Isomer-specific analysis of PCBs including toxic coplanar isomers in canned cod livers commercially processed in Poland.

Concentrations of highly toxic coplanar non-ortho (IUPAC Nos. 77, 126 and 169), mono-ortho (IUPAC Nos. 60, 105, 118 and 157), di-ortho (IUPAC Nos. 128, 139 and 170) members and 73 other isomers and congeners of PCBs were determined in canned cod livers W 9tróbka rybna po Kaukasku produced by a company in Gdańsk, Poland in early 1990. Total PCB concentrations ranged over 1.2-2.6 micrograms/g wet mass. Concentrations of ten toxic coplanar PCBs (IUPAC Nos. 77, 126, 169, 60, 105, 118, 156, 128, 138, and 170) were found to be in the range of 360 to 690 ng/g wet mass and their TCDD TEQs were 370 to 530 pg/g for aryl hydrocarbon hydroxylase and from 290-430 pg/g for ethoxy-resorufin O-deethylase. Among these toxic congeners IUPAC No. 126, which showed 84% of total TEQ values, imparted most significant toxic threat. Considering these observations, canned cod livers are likely to pose concern to human health since they exceed the tolerance limit of total PCBs (2.0 micrograms/g for edible parts of fishery products) and contain significant residue levels of toxic coplanar PCBs.

Silver content of wild-grown mushrooms from northern Poland.

Wild-grown and mostly edible species of higher mushrooms collected in 1989-1992 from the districts of Gdańsk, Elblag and Piła (northern part of Poland) have been investigated with regard to their silver content. In total, 527 samples including 25 mushroom species of six families (Agaricaceae, Boletaceae, Cantharellaceae, Hygrophoraceae, Coprinaceae and Russulaceae) were examined. Among the fungi examined only Agaricus campestris and A. augustus were heavy bioaccumulating species and showed the highest concentrations of 35 mg Ag/kg dry weight (1.1-150) and 2.0-6.9 mg/kg, respectively. Silver concentrations exceeding 1.0 mg/kg dry weight were noted in Boletus aestivalis, Lepista nuda, L. personata, and in some specimens of B. edulis, Leccinum scrabum, L. vulpinum, Cantharellus cibarius and Coprinus comatus.

Organochlorine pesticide and polychlorinated biphenyl residues in slaughtered and game animal fats from the northern part of Poland.

Pooled samples of the adipose fat of swine, cows, sheep, rabbit, ducks, geese, turkeys, wild boar, roe deer and stags collected from the northern part of Poland in 1987-88 were analysed for the presence of polychlorinated biphenyls (PCBs), hexachlorobenzene (HCB), hexachlorohexanes (HCHs), Dichlorodiphenyltrichloroethanes (DDTs), aldrin, dieldrin, heptachlor, heptachlor epoxide and chlordanes (CHLs). All the fats contained detectable, but low concentrations of organochlorine pesticides and PCBs. The mean PCB levels ranged from 9.2 to 47 micrograms/kg of which IUPAC nos. 138, 153, and 180 were dominating congeners in most of the samples. DDT concentrations were apparently lower in fat of the ruminants (45 to 84 micrograms/kg fat) while in rabbits, swine, turkeys and geese, it was from 79 to 140 micrograms/kg fat. Ducks and wild boar had the highest concentrations of 400 and 440 micrograms DDTs/kg fat, respectively. HCB was detected at concentrations ranging from 2.0 to 18 micrograms/kg fat. The total HCH concentration ranged between 15 and 77 micrograms/kg fat. Aldrin and heptachlor remained undetected while dieldrin was found only in some slaughtered species with a range of positive measurements up to 9.1 micrograms/kg fat. Similarly, heptachlor expoxide was traced up to 9.1 micrograms/kg fat and the residues of CHLs from 0.34 to 4.1 micrograms/kg fat.

Transsexualism and anatomic sex ratio reversal in Poland.

Transsexualism has been described in numerous papers as a condition appearing, on the average, four times as often with somatic males than with somatic females. Against this background we consider the situation in Poland, where, during three consecutive observation cycles over 6 years, the results obtained were reversed. Among our transsexuals the ratio was 5.5:1, with the majority being somatic females.

Karyotypic races of the common shrew (Sorex araneus L.) from northern Poland.

Karyotypic races of the common shrew which differed with respect to the combinations of chromosome arms in certain 2-armed autosomes were distinguished in Poland. Two eastern races with the arm combination ik, and one western race with the arm combination hi in the third pair of autosomes were established. In the contact area of these chromosomal forms, the fourth karyotypic race with the arm combination hk was found.

A comparison of staphylococcal nasal carrier rates in Germany and Poland.

Frequencies of different staphylococcal nasal carrier classes were studied in groups of students of nursing and of laboratory personnel in Cologne, West Germany, and in Krakow, Poland, by six consecutive samplings of their nasal vestibules. The general number of persistent carriers of S. aureus appeared to be lower than in previous studies, paralleling the diminished numbers of staphylococcal infections in populations. Differences in numbers of persistent carriers between the Cologne and Krakow groups were probably related to the incidence and types of S. aureus in the environment.

