The variable sonographic appearances of ovarian teratomas: correlation with CT.

To better understand how the internal composition of ovarian teratomas influences their sonographic features, we compared their sonographic and CT appearances in 23 cases. Sixteen lesions had a mixed echo texture, with hypoechoic as well as echogenic components; two were anechoic and five were entirely echogenic. Correlation with the CT images showed that the fat present in most teratomas had variable sonographic appearances. Pure sebum was anechoic or hypoechoic, but fat intermixed with hair strands was echogenic and often attenuated the sound beam because of the numerous tissue interfaces created within the mass. The hypoechoic component of the teratoma corresponded, on CT, to low-attenuation fat in 12 tumors and to fluid in five. Most teratomas contained a dermoid plug, a protuberance arising from the tumor wall and containing hair follicles and often fragments of bone or teeth. On sonography, all dermoid plugs were seen as an echogenic mass, often associated with distal acoustic shadowing and protruding into the hypoechoic portion of the teratoma. When the dermoid plug was large and occupied most of the tumor, the entire teratoma appeared echogenic, with no discernible anechoic component. We found that three factors greatly affected the sonographic appearance of ovarian teratomas: the size of the dermoid plug, the presence and location of calcified elements, and the histologic composition of the fatty component.

Seeing through the stratum corneum.

The stratum corneum (SC) provides a vital barrier membrane between the external environment and the vulnerable internal tissues of the skin. It impedes the flow of water, the penetration of xenobiotics, and invasion of pathogenic micro-organisms. It also has protective capacity against ultraviolet radiation and thermal injury. As routine histopathology provides a misleading picture of a disorganized and shadowy SC, we would recommend the skin surface biopsy technique. This painless technique is easy and reliable in obtaining information from the SC. It demonstrates the geometric patterns of the surface, the openings of the eccrine ducts and hair follicles. The skin surface biopsy technique is also ideal for the investigation of the in situ microbiology of skin. Staining with periodic acid Schiff reagent makes it possible to see ringworm fungi, pityriasis versicolor, candida species, or erythrasma micro-organisms. Scanning electron microscopy can be employed when the higher magnification is needed. Histochemical applications include silver staining for melanin particle, potassium ferricyanide staining for blood pigments and lipid staining with Sudan red, for sebum. The rate of movement of topically applied drugs into the skin can be measured using the skin surface biopsy technique. The concentration of radiolabelled drugs can be counted and compared. Comedogenicity and DNA analysis are other applications of this non-invasive technique.

Isotretinoin, tetracycline and circulating hormones in acne.

Isotretinoin, used to treat severe acne, has been shown to induce hormonal changes, especially to reduce 5 alpha-reductase in the production of the tissue-derived dihydrotestosterone (DHT) metabolite 3 alpha-Adiol G. However, the effects of isotretinoin on other pituitary, adrenal or gonadal hormones have not been thoroughly elucidated. In the present study, isotretinoin administered at a dose of 0.5 mg/kg/day for 4 weeks caused no marked changes in the serum levels of pituitary, adrenal or gonadal hormones or 3 alpha-Adiol G in patients with severe papulopustulotic acne (n = 19). After 12 weeks of therapy, there was a decrease in the levels of the precursor androgens androstenedione, testosterone and 3 alpha-Adiol G in 6/9 patients. Acne improved after 4.5 months in all but 2 male patients, who had very low serum hormone binding globulins (SHBG) and a high free androgen index (FAI). Isotretinoin did not affect the elevated LH/FSH ratio in a patient with the polycystic ovarian syndrome (PCOS); nor did it change the high FAI or low SHBG in the male patients. For comparison, tetracycline had no effects on the serum hormonal levels of patients with mild acne (n = 19) after 7 days of treatment. This study confirms that the effects of isotretinoin on the serum hormone levels are small and unlikely to be of relevance for the resolution of acne or the suppression of sebum excretion.

Fungal colonisation in digital silicone rubber prostheses.

The fungal discolouration of silicone rubber prostheses is reported in four cases. In two of the cases, the discolouration was caused by the fungus Candida tropicalis. In the other two cases, two different fungal organisms, namely Trichoderma sp. and Scedosporium prolificans were incriminated. The non-porous silicone rubber layers create an enclosed environment in the suction cup of the prosthesis and preclude ventilation at the prosthesis-stump interface. The moisture as a result of sweat and body warmth in the stump assists fungal growth. Residual salts from the sweat, sebum from sebaceous glands and the residues from petroleum jelly (Vaseline) applied to facilitate donning, can adhere to the surfaces of the prosthesis and provide the nutrients for fungal growth. Prolonged continuous usages of the prosthesis, the presence of sweaty palms in the users, donning the prosthesis during manual physical activities which induce perspiration, washing of hands with the prosthesis on and warm humid climatic conditions have been identified as factors predisposing the prosthesis to fungal colonisation. The fungal growth caused a black discolouration and marred the aesthetic quality of the prostheses. As a preventative measure, daily immersion of the prostheses in denture cleaner such as benzalkonium chloride, or water at 60 degrees C for 15 minutes, or decontamination with 70% alcohol is recommended. Prior cleaning to remove organic matter before decontamination is emphasised.

