Selective transarterial embolization and hysteroscopic removal of a placental polyp with preservation of reproductive capacity.

OBJECTIVE: To evaluate the role of transarterial embolization followed by hysteroscopic removal of a placental polyp with preservation of reproductive capacity. STUDY DESIGN: Three patients with placental polyps with abundant blood flow, suspected on ultrasonography with color Doppler imaging and on magnetic resonance imaging, were studied. They underwent transarterial embolization followed by hysteroscopic removal of the polyp. Transarterial embolization of the bilateral uterine arteries was carried out using an absorbable gelatin sponge. Selective removal via hysteroscopy was performed on the following day using a cutting loop without electrical stimulation. The polyp was gradually resected to the level of the surrounding endometrium. RESULTS: Complete removal of the placental polyp was achieved in all patients. The presence of a placental polyp was confirmed by pathologic examination. The operative time was ranged from 26 to 53 minutes. In all cases, no complications were noted, and bleeding was minimal during and immediately after the procedure. Postoperative ultrasonography demonstrated a uterine cavity free of residual mass in each case. CONCLUSION: Transarterial embolization of the uterine arteries followed by selective hysteroscopic removal is a safe and effective method of minimizing bleeding and preserves fertility in the treatment of placental polyps with an abundant blood flow.

[The diagnostic and prognostic significance of epithelial blood group isoantigens in carcinomas and polyps of the colon (author's transl)].

Blood group isoantigens (BG) were detected in carcinomas and polyps of the colon and were related to the degree of differentiation, the histologic type, the age of the patients and the histologic signs of malignancy. 1. BG were found in 60-70% of colon carcinomas regardless of their differentiation. 2. BG-positive carcinomas were less often surrounded by lymphocytes and plasma cells than BG-negative ones. 3. BG were present in colon polyps as mucusassociated and as membrane-associated antigens depending on the differentiation of the cells. Hence, BG-activity can be regarded as additional criterium to assess objectively the differentiation of the tumor cells in a polyp. 4. The percentage of BG-positive polyps increased with dedifferentiation. 5. Polyps with foci of malignant degeneration were consistently BG-positive. 6. Villous polyps contained BG more often than the adenomatous ones. 7. The results support the concept that both villous and adenomatous polyps of the colon are precancerous lesions.

Hysteroscopy in cases of cervical polyps.

To assess the usefulness of hysteroscopy in cases of cervical polyps, we studied 78 patients with a lesion found during a routine gynecological examination. All women were referred for diagnostic hysteroscopy followed by endoscopic polypectomy. In 83.3% of the cases intrauterine investigation confirmed primary diagnosis - a polyp peduncle in the cervical canal. Sixty-five of those lesions were the only pathologic condition, and six (7.7%) were associated with an endometrial polyp. In the remaining 16.7% of examined patients, primary polyps identified as cervical polyps appeared to be endometrial. All women admitted to the study were successfully treated by hysteroscopy. Our results suggest that endoscopic evaluation of the uterine cavity in women with cervical polyps can clarify the initial diagnosis. Hysteroscopy allows not only a precise visualisation of the polyp peduncle but also gives the possibility to identify and treat concurrent asymptomatic intrauterine pathological conditions.

[Significance and expression of transforming growth factor beta in human nasal polyp tissue].

OBJECTIVE: To explore the pathogenesis of nasal polyposis and the expression of transforming growth factor beta (TGF-beta) in human inflammatory nasal polyps. METHODS: TGF-beta 1-3 in nasal polyp tissues and inferior turbinate mucosa of twenty-five polyposis patients were detected with immunohistochemistry alkaline phosphatase and anti-alkaline phosphatase (APAAP) method. The inferior turbinate mucosa of eight healthy volunteers were selected as control. Six polyp tissues were estimated with double immunolabeling and Western-blot analysis to compare the characterization of the TGF-beta isoforms expression and the proportion of macrophages and eosinophils in nasal polyp tissues. RESULTS: The expression of TGF-beta 1-3 in nasal polyps was significantly higher than that in nasal mucosa and indetecable in nasal mucosa from healthy volunteers; TGF-beta 1 was the main isoform detected in nasal polyps; TGF-beta positively was accompanied by numerous macrophage and eosinophil infiltration. CONCLUSIONS: TGF-beta mainly TGF-beta 1 is strongly expressed in nasal polyps and its mucosa, where it could be produced by macrophages and eosinophils. TGF-beta could induce modification of epithelium and connective tissue and therefore be involved in the pathogenesis of nasal polyposis.

