Induction of experimental autoimmune keratitis by adoptive transfer of human corneal antigen-specific T-cell line.

PURPOSE: To establish a permanent human corneal antigen (HuCOAg)-specific T-cell line and to determine whether line cells are capable of inducing inflammatory keratitis by adoptive transfer. METHODS: Lymphoid cells harvested from HuCOAg-immunized Lewis rats were expanded to a permanent T-cell line by repetitive cycles of restimulation with HuCOAg and irradiated antigen-presenting cells and propagation in interleukin 2-containing medium. The phenotype and epitope specificity of the line cells were determined. Adoptive transfer was performed after seven cycles by intraperitoneal injection of activated T cells into irradiated recipient rats. RESULTS: A panel of 11 overlapping synthetic HuCOAg peptides to identify T-cell epitopes recognized by the line cells was used. The cells responded selectively to a synthetic peptide containing an immunodominant epitope of HuCOAg (peptides 69-83). Line cells bore the surface phenotype of the T-helper/inducer marker (W 3/25(+) or CD4(+)). Intraperitoneal inoculation of naive rats with 5 x 10(7) activated line cells led to maximal clinical signs of stromal keratitis 7 to 9 days after transfer, characterized by corneal haze, conjunctival and episcleral injection, corneal infiltrates, and neovascularization. Histopathologic examination of the tissues revealed numerous lymphocytes and macrophages and some polymorphonuclear leukocytes along with neovascularization. The pathologic lesions were confined to the peripheral corneal stroma. Immunohistochemical studies demonstrated that the overwhelming majority of the inflammatory cells were CD4(+) T lymphocytes and macrophages; an upregulation of major histocompatibility complex class II antigen expression was also noted. CONCLUSIONS: A long-term, rat T-cell line of CD4(+) phenotype specific for HuCOAg that can induce autoimmune keratitis by adoptive transfer of the line cells to naive syngeneic recipients is described. With the development of this cell line, the mechanisms by which T cells exert their immunopathologic effects in experimental autoimmune keratitis models can be studied.

The primary care physician's role in caring for internationally adopted children.

The rate of international adoption in the United States has been increasing. Internationally. adopted children have various medical, psychological, and developmental issues that need to be evaluated and addressed by primary care physicians. Full histories and physical examinations, developmental assessments, and screening tests are necessary for such children. Family concerns and cultural differences between the child and the adoptive family need to be addressed as well.

Genetic and environmental influences on antisocial behavior: a meta-analysis of twin and adoption studies.

A meta-analysis of 51 twin and adoption studies was conducted to estimate the magnitude of genetic and environmental influences on antisocial behavior. The best fitting model included moderate proportions of variance due to additive genetic influences (.32), nonadditive genetic influences (.09), shared environmental influences (.16), and nonshared environmental influences (.43). The magnitude of familial influences (i.e., both genetic and shared environmental influences) was lower in parent-offspring adoption studies than in both twin studies and sibling adoption studies. Operationalization, assessment method, zygosity determination method, and age were significant moderators of the magnitude of genetic and environmental influences on antisocial behavior, but there were no significant differences in the magnitude of genetic and environmental influences for males and females.

Measles outbreak among internationally adopted children arriving in the United States, February-March 2001.

On February 16, 2001, the Texas Department of Health was notified about a child aged 10 months adopted from orphanage A in China who was taken to a Texas hospital with fever, conjunctivitis, coryza, Koplik spots, and a maculopapular rash. Measles was confirmed by serologic testing. Public health authorities in Texas notified CDC, which then collaborated with health officials in other states to contact other recently adopted children from China and their adoptive families. This report summarizes the results of multistate contact investigations that identified 14 U.S. measles cases and outlines measures taken in the United States and China to control and prevent measles transmission.

Intensification of antitumor effect by T helper 1-dominant adoptive immunogene therapy for advanced orthotopic colon cancer.

