[Hip near osteosynthesis, total hip replacement and prevention of thromboembolism (author's transl)].

The effect of physiotherapy and pharmacological treatment in preventing thromboembolic disease at hip-near operations was compared from 1971 to 1978 in 787 patients being more than 60 years old. Physiotherapy alone and its completion by Dextran and Acetyl-salicylic-acid turned out to be insufficient. It was only by low-dose Heparin in combination with Dihydroergotamin (Dihydergot) and continuing physiotherapy that the rate of thromboembolism could be lowered effectively.

[Tomodensitometric aspects of hip luxation in childhood. Apropos of 100 hips].

One hundred hips in children were examined by computer assisted tomography. They were either dysplastic, subluxated or dislocated. The study resulted in more precision as to the degree of femoral anteversion and acetabular anteversion. The indications for femoral or pelvic osteotomy can thus be more accurate. Mechanical effects of some pelvic osteotomies have been analysed.

The development of dermatitis (scabby-hip) on the hip and thigh of broiler chickens.

Broiler chicks grown on litter or in cages at high bird density developed skin lesions on the thigh and hip. Changes in bird denisty markedly influenced lesion incidence (100% at 0.0185 sq m of floor space per bird). The dermatitis was characterized by crusted dry "scabs" at the base of feather follicles and between follicles. The lesions often coalesced to cover wide areas. The scab consisted of a mass of pyknotic nuclei and cellular debris, and when the epidermis was intact there was little inflammatory reaction in the dermis or subcutaneous tissue. The epidermis was destroyed in some lesions, with heterophils penetrating into the subcutaneous tissue. Colonies of gram-positive cocci were present in the lesions.

[Value of computed tomography in congenital hip dysplasia and hip luxation].

A lot of methods of measurement have been developed to record precisely the degrees of luxation and the deviations from the normal form in the hip joints of the child and adult. The orthopaedic surgeon needs those data for a preoperative "coxometric program" before performing acetabuloplastic operations with osteotomy, osteotomy of the pelvis and combined operations. Computed tomography yields a series of informations and of possibilities of measurements which can determine the operative procedure in particular. These informations concern among other things the dimensions of the dysplasia of the acetabulum in the horizontal plane with the extension of the posterior lip and the angle of the acetabular opening, furthermore the physiological or pathological congruence between the head of the femur and the acetabulum, the angle of antetorsion of the neck of the femur and, in the small infant, the direct noninvasive imaging of the tube of the capsule with imaging of possible obstacles to reposition.

The two- to six-year results with a proximally modular noncemented total hip replacement used in hip revisions.

A review of the two- to six-year results of revision cases using a proximally modular noncemented total hip replacement has been carried out. There were 91 cases. A primary stem was used in 29 and a long curved revision calcar replacement stem in 62. The results of the primary cases were 80% excellent, 6% good, 9% fair, and 5% poor. In the revision stem cases, 69.4% were excellent, 11.5% good, 7.6% fair, and 11.5% poor. One primary stem case was revised for acetabular migration. Ten of the revision stem cases were rerevised, four for femoral perforation, three for sepsis reactivation, one for acetabular migration, one acetabular component for dislocation, and one stem for trochanteric nonunion. There have been no stems or sleeve fractures, no implant dissociations and no osteolysis. In those reexplored, no metallosis has been found. Only three cases required structural allografting on the femoral side. These early results are promising and no untoward reactions have occurred as the result of the use of a modular stem.

Prophylaxis with systematic antibiotics versus gentamicin bone cement in total hip arthroplasty. A ten-year survey of 1,688 hips.

In 1976, nine Swedish orthopedic departments started a prospective, randomized, and controlled study in which the prophylactic effect of systematic antibiotics (SAs) was compared with gentamicin bone cement (GBC) in 1,688 consecutive total hip arthroplasties (THAs). After ten years, 13 deep infections had occurred in the SA group and nine in the GBC group. The earlier reported significant difference at two and five years in favor of GBC no longer existed. No allergic or toxic reactions have been reported in the GBC group.
