Conception rate in Bos taurus and Bos indicus crossbred heifers after postweaning energy manipulation and synchronization of estrus with melengestrol acetate and fenprostalene.

Conception rate in heifers after synchronization of estrus with melengestrol acetate (MGA) and fenprostalene (a prostaglandin F2 alpha analogue; PGF) was determined in pubertal Bos taurus and Bos indicus crossbred yearling heifers. Angus x Hereford (AH, n = 137) and Brahman x Hereford (BH, n = 97) heifers were sorted by body weight after weaning into light (LW) and heavy (HW) weight blocks. Heifers were assigned by age to diets to reach a target weight of 55% (LE) or 65% (HE) of their projected mature weight by the start of breeding. Heifers that exhibited estrus and had serum progesterone greater than or equal to 1 ng/ml (0 or 10 d before estrous synchronization treatment) were assigned randomly within breed and nutritional groups to either an estrous synchronization (S) or control (C) group. Heifers in the S group were fed .5 mg of MGA for 7 d and injected s.c. with 2 mg PGF on d 7 of MGA. All heifers were inseminated 12 h after first detected estrus. A greater proportion of AH (P less than .01) than of BH heifers were in estrus within 6 d after PGF, and more S heifers than C heifers (P less than .01) were in estrus. Conception rate at first service was proportionately higher (P less than .001) in AH than in BH heifers and lower (P less than .02) in S than in C heifers. There was a breed x energy level interaction (P less than .01) for conception rate at first service. Stage of the estrous cycle at the time treatment with MGA was initiated influenced (P less than .05) conception rate at first service in the S, AH heifers, with lower conception rates among heifers beginning treatment late in their estrous cycles (greater than or equal to d 12). Pregnancy rates after 21 d were higher (P less than .01) in AH than in BH heifers and higher (P less than .01) in HW than in LW heifers. More HE than LE heifers (P less than .02), and more AH than BH heifers were pregnant after 45 d. Pregnancy rates at the end of 21 d were higher among HE, BH heifers than among LE contemporaries. A higher (P less than .02) percentage of HE, HW, BH heifers were pregnant at the end of 45 d compared with other BH groups. Results indicated that a 7-d MGA-PGF treatment reduced conception rates at first service in pubertal yearling heifers. Pregnancy rate was affected by prebreeding nutrition in BH yearling heifers at the end of 45 d.

The Finnish foundry project. Background and general methodology.

The general methodology of the Finnish foundry project is presented. The project comprised dust measurements from all Finnish iron and steel foundries and a sample of nonferrous foundries, measurements of chemicals used in most foundries, and a mortality, morbidity and turnover study of active and retired foundry workers. The total budget of the project was Fmk 1,155,000 (+ 305,000). It was started in April 1972 and completed in May 1975. The project had the practical objective of developing recommendations for improvements in work conditions in individual foundries. More specific details of the methodology are reported in other communications.

ALERT-India 1981-89: nine years' experience of leprosy control in the slums of Bombay.

Bombay has a population of about 8 million people, one-half of whom live in slums. In 1981, ALERT-India started its first leprosy control project in N, S and T Wards of Greater Bombay Municipal Corporation covering an area of 122 sq km in the north-eastern suburbs of Vidhyavihar, Ghatkopar, Vikhroli, Kanjurmarg, Bhandup and Mulund, with a total population of 1,100,000 according to the 1981 census. In the 9 years of operation, over 12,000 patients have been registered and treated and of these 7425 have been released from treatment, having satisfactorily completed courses of chemotherapy. However, over 1000 cases are still identified every year by house-to-house or school surveys, or by self-reporting, including a considerable percentage in children. The origin, development, staff structure, operational procedure, administration and recording system of ALERT-India are described in detail, with emphasis on what has been accomplished with purely outpatient facilities, using paramedical workers, all of whom have received inservice training from Government recognized training centres for their specific tasks. The account includes a brief description of an expansion of the organization's work into townships in New Bombay, where preliminary surveys in 1988 confirmed the presence of leprosy cases and the need for treatment facilities. The discussion addresses: 1, the better use of the large volume of statistical information which has been collected by ALERT-India during the past 9 years, with emphasis on its value in assessing the impact on the control programme and modifying future policy; 2, the need to radically examine the present policy of survey, versus an 'education campaign approach' with regard to increasing early case-detection and self-reporting; 3, the establishment of a central coordinating body for leprosy control in Bombay to exchange information, coordinate efforts and formulate a future plan of action, the latter in association with the National Leprosy Eradication Programme; and 4, the development of a health education resource centre in association with the Bombay Municipal Corporation.

