[Relapsing parasitic embolism of the pulmonary artery in hepatic echinococcosis].

A case of multiple echinococcosis of the liver in a woman of 52 complicated by perforation of the parasite cyst into the right hepatic vein, repeated parasitic embolism of the pulmonary artery and anaphylactic shock is presented. A set of changes in the lungs in this process is described: giant-cell arteritis and thromboarteritis, hemorrhagic infarctions, giant-cell reaction in the reticular definite tissue as well as sarcoid reaction in bronchopulmonary and tracheo-bronchial lymph nodes.

[Cardiac echinococcosis. Two-dimensional echocardiographic diagnosis (author's transl)].

A case of cardiac echinococcosis is reported on a 7 year old girl, whose clinical presenting picture was an embolism of the lower limbs. M-mode echocardiography didn't reveal pathological data, whereas two-dimensional (2D) examination in apical position (four-chamber view) diagnosed a cystic structure in the left ventricular free wall. Cardiac surgery confirmed the echocardiographic findings. The necessity to perform all the 2-D sections when there is a clinical suspicion, the possibility to avoid cardiac catheterization and the use of 2-D echocardiography as a screening test in the exposed subjects are emphasized.

Pulmonary echinococcosis: a pediatric disease of the Southwestern United States.

Three cases of pulmonary echinococcosis in native American children emphasize the need to consider the diagnosis in the differential diagnosis of mass lesions of the chest when the appropriate environmental conditions are present. Two children presented with a non-productive cough, while one was asymptomatic. Surgery is the treatment of choice for pulmonary echinococcal cysts. All three children did well after thoracotomy, and one child has a normal roentgenogram six years after surgery. Recent evidence seems to indicate that transmission of the Echinococcus among native Americans in the Southwest is increasing, and thus human echinococcal disease may be of increasing clinical importance.

[A new method for the surgical treatment of hepatic echinococcosis].

A procedure for operative management of liver echinococcosis (hydatid disease) is implemented in practice, consisting in resection of the "protruding dome", resection of the altered and thinned surrounding liver tissue, and omentoplasty using tissue adhesive. The method proposed has definite advantages, namely: readily performed with minimal traumatism, the likelihood of a nonparasitic cyst formation is ruled out, shortened postoperative period and quicker recovery of the patients with resuming of the previous jobs. The indications and contraindications for performing the operation, and possible postoperative complications are discussed.

Vertebral localization of echinococcosis (case report with medullary compression).

A case is presented of cord compression due to vertebral echinococcosis which relapsed after a first laminectomy elsewhere. The authors carried out a second excision with decompression from the anterior approach, supplemented by grafting. After a period of recovery and well being, relapse occurred with renewal of the cord compression. A second decompressive laminectomy and excision of the relapsed cyst was carried out. The present case illustrates the limitations of surgery in this parasitology, because of the extreme liability to relapses.

[Isolated echinococcosis of the lung].

This is a report of a 81 year old women with a cyst of echinococcus granulosus, that was almost completely taking over the lower lobe of the left lung. This case study shows on one hand the diagnosis of echinococcosis is a part of the differential diagnosis even for an elderly patient. On the other hand we were able to demonstrate, that elderly patients can benefit from the surgical treatment especially since medication has a lot of side effects and a fairly high rate of failures.

Pulmonary echinococcosis--a case report.

Pulmonary echinococcosis is a rare disease in Taiwan and has never been reported in Taiwan's literatures. Here we report an overseas Chinese from Calcutta, India, presenting with recurrent hemoptysis and chest pain and with two big lung masses on chest roentgenogram. Rupture and spike fever ensured after fine needle aspiration of the cysts. Successful surgical intervention was obtained and echinococcal infection was documented pathologically.

[Isolated echinococcosis of the left hepatic lobe].

The authors analyse 47 patients with isolated echinococcosis of the left hepatic lobe. There were 36 males and 11 females. One cyst was found in 39 patients, two cysts in 8 patients. The volume and character of the operative interventions were determined by the peculiarities of the case and its complications. Cholectomy together with the parasite but without opening the cavity was conducted in 8 patients with cirrhosis and deficiency of the left lobe. In rupture of the cysts into the bile ducts echinococcectomy was combined with cleansing and drainage of the ducts: retrograde drainage after Abu proved to be the best method. Wedge resection was conducted in 7 patients and thermal treatment in 8 patients. In the remaining cases various methods of echinococcectomy were applied.

Echinococcus multilocularis (Cestoda), the causative agent of alveolar echinococcosis in humans: first record in Poland.

Echinococcus multilocularis, the causative agent of human alveolar echinococcosis, was found for the first time in Poland in 2 of 20 red foxes (Vulpes vulpes), originating from the area of Gdansk (northern Poland). This finding supports the hypothesis that the endemic area of this parasite in central Europe (Belgium, France, Luxembourg, Switzerland, Liechtenstein, Germany, and Austria) is not an isolated focus as previously anticipated but is connected with the large endemic zone in Russia.

[Treatment of 842 cases with cystic echinococcosis of chest].

842 cases of thoracic echinococcosis were surgically treated by endocystectomy (79%), pneumonectomy (12%), drainage and other operations (9%). Among a total of 1,010 operations, the mortality was 0.6% (5/807). Of 637 cases received 798 endocystectomies, the recurrence rate was 4.2% (27/638). Up to 1979, the first series of 323 cases were followed-up for 3-20 years, no single case died of thoracic echnococcosis. Among the 32 non-operation cases, the mortality was 22.2% (6/27) and 9 cases were spontaneously cured. 22 cases were given praziquantel at two dosages, Group A received 120-150 mg/kg and Group B received 210 mg/kg in divided doses for 5-6 days. The death rate of protoscolices inside the operatively removed cystic fluid from the above two groups was 43.1 +/- 9.6% and 64.2 +/- 3.5%, respectively, while in the untreated Group C 9.2 +/- 2% (P < 0.01). With total dosage of praziquantel 300 mg/kg in divided doses for 7 days, the concentration of praziquantel in patient's blood and cystic fluid was 0.49 +/- 0.075 microgram/ml and 0.006 microgram/ml, respectively.