[Expression and distribution of Aquaporin 1 in the nasal polyps].

OBJECTIVE: To confirm the expression and distribution of Aquaporin 1 (AQP1) in the nasal polyps and to investigate the relation between AQP1 and the nasal polyp edema. METHODS: fourteen cases of normal inferior turbinates and 26 cases of nasal polyps were used. The expression and distribution of AQP1 in nasal polyps were examined by immunohistochemical technique. RESULTS: The level of AQP1 in the epithelial cells and serous cells from nasal polyps was higher than that in inferior turbinates, and the level of AQP1 in the epithelial cells and cilium cells from inferior turbinate was higher than that in nasal polyps. CONCLUSION: There is close relationship between AQP1 and nasal polyp edema.

[Expression and significance of interleukin-6,8 in human nasal polyps].

OBJECTIVE: To explore the role of interleukin-6,8 in nasal polyp formation and to search into the effect of allergy in the pathogenesis of nasal polyps (NP). METHODS: The expression and significance of interleukin-6,8 were studied in 36 nasal polyps and 36 serum samples of NP patients by radioimmunoassay. RESULTS: The mean value of IL-6 and IL-8 was (2.7658 +/- 0.3797) ng/L and (4.1877 +/- 0.1758) ng/L in all nasal polyp tissue homogenates. As compared with serum of NP patients, IL-6 and IL-8 were over expressed in nasal polyp tissue homogenates. No relation was found between the expression of IL-6/IL-8 and patients' gender, age and clinical stage. The expression of IL-6 and IL-8 in patients' serum, cord blood and normal serum showed no significant difference. CONCLUSION: IL-6 and IL-8 are strongly correlated with the formation of nasal polyp. Neither allergy nor infection play a role in the pathogenesis of nasal polyps.

[Effects of intranasal budesonide on the infiltration and activation of eosinophils in nasal polyps].

OBJECTIVE: To assess the effects of topical nasal steroid application on the infiltration and activation of eosinophils in nasal polyps. METHODS: Nasal polyps from intranasal budesonide treated patients (n = 16) and untreated patients (n = 16) were investigated. The samples were stained with histochemical and ABC immunohistochemical methods. RESULTS: The majority of eosinophils in nasal polyps were activated eosinophils. Compared with untreated polyps, the proportion of activated eosinophils (EG2+/eosinophil ratio) was significantly less in steroid-treated polyps. Although the numbers of total eosinophils and actived eosinophils were lower in steroid-treated polyps, both of the differences were not statistically significant. CONCLUSIONS: It emphasized that the actived eosinophils play a key role in the pathogenesis of nasal polyps. The effects of intranasal steroid treatment on the number of eosinophils (total and activated) was more profound then on the proportion of activated eosinophils.

[The secretion of chemokine RANTES in epithelial cells of nasal polyps and its significance].

OBJECTIVE: To investigate the effect of RANTES(regulated upon activation, normal T cell expressed and secreted) on chemotaxis, migration and accumulation of eosinophils in nasal polyps during epithelial immune responses. METHODS: The epithelial cells were obtained from nasal polyps and inferior turbinates in patients with snoring, and cultured for 24 and 48 hours with IL-1 beta(25 micrograms/L, 50 micrograms/L). RANTES was measured in the culture supernatant by ELISA. RESULTS: 1. On incubation with IL-1 beta, the epithelial cells from inferior turbinate and nasal polyps released 1.5-10 fold and 10-20 fold greater amounts of RANTES respectively than unstimulated samples. Significant increase in levels of RANTES was found in both groups(P < 0.001); 2. There was no difference between nasal polyp and inferior turbinate before incubation with IL-1 beta(P > 0.05) in the level of RANTES. After IL-1 beta stimulation, the concentration of RANTES was higher in polyp than in inferior turbinate (P < 0.001); 3. The expression of RANTES in both groups was dose-dependent and time-dependent upon stimulation of IL-1 beta. CONCLUSION: The epithelial cells of nasal mucosa are active end-organs, capable of releasing chemokine RANTES. In nasal polyps, RANTES has chemotactic activity for eosinophils and other inflammatory cells. It could affect the chemotaxis and eosinophil function and may play an important role in polyp formation.

