Hostile personality characteristics of duodenal ulcer patients.

The aggression profile of patients with Duodenal Peptic Ulcer (DPU) (n = 39) was assessed by means of five self-rating scales (FAF 1, FAF 3, PDS-Extp, PDS-Intp, PDS-Dom) and was compared with a matched normal control group (n = 39) and a psychiatric outpatient sample (n = 67). The DPU group scored significantly higher than normal controls on aggressive excitability and extrapunitiveness (trend in the same direction for potential aggressions), and significantly lower than the psychiatric group on potential aggressions and intropunitiveness. Contrary to expectations, DPU patients did not differ from the other groups on dominance; in this dimension, however, the psychiatric group scored significantly lower than normals. The findings are discussed in the context of the available literature and it is suggested that intropunitiveness is drained off by the somatic symptom (DPU).

[The postsynaptic components of the paroxysmal reactions of the neurons in the strychninized neocortex].

Intracellular responses of neurons of cortical strychninized suprasylvian gyrus were investigated in experiments on immobilized and unanaesthetized cats. Paroxysmal depolarizing shifts (PDS) of the neuronal membrane potential were registered. They consisted of the burst discharge (BD) and slow depolarization wave. By means of intracellular stimulation it was shown that PDS could be summarized and were able to change their form and size. BD in PDS were triggered by large EPSPs which could be elicited from paroxysmal responses. Presumably, the intradendritic recordings have shown the presence of large EPSPs during generation of epileptiform discharges in the neocortex. In some neurons PDSs were accompanied by hyperpolarizing potentials which were apparently IPSPs because they were reversed at the intracellular Cl- injections. The contribution of excitatory and inhibitory synaptic influences to the neuronal paroxysmal responses is discussed.

Basophil histamine release induced by a substance from stimulated human platelets.

Platelet activation may occur during immunoglobulin E antibody (IgE)-mediated reactions. In these studies, we confirm that platelet-derived supernatants (PDS) induce histamine release from human mixed leukocytes containing basophils, one of the initial target cells in IgE-mediated reactions. In extending this observation, we have shown that this PDS-induced histamine release is both temperature- and calcium-dependent. Kinetic studies of release induced by PDS indicate that release is more rapid than that associated with IgE-dependent mechanisms. This platelet-derived, histamine-releasing activity is produced by platelet stimulation with collagen (5 micrograms/ml) and acetylglyceryl ether phosphorylcholine (10(-7)), as well as thrombin (1 U/ml). Initial characterization has shown that it is stable to acid and to freeze-thawing but not to boiling for 10 min. In addition, although this histamine-releasing activity is nondialyzable (i.e., greater than 3500 m.w.), it cannot be attributed to platelet factor 4. Thus, platelets, once activated, can produce a soluble substance or substances which can initiate basophil-mediated reactions, further suggesting that platelet activation can enhance allergic and inflammatory reactions.

Measurement of quality-of-life changes in patients with Alzheimer's disease.

A scale was developed that appears to be a reliable and sensitive indicator of differences and changes in the quality of life of patients with Alzheimer's disease (AD) as the disease progresses. The instrument was generated from interviews with spouses and/or caregivers of AD patients, and was designed to be completed easily by the caregiver. The bipolar analog scale was based on the daily behavior of AD patients and was developed in three steps involving interviews with caregivers (step I), testing of questionnaires, and validation of the instrument. At step II, four iterative trials, each with a different version of the questionnaire, were performed for validation. The final form of the Progressive Deterioration Scale (PDS) contained 27 quality-of-life factors and was cross-validated in a second, separate study. The Global Deterioration Scale (GDS) for cognitive function was used as the external criterion for the analysis of the quality-of-life scales. Statistical analysis indicates that the PDS provides a reliable, valid, and sensitive measure of changes in the quality of life as AD progresses. The PDS achieved 80% overall accuracy in discriminating non-AD elderly patients from patients in the early, middle, and late stages of AD. This scale may provide a useful means for clinical evaluation (phase III) of potential medications to alleviate the symptoms of AD.

Pyloric exclusion. Suture material of choice.

Pyloric exclusion was first reported in 1977 as a temporary method of providing duodenal decompression in an attempt to protect the duodenum in the early postoperative period during the healing phase, followed by the return of normal gastrointestinal transit. It has been adopted by several trauma centers across the country as part of their armamentarium for managing moderate to severe duodenal injuries. Most series report using a polyglycolic acid (PGA) suture in performing the exclusion, anticipating a 3- to 4-week interval before pyloric patency is re-established. A comparison of polypropylene (PP), polyglycolic acid (PGA), and polydioxanone (PDS) sutures in the dog model suggests, however, that only PDS reliably accomplishes this goal. The low (approximately 5-10%) but finite incidence of fistula formation reported with pyloric exclusion using PGA may be improved by using PDS instead.

