Nurse-led shared care diabetes projects: lessons from the nurses' viewpoint.

This paper explores the experiences of four nurse practitioners specialised in diabetes care, in the development and implementation of two Dutch nurse-led shared care projects to improve quality of care. The focus is on the impeding factors involved. The nurses' views are compared to those of the 38 participating physicians by using instruments of qualitative research. Both nurses and physicians consider the way shared care delivery has been structured as the most impeding factor, particularly downward substitution of care from doctor to nurse. In the end, lessons are drawn for nurses, doctors and managers, to solve the assessed impediments to shared care.

Developing postgraduate awards for critical care.

This report outlines a qualitative study that explored the academics' perceptions of postgraduate education for critical care. A framework was generated using models of curriculum design. Focus groups and interviews were undertaken to explore four areas within this framework - taught theory/professional practice, advanced practice skills, assessment and delivery. The findings revealed that ethical and legal aspects of role, research, physiology decision-making and practice skills such as defibrillation, first line treatment and patient assessment were considered extremely relevant to such a programme. Practice assessment was considered essential to ensure clinical credibility of the programme and the practitioner. Frameworks were generated to define the nature of advanced critical care practice. As such, work-based learning and interprofessional supervision were seen as key elements of the programme. Finally areas for further research were identified.

Psychotherapy and social support: integrating research on psychological helping.

Psychotherapy interactions and social support conversations have many similarities, as well as some important differences. Researchers studying these two manifestations of psychological helping--often known as formal and informal helping--usually apply a separate set of concepts and methods to each and tend to locate their work in separate bodies of literature. This paper argues that such a division of the field is unnecessary and unproductive. It outlines several ways in which the two bodies of literature might inform each other and argues for conceptual integration of the two fields.

Changing the nature of physician referral relationships in the US: the impact of managed care.

Prior to the era of managed care in the US, health care delivery was managed by the professional activities of physicians. Managed care replaces management by profession with bureaucratic management structures and oversight, such as utilization review and gatekeeping (required referrals to specialty care). Practically, this means that physicians cannot use the professional relationships that typified practice under fee-for-service medicine, potentially changing not only what physicians do (e.g., order test or not, refer or not), but also how they do what they do. In this paper I look at just one of the changes brought about by managed care: contractual arrangements that require primary care providers to refer patients to a closed panel of specialist physicians. Through an in-depth case study of 45 primary care providers' in the USA who face restricted specialist panels for their managed care patients, but not for their fee-for-service patients, I investigate how the practice of referring is changed by this requirement. First, I use interview data to describe primary care providers general preferences for referral consultants, as well as their views of the referral process and potential barriers in it. Next I present data from all referrals over a four-week period to analyze the extent of referral relationships in actual referrals. Finally, I conclude by discussing some ways that managed care entities can facilitate rather than diminish referral relationships among physicians.

Home-birth emergencies in the US and Mexico: the trouble with transport.

Proponents of the global Safe Motherhood Initiative stress that primary keys to safe home birth include transport to the hospital in cases of need and effective care on arrival. In this article, which is based on interviews with American direct-entry midwives and Mexican traditional midwives, I examine what happens when transport occurs, how the outcomes of prior transports affect future decision-making, and how the lessons derived from the transport experiences of birthing women and midwives in the US and Mexico could be translated into improvements in maternity care. My focus is on home birth in urban areas in Mexico and the US. In both countries, biomedicine and home-birth midwifery exist in separate cultural domains and are based on distinctively different knowledge systems. When a midwife transports a client to the hospital, she brings specific prior knowledge that can be vital to the mother's successful treatment by the hospital system. But the culture of biomedicine in general tends not to understand or recognize as valid the knowledge of midwifery. The tensions and dysfunctions that often result are displayed in midwives' transport stories, which I identify as a narrative genre and analyze to show how reproduction can go unnecessarily awry when domains of knowledge conflict and existing power structures ensure that only one kind of knowledge counts. This article describes: (1) disarticulations that occur when there is no correspondence of information or action between the midwife and the hospital staff; and (2) fractured articulations of biomedical and midwifery knowledge systems that result from partial and incomplete correspondences. These two kinds of disjuncture are contrasted with the smooth articulation of systems that results when mutual accommodation characterizes the interactions between midwife and medical personnel. The conclusion links these American and Mexican transport stories to their international context, describing how they index crosscultural markers, and suggest solutions, for "the trouble with transport."

Dissent and heresy in medicine: models, methods, and strategies.

Understanding the dynamics of dissent and heresy in medicine can be aided by the use of suitable frameworks. The dynamics of the search for truth vary considerably depending on whether the search is competitive or cooperative and on whether truth is assumed to be unitary or plural. Insights about dissent and heresy in medicine can be gained by making comparisons to politics and religion. To explain adherence to either orthodoxy or a challenging view, partisans use a standard set of explanations; social scientists use these plus others, especially symmetrical analyses. There is a wide array of methods by which orthodoxy maintains its domination and marginalises challengers. Finally, challengers can adopt various strategies in order to gain a hearing.

[Interdisciplinarity of Spanish cardiovascular research teams].

OBJECTIVES: The incidence of interdisciplinarity (ID) in Spanish cardiovascular research teams was analyzed and scientists' opinions about interdisciplinary relationships were examined. METHODS: The data analyzed were obtained in a survey sent in 1999 to a sample of 310 researchers. They were selected using bibliometric techniques and/or for being research project leaders in recent years. RESULTS: The response rate was 61%. Data were obtained from 130 research teams located mainly in Madrid and Catalonia. Teams doing clinical research (81%) and those working in hospitals (64%) predominated. Different facets of the interdisciplinary nature of the teams were analyzed: scientist training, team composition, behavior patterns, collaboration, and publication and reading habits. A high ID was observed in the area: more than 70% of the teams are interdisciplinary according to the training of scientists, around 80% make use of knowledge or techniques from other disciplines, and around 90% read and publish in journals outside their own disciplines. Basic research teams had a higher ID than clinical ones. A total of 37 highly interdisciplinary teams were identified. These teams had a greater tendency towards collaboration. CONCLUSIONS: Interdisciplinary reading and publishing habits were the norm among Spanish researchers, even in single-discipline groups. The scattered nature of teams, their high rate of external collaborations, and the multidisciplinary context of centers enhance interdisciplinary relationships. Administrative barriers do not seem to be a major obstacle to establishing interdisciplinary contacts.

Interdisciplinary consensus on the use and technique of vacuum-assisted stereotactic breast biopsy.

PURPOSE: Quality assurance of stereotactic vacuum-assisted breast biopsy (VB). METHOD: A consensus was achieved based on the existing literature and the experience of VB users (Ethicon Endosurgery, Norderstedt). RESULTS: The imaging work-up must be completed according to existing standards before an indication for stereotactic VB is established. Indications include microcalcifications and small non-palpable masses; for the time being lesions very close to the skin and architectural distortions (radial scar) are considered less suitable. Acquisition of >20 cores (11 Gauge) should be routinely attempted (goals: as complete a removal of small lesions as possible, thereby increasing diagnostic confidence and reducing so-called 'underestimates'). The pre/post-fire and post-biopsy stereotactic images and a post-biopsy orthogonal mammogram must be documented. All cases with no or uncertain histopathological correlation require discussion in a regular interdisciplinary conference and a documented consensus concerning further work-up or therapy. Standardised documentation of the primary findings and follow-up mammography after approximately 6 months is requested. CONCLUSION: This consensus includes protocols for the establishment of an indication, performance indicators, interdisciplinary interpretation and therapeutic recommendation, documentation and follow-up. It does not replace official recommendations for percutaneous biopsy.