[Expression of cell proliferation and apoptosis gene associated protein on nasal polyps and its significance].

OBJECTIVE: To further understand the pathogenic mechanism of nasal polyps, namely the cells proliferation and apoptosis in nasal polyps tissue. METHODS: The proliferation cell nuclear antigen (PCNA), apoptosis associated gene protein (Bcl-2, Bax) were determined in 26 tissue samples of nasal polyps and 14 controls from normal inferior turbinates respectively. RESULTS: (1) The positive expression e of PCNA, Bcl-2 and Bax were significantly higher in epithelium of nasal polyps than the controls, and the expression rate of Bcl-2/Bax, however, tended equipoise. (2) The expression of Bcl-2 were significantly stronger than Bax in glands, eosinophils of nasal polyps tissue, and there were not significant differences in inferior turbinates. CONCLUSION: There is strongly proliferation activity in the epithelium of nasal polyps, and expression imbalance of Bcl-2/Bax may be one of the important factors of eosinophilia in nasal polyps tissue.

Pit pattern analysis by magnifying chromoendoscopy for the diagnosis of colorectal polyps.

BACKGROUND AND PURPOSE: The development of magnifying chromoendoscopy has facilitated the observation of mucosal pit patterns. This study investigated the value of this technology in predicting the histologic findings of colorectal lesions. METHODS: A total of 954 colorectal polyps were included. After identifying the lesions at colonoscopy, 0.2% indigocarmine solution was sprayed and then the zoom apparatus was switched to make a magnified view of the stained crypt orifice at a maximum 100 times magnification. The observed pit patterns were classified into 6 categories (I, II, IIIL, IIIS, IV, and V) according to Kudo's classification. Type I and II were designated as non-neoplastic patterns whereas other types were neoplastic. Correlation of the pit pattern with the findings of histologic examinations of resected or biopsied polyps was performed. RESULTS: There were 678 diminutive (</= 5 mm) polyps (71.1%) and 705 neoplastic polyps (73.9%), including 695 adenomas and 10 carcinomas. When comparing histologically confirmed neoplastic lesions to non-neoplastic lesions, prediction of neoplastic lesions by endoscopists based on magnifying chromoendoscopy had a sensitivity of 90.8%, a specificity of 72.7%, a positive predictive value of 90.4%, a negative predictive value of 73.6%, and an overall accuracy of 86.1%. The diagnostic accuracy for neoplastic lesions was not associated with polyp size and location but was related to the operator's experience. CONCLUSIONS: Characteristic pit patterns obtained by magnifying chromoendoscopy provide useful clues for differentiation of adenomatous from non-adenomatous polyps. Used appropriately in experienced hands, this technique offers a valuable adjunct to standard colonoscopy in predicting the histologic characteristics of colorectal polyps.

[Study on the role of interleukin-5 and eosinophil cationic protein and the correlation between them in nasal polyp].

OBJECTIVE: To evaluate the degree of recruitment and activation of eosinophils; to confirm the concentration and expression of IL-5 in nasal polyps; to study the correlation between eosinophilic activation and cytokine such as IL-5. METHOD: Blood and nasal specimens were collected from 30 patients with nasal polyps and 8 controls with deviation of nasal septum or OSAS. The serum ECP levels in two groups and the IL-5 and ECP in nasal polyp specimens or inferior turbinate mucosa were examined. RESULT: Concentration of IL-5 and ECP in nasal polyps tissue homogenates were significantly higher than those in inferior turbinate mucosas (P < 0.05). The same result was showed in serum ECP. The elevated IL-5 levels in polyp homogenates had a significant correlation with serum ECP(r = 0.598, P < 0.05) and polyp tissues ECP(r = 0.451, P < 0.05). CONCLUSION: The concentration of IL-5 increased in the nasal polyp tissues and the degree of expression showed a significant correlation with activated eosinophils.