[Resorbable implants for refixation of osteochondral fragments in joint surfaces].

Resorbable pins made of solid polydioxanone (PDS) were tested for refixation of osteochondral fragments. In both medial femoral condyles of sheep osteochondral fragments were chiselled and refixed by 3 PDS pins. After 3 months the sheep were sacrificed and healing of the fragments investigated histologically and biomechanically. Fragment dislocation was not seen in any of the cases and the cartilage surface could be described as normal. The strength of the bone healing zone reached 80% of the strength of the normal bone. The contact surface between fragment and subchondral bone showed complete bony healing. Only the border of the cartilage flake was still visible. The PDS pins had not been completely biodegraded after 3 months.

[Experimental evaluation of 4 different suture materials in colo-colic anastomoses (in the rat)].

The Authors carry out an experimental comparative study on four different suture materials (collagen cromic, polyglactin 910, PDS and nylon), used to perform end-to-end colonic anastomoses. They assess the resistence to the endoluminar pressure. The results show that during the first four days the anastomoses performed with PDS display a vere high resistence, while those with collagen cromic exhibit still lower value than Vicryl and nylon. After eleven days the anastomoses with the PDS show most resistence again, but the other three materials get similar value.

Effect of commercial peritoneal dialysis fluids on the lytic function of lymphokine-activated killer cells.

Intraperitoneal (ip) immunotherapies are often administered in saline or peritoneal dialysis solutions. In preparation for a Phase I i.p. interleukin-2/lymphokine-activated killer cell (IL-2/LAK) trial in patients with recurrent ovarian cancer, we evaluated the effect of six solutions, including two 1.5% dextrose peritoneal dialysis solutions (PDS), on the lytic function of LAK. In vitro IL-2-activated LAK cells were exposed to solutions, washed with Hanks' balanced salt solution, and lytic activity was measured in a standard 4-h chromium release assay using Daudi as a tumor target. LAK function was abrogated after 2 h of exposure to Inpersol and after only 20 min of exposure to Dianeal PD-2. Five percent dextrose also significantly decreased lytic capabilities of LAK, whereas 0.9% sodium chloride or lactated Ringers had no deleterious effect on function. Adjustment of PDS to pH greater than 6.5 decreased the damaging effect on LAK function, suggesting that the low pH of PDS and 5% dextrose results in a loss of LAK cell viability and therefore lytic function. Based on these data, we have chosen to administer IL-2/LAK i.p. in lactated Ringers.

Validity of self-report measures of girls' pubertal status.

To understand the meaning of somatic changes to the adolescent and their possible effects on behavior, developmentalists have begun to examine maturational timing and status in detail. Efforts have been hampered by the necessity of obtaining somatic growth data by physician examination. In the present study, 3 self-report methods for rating secondary sexual characteristic growth were compared to physician ratings; the accuracy of self-reported height and weight also was assessed. Specifically, 151 11-, 12-, and 13-year-old girls rated their breast and pubic hair development using schematics of the 5 Tanner stages and filled out the Pubertal Development Scale (PDS); their mothers also rated their daughters' development using the Tanner stage schematics. The correlation with physician Tanner ratings was .82 for self-ratings, .85 for ratings by the mother, and between .61 and .67 for self-reports on the PDS. The correlations for self- and actual reports of weight and height were .98 and .75, respectively. The usefulness of the PDS, Tanner ratings, and self-reported height and weight as adequate estimates of pubertal development is discussed.

[Use of absorbable suture material in dermatologic surgery].

Absorbable synthetic suture material (Vicryl, PDS) alone was used for wound closure in 1860 surgical interventions on the skin. The PDS suture is totally inert in intracutaneous tissue, and proved superior to the Vicryl suture. In combination, PDS sutures can also be used intracutaneously as well as conventionally. Additional advantages of absorbable material are the possibility of meandering and everted stitches in continuous intracutaneous sutures, no need for removal in most cases, and avoidance of intracutaneous scar dehiscences because of the good durability of the suture material.

Asymmetric pigment dispersion syndrome.