[Seborrhoeic dermatitis].

UNLABELLED: The cause of seborrhoeic dermatitis (SD) is still unknown, although many attempts have been made to relate it to infection by Malassezia. Recently Midgley, Gueho & Guillot revised the genus Malassezia to 7 species. We detected Malassezia spores by direct counting using cellotape stripped skin of healthy students or patients; we also cultured Malassezia from these subjects. Strains were classified as species according to Guillot's Tween test. RESULTS AND CONCLUSION: Male students had many more spores than females. Spore numbers were not correlated to sebum levels. M. globosa or M. furfur appeared to be the main strain associated with SD. Treatment with anti-fungal agents showed that the elimination of lesions paralleled the reduction innumber of Malassezia spores. In contrast to corticosteroid treatment, anti-fungal agents showed low recurrence rates. Anti-fungal agents thus have potential value as a new therapeutic agent -an alternative to corticosteroid- for the treatment of SD.

Deposition of [3H]cocaine, [3H]nicotine, and [3H]flunitrazepam in mouse hair melanosomes after systemic administration.

Microautoradiography was employed to show that association of drugs from the serum directly with forming hair pigment is a primary pathway of deposition into the hair. After systemic administration of [3H]flunitrazepam, [3H]nicotine, and [3H]cocaine, association of all three drugs with melanin in the forming hair was observed within minutes of dosage. Sebum was determined to be an insignificant deposition route for all three drugs. Pigmented mice had significantly higher concentrations of all three drugs than did nonpigmented mice. The results provide a better basis for ultimately using hair for reliable analysis of drug and environmental toxin exposure.

Application of acupuncture and moxibustion for keeping shape.

On the basis of TCM diferentiation, the authors have treated 359 adult female cases of non-obesity with undesirable body shape by combined application of body acupuncture and moxibustion and auricular acupuncture, and achieved quite good remoulding and orthopedic results, suggesting that acupuncture and moxibustion can very effectively regulate the somatotypic indexes of body weight, circumference of the chest, loin, hip and thigh, the ratio of the loin to hip, sebum thickness, obesity degree, body mass index and body fat percentage of the cases in the overweight group and orthopedic groups 1, 2. It is therefore concluded that acupuncture and moxibustion is a good therapy for obtaining a graceful body shape.

[Sebaceous gland function in transplanted microvascular flaps].

Sebaceous glands produce sebum that contains: triglycerides, esters of cholesterol, and sometimes even pure cholesterol, squalene and esters of wax. Microvascular transplantation exposed the tissue of free flaps to the temporary ischemia and anaerobic metabolism, inflammatory, denervative and reinnervative processes. The sebaceous glands of the free flaps were also exposed to the action of physiopathologic processes during the micro vascular transfer. Because of that, the function of the sebaceous glands of the transplanted free flaps was estimated compared to the function of the sebaceous glands of the recipients and donor's region environment. The investigation of the sebaceous glands of free flaps function has been performed in 32 wounded patients at the Clinic for Plastic Surgery and Burns, and Institute for Medical Biochemistry of the Military Medical Academy. All the wounded have undergone the microvascular transplantation of the cutaneous, myocutaneous and osteocutaneous flaps. Dry residue in either of the soluble substances was measured in the period from six to thirty six months after the transplantation. The obtained results indicated that the function of the sebaceous glands of the transferred flaps 25.36 months after the transplantation was weaker than the function of the sebaceous glands of the recipients and donor's region's environment and amounted to 98% of the function of the sebaceous glands of the recipient is region's environment.

Use of systemic agents in the treatment of acne vulgaris.

Effective treatment of acne vulgaris can prevent emotional and physical scarring. Therapy varies according to the severity of the disease. Topical medication is generally adequate in clearing comedonal acne, while inflammatory acne usually requires the addition of oral medication. Systemic antibiotics are used most frequently and can be highly effective. Newer formulations of combined oral contraceptives are also helpful in modulating sebum production in the female patient. Severe nodulocystic acne that does not respond to topical retinoids and systemic antibiotics may be treated with isotretinoin. However, the side effect profile of this medication is extensive, and physicians should be well-versed in its potential adverse effects.

