[Photographic pupil diagnosis in Horner syndrome].

In cases of miosis without other clinical signs and not contributory medical history it is often hard to differentiate Horner's syndrome. In these cases the pharmacological testing with cocaine eye drops is a well established and validated method. The physiological testing, i.e. the detection and measurement of a dilatation lag in Horner's syndrome, is not so common. For this reason we measured the dilatation of pupils in 22 Horner's syndromes and 16 physiological anisocorias to find limit ("cut off") values. We used flash photography and a commercial available camcorder and took photos in bright light (400 cd/m2) as well as 4 and 15 seconds after cessation of light stimulus in darkness. An anisocoria after 4 seconds of darkness greater than 0.6 mm distinguishes both groups with a sensitivity of 82% and a specifity of 69%. A dilatation speed of the smaller pupil during the first 4 seconds of darkness lower than 1.1 mm/4 sec is also a good threshold value. Because of the additional good correlation of these parameters to the anisocoria after cocaine the photographic methods are at least a supplement to pharmacological testing with cocaine, which is sometimes equivocal and always delays the localizing hydroxyamphetamine test. The advantage of using a camcorder is the immediate evaluation on a TV screen. Because of the similar price it is more useful to buy an infrared camera to perform real pupillography. Because of the missing possibility of magnification we prefer the evaluation of slides instead of Polaroid flash photographs.

[Survey and analysis on correlative factors with CHD in 12,468 pupils in Yimeng mountain area].

The risk factors correlated with congenital heart diseases (CHD) in 12,468 pupils in Yimeng mountain area were surveyed and analysed. The morbidity rate of CHD was 3.05%, which was higher than those of other heart diseases; ventricular septal defect (VSD) was more frequent than other types of CHD. The frequency in pupils in city is higher than in rural areas. In 38 cases with CHD, through analysis (1:1) on the risk of single factor and the progressive regression analysis on compound factors, the results showed that the age of pregnant women (> or = 35 years), the job of mother (chemical worker), the history of coryza in pregnancy (within 3 months), the season of birth (August, September), and the family history of malformation are notably correlated with CHD.

Optical zone centration in keratorefractive surgery. Entrance pupil center, visual axis, coaxially sighted corneal reflex, or geometric corneal center?

PURPOSE: To study the differences in optical zone marking using the geometric corneal center, entrance pupil center, visual axis, and the coaxially sighted corneal reflex as centration points. METHODS: A modified autokeratometer was used to photograph the cornea in 50 volunteers under standardized levels of illumination, with the subject fixing on the keratometer target. These photographs enabled us to mark the above-mentioned centration points and measure the direction and degree of decentration. RESULTS: From the corneal intercept of the visual axis, the entrance pupil center was found up to 0.75 mm (mean, 0.34 mm) temporally, the corneal reflex was found up to 0.62 mm (mean, 0.02 mm) nasally, and the geometric corneal center was found up to 1.06 mm (mean, 0.55 mm) temporally. CONCLUSION: The ideal physiologic centration point is the corneal intercept of the visual axis. The decentration from the visual axis was least if the coaxially sighted corneal reflex was used for centration.

Weight, height and triceps skindfold in South African black, Indian and White School pupils of 18 years.

In representative series of Sout African Black, Indian and White High School pupils of 18 years, data were secured on weight, height and triceps skinfold. Values among Black boys were significantly lower than those among White boys. Yet, obesity was far commoner among Black compared with White girls. Among Indian pupils, there were no significant differences in mean anthropometric values between groups relatively affluent and relatively poor. Values on Indian boys were similar to those of Black boys, although the former were far more favourably placed socioeconomically. Indian girls, compared with Black girls, had highly significantly lower values for weight and skinfold thickness, but not for height. However, mean weight for height values for Black and Indian boys were similar to values for White boys. Weight for height values for Indian girls were similar to data for White girls, although values on these two ethnic groups were significantly lower than those for Black girls. Findings question the insistence by many (i) of the need to attain maximum potential growth, and (ii) apart from extreme wasting and stunting, of the need to conform to international growth standards.

[Biological, behavioral and intellectual rhythms in pupils during the school-day].