Boston building boom--is it an arms race?

A massive healthcare construction boom is under way in downtown Boston, prompting some observers to declare it a "medical arms race." Others, however, say the new projects are critical to some hospitals' ability to remain competitive. Whatever the case may be, there's big money involved--some $1.3 billion in projects that already have been started or soon will be.

Day care unit--Epsom General Hospital.

The project took 12 months from start of design to handover of the building, including all enabling works, with the construction period being from October 1992 to May 1993. The building contract value was L650,000, the main contractor being E Martin & Sons Ltd. The ventilation subcontractor was C A Goodwin Ltd, the electrical subcontractor was Cowleys Sanitary and Heating Engineers Ltd. Since opening its doors on 12 July 1993 the Day Case Unit has seen approximately 700 patients and a further 300 patients have been assessed.

Issues in identification and linkage of patient records across an integrated delivery system.

Historically, the health information systems community has viewed linking personal records as a mundane task. The oversimplified view that routine database manipulation can accurately identify multiple records for a single individual is erroneous, an assumption based on a misperception of the quality of the underlying data. Such data have been adversely affected by the evolution of individual facility patient indexes from multiple systems and the results of backload procedures, and the lack of focus on the need for data integrity by users of the automated systems. Much of the random, invalid data we identify on a daily basis is directly associated with the need for system users to place data in the patient record while they face the situation of having no obvious data field in which to place them. Combined with an underlying lack of standards for the collection of personal identification information, this results in pure chaos when reviewing an MPI file containing a million records at the start of a linkage evaluation project. We have documented the considerable effort that must therefore be made in standardizing the MPI files using stringent analytical procedures and applying common edit routines before commencing record linkage. This preprocessing effort must then be supplemented with sophisticated matching procedures that can handle the dual challenge of minimizing false negatives (the failure to identify true linkages) and false positives (the incorrect linking of records that do not represent the same person). The identification of pairs of linked records does not, however, complete an EPI loading. Because it is fairly common for a multiple facility linkage evaluation to identify more than two medical record numbers for the same patient, and the primary goal of an EPI is to assign a unique identifier for the patient which will link that patient's multiple files, it becomes necessary to develop a means of readily associating three or more records for the same patient. One approach we have used with great success is to assign a common, sequential identification number to all linked medical record numbers for the same patient regardless of facility. The assignment of linkage identification numbers is computer-intensive and is generally accomplished with a highly iterative process. Both system memory and hard disk resources are fully tested as the number of good linkages in an overlap evaluation reaches the half-million mark or greater. Because the primary linkage analysis goal is to develop linkages on pairs of records, with confidence levels based on the comparison of information for those two records, thresholds must be set to decide which linkages should be accepted as true without any human evaluation. If the threshold is set too low, the defined linkage groups may incorrectly join the medical record numbers for different persons. But if the threshold is set too high, there will be undesired duplication of persons in the enterprise system. As in the identification of the underlying linkage pairs, the development of a confidence measure greatly facilitates the assignment of the unique identification numbers needed in the EPI implementation.

Computing for the next millennium.

Computer technology has changed our lives, even that of physicians. In a few years time, a physician can expect to have a new tool by the bedside: a hand-held computer small enough to put into a pocket and powerful enough for all everyday activities, including highly specialized and sophisticated activities such as prevention of adverse drug reactions. The Croatian Academic and Research Network (CARNet) was crucial in bringing the benefits of the information technology to the Croatian scientists. At the Split University School of Medicine, we started the Virtual Medical School project, which now also includes the Mostar University School of Medicine in neighboring Bosnia and Herzegovina. Virtual Medical School aims to promote free dissemination of medical knowledge by creating medical education network as a gateway to the Internet for health care professionals.

Establishment of the FMRFamide-immunoreactive olfacto-retinalis pathway during ontogeny of the rainbow trout (Oncorhynchus mykiss).

Migration of neurons is one of the mechanisms establishing normal central nervous system connectivity during ontogeny. Proper timing of axonal sprouting is relevant in the same context. In the present study, we used the immunoreactivity of the tetrapeptide FMRFamide (Phe-Met-Arg-Phe-NH2) to visualize the olfacto-retinalis projection during trout ontogeny. It starts to innervate the retina two to four weeks after hatching, in contrast to reports on salmon where it only appears after the fish are imprinted on their natal stream.

[Sex education. An experience in elementary and middle schools].

