Spontaneous Clostridium perfringens lung abscess unresponsive to penicillin.

Clostridial necrotising pneumonia is a rare complication of aspiration, bronchial tumour or foreign body, pulmonary infarction, trauma and debilitating medical conditions. Although spontaneous clostridial pneumonia has been reported previously, close scrutiny of those case reports suggests that most of the patients had a recognised predisposing cause. We report a case of true spontaneous Clostridium perfringens pneumonia complicated by septic shock, pneumothorax and pulmonary necrosis. The patient responded poorly to conventional treatment with benzylpenicillin, and although the addition of metronidazole produced dramatic resolution of the sepsis, lobectomy was required to effect cure.

[Rehabilitation of patients and prevention of invalidism after surgical treatment of lung abscess].

The number of operated on patients who are referred to a commission for expert medical assessment of working capacity is growing at present. Correct expert assessment of the working capacity allows the results of surgery to be reliably consolidated and promotes restoration of working ability. The duration and severity of working ability. The duration and severity of the pulmonary abscess, the method of surgical treatment, the lapse of time after the operation, the course of the restorative processes, complications and concomitant diseases, the degree or respiratory and circulatory insufficiency, the patients' age, profession, and the conditions and character of work are taken into account during examination. Timely and complete treatment and rational choice of occupation are important factors in the prevention of invalidism and enable most patients (86.8%) to resume work in full volume or with limitations as specified by the commission.

[Immunologic disorders in the pathogenesis of acute lung abscesses].

Immune response was investigated in patients with acute pulmonary abscesses (APA) treated according to the discrete-dynamic scheme. The study of pathogenetic significance of immune response and chronic alcohol intoxication in APA demonstrated that failure of antiinfectious resistance of the body is one of the factors responsible for the onset, course and outcome of APA. This failure is characterized by unbalance of immune parameters, strain of immune system which is more pronounced in the presence of chronic alcoholic intoxication. It is evident that suppression of immune system shown in APA patients is an underlying cause of immunity deficiency in them.

[Clinical report of an aged patient with lung abscess who show an excellent response after long-term administration of amikacin (author's transl)].

UNLABELLED: The patient we report here is a 71 year-old female. After hospitalization, she had clouding of consciousness as an accompanying symptom of DIC (disseminated intravascular coagulation) complicated by septicemia. Her clouding of consciousness caused aspiration pneumonia which developed into pulmonary abscess. Furthermore, she had a large decubital ulcer on the gluteal region. Sputum cultures revealed P. aeruginosa, Serratia and Klebsiella and those from decubital pus grew Proteus, showing to be a mixed infection due to Gram-negative bacilli which brings on medical discussion at present. To the patient, repeated intramuscular AMK was administered at a daily dose of 200 mg ( DURATION: 124 days, Total amount: 24,800 mg). Chest X-P showed a marked improvement. Similarly, the large decubital ulcer was improved. No side-effects were observed in the kidney and eighth nerve though she was a patient with the advanced age of 71. It is thought, therefore, that patients can tolerate long-term treatment with AMK.

[3 cases of hematogenous lung abscess of amebic origin].

Invasive amebiasis is a very serious health problem in Mexico as it is presumably related to the presence of virulent strains of Entamoeba histolytica and poor hygienic and sanitary conditions; other factors related to invasive amebiasis are undernutrition, alcoholism, and homosexuality. We present three patients with pulmonary amebic hematogenous abscess. Clinically all patients had the typical "chocolate" exudate. The three patients had pulmonary consolidations by chest roentgenogram; one of them had multiple opacities with air fluid level, and the others had an isolated opacity with air fluid level. The ultrasound and hepatogammagram were negative for diaphragmatic communication in all; in one of them the pneumoperitoneum was negative for diaphragmatic communication. The transthoracic needle biopsy of the lesions was positive to ameba in two patients. The serologic tests were positive in all. We treated the patients with metronidazole and emetine during 10 days; since the clinical picture and the radiologic findings did not remit completely, we gave a second course of metronidazole during 10 days more and achieved complete resolution.

[The efficacy of the natural antibacterial preparation chlorophyllypt in the combined treatment of acute lung abscesses].

The complexity of the treatment of acute suppurative pulmonary diseases has been aggravated recently by the growth of microbial resistance to antibiotics and enhancement of the allergy incidence among the population. This circumstance restricts the use of antibiotic on a broad scale in clinical practice and makes the researchers go in studies of adequate substitutes. In the given case, use was made of chlorophyllipt. It was given to patients by intravenous drip in the form of a 0.25% solution based on saline twice a day. To increase local deposition of the administered antimicrobial drug in the affected area, interstitial electrophoresis was employed. In the patients' group on chlorophyllipt, the clinico-laboratory and x-ray parameters returned to normal earlier. Chlorophyllipt was noted to produce an immunocorrective action characterized by the normalization of the absolute count and percentage of E-RFC and theophylline-resistant-RFC. This action could not be observed in the patients' group receiving only antibiotics.

Lung abscess due to retained gallstones with an adenocarcinoma.

We describe a patient who had a right lower lobe mass containing calcifications consistent with gallstones develop 3(1)/(2) years after laparoscopic cholecystectomy. Thoracotomy revealed a chronic abscess containing pigmented gallstones and an adjacent area of bronchoalveolar adenocarcinoma involving both N1 and N2 lymph nodes.