Contribution to the epidemiology of multiple sclerosis in Poland.

An epidemiological survey of multiple sclerosis was carried out in western Poland. Uneven local distribution of MS was found within 198 communes making up the territory. The disease was less frequent in 1981 than in 1965. A marked increase of population in age classes under 20 during the last decade may account for the observed differences in the prevalence rate. In the town of Gniezno, a new MS cluster was discovered.

Recent health system development in Poland and Hungary.

Before 1989, the health systems of both Poland and Hungary were fully socialist, with all resources being governmental. The total populations of these countries were entitled to comprehensive health services from regionalized networks of hospitals, polyclinics, and primary health stations. Preventive environmental and epidemiological services were provided through special small facilities. Since the termination of socialism in 1989, the Polish system has been largely unchanged, except for greater emphasis on primary health care. About 5 percent of polyclinics have been privatized. In Hungary, financing has been transferred to a Social Security Fund. Polyclinics have been absorbed by hospital outpatient departments, and patients may use them only on referral by a family doctor. Public health officers have a wider scope of responsibilities in their districts. Both health systems still stress equitable distribution of services to everyone.

Frequencies for five short tandem repeat (STR) systems in a population from north Poland.

A population study of unrelated individuals from North Poland (Gdansk area) was carried out to investigate the allele distributions of the five STR systems HUMCD4, HUMFES/FPS, HUMVWA31, HUMTH01 and ACTBP2. PCR products were separated on horizontal non-denaturing polyacrylamide gels followed by silver staining. For all STR systems analysed the distribution of observed phenotypes did not deviate from Hardy-Weinberg equilibrium. A comparison of allele distributions between Polish and other European Caucasian population samples is presented.

Hepatitis G virus coinfection in chronic hepatitis B and C patients in Poland.

This study evaluated the epidemiology and impact of hepatitis G virus (HGV) infection in patients with chronic hepatitis B and C. Serum samples were obtained from 128 consecutive untreated patients with chronic hepatitis B (72 cases) or C (56 cases). The presence of HGV RNA was determined by PCR amplification of the 5'untranslated region; the sensitivity of the assays was ten template copy equivalents. The prevalence of HGV RNA in hepatitis B and C was found to be 25% and 34%, respectively. HGV-positive and HGV-negative patients did not differ with respect to risk factors for infection, age, sex, or alanine aminotransferase activity. Similarly, there was no difference in the severity of liver disease, as assessed with HAI score. In conclusion, we found a very high prevalence of HGV infection in chronic hepatitis B and C patients in Poland. Nevertheless, no evidence was found that HGV coinfection has any impact on the severity of the underlying disease.

Renal oncocytoma. A rare entity in Poland.

Renal oncocytoma represents a special category of renal tumours and accounts for 3-5% of renal neoplasms. Usually they are incidental findings. Most reports have demonstrated their benign nature but a few cases of high-grade oncocytomas were reported to have metastasized. In Poland, between 1971 and 1991, 7 patients (4 women, 3 men) underwent radical nephrectomy for oncocytoma. The author describes here one case of rare oncocytoma malignum.

Lead and cadmium in the hair and blood of children from a highly industrial area in Poland.

The study covered the children living in Miasteczko Slaskie, near the largest Zn plant in Poland. This is one of the areas highly contaminated with heavy metals. The subjects were 158 children aged from 8 to 15 (98 boys and 60 girls). The average Pb and Cd levels in the hair of the entire children population was 8.21 +/- 5.59 microg/g, and 0.91 +/- 0.61 microg/g, and the average Pb and Cd levels in their blood were 14.32 +/- 3.98 and 0.52 +/- 0.24 microg/dL(-1), respectively. The children population under investigation was divided according to their sex. The hair of the girls contained, on the average, 5.06 +/- 2.81 microg/g of Pb and 0.74 +/- 0.48 microg/g of Cd and the hair of the boys 10.14 +/- 6.0 microg/g of Pb and 1.01 +/- 0.65 microg/g of Cd. The blood of the girls contained, on the average, 13.23 +/- 4.23 microg/dL of Pb and 0.48 +/- 0.21 microg/dL of Cd, and the blood of the boys 14.99 +/- 3.68 microg/dL of Pb and 0.55 +/- 0.24 microg/dL of Cd. Thus, both the hair and blood of the boys accumulated more Pb and Cd than those of the girls. A correlation between the concentrations of these metals was confirmed.

Chromium content in the hair of children and students in southern Poland.

Chromium concentrations in hair were compared in groups of children and students from Southern Poland. There were no statistically significant differences (at p greater than 0.05) between girls and boys. The results are similar to those found by other authors in different countries.

Protein profile and biochemical properties of Bacillus circulans isolated from intestines of small free-living animals in Poland.

