A health program for commercial dairy herds. 1. Objectives and methods.

A health program for dairy herds is defined as a planned and coordinated approach to achieving and maintaining optimal health and productive efficiency of livestock. A program is described, which has been developed over a period of eight years to fulfill this definition in providing veterinary services to commercial dairy herds. It is demonstrated that clinical veterinary services to participating herds did not satisfactorily achieve the stated aim and therefore an alternative approach was developed. This approach is centred around the concept of performance targets, and each herd is monitored closely by means of a computer-based health and productivity monitoring system in order to detect deviation from target performance. When such deviations occur, the monitoring system is used to diagnose the nature of the problem. The detailed targets adopted in this study are described and the examinations which are undertaken on cattle are outlined. These examinations are aimed at correcting problems, or at confirming satisfactory performance. Further details of the findings of the study are outlined in following papers.

[A longitudinal analysis of dysfunctional TMJ pathology: an assessment of a sample of patients undergoing nonsurgical therapy].

One the different interpretation of the pathological and clinical evolution of TMJ dysfunctions, the most-common of these pathologies is the condyle-disk uncoordination. This study analyzes the evolution of the treatment on the sample of patients affected by TMJ dysfunction and examined at the maxillo-facial department of the University of Rome "La Sapienza". After the TMJ dysfunction has been diagnosed the patients were informed of the individual therapeutic program which consisted of a temporary and definitive occlusal therapy. The aim of this scientific work is to control the efficiency within this therapeutic program in relation to the uncoordination of TMJ. In order to validate this approach the study was compared with another that considered the evolution of TMJ pathology in patients examined at the first diagnostic, visit but who had not undergone any treatment.

Ventilation efficiency and pulmonary function after a wheelchair interval-training program in subjects with recent spinal cord injury.

OBJECTIVE: To study the effect of a wheelchair interval-training program on the ventilatory function of subjects with recent spinal cord injury (SCI). DESIGN: Evaluation trial before and after a training program. SETTING: Center of reeducation and university hospital. PARTICIPANTS: Six subjects (5 men, 1 woman) hospitalized after a recent SCI. INTERVENTION: On a wheelchair ergometer, subjects with SCI performed 30 minutes of interval training 3 times a week for 6 weeks. The training program was part of their reeducation program. MAIN OUTCOME MEASURES: Spirometric values at rest and dynamic ventilatory responses were studied before and after this training program with a spirometric test, a maximal exercise test that increased by 5W every 2 minutes, and a submaximal test. RESULTS: Spirometric values at rest did not change after training. At maximal exercise, peak ventilation (Vepeak, 7.5%), peak breathing frequency (f peak) (-13.4%), peak tidal volume (Vtpeak +28.9%), and the ventilatory reserve (12.9%) improved after training. The oxygen cost of Ve decreased significantly (-20%) after training. We observed for the wheelchair tests that, at the same workload after training, Ve and f decreased and Vt increased. CONCLUSIONS: After 6 weeks of our interval-training program in subjects with recent SCI, the increase of Vt and the decrease of oxygen cost of Ve indicated better ventilatory efficiency.

An algorithm for identifying regions of a DNA sequence that satisfy a content requirement.

We present a dynamic programming algorithm for identifying regions of a DNA sequence that meet a user-specified compositional requirement. Applications of the algorithm include finding C + G-rich regions, locating TA + CG-deficient regions, identifying CpG islands, and finding regions rich in periodical three-base patterns. The algorithm has an advantage over the simple window method in that the algorithm shows the exact location of each identified region. The algorithm has been implemented as a portable C program called LCP (Local Content Program). LCP is extremely efficient in computer time and memory; it instantly locates all regions of a long DNA sequence meeting a given requirement. The LCP program was used to analyze the rabbit alpha-like globin gene cluster sequence.

Data analyst technician: an innovative role for the pharmacy technician.