PURPOSE: T helper (Th) 1/Th2 balance, controlled by Th1 or Th2 cells producing cytokines, plays important roles in antitumor immunity. Interleukin-18 (IL-18) can act together with IL-12 in promoting the generation of IFN-gamma producing Th1 cells. The goal of this study was to determine whether cytotoxic T lymphocyte (CTL) secreted in a murine IL-18-induced Th1-dominant state inhibited the development of primary tumors and synchronous liver metastases in orthotopic colon cancer model. EXPERIMENTAL DESIGN: Murine IL-18 gene was transduced into activated T lymphocytes by an adenovirus vector encoding IL-18 (AdIL-18) liposome complex method. Efficacy of adoptive immunogene therapy using AdIL-18 with or without IL-12 was tested in advanced orthotopic xenograft of murine colon cancer. To elucidate the mechanism responsible for the adoptive immunogene therapy, serum IL-4, IL-6, IFN-gamma, and tumor necrosis factor alpha production in Th1/Th2 cytokine balance and quantification of tumor vascularity were investigated. RESULTS: By a modified method of adenoviral gene transduction, T lymphocytes achieved efficient IL-18 production without cell toxicity. Against orthotopic colon cancer, when combined with low dose of recombinant (r) IL-12 (AdIL-18-CTL/rIL-12), the therapeutic efficacy showed much smaller tumors with no liver metastases and no disseminated tumors. There was a significant difference in the volume of primary tumors and the number of liver metastases compared with the group treated with AdIL-18-CTL alone or other group (P < 0.01). In addition, the median survival time of the group treated with AdIL-18-CTL was 53.7 +/- 5.8 days and that of AdIL-18-CTL/rIL-12 was 78.4 +/- 6.1 days, which was also a significant difference (P < 0.01). These antitumor mechanisms were involved with Th1-dominant response in serum Th1/Th2 cytokine balance and suppression of neovascularization at primary tumor site. CONCLUSION: These data suggest that a strategy of Th1/Th2 balance-based adoptive immunogene therapy might be useful for advanced cancer patients.

[Autologous adoptive immunotherapy without destruction of infected cells during the treatment of chronic hepatitis B].

OBJECTIVES: To investigate the profile of liver histological damage after autologous adoptive immunotherapy with cytokine-induced killer cells (CIK cells) in patients with chronic hepatitis B (CHB). METHODS: 16 CHB patients were randomly enrolled and received autologous adoptive immunotherapy, then followed up 52 weeks. Liver samples were taken from the patients to evaluate the degree of inflammation and fibrosis. The markers of hepatitis B virus and liver function were also detected. RESULTS: 4 patients had two liver-biopsied samples before therapy and after 52 weeks follow-up. One patient's histological assessment revealed a significant improvement in intralobular necroinflammation (G2 --> G1) and fibrosis (S2 --> S1). The others failed to show obvious changes in liver histology. After 10 days culture in vitro, phenotypic characterization of CIK cells changed significantly by flow cytometry. The percentage of CD4+ cells decreased gradually, while the percentage of CD8+ cells increased from 20% to 60% - 80%. After 52 weeks follow-up, HBV DNA was negative (HBV DNA<4pg/ml in serum) in 6 out of 14 patients. The rates of both HBeAg/anti-HBe seroconversion and alanine aminotransferase normalization were 42.86%(6/14). There was no HBsAg/anti-HBs seroconversion. There was few severe treatment-related adverse events. CONCLUSION: Autologous adoptive immunotherapy doesn't induce the damage of liver histology in chronic hepatitis B patients, which inhibits hepatitis B virus replication by a certain noncytotoxic mechanism.

Correlations of body mass index of adult adoptees and their biological and adoptive relatives.

Adult family members resemble each other in body mass index (weight/height2). Our aim was to assess the extent to which the genetic relationship can account for the resemblance in natural families. We estimated the correlations in body mass index between adult adoptees, their adoptive parents, and their biological parents and siblings and compared the correlations to those between the biological relatives. Height, current weight, and greatest ever weight were obtained for 3651 adoptees. Groups representing thin, medium weight, overweight and obese subjects were selected by current body mass index (n = 540) and similarly selected by maximum body mass index (n = 524). The heights and weights of their biological and adoptive family members were assessed. The correlation in current body mass index between the adoptees and biological mothers, fathers, and full siblings were 0.15, 0.11, and 0.23, respectively (all P less than 0.001). These correlations were compatible with those found among the biological relatives living together. The correlations exhibited no consistent gender-related pattern. There was no correlation in body mass index between the adoptees and their adoptive parents. Similar results were obtained for maximum body mass index. Moreover, the correlations between the adoptees and their biological relatives are very similar to those found in large population studies of natural families. This study suggests that the genetic relationship fully accounts for the familial resemblance in body mass index among adults.

[Studies on lymphokine-activated killer (LAK) cell: accumulation in tumor tissue and the therapeutic effects of adoptive immunotherapy].

