serotype 7). SDS-PAGE with LPS from strains 1536, 4226, WF83 and strain 7317 (representative of the 9 isolates examined) showed that strains WF83 and 7317 had an identical smooth ladder pattern whereas LPS from strains 1536 and 4226 showed a distinctly different pattern. The antigenic similarities of the LPS of strains WF83 and 7317 were confirmed by immunoblots using rabbit or pig antisera prepared against the 3 strains. No antigenic similarities in the LPS of strains 1536 and 7317 were revealed. Since an antigenic determinant specific for the 9 isolates could not be demonstrated with the methods used, the strains are proposed to be designated K2:O7.

Evaluation of a commercial latex agglutination test for identification of Staphylococcus aureus.

A new, commercially available latex agglutination test (SeroSTAT Staph; Scott Laboratories, Inc., Fiskeville, R.I.) was compared with the tube coagulase and slide coagulase tests as means for identifying Staphylococcus aureus. Of 160 clinical isolates of S. aureus, 159 (99.4%) yielded positive results with the latex agglutination test. Negative latex agglutination test results were obtained with 266 of 267 clinical isolates of Micrococcus spp. and staphylococcal species other than S. aureus (99.6%). The latex agglutination test was found to be a rapid, technically nondemanding method for identifying S. aureus. It was as accurate as the tube coagulase test and more accurate than the slide coagulase test.

Investigations of outbreaks of contagious caprine pleuropneumonia in Eritrea.

Mycoplasmas were isolated from freeze-dried lung samples from goats from the western lowlands of Eritea suspected of being affected by contagious caprine pleuropneumonia. The goats belonged to two herds in which mortality and morbidity rates were high. Mycoplasma capricolum subsp. capripneumoniae was identified in most samples by the polymerase chain reaction and by conventional serological tests. The latex agglutination test detected more positive serum samples in both herds than did the complement fixation test. Following cloning, the isolates of M. capricolum subsp. capripneumoniae were analysed biochemically and shown to be metabolically similar. They oxidized glucose, N-acetylglucosamine, pyruvate and L-lactate with high affinity and mannose, glucosamine and 2-oxobutyrate with low affinity; they were unable to utilize maltose, trehalose, fructose or ethanol. Major improvements were seen in the growth yield of the Eritrean strains with the addition of pyruvate to the medium. Thus, it may be that organic acids are important energy sources for these strains and may be used in addition to or in place of glucose. In contrast to most other strains of the M. mycoides cluster, the Eritrean strains produced large amounts of hydrogen peroxide during the oxidation of NADH by lysed cells. This characteristic had previously been reported for strain M. F38, the type strain of M. capricolum subsp. capripneumoniae, although strain F38 did not metabolize sugars. Hydrogen peroxide has long been considered a pathogenicity factor in mycoplasma infections. This is the first isolation of M. capricolum subsp. capripneumoniae from Eritrea.