We report in this paper the data separately collected for 15 years by the research teams of Montagner and Testu on the daily fluctuations of the periodical type and ultradian and circadian rhythms of biological, behavioural and mental variables which are or could be linked to or correlated with the fluctuations of intellectual activity in pupils throughout the school-day. The variables under study are the behaviours which are considered to be indicators of vigilance and non vigilance (arousal and drowsiness), corporal activation, heart rate, blood pressure, performances in tests of vigilance, targeted attention, spatial structuration and logical construction, and also in usual school tasks. Studies were carried out on different children populations at all ages (in this article we only report data on children attending primary school and partly kindergarten) in different French schools and, with regard to Testu's studies, several european schools in England, Germany and Spain. The main data collected by both teams are comparable in many respects: 1) the vigilance level and intellectual performances of pupils of all ages are weak or relatively weak at the beginning of the school activities, i.e. from 8 to 9 or 9:30 according to the studies and measured variables, as the biological constraints are not at a high level (heart rate and blood pressure); 2) the same items or patterns significantly increase throughout school time up to 11:30 or noon according to the studies with the exception of the 11:00-11:30 span in Montagner's research in children attending the first level of primary school, as the biological constraints decrease; 3) in most studies the vigilance level and intellectual performances are weak or relatively weak at the beginning of the school-time following the lunch, and the biological constraints are the strongest of the day (from 13:30 to 15:00) ; 4) the vigilance level and intellectual performances are always higher in the middle of the school-day (i.e. from 15:00 to 17:00) than at the beginning of the afternoon, and the biological constraints are weaker.

Trends in prevalence and pattern of substance use among secondary school pupils in Ilorin, Nigeria.

We present trends data on the prevalence and pattern of substance use among secondary school pupils in Ilorin, derived from comparing the finding of two consecutive cross-sectional surveys. From a sample of six schools, 1041 and 848 pupils 1988 and 1993 respectively, completed anonymously a 117-item WHO self-report substance-use questionnaire. The analyses cover responses on the current and lifetime use of eleven substances, their frequency of use, and the effect of gender and school location on use trends. Although a significant increase in current use rates was recorded for alcohol, cannabis, mild stimulants and hypnosedatives, all of these substances (except stimulants) showed a shift towards less frequent use in 1993. The only consistent gender effect was found for smoking, which remained significantly a male activity. The significant increases found in the current use of cocaine, organic solvents and hallucinogens are difficult to substantiate, thus prompting the suggestion for further corroborative qualitative studies. There was a trend towards greater involvement of the rural school's respondents in substance use in general. The implications of the findings with respect to policy issues on substance-use prevention programmes for youths in the Ilorin metropolis are discussed within the context of the limitations of the study design.

[A comparison of incidence of dental caries among primary and middle school pupils in Shanghai at 1982 and 1990].

In 1982 and 1990, two epidemiological surveys of dental caries of primary and middle school in Shanghai were carried out, using similar standards and methods. A comparison of the results revealed that: the incidence of caries of milk teeth continued to stay at a high level; the incidence of caries of permanent teeth showed a marked reduction. There was no difference between the two sexes. The incidence of caries of permanent teeth of the urban pupils is higher than that of the rural ones. Conversely, the incidence of caries of milk teeth of the rural pupils is still higher than that of the urban ones.

[School lunch and nutrition of pupils ended in disadvantaged environments: reports, stakes, assistance with the decision].

Because of a growing poverty in France, the most disadvantaged families are having to give up school lunches for their children, with negative consequences from the point of view of nutrition. The data collected and worked by the authors calls for a more qualified interpretation. A drop in school meal attendance is a reality in some establishments and in some departments. Falling incomes in families experiencing the greatest difficulties undoubtedly influence the choice of expenditure, but it is not possible to generalise it at national level or to confirm any downward trend in attendance. Beside socio-economic factors, it is wide to consider also the food supply, school catering conditions and others factors bound up with changes in eating habits among young and their families. In any case, there is no proof for the moment of any repercussions on pupil's nutritional health, even though this will have to be structurally monitored. Neither has it been proved that pupils who do not take school meals are any the less well fed.

Adie's pupils in paraneoplastic ganglionopathy with ANNA-1.