[Analysis of the cellular infiltrate and epithelial class I and II molecular expression in edematous type nasal polyps].

Although in the last few years histopathological, immunohistochemical and immunological studies on nasal polyps have been carried out by several Authors, the etiology of these formations still remains unknown. Nasal polyps have a very characteristic structure and have been classified in three histologic types: edematous, glandular and fibrous. In the present report, 11 nasal polyps of edematous type, representing 61% of total number of collected polyps, were studied employing immunohistochemical methods. All the examined polyps were similar in histology and positivity pattern for HLA molecule expression. The edematous core appeared infiltrated (149 cells/mm.2) mainly by eosinophils (> 90%), whereas the peripheral subepithelial connective tissue contained cellular clusters (416 cells/mm2.) made up of different subsets of hematic cells (30.8% were monocytes-macrophages and 48.6% were lymphocytes largely represented by CD4+ cells). On the contrary, mast cells were quite rare (on the average 1.7 cells/mm2.) and located near T cell clusters. The epithelial positivity for HLA-DR and HLA-A,B,C molecules showed a characteristic discontinuous pattern. In the same patient, controlateral nasal mucosa showed a histological structure very similar to that of polyps. The above data suggest that the presence of polyps is the result of an inflammatory process brought about by a complex array of cellular and humoral components.

[Colonic polyps: an analysis of the endoscopic and histopathological aspects].

The present study is a retrospective analysis of the endoscopic and histologic features of 728 polyps resected from 422 patients during the years 1980 to 1991. Mean age of the patients was 52 years (5 to 87), 63% male and 37% female. The histology of the lesions showed: adenoma: 60.3%, hyperplastic: 31.7; juvenile: 2.3% and miscellaneous: 5.7%. The 439 adenomas where found in 270 patients of which 151 (55.9%) were male and 119 (44.1%) female. The 231 hyperplastic polyps where found in 113 patients: 66 (58.4) male and 47 (41.6) female. All the hyperplastic polyps where less than 20 mm in diameter. Of the adenomas, 385 (87.7%) where less than 20 mm in diameter and had macroscopic appearance similar to the hyperplastic polyps. Of the 439 adenomas, 218 (49.6) where pediculated and 221 (50.4%) sessile. Of the hyperplastic polyps 53 (22.9%) where pediculated and 178 (77%) sessile. Of the adenomas, 351 (80%) where tubular and 88 (22%) had a villous component, most of the tubular adenomas where less than 15 mm in diameter and the villous component was more frequent as the polyps increased in size. Of the adenomas, 139 (30%) had dysplasia and of these 10% had cancer which represents a total of 3% of all the adenomas in this series. The grade of dysplasia was more severe as the adenomas was larger in size or had villous component. The polyps were located: 69.6 in the distal colon at the reach of the flexible sigmoidoscope and 30.4% in the proximal colon.(ABSTRACT TRUNCATED AT 250 WORDS)

[The effects of intranasal FP on the expression of aquaporin-2 in nasal polyps].

OBJECTIVE: To study the effects of intranasal FP on the expression of AQP-2 in nasal polyps. METHOD: Fourteen cases of type II phase 3 and 12 cases of type III nasal polyps were selected. FP were intranasally used in 7 cases of type II phase 3 and 6 cases of type III nasal polyps for 7 days before operation. The others, treated with nothing, were used as control. 26 nasal polyps were obtained and studied with immunohistochemical technique. RESULT: The volume of nasal polyps in FP-using group became smaller. Both the number of AQP-2 positive cells and the area density were significantly lower in the FP-using group than in the control group (P < 0.01). CONCLUSION: Under the action of intranasal FP the expression of AQP-2 reduced, so is the volume of nasal polyps. It indicated that AQP-2 contributed to edema of nasal polyps and FP may lesson edema through changing the expression of AQP-2.

[Roles of CD45RO, CD20 and local IgE in the pathogenesis of vocal fold polyps].