We studied the therapeutic effects of adoptive immunotherapy with lymphokine-activated killer (LAK) cells combined with chemotherapy on BMT-11 fibrosarcoma in C57BL/6 mice. Compared with the untreated group, no significant therapeutic effect was brought about by CY therapy alone or LAK.rIL-2 alone and all mice belonging to these three groups died with a mean survival time (MST) of 45.3, 51.8 and 45.9 days respectively. CY plus LAK.rIL-2 brought about complete cures in 3 out of 8 mice (37.5%) and a significant prolongation of MST of mice which died (64.4 days) and the accumulation of LAK cells (% Dose/g) at tumor sites was enhanced more than 7-fold by combination with CY. On the other hand, the therapeutic effects of cytotoxic T lymphocytes (CTLs) was sufficiently high even in the CTL.rIL-2 alone and were only slightly enhanced by combination with CY compared with LAK cells. Also, we detected LAK-attractant activity in the conditioned medium (CM) of CY-treated tumor tissues but not in that of untreated tumor tissues, and peak activity was reached 5 days after CY-treatment. This attractant activity was located in two major 10,000-50,000 M. W. fractions of CM. We then observed that LAK-attractant was produced in CM of host reactive cell enriched fractions from CY-treated tumor tissues, but not in that of tumor cell enriched fractions. The above findings imply that the effects of adoptive immunotherapy depend upon the accumulation of transferred effector cells at tumor sites, and we believe that the production of LAK-attractant by tumor tissue, facilitated by chemotherapy, is one of the mechanisms responsible for enhanced LAK-cell-accumulation at tumor sites. We performed a preliminary clinical trial with adriamycin, autologous spleen-LAK cells and rIL-2 on 30 hepatocellular carcinoma patients after radical resection on the basis of the experimental results above. There were no significant therapeutic effects after adoptive immunotherapy during the postoperative course but tendency for temporary inhibition of recurrence. Thus it is shown that this method has probable value as effective adjuvant postoperative therapy.

Integrating special-needs adoption with residential treatment.

Current estimates indicate that 76% of Iowa children whose parents' rights have been terminated and who are listed on the adoption exchange are older children with emotional/behavioral disabilities. Residential treatment can play a significant role in preparing difficult-to-place children (e.g., seven- to 14-year-olds with moderate to severe emotional or behavioral problems) for adoptive placement [Powers and Powell 1982]. The SATT model offers the opportunity to effectively manage the risks in adoptive placement for these children.

Adoptive immuno-gene therapy of cancer with single chain antibody [scFv(Ig)] gene modified T lymphocytes.

Adoptive transfer of antigen-specific T cells has recently shown therapeutic successes in the treatment of viral infections and tumors. T cells specific for the antigen of interest can be generated in vitro, and adoptively transferred back to provide patients with large numbers of immune-competent T cells. Adoptive T cell therapy, however, is a patient-tailored treatment that unfortunately is not universally applicable to treat viral infections and tumors. We and others have demonstrated that the transfer of genes encoding antigen-specific receptors into T cells (i.e., genetic retargeting) represents an attractive alternative to induce antigen-specific immunity. Currently, we evaluate this concept in a clinical protocol to treat patients with metastatic renal cell cancer (RCC) using autologous RCC-specific gene-modified T lymphocytes.

[Biodistribution of immune cells in adoptive immunotherapy by using GFP transgenic mice].

All of the cells and tissues from GFP transgenic mice, with the exception of erythrocytes and hair, express green fluorescent protein. Additionally, lymphokine activated killer cells induced from splenocytes of the mice (GFP-LAK) express green fluorescence under the observation by fluorescent microscopy. In the present study, we studied the biodistribution of LAK in the two adoptive immunotherapy models by injecting GFP-LAK into non-GFP expressing syngeneic mice. In peritoneal dissemination model of B16 melanoma cells, intraperitoneally injected GFP-LAK accumulated densely on the tumor. On the other hand, in the lung metastases model, intravenously injected GFP-LAK stayed scattered around the tumor, although they were observed abundantly in the spleen. Our adoptive transfer model using GFP transgenic mice is useful for understanding antitumor mechanisms induced by adoptively transferred immune cells, without any troublesome marking procedures.

Did I make the grade? Ethical issues in psychosocial screening of children for adoptive placement.

This article suggests that psychological evaluations as a "screen for normalcy" with a view to adoption are a breach of the child's right to psychological integrity and privacy under international human rights law. A foreseeable outcome of such psychological screens, especially for the older foster child who has experienced multiple placements, is an unreliable mental health diagnosis. Normal, albeit maladaptive, potentially modifiable coping strategies arising in the context of family disruption come after the psychological screen to be labeled as an indicator of mental disorder. This, in turn, may inappropriately interfere with the child's adoption prospects. It is suggested that psychological screens for normalcy of preadoptive children represent a misuse both of psychology and psychiatry for they are motivated more by the needs and interests of social institutions involved in the adoption process rather than those of the child.

Imported giardiasis: impact of international travel, immigration, and adoption.