Autonomic disturbances are common in patients with paraneoplastic syndromes associated with type-1 antineuronal nuclear autoantibodies (ANNA-1), although pupillary disturbances are infrequent. The authors describe a patient with ANNA-1 associated paraneoplastic sensory neuronopathy and bilateral Adie's pupils.

Pupil size as related to interest value of visual stimuli.

Increases in the size of the pupil of the eye have been found to accompany the viewing of emotionally toned or interesting visual stimuli. A technique for recording such changes has been developed, and preliminary results with cats and human beings are reported with attention being given to differences between the sexes in response to particular types of material.

Relative afferent pupillary defect and edge light pupil cycle time in the early differentiation of central retinal vein occlusion.

A total of 37 patients of unilateral fresh central retinal vein occlusion (CRVO) of less than one month duration were examined and re-evaluated after 8 weeks. All eyes with relative afferent pupillary defect (RAPD) < or = 0.6 log units with edge light pupil cycle time (ELPCT) of 925 +/- 94 ms, visual acuity > or = 6/60 with minimal field defects proved to be of nonischemic CRVO, while eyes with RAPD > or = 0.9 log units with ELPCT of 4005 +/- 712 ms, visual acuity 3/60 with marked field defects proved to be of ischemic CRVO with evidence of retinal ischemia on fluorescein angiography. RAPD and ELPCT were found to be extremely reliable indicators for early differentiation of ischemic from nonischemic CRVO and the values remained stable over a period of time. The electroretinography (ERG) correlated well with these parameters.

Isolated pupil-sparing third-nerve palsy as the presenting sign of multiple sclerosis.

A 27-year-old man presented with an isolated, pupil-sparing, partial third-nerve palsy. Etiologic investigations proved unrevealing. Subsequent development of an optic neuropathy prompted magnetic resonance imaging, which confirmed the clinical suspicion of multiple sclerosis. Rarely does multiple sclerosis present with oculomotor nerve involvement.

Regulation of intraocular pressure and pupil size by beta-blockers and epinephrine.

The interaction of adrenergic beta-receptor blockers and epinephrine on intraocular pressure and pupil size was investigated in healthy volunteers. Oral (atenolol and propranolol) and local (timolol) beta-blockers were studied. The experiment was carried out under double-blind randomized crossover conditions. The results showed that the combination of oral beta-blockers with topical epinephrine had an additive hypotensive effect on IOP, while timolol and epinephrine eyedrops both reduced the IOP used alone, but combined had an antagonistic effect, resulting in the disappearance of the epinephrine-mediated IOP reduction.

Peninsula pupil. Anomaly unique to Newfoundland and Labrador?

Forty cases of an unusual form of iris atrophy that results in a characteristically oval pupil have been seen in Newfoundland and Labrador. The anomaly is an atrophy of part of the sphincter pupillae, is confined to the iris, and is not associated with any systemic disorder. The great majority of those affected are male, and all have blue eyes. We suggest that it may be an inherited trait.

Pupil-sparing oculomotor palsies with internal carotid-posterior communicating artery aneurysms.

The charts of 84 patients admitted to the Neurosurgery Service of the Washington University Medical Center between January, 1960, and July, 1981, with aneurysms at or near the junction of the internal carotid and posterior communicating arteries (ICA-PoCA) were reviewed. Special emphasis was placed on the pupillary size and reactivity of 51 patients with oculomotor nerve involvement. Seven initially had normal pupils, representing 8% of the total group and 14% of those presenting with oculomotor palsies. In 4 of these patients pupillary involvement developed within 5 days, and in 1 pupillary involvement developed in 4 months. Pupillary sparing appears to be more common than previously appreciated in patients with ICA-PoCA aneurysms and oculomotor nerve involvement. Patients with acute somatic oculomotor paresis should be observed closely for at least one week for the development of pupillary involvement. Arteriography may be indicated more frequently than previously recommended.

Paradoxical pupil in congenital achromatopsia.

Children with congenital achromatopsia possess an interesting paradoxical pupillary constriction to darkness that has not been previously described. This paper describes three children in whom this paradoxical pupillary reaction was documented with infrared pupillometry. The literature on the pupil in achromatopsia is reviewed and a tentative testable model of the reaction is hypothesized.