The development of an innovative role for the pharmacy technician is described. The role of the pharmacy technician was based on a needs assessment and the expertise of the pharmacy technician selected. Initial responsibilities of the technician included chart reviews, benchmarking surveys, monthly financial impact analysis, initiative assessment, and quality improvement reporting. As the drug-use and disease-state management (DUDSM) program expanded, pharmacist activities increased, requiring the expansion of data analyst technician (DAT) duties. These new responsibilities included participation in patient assessment, data collection and interpretation, and formulary enforcement. Most recently, technicians' expanded duties include maintenance of a physician compliance profiling database, quality improvement reporting and graphing, active role in patient risk assessment and database management for adult vaccination, and support of financial impact monitoring for other institutions within the health system. This pharmacist-technician collaboration resulted a threefold increase in patient assessments completed per day. In addition, as the DUDSM program continues to expand across the health system, an increase in DAT resources from 0.5 to 1.0 full-time equivalent was obtained. The role of the DAT has increased the efficiency of the DUDSM program and has provided an innovative role for the pharmacy technician.

[Community-based programs for hereditary hemoglobinopathies in Brazilian high school students].

This study tested the viability and efficiency of a hereditary hemoglobinopathy program in which a Brazilian community the town of Bragança Paulista, SP (Brazil)-was involved, were tested focusing school students. The screening of the hemoglobinopathies was done through optional exams for patients by using electrophoresis of hemoglobins and complementary hematological tests. A total of 1,171 individuals, including, 1,118 pupils and 53 of their relatives were tested over a period of 24 months. A total of 47 individuals with hereditary hemoglobin changes were detected-4.0% of the samples examined. The community was fairly receptive to the program which showed a general level of acceptance of 55.4%. The study aroused the interest of the community and motivated the implantation of a special service for diagnosis, genetic counseling and treatment of the hemoglobinopathy carriers in the community where it was undertaken.

Efficient analysis of macromolecular rotational diffusion from heteronuclear relaxation data.

A novel program has been developed for the interpretation of 15N relaxation rates in terms of macromolecular anisotropic rotational diffusion. The program is based on a highly efficient simulated annealing/minimization algorithm, designed specifically to search the parametric space described by the isotropic, axially symmetric and fully anisotropic rotational diffusion tensor models. The high efficiency of this algorithm allows extensive noise-based Monte Carlo error analysis. Relevant statistical tests are systematically applied to provide confidence limits for the proposed tensorial models. The program is illustrated here using the example of the cytochrome c' from Rhodobacter capsulatus, a four-helix bundle heme protein, for which data at three different field strengths were independently analysed and compared.

Accelerated surgical stay programs. A mechanism to reduce health care costs.

OBJECTIVE: To increase cost-efficiency while maintaining the standard of medical care, an accelerated surgical stay program for patients having breast surgery was instituted. SUMMARY BACKGROUND DATA: In the past 20 years, annual health care costs have soared and now comprise 12.2% of the United States gross national product. The annual inflation rate of almost 11% has prompted third-party payers to scrutinize hospital costs as hospitals now consume 38% of health care costs. METHODS: A multidisciplinary task force was formed to analyze and reduce lengths of stay for breast surgeries and to standardize clinical protocols to address ensuing issues. RESULTS: After 1 year, this prospective study found a 39% decrease in average length of stay and a 22% increase in patient volume. This was also accompanied by a low incidence of surgical complications. Through information obtained from 373 consecutive patients through telephone surveys and questionnaires, the Center for Cost-Effective Care reported high patient acceptance and satisfaction. CONCLUSIONS: As a management strategy, accelerated surgical stay programs increase operating efficiency and reduce medical care costs without compromising quality of patient care. Success of this program was attributed to support from senior management, expansion of available educational resources for patients, and to a carefully planned transition from the program's developmental to its operational phase.

Defining the elements of a successful volunteer program in the provision of AIDS services.

Volunteerism has been the cornerstone of HIV/AIDS services since the founding of the first AIDS service organizations (ASOs) in the early 1980s. The work of volunteers continues to be an integral part of care in the face of limited resources and the spreading epidemic. As existing ASOs seek to improve and expand services, and communities seek to establish new organizations to combat the disease, guidelines for practice are necessary to facilitate the implementation of effective, efficient, and successful volunteer programs. This article provides a synthesis of a diverse literature base on the subject and describes the current practical experience at three large American ASOs.