Forty-seven strains of Bacillus circulans isolated from the intestinal tract of free-living small mammals from the Narvia and Biebrza National Park (NE Poland) were compared with the electrophoretic whole-cell protein patterns on the basis of SDS-PAGE and biochemical characteristic using API tests (50 CHB and 20E). The strains were grouped into two clusters (I and II) at the similarity of protein pattern of 78% using the simple matching coefficient and clustering on unweighted pair group arithmetic average algorithm (SSM/UPGMA). Each of the clusters comprised reference strains. The rods included in the cluster I are characterized by higher biochemical activity in comparison with the bacilli II.

Anaesthesia in Poland syndrome.

PURPOSE: To describe the anaesthetic management of a child with Poland syndrome for CT scan. CLINICAL FEATURES: An eight-month-old child presented with left upper limb hypoplasia and chest wall deformity with absence of ribs on the left side for CT scan of thorax. Pulsations of the heart could be seen along with paradoxical respiration over the defect. The trachea was intubated and the lungs ventilated manually to avoid inadequate ventilation and hypoxia. CONCLUSIONS: The unilateral absence of ribs leads to poor development of subatmospheric pressure in the thorax and paradoxical respiration, and may cause inadequate pulmonary ventilation and hypoxia. In the present case, positive pressure ventilation was chosen to maintain ventilation during the procedure.

Regional distribution of heterozygous 657del5 mutation carriers of the NBS1 gene in Wielkopolska province (Poland).

The homozygous 657del5 mutation, called Slavic mutation, of the NBS1 gene, causes the Nijmegen Breakage Syndrome (NBS). This syndrome is connected with a high incidence of malignancies in early childhood. A high frequency of NBS heterozygotes was found among patients with melanoma, breast, ovary and prostate cancer. The aim of our research was to determine the frequency of 657del5 mutation of the NBS1 gene in the population of Wielkopolska province. For this purpose, we analysed blood samples from anonymous Guthrie cards. In a group of 2090 newborns from the whole province, we found 16 heterozygous mutation carriers. The frequency of 1/131 is higher than 1/190 reported for populations from other regions in Poland. We observed differential regional distribution of heterozygous 657del5 mutation carriers within the province: among 464 samples from the eastern part of Wielkopolska we found 6 carriers (1/77), in contrast to the southern part without any carrier among 625 samples analysed. The high mean frequency of heterozygous 657del5 mutation (1/131) in Wielkopolska province may be associated with cancer incidence in this region.

Cluster analysis of Neisseria meningitidis type 22 strains isolated in Poland.

The aim of this study was to investigate the genetic relationships among strains belonging to Neisseria meningitidis serotype 22 recovered in Poland from 1995 to 1998. Phenotypical characterisation of meningococcal strains isolated from patients with meningococcal disease has been performed since 1995. Strains belonging to Neisseria meningitidis type 22 were most frequently recovered from severe cases of meningitis and septicaemia. The type 22 strains were separated into clusters by serosubtyping and DNA analyses by pulsed-field gel electrophoresis and ribotyping. However, the grouping according to serosubtypes was quite different from that obtained by ribotyping and pulsed-field gel electrophoresis. Furthermore, the two genotyping methods seldom identified the same clusters, nor were clusters identified among 26 Neisseria meningitidis type 22 strains recovered from patients hospitalised in five cities. Determination of the susceptibility to penicillin showed that a single strain was resistant (MIC, 2 mg/l) and that one-third (24/71) of the strains tested had reduced susceptibility (MIC range, 0.1-1 mg/l).

Longitudinal study on early diagnosis and treatment of phenylketonuria in Poland.

Early diagnosis and treatment of phenylketonuria (PKU) in Poland was started in 1965, initially on a voluntary and then on a obligatory basis. Guthrie tests have been used for newborn screening. For confirmation of diagnosis changing with time methods of blood phenylalanine (Phe) and tyrosine estimation were used. In addition, Phe and its urinary metabolites were estimated. A total of 560 cases of classical PKU, 99 mild hyperphenylalaninaemia and six atypical PKU cases were detected. The age of confirmatory estimations was from the 1st to 3rd month, in some cases above 3 months. Pretreatment Phe levels ranged from 12.5 to 70.7 mg/dl. The diet was started on a hospital basis and continued at home, controlled with Guthrie tests with age dependent frequency. Periodical multidimensional controls allowed evaluation of physical and mental development, together with biochemical scores (Hb, RBC, total plasma protein, Phe and tyrosin). In some patients trace elements (copper, iron, zinc and selenium) were also determined. Physical development of treated patients was normal. Biochemical scores presented transient relatively low Hb and iron concentrations especially in the youngest patients. DQ/IQ scores correlated with dietary control and social environment. In adults (age 19-26 years) off diet, mean IQ was 97.2 +/- 15.5 in those with good dietary control and 81.0 +/- 13.8 in those poorly controlled during treatment. In adults the average level of education attained was various types of professional school. Some patients attended or graduated from university.