The hypothesis proposed by Campbell goes a long way to clarify the loss of pigment from the posterior iris pigment epithelium in PDS. However, the present series of cases of either asymmetric PDS (cases 1 to 3) or early developing PDS (cases 5 to 7) suggest that other factors appear to play a role in addition to mechanical rubbing. These other factors await further identification.

[Experimental study in the urinary tract of a new smooth surface absorbable synthetic monofilament (author's transl)].

The authors used 27 dogs to test the behaviour in the urinary tract of a new synthetic monofilament and absorbable suture material. Polydioxanone (PDS). The experiment was aimed at assessing the strength, ease of use, rate of absorption and lithogenic power of this material, in comparison with chromic catgut. Four loops of each material studied were inserted in the bladder, floating freely in the lumen. Absorption was evaluated by periodic cystoscopies. In the kidneys, one suture of each type was inserted into the excretory cavities by the transparenchymatous route. Development of a possible calculus was sought by urography and repeated plain X-rays of the abdomen. All the animals were killed at 6 months and routine pathological study made of the kidneys and bladder. Certain dogs were infected with proteus and subjected to fluid restriction. The lithogenic power of PDS and chromic catgut appeared to be identical. In contrast with urine, the absorption of chromic catgut by enzyme digestion was more rapid than that of polydioxanone by hydrolysis. By contrast, within the thickness of the tissues, absorption of the latter was more constant and more complete than that of chromic catgut. Polydioxanone caused a less marked tissue inflammatory reaction than the former. Scars were of equal strength. Finally, PDS suture material appears to be remarkably strong and very much easier to use than woven absorbable synthetic materials currently in use.

Activated human mononuclear phagocytes release a substance(s) that induces replication of quiescent human fibroblasts.

Human mononuclear phagocytes (HMP) treated in vitro with N-acetylmuramyl-L-alanyl-D-isoglutamine (MDP) released a soluble substance(s) that induced in vitro replication of subconfluent, quiescent, serum-free human fibroblasts. The response was enhanced if nonmitogenic plasma-derived serum (PDS) was added to the HMP-conditioned media after harvest. The capacity of serum-free, quiescent fibroblasts to respond to the HMP-released substance(s) diminished with time in culture. The diminished proliferative response was reversed by adding PDS to the MDP-HMP-conditioned media. Similar results were obtained in parallel experiments using platelet-derived growth factor (PDGF). These data suggest that the HMP-associated growth-promoting substances may, like PDGF, act co-ordinately with substances in PDS to stimulate the proliferation of cells in vitro.

Knotting abilities of a new absorbable monofilament suture: poliglecaprone 25 (Monocryl).

OBJECTIVE: To study the mechanical knotting abilities of the new synthetic absorbable monofilament suture poliglecaprone 25 (Monocryl) compared with other absorbable suture materials. DESIGN: Experimental laboratory study. MATERIAL: Knotted loops (3/0 USP) of five synthetic absorbable sutures: Monocryl (poliglecaprone 25), PDS-2 (polydioxanone), Maxon (polyglyconate), Vicryl (polyglactin-910) and Dexon-Plus (polyglycolic acid). Three different knots were tested in a total of 150 experiments. MAIN OUTCOME MEASURES: Mean loop holding capacity (LHC) of 10 experiments, expressed in Newtons (N). RESULTS: Monofilament Maxon tied significantly more secure knots than the other sutures. Sliding knots of Monocryl (poliglecaprone 25) were similar to, and three-throw square knots were less secure than those tied with monofilament PDS-2. Braided Dexon-Plus was superior to Monocryl for sliding knots but comparable for square knots. Braided Vicryl did not differ significantly from Monocryl. CONCLUSION: The mechanical knot performance of Monocryl is identical to that of Vicryl, less than that of one other synthetic absorbable monofilament suture (Maxon) and comparable to or somewhat less than the other alternative (PDS-2). These findings, together with other characteristics such as degradation rate, should be used to define the application of Monocryl.

Relationship between affective disorders and Axis II diagnoses in geropsychiatric patients.

Twenty-four geropsychiatric outpatients and 52 inpatients diagnosed with affective disorders (ADs) via psychiatric evaluation were administered the Structured Interview for Disorders of Personality-Revised (SIDP-R). A chart review yielded 1-year relapse rates for the inpatient sample. Data indicate a 63% personality disorder prevalence rate for the total sample, and a 21% relapse rate for inpatients. No significant differences were found in PD rates between elderly depressed and bipolar patients, and there were no differences in relapse rates for those with and without PDs. Results were discussed in relation to the sizable frequency of PDs in this elderly sample of ADs, the possible effects of severe depression on the personality ratings of older adults, and the differential prognostic value of PDs for geropsychiatric inpatients versus outpatients.