Androgen physiology and the cutaneous pilosebaceous unit.

Hormones play an important role in cutaneous physiology. The androgen hormones are of particular relevance in modulating hair growth and sebum production in the pilosebaceous unit, which is comprised of the hair follicle and sebaceous gland. Testosterone arising from the circulation is converted peripherally to its more potent, reduced form, dihydrotestosterone, by the enzyme 5 alpha-reductase. Dihydrotestosterone is primarily responsible for androgen receptor binding and exerting end-organ effects. The clinical sequelae of enhanced local androgen production include androgenetic alopecia, hirsutism, and acne. These disorders can be fraught with significant psychosocial ramifications because of their highly visible nature and impact on perceptions of masculinity and femininity.

[Pharmacokinetics of pramiracetam in animals].

Pharmacokinetic rules of pramiracetam were studied here. After giving pramiracetam orally to dogs, we drew their blood at various times. The drug concentrations in blood plasma were detected by HPLC. 3p87 program was used to calculate the pharmacokinetic parameters. The time-concentration curve corresponded to one apartment model. T1/2 was about 2.3-3.9 hours in various doses. After pramiracetam was given to rats per os, high concentrations of pramiracetam were detected in the rats' tissues. The kidney had the highest concentration of pramiracetam; the liver had the next highest concentration, and then the intestine, lung, muscle, heart, gonad, spleen and sebum had the high concentration in order. The drug was also detected in the brain. 0.7% of the given dose was excreted in unchanged form in bile in 24 hours. 28.26% and 6.35% were excreted in urine and feces respectively in 72 hours. The plasma protein combining rate detected by the method of balance dialysis was 20.1-22.2%.

[Kinetics of refatting of human hair (author's transl)].

The sensitivity of the ground glass method for measurement of lipids can be considerably increased by a more sensitive polish of the glass plates. Therefore lipid measurements are possible in areas with only few sebaceous glands and even on scalp hairs. The hair lipids are continuously measureable without disturbance by sebum from the skin of the head and without extracting the hair of the test person; the time needed for measurement is only short. The curve for the refatting of the hair can be quantitatively recorded. Beside the measurement of the dynamics of hair fatting, we now have a method to prove the cosmetical treatment of hair which also allows experiments to be carried out on a larger scale.

Fatty acid ethyl esters in hair as markers of alcohol consumption. Segmental hair analysis of alcoholics, social drinkers, and teetotalers.

BACKGROUND: Fatty acid ethyl esters (FAEEs) are products of nonoxidative ethanol metabolism. After incorporation in hair, they should be suitable long-term markers of alcohol abuse. METHODS: Hair samples from 19 alcoholics in a treatment program, 10 fatalities with verified excessive alcohol consumption, 13 moderate social drinkers who consumed up to 20 g ethanol/day, and 5 strict teetotalers were analyzed in 1-12 segments for four FAEEs (ethyl myristate, ethyl palmitate, ethyl oleate, and ethyl stearate) by external degreasing with n-heptane, extraction with a dimethyl sulfoxide-n-heptane mixture, headspace solid-phase microextraction of the extracts, and gas chromatography-mass spectrometry with deuterated internal standards. The n-heptane washings were analyzed in the same way for FAEEs from the hair surface. RESULTS: The sum of the four ester concentrations in hair calculated for the proximal 0-6 cm segment was 2.5-13.5 ng/mg (mean, 6.8 ng/mg) for the fatalities, 0.92-11.6 ng/mg (mean, 4.0 ng/mg) for 17 of the alcoholics in treatment, 0.20-0.85 ng/mg (mean, 0.41 ng/mg) for the moderate social drinkers, and 0.06-0.37 ng/mg (mean, 0.16 ng/mg) for the teetotalers. In almost all cases the segmental concentrations increased from proximal to distal. There was no agreement between the self-reported drinking histories of the participants and the FAEE concentrations along the hair length. Ethyl oleate was the dominant ester in all samples. CONCLUSIONS: FAEEs are deposited in hair mainly from sebum. Despite large individual differences, FAEE hair concentrations can be used as markers for excessive alcohol consumption with relatively high accuracy.

The effect of benzoyl peroxide and benzoyl peroxide/erythromycin combination on the antioxidative defence system in papulopustular acne.

