Aortic root pressure and flow data can be used to assess left ventricular (LV) performance and properties of the systemic arterial tree. The calibrated subclavian arterial pulse trace was combined with echocardiographic imaging and Doppler velocity recordings to obtain noninvasive estimates of aortic root pressure and flow in 8 healthy subjects (group A), 12 patients with recent myocardial infarction (group B), and 8 with healed myocardial infarction and a dilated left ventricle (group C). The pressure and flow data were transferred to a computer and processed in specially designed software, including a new procedure for estimation of 3-element windkessel model parameters. There were no significant group differences for either aortic root pressure estimates or heart rate. In groups B and C, stroke and cardiac indexes were lower and total peripheral resistance higher than in group A. There were no group differences in the model estimates of total arterial compliance, whereas the characteristic impedance was greater in group C than in A, indicating a less compliant aorta in C. Both LV total and steady power were less in groups B and C than in A, whereas no group difference was found for percent oscillatory power. The reproducibility for recording was good for the aortic root pressure estimates, and lower for the derived parameters (stroke and cardiac indexes, windkessel model parameters and LV power), whereas that for interpretation was generally good. This method provides a unique noninvasive access to important parameters of LV function and the systemic circulation.

The plasma concentrations for unconjugated and conjugated oestrone, oestradiol-17 beta and oestriol, were measured in 25 male patients with chronic alcoholism and 25 healthy male blood donors of the same age. All persons had normal weight and were free of medication. The patients with chronic alcoholism consisted of 15 persons with and 10 without hepatomegaly. Liver biopsy was performed in the persons with hepatomegaly. Unconjugated oestrogens were separated on LH-20 micro columns after extraction with diethyl ether. Oestrogen sulphates were extracted and separated after hydrolysis with sulphatase, and glucuronides were extracted and separated after hydrolysis with glucuronidase. In the patients with hepatomegaly the plasma concentrations of unconjugated oestrone, oestradiol-17 beta and oestriol were significantly increased, and the plasma levels for the same oestrogen sulphates were significantly decreased, whereas a moderate increase was observed for oestriol glucuronide. The patients without hepatomegaly revealed a significantly lower plasma concentration for oestrone sulphate, whereas no difference was seen for the remaining oestrogen components. The ratio oestrone sulphate to oestrone was significantly reduced in both groups of patients. A significant decrease for the ratio oestradiol sulphate to oestradiol and oestriol sulphate to oestriol was seen only in the patients with hepatomegaly.

During January and February 2003, 20 non-treatment seeking homeless, substance-using MSM accessing community-based prevention services in West Hollywood, California were assessed to characterize demographics, addiction and psychiatric severity using structured and semi-structured clinical interviews, and high-risk drug and sexual behavior. Participants averaged 37 years old, were mostly Caucasian/white (65%) and most identified as bisexual (58%) or gay (37%). Self-reported HIV seroprevalence was 21%. Most (53%) exchanged sex for money and/or drugs within the previous 30 days. All were diagnosed with current DSM-IV Substance Dependence Disorders, primarily alcohol (62%), amphetamine (57%), cocaine (52%), and marijuana (38%). Participants reported many (35.7) positive psychiatric symptoms on the Brief Symptom Inventory and averaged moderate Beck Depression Inventory scores (19.1). Most (75%) met criteria for Mood Disorder, 33% for Major Depressive Disorder, 43% for Antisocial Personality Disorder and averaged low (48.5) Global Assessment of Functioning scores. While the small convenience sample limits generalizability of the findings, these data provide an opportunity to gain insight into this at-risk population and, thereby, assess appropriate intervention strategies.