[Esthetic suture of the lower limbs after a saphenous vein graft in cardiac surgery. Polydioxanone vs Vicryl].

The vena saphena magna is widely used as a passage during myocardial revascularisation surgery (CABG). The preparation of the vein involves a long incision on the thigh and leg which is routinely closed using a continuous suture thread in Vicryl of the subcutis and continuous intradermal suture of the cutis. The authors retrospectively evaluated the functional and esthetic results of intradermal suture performed using two different types of reabsorbable thread: PDS II and Vicryl. A total of 178 patients underwent CABG surgery at the Heart Surgery Division of Tor Vergata University of Rome during the period January-September 1992. Mean age was 63 year +/- 7 (SD), 140 were males and 38 females. PDS II 3/0 was used for intradermal suture in 88 patients, whereas Vicryl 3/0 was used in 90. There were no significant differences between the two groups with regard to age, sex, number of grafts, associated pathologies or wound length. After 1-9 months (mean 5.6 months) the surgical wound on the saphenectomized limb was evaluated. It was found that compared to Vicryl the use of PDS II presented a higher incidence of keloids (p > 0.05). This was probably due to the greater reaction provoked in the dermis by the monofilament compared to twisted thread. The authors conclude that the use of a reabsorbable twisted thread, such as Vicryl or Vicryl Rapid, is preferable owing to the higher rate of compliance and lower incidence of complications.

Homeodynamics versus homeostasis: periodicities superimposed on non-linear dynamic sympathetic tone generated in ventral medulla.

Homeodynamics based on theories of complexity and chaos and its impact on mechanisms generating sympathetic activity are presented. Activity in rats cervical, lumbar and renal sympathetic nerves was analyzed. In time domain glutamate stimulation of neurons within medullary periambigual area (PAA) disturbed temporal pattern of respiratory-sympathetic synchronization. Divalént calcium antagonists, Co2+ and Mg2+, blockers of synaptic transmission, uncoupled respiratory oscillator and sympathetic activity. PAA neurons act as an interphase between different subsets of respiratory neurons and bulbospinal sympathoexcitatory neurons in rostral ventrolateral medulla (RVLM). In frequency domain sympathetic activity analyzed by FFT algorithm and power density spectra (PDS) exhibited periodicities at the range from 0.4 Hz to 7.5 Hz. Blockers of synaptic transmission microinjected bilaterally into RVLM reduced total power exhibited in PDS to low level of magnitude generated in spinal cord and increased total, yet non-synchronized sympathetic activity and arterial blood pressure. A two component hybrid model of generation of sympathetic activity was proposed: a tone-generating system confined mainly to intrinsic activity of RVLM pacemaker neurons responsible for chaos-like discharges and a second component-neuronal circuits superimposed on tone-generating neurons and shaping the pattern of PDS. Contribution of spinal cord oscillatory mechanism to overall power of sympathetic periodicities was discussed.

[Comparison of surgical techniques and various suture materials for correction of aortic isthmus stenosis].

UNLABELLED: Clinical examination and Doppler ultrasound were performed in 31 children after repair of coarctation of the aorta. Median postoperative follow-up period was 4.5 years. The aim of our study was a comparison of different operation techniques and suture materials. In 16 infants subclavian flap repair had been performed using polydioxanone absorbable sutures (PDS) in 8 cases and polypropylene (Prolene) sutures in the other 8 cases. Resection and end-to-end repair had been carried out in 8 infants using PDS and in 7 using Prolene sutures. Doppler-echocardiographically derived gradients across the reconstructed aorta were significantly lower in infants operated with the subclavian flap technique (p < 0.05). The length of the arm on the side of the subclavian flap operation was shorter (median 1.2 cm), but there were no signs of ischaemic complications. Using PDS sutures the aortic arch and the aortic isthmus were each morphologically significantly wider in both operation techniques. Noninvasive two-dimensional echocardiography demonstrated good anatomical repair and no anastomotic aneurysm formation after aortic repair using polydioxanone. CONCLUSION: Regarding the doppler-echocardiographically derived gradients in the anastomotic region this intermediate follow-up study reveals better results using the subclavian flap technique; absorbable polydioxanone sutures favour normal growth of the anastomotic site without vascular complications.