OBJECTIVE: To investigate the roles of CD45RO, CD20 and local IgE in the pathogenesis of vocal fold polyps. METHOD: Thirty cases of vocal fold polyps and ten cases of normal vocal folds were used in present study by immunohistochemical method to detect the CD45RO, CD20 and IgE cells. A mean numbers of positive cells per high power field(X 400) were counted. All data were analyzed with t-test and correl-test by SPSS10.0 software. RESULT: The number of CD45RO+ cells, CD20+ cells and local IgE cells in vocal fold polyps was significantly more than in normal vocal folds (P < 0.01). The expression of CD45RO+ cells, CD20+ cells and local IgE+ cells in vocal fold polyps was no connection relations. The expression of CD45RO+ cells, CD20+ cells and local IgE cells between male and female vocal fold polyps was no significant(P > 0.05). CONCLUSION: There were active cellular immunity and humoral immunity in vocal fold polyps. Local allergy may play a role in the pathogenesis of vocal fold polyps.

Evaluation of telomerase activity in nasal polyps.

BACKGROUND: The objective of this study was to assess if nasal polyps express telomerase activity and whether a difference could be found between the polyp and the surrounding mucosa of the middle meatus and between different portions of the polyp itself METHODS: Nine patients affected by nasal polyposis were included in this study; four of these patients had recurring polyposis. Telomerase activity was measured by telomeric repeat amplification protocol assay. In six patients, the telomeric repeat amplification protocol assay was performed on the polyp and on the mucosa from the ipsilateral middle meatus. In a polyp, we were able to investigate telomerase activity of its different portions, corresponding to pedicle and fundus. RESULTS: Telomerase activity observed in nasal polyps was higher than that observed in samples from the ipsilateral middle meatus mucosa. High or intermediate telomerase activity was found to be related to predominant recurring polyposis. CONCLUSIONS: Therefore, it could be postulated that telomerase activity could be related with the tendency of polyps to recur.

[Expression of aquaporin-1 and Bcl-2 mRNA in eosinophils of the nasal polyps and its significance].

OBJECTIVE: To study the significance of aquaporin-1 (AQP-1) expression in the eosinophils of nasal polyps. The expression and location of AQP-1 mRNA and apoptosis associated gene Bcl-2 mRNA in nasal polyps were explored. METHODS: Sixteen nasal polyp samples were collected from 11 women and 5 men aged 20-65 years during routine endonasal surgery. Nasal mucosa specimens from the inferior turbinates of 10 patients with allergic rhinitis (7 women and 3 men, aged 16-58 years), collected during septoplasty, were used as controls. The expression of AQP-1 mRNA and Bcl-2 mRNA was detected in serial adjacent sections by in situ hybridization and eosinophils were examined by stain MGG. RESULTS: AQP-1 mRNA expression was found in all 16 nasal polyps and in 4 of 10 inferior turbinate tissues, the mean expression rates were (93.16 +/- 13.25)% and (19.54 +/- 4.98)%, respectively. All 16 nasal polyps and 10 control nasal tissues expressed Bcl-2 mRNA, by the average rates of (84.74 +/- 12.10)% and (16.45 +/- 3.12)%, respectively. The expression of AQP-1 mRNA was positively correlated with Bcl-2 mRNA expression in nasal polyps (r = 0.875, P < 0.01). CONCLUSIONS: AQP-1 contributes to the survival of eosinophils in nasal polyps by keeping the permeation balance of eosinophils.

[The recurrent nasal polyp analysed by mRNA difference to demonstrate].

OBJECTIVE: To look for the correlated gene of nasal polyp. METHOD: The nasal polyp was analysed by mRNA difference to demonstrate, and the fragment of writing was analysed and appraised. RESULT: The amplification products of normal conchae tissue were compared with the nasal polyp tissue under the condition of background with one accord. The total 11 difference fragments were discovered during the 150-700 bp. One of the fragments (X-1) was completely homology with the section series of the eight chromosome long arm. There was a zine finger protein in the upper stream and the down stream. The fragment expressed in nasal polyp tissue. CONCLUSION: Based on polymerase chain reaction (pcr), using differential display R. T. PCR (DD-RTPCR) technique, the difference express gene of nasal polyp could be effectively compared with normal nasal conchae tissue or cells, the correlated gene of nasal polyp tissue could screened out. The fragment (X-1) that was completely homology with the section series of the eight chromosome long arm may be a new identified gene which cause the nasal polyp.
