Luteoma in pregnancy is a rare tumour (about 100 published cases) and it is rarely associated with signs of virilisation (28 published cases). A visiting luteoma in pregnancy in primigravid woman of 27 years of age is reported and compared with other cases in the literature. A study of the androgen secretions before and after pregnancy and especially the levels of androgens in the peripheral blood, the ovarian vein and in the tumour ware carried out so that metabolic pathways of the production of the androgens could be worked out. The particular circumstances of the unexpected appearance of this luteoma during an H.C.G. stimulation test (Human Chorionic Gonadotrophin) gives an opportunity to define the role of this hormone in its occurrence. Finally, a review of the literature makes it possible to draw up a history of the usual behaviour of these virilising luteomas in pregnancy: they usually show up in the second trimester of pregnancy, and the overriding role is played by testosterone with virilisation of female fetuses.

401 cases of hormone-producing tumors of the ovary were studied (190 theca-cell, 169--granulose cell, 25--mixed thecagranulosa cell and 17--virilising tumors). Histological diagnosis, clinical stage and end results of their treatment were compared. Theca-granulosa cell tumor is considered to be the most malignant form of these neoplasms. Pure theca-cell tumor proved to be benign in most cases. Of special interest is a high percentage of patients being cured at stage 1 (FIGO classification) of neoplastic process, that stands, in the authors' opinion, in close relationship with comparatively good results obtained in these patients.

Female C3H strain mice aged 5 to 6 weeks were exposed to a single dose of 400 Rad irradiation to their lower abdomens and the morphological changes of their ovaries were observed until 72 weeks after irradiation. The incidence of tumor development increased gradually from 34 weeks after irradiation till 72 weeks, when ovarian tumors were noted in more than 90% of the animals. Three types of ovarian tumors developed after a prolonged period, postirradiation: tubular adenoma, granulosa cell tumor and luteinizing tumor. Mixed tumor types account for 68% of the tissue examined. The tubular adenoma is believed to arise from the germinal epithelial inclusion cyst, the granulosa cell tumor and luteinizing cell tumor arise from the interstitial cells. In the case of the granulosa cell tumor, it is believed that tumor cells one capable of secreting estrogen, because vaginal smears demonstrated estrus and histological sections of endometrium demonstrated hypertrophy.

Endocrine studies were performed in 2 cases of luteoma of pregnancy. Even though both luteomas were discovered incidentally at the time of postpartum tubal ligation and cesarean section, they were found to be functionally active, secreting several androgens. Steroid concentrations were measured in the ovarian vein blood draining the luteoma in one patient and in peripheral vein blood in both patients. Peripheral testosterone, androstenedione, and dihydrotestosterone concentrations were increased severalfold higher than the normal controls and were comparable to the concentrations reported previously in virilizing luteomas. Progesterone, 17 alpha-hydroxyprogesterone, and estradiol concentrations were increased as well. The concentrations in the ovarian vein were significantly greater than those in the peripheral vein, which indicates active secretion of these steroids by the luteoma. In spite of the high concentrations of androgens, neither the mothers nor the infants were virilized. Concentrations of androgenic steroids in the cord blood were in the normal range. Determination of steroid concentrations in the serum, taken at intervals post partum, indicated spontaneous regression of the luteomas.