Two nurses at a Primary Health Care Center started up a "Sexual Education Project" in schools in an urban Basic Health Zone. Over the course of the five years this project has been in operation, classes in Sexual Education have been taught in two Middle Schools and six Elementary Schools. During this time, some 4500 questions and doubts have been received and processed. The teaching staffs have shown tremendous interest in this project; furthermore, no school placed any impediments for the development of this program. The types of questions asked did not vary substantially according to the different ages of students taught. The program instructors became aware of the need to provide sexual education information to adults beyond school age. The program directors discovered that it was necessary to establish coordination among the schools and health centers as well as to enlarge this kind of program to other areas of the general public. This experience demonstrated that teaching activities employing nursing personnel as educators in sexuality provides very satisfactory results.

[Taste sensitivity in homeotic mutants of the drosophila leg-arista-wing complex].

It is well established that taste chemoreceptors in Drosophila are located on the tarsi of the first leg pair. In order to investigate the influence of the novel homeotic arista-tarsus transformation on behavior, an analysis of taste perception in the lawc-mutants, characterized by the transformation of the arista into tarsus elements, was carried out. The data were subjected to thorough statistical treatment. It was shown that elements of an additional leg that appeared as a result of homeotic transformation of the arista were sensitive to gustatory stimuli. Analysis of the innervation of the homeotic organs by means of cobalt staining of afferent projections showed that the afferents starting from the homeotic leg reached the thoracic ganglion.

[Experience in the development of information systems at the canton level].

In the aim of need and necessity of stronger orientation and more efficient aim realization 35 Strategy SZO (health information system) country members should have the health information system able to provide information support to their National strategy "Health for All until 2005". Such a health information system should provide the information support for the planning process, monitoring and health evaluation, health care and work in public-health services sector. The experiences in the work of information system organization in Tuzla Canton Public Health Institute should be seen from two aspects: Aspect of the information system significance in the framework of the social-medical, epidemiological, hygiene-ecological and other activities of the Institute; Aspect of the information system development within the Public Health facilities of Canton in relation to the automatic data processing important for registration, tracking and recognition of success and population health care needs. From the aspect of establishing the information system in the area of Tuzla Canton and the Public Health Institute role, there is a need for adequate high technology technical equipment, with the task to enable practical realization of defined projects and in such a way, varied acceleration of work giving as a result usable information. The canton Public Health Institute needs as follows: necessary resources for free information system work; to provide free computer access to all; to ensure connection with other PHI through communications. In this way the canton Public Health Institute information system would be connected with other information systems in the field of health. In would be a base for development of integrated health information system. The information system is desirable aim for the work of Public Health Institute, method of selection and leading content of modernization. Analysing the Bill of Law on Health Care, a defectiveness has been noticed, since the information system is not regulated. Therefore, the need to be issued by Lay is of great importance. A statement that all projects on local information systems, started before war or during the war have been stooped, joins to the aforesaid, and also a series of independent information system in different health segments, which are not connected into one, unique information system. In any case, this disparaged the quality of medical decision making and therefore, providing of high-quality services at all levels of health work.

Veterinary epidemiology--a key to sustainable pig production in Switzerland.

Sustainable animal production requires a healthy production system including healthy animals producing wholesome products. Epidemiology as a discipline is concerned with the occurrence of disease or disease indicators in populations as well as with factors influencing disease occurrence. By providing this information, epidemiology contributes significantly to the development and maintenance of healthy livestock. This is illustrated with the example of the enzootic pneumonia (EP) eradication programme of Switzerland. A series of observational studies were conducted in order to obtain the necessary knowledge to implement an area-wide EP eradication programme. The issues of economical eradication strategies at the herd level and the risk of re-infections were addressed. Simulation modelling and economical analysis were also applied. These research projects led to the successful start of a national eradication programme. The example of the EP eradication programme illustrates that applied epidemiological research provides critical information for the planning, implementation and evaluation of disease control programmes and is thus a key tool in developing sustainable animal production.

Implications of oncogenomics for cancer research and clinical oncology.

The explosion of information generated by large-scale functional genomics technologies has resulted in an exponential increase in the number of potential genes and proteins available for pharmaceutical and diagnostic research development. In order to tap this potential, the primary challenge is to develop a strategy to effectively integrate and extract meaning from the human genomic sequence information that has been generated since the start of the Human Genome Project. This article deals with the strategies being applied by academics and by the biotechnology sector to sort and triage this information with the ultimate goal of identifying future therapeutic targets for cancer and other diseases.

Community-based noncommunicable disease interventions: lessons from developed countries for developing ones.