Acne vulgaris is a common, inflammatory disease of the pilosebaceous duct. Propionibacterium acnes proliferated in sebum, produces chemotactic factors followed by phagocytosis and this process results in the production of reactive oxygen species which contribute to the inflammatory reaction in papulopustular type acne. Benzoyl peroxide (BP) and BP combination with erythromycin (BP/E) are effective topical medications for the treatment of mild and moderate acne vulgaris. In the present study, the effects of BP and BP/E on antioxidant defence enzymes in 40 patients with papulopustular type acne were evaluated. The activities of superoxide dismutase (SOD), glutathione peroxidase (GSH-Px) and catalase (CAT), and also thiobarbituric acid reactive substance (TBARS) levels were determined in peripheral blood leukocytes in all patients, as well as in tissues of a small group of patients before and at the end of 4 weeks of treatment. No difference was detected in leukocyte antioxidant enzyme activities and TBARS levels due to BP treatment. However, SOD, CAT and GSH-Px activities in leukocytes decreased and TBARS levels increased in BP/E-treated patients (p < 0.05). There was no statistically significant difference between pretreatment and posttreatment enzyme activities in tissue samples. The results of this preliminary study may be attributable to in vivo conditions and possible stability problems while compounding the mixture of the BP/E. Influence of the other ingredients of the formulations applied in the study must also be considered.

[Xerosis].

The clinical characteristic of xerosis is rough or coarse skin. Physiopathologically, the structure of the stratum corneum is modified and abnormalities in keratinization, proliferation, surface lipid, water metabolism and also pH and sebum exist. There are two forms of xerosis: dry skin forms and ichtyosis or ichtyosis-like forms. Xerosis has many etiologies including external aggression, senescence, drugs, infection, atopy, deficiencies, malignant diseases, endocrine affections, eating disorders and renal failure in dialyzed patients. It provokes cutaneous discomfort and unaesthetic appearance that justify appropriate treatment. Treatment is essentially local, symptomatic and must be accompanied by general measures. Many products include moisturizers and emollients (keratolytics: salicylic acid, urea and alpha-hydroxy acid).

Distribution of acid and alkaline phosphatases in canine skin.

Azo dye methods were used to determine the distribution of acid phosphatase (ACP) and alkaline phosphatase (ALP) in the skin of 25 Beagle dogs. ALP activity was found in dermal papillae of hair follicles regardless of their state of activity, in Huxley's layer of the inner root sheath of anagen stages, in myoepithelial cells of apocrine sweat glands, in germinative cells of sebaceous glands, in vascular endothelium, and in mast cells. The ACP activity was found in the epidermis, outer and inner root sheaths, keratogenous zone, hair cuticle and medulla, duct of sebaceous gland, and sebum. The results indicate that ACP and ALP are distinctive enzymes serving different biologic functions. The principal role of ALP in the skin appears to be dephosphorylation for adsorption and transport of chemical substances necessary for growth and maintainence of the pilary system and glandular adnexa. The ACP appears to be primarily involved in the breakdown of phospholipids and in necrobiosis of keratinocytes.

[Acne and hormones].

Androgens stimulate sebum production which is necessary for the development of acne. Acne in women may thus be considered as a manifestation of cutaneous androgenization. Most of acnes may be related to an idiopathic skin hyperandrogenism due to in situ enzyme activity and androgen receptor hypersensitivity, as also noted in idiopathic hirsutism. Some acne may correspond to elevated ovarian or adrenal androgen secretion. The presence of acne in women may lead to a diagnosis of functional hyperandrogenism, either polycysticovary syndrome or nonclassical 21-hydroxylase deficiency. Plasma level assays for testosterone, delta 4 androstenedione and 17-OH progesterone and ovarian echography are necessary to determine the possibility for an ovarian or adrenal hyperandrogenism, but not to better treat acne. The goal of hormonal therapy in acne is to oppose the effects of androgens on the sebaceous gland. Hormones may be used in female acne in the absence of endocrine abnormalities. Antiandrogens (cyproterone acetate or aldactone) may be useful in severe acne, hormonal contraceptives with cyproterone acetate or non androgenic progestins in mild or common acne often in association with other anti-acneic drugs. Glucocorticoids have to be administered in acne fulminans and other forms of acute, severe, inflammatory acne, for their anti-inflammatory properties.

Systemic antibiotic therapy for acne: a review.

Acne is a multifactorial disease of the pilosebaceous unit in the skin. Four contributing pathogenic factors need to be elucidated and include excess sebum production, follicular hyperkeratinization, colonization of the pilosebaceous unit by Propionibacterium acnes, which is a gram positive anaerobic diphtheroid, and the release of inflammatory mediators into the follicle and dermis. One or more of these factors are targeted by each of the systemic therapies for this disease and its variant, including systemic antibiotic therapies, which will be reviewed here.
