[Thumb reconstruction by means of transposition of a damaged little finger and interposition splint after an explosion injury to the hand].

This case report concerns the loss of the thumb, the middle and ring fingers as well as the distal phalanx of the little finger as a result of an explosion. A combined procedure for the reconstruction of a thumb was performed employing interposition lengthening of the first metacarpal by an iliac crest bone graft followed by the transposition of the distal part of the remainder of the little finger. The proximal portion of that digit was preserved in order to obtain better span grasp.

[Intraoperative explosive choroidal hemorrhage (report of four cases)].

Intraoperative explosive choroidal hemorrhage (ECH) is a rare, but severe complication of intraocular surgery. This complication often occurs during or after intraocular surgery. We report four cases that developed this complication during surgery. Once the complication occurs, we should use the dehydration medicine intravenously, suture the incision, application of pressure directly to the eye and incise the postsclera. If the bleeding can't be stopped, enucleation is the indication.

[Treatment strategy in explosion and gunshot injuries of the upper extremity].

Explosion and gunshot injuries represent a challenging problem with regard to preserve optimal hand function. These wounds exhibit a spectrum of complexity and mostly include extensive soft tissue trauma complicated by burns, foreign bodies, fractures and concomitant trauma. To gain maximal restoration of hand function the use of microsurgical technique, grafting of nerves, vessels and soft tissue as an acute free flap to cover a large defect immediately is necessary. The experience in treatment of 38 cases of blast and gunshot injuries of the hand are reported. The results are discussed in relation to literature, complication, indication and technique.

Changes in physical fitness profile in female basketball players during the competitive season including explosive type strength training.

Ten female basketball players served as subjects in order to examine changes in a physical fitness profile during a 22-week official competitive season. Specific explosive type strength training (1-2 sessions per week) was utilized throughout the season. The present findings showed that the entire competitive season led to no systematic changes in the maximum oxygen uptake (from 48.0 +/- 6.6 to 47.0 +/- 6.0 ml.kg-1.min-1), in anthropometric characteristics or in maximal isometric force of the leg extensor muscles (from 2567 +/- 490 to 2622 +/- 747 N). However, significant (p < 0.05) increases occurred during the season both in the average power output during the first 15 s work in an anaerobic jumping test and in the maximal vertical jumping heights in the squat jump (from 21.7 +/- 2.3 to 24.2 +/- 2.4 cm) and in the counter movement jump (from 24.9 +/- 2.6 to 26.3 +/- 2.9 cm). A considerable change occurred also in the shape of the isometric force-time curve of the leg extensor muscles so that the times to produce submaximal force level shortened (p < 0.05). The individual changes during the competitive season both in the power output and in the times of rapid force production correlated negatively (p < 0.05) with the individual initial values recorded before the season. The present findings suggest that the prolonged competitive season in the present female basketball players seemingly placed sufficient demands on the athletes in energy production through aerobic processes to maintain their level of VO2max.(ABSTRACT TRUNCATED AT 250 WORDS)

[Malignant sacrococcygeal teratoma: report of a case of "explosive" intrauterine growth].

An exceptional case of fetal sacrococcygeal teratoma with an exclusive abdominopelvic growth is described. The tumor was not detected on a "routine" ultrasound study obtained at 31 weeks of gestation showing findings of a severe congenital uropathy. It remained unrecognized until the 34 weeks of gestation when the mass began an explosive growth causing a rectouretral fistula. Despite appropriate obstetrical and surgical management the baby died 24 hours later because inadequate pulmonary maturity. The istological findings showing malignant elements such as embryonal carcinomas.

Tire explosion injuries to the upper extremity.

We describe a protocol for treating tire explosion injuries to the hands on the basis of treatment of five cases over a 3-year period at Emory University Hospital. These high-energy wounds frequently require emergent initial debridement and stabilization, followed by complex soft tissue and skeletal reconstruction to maximize function. Data from all cases are summarized to show similarities among these injuries and differences in their response to treatment. We define the stages of treatment, emphasizing prevention, complete assessment, and early intervention, especially for limb-threatening conditions with fasciotomy, revascularization, thorough debridement, and rigid fixation. We also discuss the importance of aggressive therapy, close follow-up, and treatment of late complications. Functional reconstruction was achieved in each case, suggesting that our treatment protocol can be used successfully.

[The microflora of gunshot and explosive mine wounds in victims delayed for a long time at the prehospital stage].

The clinico-microbiological research of gunshot and mine-explosive injuries in wounded servicemen of Afghanistan army, arriving on stage of the qualified surgical care for 2-5 days after wound is lead. The quantitative and qualitative structure of wound microflora at modern combat trauma on a prehospital stage is determined. It was established, that main kinds of wound infectious complications in injuries are suppuration of wounds and clostridial infection. The advantages and the lacks of benzyl penicillin for prophylaxis of wound infection are investigated.

