Endodontic and surgical treatment of a geminated maxillary incisor.

AIM: A geminated maxillary incisor required complex multidisciplinary treatment to preserve health and restore aesthetics. This report describes the coordination of coronal division, root extraction, bone grafting, endodontic treatment and orthodontics in achieving treatment goals. It is the intention of this report to show how a difficult case could be managed by properly coordinated multidisciplinary care. SUMMARY: Pulp testing of the geminated central right maxillary incisor gave a normal response. Radiographic investigation indicated a connection of the pulp chambers. Both the mesial and distal root were filled with gutta-percha. The connection between the two root canals was sealed with a flowable dentine-bonded resin. After dividing the crown with a diamond bur, the mesial part of the tooth was removed and the extraction socket was filled with beta-tricalcium phosphate ceramic (Cerasorb). Radiographs taken immediately after surgery and after 6 months showed no periodontal or periapical lesions. No signs of external resorption were identified. The diastema between the central incisors was closed by orthodontic treatment. KEY LEARNING POINTS: Geminated teeth may present aesthetic and functional problems which require multidisciplinary care. Careful clinical and radiographic examination is essential to decide on the fate of the coronal and root halves involved. A proper coordination between endodontic and surgical treatment may result in maintaining one tooth half, even if a midroot connection between the pulp chambers becomes evident. Orthograde endodontic treatment, hemisection and orthodontics may solve the aesthetic problem of a geminated tooth.

In vivo microleakage of luting cements for cast crowns.

Standardized tooth preparations were completed on previously intact human molars in vivo, and castings were made with a precious metal ceramic alloy by conventional techniques. The castings were randomly assigned to the following luting agents: zinc phosphate, composite resin-glass ionomer hybrid, and a composite resin-glass ionomer hybrid with a dentinal bonding agent and were cemented in a standardized manner to periodontally compromised molars. After 6 months the teeth were carefully extracted, stained, embedded, and sectioned, and the in vivo microleakage was measured. ANOVA disclosed significant differences between groups, and a multiple comparisons test revealed that the zinc phosphate group leaked significantly more than other cement groups.

Treating the cracked tooth with a full crown.

Twenty-eight teeth diagnosed as having a crack were immediately treated with full coverage acrylic resin crowns. Twenty-five of these teeth became asymptomatic after full coverage acrylic crown placement and continued to be asymptomatic one year after restoration.

Clinical procedures for the immediate reattachment of a tooth fragment.

Uncomplicated crown fractures are frequent dental injuries, especially in young patients. It is beneficial to quickly restore the function and the aesthetics of the traumatized tooth. The immediate fragment reattachment is a therapeutic choice for uncomplicated anterior crown fractures, when the tooth is not luxated and the fragment is correctly stored. The aim of this article is to present the clinical procedures for the immediate fragment reattachment by accurate bonding procedures. The immediate fragment reattachment is a very conservative treatment: it allows the restoration of the original dental anatomy, thus rehabilitating function and aesthetics in a short time, by preserving dental tissues.

Variation in mesiodistal crown width and amount of tooth exposure between right and left maxillary anterior teeth.

Facial aesthetic depends greatly on the presence of teeth specially maxillary anterior. Four-hundred-six patients (191 males and 215 females) were examined to investigate the variation of mesiodistal crown width and amount of tooth display between right and left maxillary anterior teeth. The result of this survey indicated that right and left maxillary anterior teeth are not always identical. A difference of up to 1 mm in mesiodistal width and the amount of tooth exposure between the right and left maxillary central incisors is considered to be within the range of normal appearance. Right and left maxillary lateral incisors and canines have the same range of difference as that of central incisors.

[Experimental studies of the proper indication of inlay and crown therapy in lateral tooth areas].

In the framework of a study involving both experimental and clinical tests, two preliminary tests were made to measure and compare the edge lengths of crowns and inlays and the substance losses after tangential and circular stage-type preparations. As revealed by the results, both the three-face and two-face cast fillings in the lateral tooth area have a longer restoration edge than corresponding cast crowns. The weight loss after crown preparations involving a circular stage is greater by one third than after a tangential preparation. In this context, ways of properly using restorative means of therapy in clinical dentistry are discussed.

Potassium nitrate-zinc oxide eugenol temporary cement for provisional crowns to diminish postpreparation tooth pain.

Pulpal injury commonly occurs with tooth preparation for complete fixed partial dentures. This can be documented by the substantial incidence of pain after tooth preparation. In this study, a 4% potassium nitrate-zinc oxide eugenol temporary cement was used to secure provisional crowns over recently prepared teeth and it significantly reduced the incidence and severity of pain after tooth preparation and impression taking.