Size of donor chromosome segments around introgressed loci and reduction of linkage drag in marker-assisted backcross programs.

This article investigates the efficiency of marker-assisted selection in reducing the length of the donor chromosome segment retained around a locus held heterozygous by backcrossing. First, the efficiency of marker-assisted selection is evaluated from the length of the donor segment in backcrossed individuals that are (double) recombinants for two markers flanking the introgressed gene on each side. Analytical expressions for the probability density function, the mean, and the variance of this length are given for any number of backcross generations, as well as numerical applications. For a given marker distance, the number of backcross generations performed has little impact on the reduction of donor segment length, except for distant markers. In practical situations, the most important parameter is the distance between the introgressed gene and the flanking markers, which should be chosen to be as closely linked as possible to the introgressed gene. Second, the minimal population sizes required to obtain double recombinants for such closely linked markers are computed and optimized in the context of a multigeneration backcross program. The results indicate that it is generally more profitable to allow for three or more successive backcross generations rather than to favor recombinations in early generations.

Implementing the Health Care Quality Improvement Program in the Medicare ESRD Program: a new era of quality improvement in ESRD.

Improving the quality of health care is a central challenge for America's health care system. The mission of the End-Stage Renal Disease (ESRD) program is to promote the quality, effectiveness, and efficiency of ESRD patient care and program administration. The program provides an ideal opportunity to demonstrate the use of information to help clinicians analyze and improve the care they deliver to patients in an ambulatory setting. This is possible because the program has established regional surveillance systems, called ESRD Networks, that gather information on the occurrence and outcomes of treatment of Medicare beneficiaries with ESRD. The Health Care Financing Administration, which is responsible for the administration of the program, and the renal community have worked together since 1990 to identify ways of incorporating new methods of quality improvement into the program. These methods include statistical evaluation of the processes and outcomes of care in dialysis populations; communicating recommended practices with clinical guidelines and algorithms; regional peer review and feedback (ie, technical assistance and/or collaborations for quality improvement); interventions that focus on the provision of assistance for quality improvement efforts; continuing collection and active feedback of data to providers; and a commitment to continue to evaluate and revise quality improvement activities to reflect lessons learned and newly identified needs. These ideas have been included in the 1994-1997 scope of work for the ESRD Networks and is called the ESRD Health Care Quality Improvement Program (HCQIP). This article describes the background for the ESRD HCQIP and the program's elements.

Prescription of resistance training for health and disease.

When prescribed appropriately, resistance training is effective for developing fitness, health, and for the prevention and rehabilitation of orthopedic injuries. Because resistance training is an integral component in the comprehensive health program promoted by the major health organizations (e.g., American College of Sports Medicine, American Heart Association, American Association of Cardiovascular and Cardiopulmonary Rehabilitation, Surgeon General's Office), population-specific guidelines have recently been published. The current research indicates that, for healthy persons of all ages and many patients with chronic diseases, single set programs of up to 15 repetitions performed a minimum of 2 d x wk(-1) are recommended. Each workout session should consist of 8-10 different exercises that train the major muscle groups. Single set programs are less time consuming and more cost efficient, which generally translates into improved program compliance. Further, single set programs are recommended for the above-mentioned populations because they produce most of the health and fitness benefits of multiple set programs. The goal of this type of program is to develop and maintain a significant amount of muscle mass, endurance, and strength to contribute to overall fitness and health. Patients with chronic diseases (e.g., arthritis) may have to limit range of motion for some exercises and use lighter weights with more repetitions.

Weighted estimating equations for linear regression analysis of clustered failure time data.

Estimation of regression parameters in linear survival models is considered in the clustered data setting. One step updates from an initial consistent estimator are proposed. The updates are based on scores that are functions of ranks of the residuals, and that incorporate weight matrices to improve efficiency. Optimal weights are approximated as the solution to a quadratic programming problem, and asymptotic relative efficiencies to various other weights computed. Except under strong dependence, simpler methods are found to be nearly as efficient as the optimal weights. The performance of several practical estimators based on exchangeable and independence working models is explored in simulations.

Estimation of genetic parameters for quantitative trait loci for dairy traits in the French Holstein population.