Modeling of erbium: YAG laser-mediated explosive photovaporization: implications for vitreoretinal surgery.

BACKGROUND AND OBJECTIVE: Motivated by the potential for highly precise tissue removal, investigators are exploring fiberoptic microsurgical maneuvers with the erbium:YAG (Er:YAG) laser. Tissue is disrupted and removed by direct ablation and the acousto-mechanical sequelae of explosive vaporization of the tissue water. The authors investigated the scaling laws for photoablative and photodisruptive interactions and interpreted these results to optimize energy delivery for vitreoretinal surgical maneuvers. MATERIALS AND METHODS: A model for laser-generated-bubble expansion is presented based on energy principles and adiabatic gas expansion. Comparisons are made with the authors' previous studies of ablation rate and thermometry. RESULTS: The authors' modeling studies generated predictions similar to experimental data. The maximum bubble diameter increases as the cube root of the pulse energy. At constant radiant exposure, the maximum bubble diameter increases as the probe tip diameter raised to the two-thirds power. The authors demonstrated that tissue ablation depends on radiant exposure (J/cm2), whereas temperature increases, bubble size, and peak pressure depend on total pulse energy. CONCLUSIONS: Mechanical injury should be minimized and efficient ablation preserved by delivering low-pulse energy through small-diameter probe tips at high repetition rates.

The Peak Flow Working Group: test of portable peak flow meters by explosive decompression.

In 1991, 50 new Vitalograph peak flow meters and 27 previously used mini-Wright peak flow meters were tested at three peak flows by use of a calibrator applying explosive decompression. The mini-Wright peak flow meters were also compared with eight new meters. For both makes of meter there was an excellent within-meter and between-meter variation. The accuracy, however, was poor, with a maximal overestimation of true flows of 50 and 70 L.min-1 in the interval from 200 to 400 L.min-1 for the Vitalograph and mini-Wright meters, respectively. The deviation is explained by the physical characteristics of the variable orifice peak flow meters. They have been supplied with equidistant scales, which give non-linear readings.

Risk of late perforation in intestinal contusions caused by explosive blast.

BACKGROUND: Despite the predominance of superficial injuries after explosive blast exposure, major morbidity or mortality among immediate survivors is caused by delayed perforation of intestinal mural contusions. Previous studies have suggested that small bowel and colonic contusions larger than 10 mm in diameter are at high risk. This experimental study aimed to identify contusions at high risk of late perforation. METHODS: Histological features of injury were classified in 188 blast-induced intestinal contusions in 16 anaesthetized Large White pigs. RESULTS: Some 16 per cent of small bowel and 12 per cent of colonic contusions were at high risk of late perforation. Small bowel contusions larger than 15 mm in diameter had a worse histological grading than those smaller than 15 mm (chi 2 = 0.09, 2 d.f., P = 0.01). Contusions that extended over more than half the bowel circumference (chi 2 = 14.79, 2 d.f., P = 0.0006) and those affecting the mesenteric border (chi 2 = 7.5, 2 d.f., P = 0.024) were more severe injuries. Colonic contusions larger than 20 mm in diameter had a worse histological grading than smaller ones (chi 2 = 14.95, 2 d.f., P = 0.0006). Confluent, rather than diffuse, colonic contusions were more severe injuries (chi 2 = 6.37, 2 d.f., P = 0.04). CONCLUSION: Once identified at laparotomy, the number of small bowel contusions requiring excision may be reduced from 86 to 60 per cent; similarly, excision of colonic contusions can be reduced from 73 to 27 per cent if small bowel contusions smaller than 15 mm in diameter and colonic contusions of less than 20 mm are managed conservatively.

An explosive outbreak of Q-fever in Jedl'ové Kostol'any, Slovakia.

An explosive epidemic of Q-fever that occurred at Jedl'ové Kostol'any (Nitra District) in April 1993, had an unusual mode of transmission, unprecedented in Slovakia. The submitted case-reports can be very instructive for both health workers and the lay public. The bulk of infection was spread in the local pub through contaminated garments of animal attendants assisting abortions and births of goats in a large capacity breeding centre of Gemersan Co. By their repeated visits to the local pub the infection most probably spread to other guests by aerosol. A total of 113 persons (103 males, 10 females) contracted Q-fever. Out of them 95 were infected by contact with the goat attendants (84%), and 18 were occupational diseases after direct contact with the infected goats. The human epidemic of Q-fever facilitated to trace an ongoing epizootic of coxiellosis in the herd of goats of Gemersan breed. In order to stop further spread of infection among humans and to contain the outbreak of coxiellosis among animals extensive epidemic and epizootic measures were taken.