Community-based programmes for prevention and control of cardiovascular diseases (CVD) started in Europe and the USA in the early 1970s. High mortality from CVD in Finland led to the start of the North Karelia Project. Since then, a vast amount of scientific literature has accumulated to present results and discuss experience. The results indicate that heart health programmes have a high degree of generalizability, are cost-effective and can influence health policy. In the 1980s the focus of programmes expanded from CVD to noncommunicable diseases (NCD), mainly because of the common risk factors. Attention has now turned to promoting this approach in developing countries, where the prevalence of NCD is growing. Theory and experience show that community-based NCD programmes should be planned, run and evaluated according to clear principles and rules, collaborate with all sectors of the community, and maintain close contact with the national authorities. In view of the burden of disease they represent and of globalization, there is a great need for international collaboration. Practical networks with common guidelines but adaptable to local cultures in a flexible way have proved to be very useful.

Nutrient removal process development and full scale implementation at the 4 million p.e. main treatment plant of Vienna, Austria.

The Main Treatment Plant of Vienna is in extension for 4 million p.e. and very stringent nutrient removal requirements. The existing high rate BOD removal activated sludge plant (in operation since 1980) is extended by a second stage activated sludge plant and a newly developed flow scheme for nitrogen removal optimisation adaptable to the temperature variations over the year. For this plant pilot investigations have been performed for the development of a specific mathematical model (ASMV) and a specific aeration control strategy. The civil work of the extension is already finished and the installation of the equipment has started. Operation should start in 2004. The whole project will cost about [see symbol in text]264 million of which about one half is for civil work. The effluent standards correspond to the requirements for sensitive areas in EU Directive for Municipal Waste Water. The raw primary and excess sludge are incinerated after thickening and dewatering. This paper tries to condense the already existing literature with the construction progress and the cost situation.

Wind tunnel evaluation of several tracer and collection techniques for the measurement of spray drift.

In the history of pesticide drift measuring techniques, different tracers and a lot of different collection techniques have been used. At the start of a new Flemish project 'Protecting the Flemish environment against drift - The importance of drift-reducing techniques', wind tunnel tests have been executed to select the most efficient tracer and collection technique. As tracer types a fluorescent tracer Renaissance W15, 2 different chelates, a NaCl-solution and a fungicide Tolylfluanide were used. 2 different collection techniques were tested: drains incorporated in the wind tunnel floor filled with filter paper and filled with cloths. The recovery of the different tracers combined with the 2 collection materials was calculated. The advantages and disadvantages of the tracers and collection materials are enumerated in this article.

[Construction and application of an in silico elongation system of nucleic acid sequence].

Normally it is difficult to obtain full-length cDNA sequence of novel genes. More and more expressed sequence tags(ESTs) have been obtained since the start-up of human genome project. Powerful system is badly needed for data mining on these EST sequences. Based on a personal computer coupled with Linux operating system and EST database, the Blast software and Phrap software were used to construct a platform for in silico elongation of ESTs in our lab. The performance was tested using 11386 EST sequences and 511 partial-length cDNA sequences. Results demonstrated that 8373 EST and 389 cDNA sequence were elongated using this system. Thus the platform seems to be a fast way for full-length cDNA sequence cloning of new genes.

Newborn screening in Indonesia.

In Indonesia, newborn screening is not yet a policy, and the incidence of preventable causes of mental retardation detected by newborn screening is not known. Congenital hypothyroidism (CH) is not infrequent. Without a screening program, unrecognized CH patients were neglected for years. Since May 1999, the International Atomic Energy Agency (IAEA) has assisted in starting a CH Newborn Screening Project to estimate the local incidence of CH and to evaluate the problems associated with the screening. In June 2000, a pilot study was conducted using primary TSH measurement, supplemented by T4 in infants with elevated TSH. The target was to screen 12,000 newborn infants, using cord blood serum taken at birth, or a heel prick between 2 to 6 days of age. Between June 2000 and February 2001, 3,534 neonates born in 4 hospitals were screened using cord blood serum taken at birth (recall rate 3.3%). From March 2001 onwards, the heel prick method was used and participating hospitals increased from 4 to 7. Using this approach, until August 2001, 3,309 samples were analysed and the recall rate was much lower (0.64%). The number of unsatisfactory samples was relatively high due to an unstable process of blood collection. Parental refusal and low acceptance of screening among policy makers resulted from lack of awareness of the dangers of CH, and the screening program was not considered a health priority. Recall of patients after screening was a major barrier, with problems in tracking patients arising from urbanization and a high rate of relocation. To advance the CH screening program nationwide, infrastructure must be improved along with the recall system, and education as well as information campaigns for parents and medical professionals must be intensified. The Department of Health must be persuaded to give a national mandate.