A marker-assisted selection program (MAS) has been implemented in dairy cattle in France. The efficiency of such a selection program depends on the use of correct genetic parameters for the marked quantitative trait loci (QTL). Therefore, the objective of this study was to estimate the proportion of genetic variance explained by 4 QTL described in previous studies (these QTL are segregating on chromosomes 6, 14, 20, and 26). Genotypes for 11 markers were available for 3,974 bulls grouped within 54 sire families of the French Holstein population undergoing MAS. The parameters were estimated for 4 QTL and 5 dairy traits: milk, fat and protein yields, and fat and protein percentages. The proportion of genetic variance explained by the QTL ranged from as low as 0.03 to 0.36%. Both lack of marker informativity and poor monitoring of QTL transmission might limit the accuracy of estimation. The QTL explained a larger proportion of genetic variance for milk composition traits. The QTL on chromosome 14 and chromosomes 6 and 20 have their largest influence on fat and protein percentages, respectively. The overall proportions of genetic variance explained by the QTL were 27.0, 30.7, 24.1, 48.2, and 33.6% for milk, fat and protein yields, and fat and protein percentages, respectively. These results clearly indicated that a large part of the genetic variance is explained by a small number of QTL and that their use in MAS might be beneficial for dairy cattle breeding programs.

Collaborative preceptor training: a creative approach in tough times.

Rapid changes in the health care environment have challenged educators to find more efficient methods of developing nurses. These demands are leading to the use of collaborative approaches to education. Nurse educators have also found that they have to rely on preceptors for orientation of new employees. The Texas Medical Center Collaborative Preceptor Program was developed in 1989 to prepare nursing preceptors for all nine participating institutions. The goals of the program were to increase efficient use of instructor time, standardize preceptor preparation in the Texas Medical Center, and offer more programs and prepare more nurses per year than each individual institution could do alone. There were many unexpected benefits as well, and future plans for collaboration are already being formulated.

Image reconstruction by linear programming.

One way of image denoising is to project a noisy image to the subspace of admissible images derived, for instance, by PCA. However, a major drawback of this method is that all pixels are updated by the projection, even when only a few pixels are corrupted by noise or occlusion. We propose a new method to identify the noisy pixels by l1-norm penalization and to update the identified pixels only. The identification and updating of noisy pixels are formulated as one linear program which can be efficiently solved. In particular, one can apply the upsilon trick to directly specify the fraction of pixels to be reconstructed. Moreover, we extend the linear program to be able to exploit prior knowledge that occlusions often appear in contiguous blocks (e.g., sunglasses on faces). The basic idea is to penalize boundary points and interior points of the occluded area differently. We are also able to show the upsilon property for this extended LP leading to a method which is easy to use. Experimental results demonstrate the power of our approach.

Better use of data to define nutrition priority and action.

Nutrition and dietary survey data has been useful in defining the magnitude of malnutrition and dietary deficit. Such information has been helpful in gaining attention to support or to do more to improve nutrition-advocacy. However, there are also greater opportunities to use the common data to guide the program action. In summary, we have plenty of good nutrition and dietary information available to us in the planning and operation of nutrition intervention program. If we go beyond the use of data to justify area or subject of engagement, we can also use the same data for more efficient and more effective program without additional cost to us.

[Autologous blood transfusion and hemodilution in planned large interventions exemplified by 132 radical prostatectomies].

The hospital records of 132 consecutive patients between May 1988 and August 1991 with biopsy-proven local prostate carcinoma, who were scheduled for radical prostatectomy were reviewed. Seventeen patients had positive lymph nodes at pelvic lymphadenectomy and were subsequently given hormone therapy. The mean blood loss in 115 patients undergoing pelvic lymphadenectomy and radical prostatectomy was 1379 ml. Since September 1988 we have operated a program for predeposit autologous blood donation in all patients prior to radical prostatectomy. Sixty patients were entered into this program and 59 received their autologous blood back during operation. Seventy-eight percent of our patients received only autologous blood. The basis of an efficient autologous blood donation program is a simple logistic schedule for all patients (risk and non-risk), who can choose their dates for autologous blood donation. We hope with this study to encourage other centers to introduce autologous blood donation.