From the Swedish national database, regarding notified Giardia cases, we could for the period 1997-2003 identify 3,697 cases of travel-associated giardiasis, 4,151 cases in newly arrived immigrants and refugees, and 455 cases in internationally adopted children. These were compared with data sets on the number of international travelers, immigrants/refugees, and adopted children. The overall risk of being notified with giardiasis in returning travelers was 5.3 of 100,000, with the highest incidences in travelers from the Indian Subcontinent (628 of 100,000), East Africa (358 of 100,000), and West Africa (169 of 100,000). A large proportion of the travel-related cases were seen in persons with family roots in the country of infection-a risk group deserving special attention. The overall risk in immigrants and refugees was 1,180/100,000 with the highest risk in persons from Afghanistan (3,800 of 100,000) and Iraq (2,990 of 100,000). The incidence was highest among internationally adopted children (8,110 of 100,000), with geographical risks not entirely correlating to those in travelers and immigrants.

Adoption of children with developmental disabilities.

United Methodist Family Services of Virginia placed 41 developmentally disabled children in adoptive homes between August 1985 and June 1988, representing a range of chronic emotional, intellectual, and physical disabilities. This article describes the need for specialized adoption programs for children with developmental disabilities and the methods used in placing these children. The obstacles to adoption of children with developmental disabilities are also discussed.

Murine studies using polyethylene glycol-modified recombinant human interleukin 2 (PEG-IL-2): antitumor effects of PEG-IL2 alone and in combination with adoptive cellular transfer.

A polyethylene glycol-modified form of recombinant human IL-2 (PEG-IL-2) was tested for murine antitumor effects in vitro and in vivo. This PEG-IL-2 was demonstrated to retain the in vitro ability to support T cell proliferation, enhance a mixed lymphocyte reaction, and generate lymphokine-activated killer (LAK) cells. It was found to have a circulating half-life in mice 25 times longer than unmodified recombinant IL-2 (RIL-2). Serum levels were detected up to 60 h after a single intravenous injection. When given as a single, intravenous administration the antitumor effect of this material was similar to multiple, repeated bolus doses of RIL-2. PEG-IL-2 was also found to support the in vivo efficacy of adoptively transferred LAK cells and tumor infiltrating lymphocytes (TIL). Using a congenic TIL (Thy 1.1), persistence of adoptively transferred TIL was found to be prolonged with PEG-IL-2 compared to repeated boluses of RIL-2. Four days after transfer, twice as many Thy 1.1 TIL were recoverable from the lungs of mice given PEG-IL-2. These studies show that PEG-IL-2 is a modified lymphokine with significant antitumor activity in murine systems and is superior to bolus RIL-2 in enhancing the survival of adoptively transferred TIL.

[Abandonment-adoption: experience at the neonatology service of the University Hospital Center in Sousse, Tunisia].

We studied the outcome of 178 newborns abandoned in the maternity hospital of Sousse, Tunisia during the past eight years. The natural mothers are frequently very young: 40% less than 20 years of age, 10% less than 16 years. In most cases (65%), there is no medical supervision of the pregnancy. There is a high rate of pathological deliveries (11%) and a small for date infants. As a consequence of a long hospitalization in neonatal unit, these infants have important morbidity and mortality. The minimum annual adoption rate is 77%. Most adoptive parents are middle class Tunisians and the large majority of adoptions are successful.

Use of adoptive transfer of tumor-infiltrating lymphocytes alone or in combination with cisplatin-containing chemotherapy in patients with epithelial ovarian cancer.

Lymphocytes isolated from fresh human epithelial ovarian tumors can be expanded in the presence of recombinant interleukin-2, and some CD8 antigen-positive lymphocytes can lyse autologous fresh tumor cells. We treated 7 patients with advanced or recurrent epithelial ovarian cancers, using the adoptive transfer of these tumor-infiltrating lymphocytes (TILs) following a single i.v. injection of cyclophosphamide. One case of disappearance (complete response) and 4 cases of greater than 50% decrease of tumor (partial response) were reported (14.3 and 57.1%, respectively). Regression of tumors in the ovary, liver, lung, and lymph node, both primaries and metastases, lasted for 3-5 months. Furthermore, 10 patients were treated alternately with a cisplatin-containing chemotherapeutic regimen and the adoptive transfer of TILs. Seven cases of complete regression and 2 cases of partial regression of cancer were reported. Four of the 7 patients with a complete response have had no recurrence for greater than 15 months of follow-up. It appears that this experimental technique of adoptive transfer of TILs achieves high response rates even without recombinant interleukin-2 administration and that the prospect of combined therapy using TILs and cisplatin offers hope for increasing the cure rate and long-term survival.

Adoption and single parents: a review.

This review examines what is known about those who choose to become single adoptive parents. The demographic and personal characteristics of single parents who adopt are reviewed and the experiences of single parents with the children they adopt are summarized.