[A clinicopathological analysis of 70 cases of liver alveolar echinococcosis with a comment of the prognostic factors].

As to the geographical distribution of the 70 cases of liver alveolar echinococcosis in the present series, the highest incidence was found in Gansu province (82.8%), where most of the patients came from Zhang county (65.6%). Female outnumbered slightly male. Most patients were young and in robust years of their life (98.8%). The most frequent symptom was liver mass (56.4%) and the next abdominal pain (28.2%). Imaging examination and immunological test were helpful to the diagnosis. Under microscopic observation the hyperplastic bud from germinal membrane of liver alveococcus cyst wall comprised a few nuclei and showed exogenous or endogenous budding. According to whether alveococcus proliferation was present or not, pathological types of regressive grade I and hyperplastic grade II were discerned. Resection was not done in 34 cases during exploratory laparotomy, but drainage was performed in 8 cases for alveococcus necrotic pseudocyst and cholecstostomy was done in one case for jaundice. Albendazole or/and traditional Chinese medicine were administered in 35 cases. The prognosis of the patients in relevant to some factors such as sex, age, pathology, symptom and treatment was discussed.

Cellular localisations of interleukin-1 beta, interleukin-6 and tumor necrosis factor-alpha mRNA in a parasitic granulomatous disease of the liver, alveolar echinococcosis.

Alveolar echinococcosis (AE), an uncommon and very severe parasitic liver disease, can be considered as an "infectious model" of granulomatous disease, where cellular immunity plays a key role in the defence against Echinococcus multilocularis, the larval cestode responsible for the disease. We analysed the localisation of the expression of the pro-inflammatory cytokines IL-1 beta, IL-6 and TNF-alpha mRNA in human AE liver lesions, in the periparasitic granulomas and in the hepatic parenchyma, as well as the phenotypic characteristics of the cells on serial sections. In situ hybridizations, using anti-sense 35S dUTP-labeled IL-1 beta, IL-6 and TNF-alpha riboprobes, were performed on cryostat liver sections; the sense probes were used as negative controls. IL-1 beta, IL-6 and TNF-alpha mRNA were observed in macrophages located at the extreme periphery of the granuloma, between the lymphocytic ring and the liver parenchyma, in patients with active AE. No cytokine mRNA expression was observed in a patient with an abortive case. Only TNF-alpha mRNA was located in the periparasitic area, in cells morphologically identified as macrophages but exhibiting an unusual phenotype (CD 11b-, CD 25+); this particular expression was observed only in those patients with very fertile lesions, associated with centro-granulomatous necrosis. These results show that pro-inflammatory cytokines are consistently produced by macrophages at the periphery of the periparasitic granuloma and can serve as mediators of acute-phase protein secretion and of fibrogenesis in that location.(ABSTRACT TRUNCATED AT 250 WORDS)

[Retrospective study of echinococcosis in the Zadar region from 1960 to 1990].

Seven hundred and thirty-six patients with human echinococcosis admitted to the Zadar General Hospital between 1960 and 1990 were studied. Seven hundred and seventeen patients were residents of Zadar region, while 19 were referred from other areas. There were more females (61%) than males (39%) in our study, with the majority of patients (208 individuals) being in the age group of 60 years and more. Among the examinees, the largest number were housewives (42%). 59.1 percent of echinococcus was found in the liver, 22.7% in the lungs and 3.4% in the spleen. There was a significant difference between surgically treated and untreated patients (p < 0.01) as well as between cured and uncured ones (p < 0.01). Over the studied period, there were approximately 24 new cases and 23.74, respectively. The incidence rate per 100,000 inhabitants for 1990 was 12.74. The decrease of incidence was 83.13%. The prognosis made in 1978 for specific morbidity in 1990 was 9.051 approximately 9, and 3.682 approximately 4 for realized specific morbidity. In 1990, the density of population in Zadar region was 68.2 inhabitants/km2. The number of inhabitants per 1 km2 on the mainland within the same year was 79.9, while on the islands it was 29.2 inhabitants/km2. In order to make a comparison, density of population in 1980 was calculated. The number of population amounted to 66.1 inhabitants/km2. In 1980, there were 75.8 inhabitants/km2 on the mainland, and 35.2 inhabitants per 1 km2 on the islands of the Zadar region. In 1990 the number of inhabitants on the islands decreased, although the overall number of inhabitants of that region increased.

[Experimental chemotherapy of echinococcosis. 13. The effect of mebendazole in combination with a vegetable oil on the larval cysts of Echinococcus multilocularis and the reaction of the infested host to long-term administration of the preparation].

The use of mebendazole as a suspension in vegetable oil enhanced the efficiency of treating experimental Echinococcus granulosus infection in outbred albino rats without increasing the dose of the drug. A sharp reduction in platelet count in the blood of the infected animals treated and untreated suggests that thrombohemorrhagic complications might be in echinococcosis. Thrombopenia in the treated animals appeared to be steady-state. A decrease in a rapid immune response in the mebendazole-treated rats, as compared to the untreated ones, is likely to be a consequence of a significant fall of antigenic immune stimulation due to suppression of the parasite's viability with the drug and to be an indicator of therapeutical efficiency.

[Human echinococcosis: the role of histocompatibility antigens in realizing infestations and the characteristics of their course].

The role of histocompatibility antigens in the realization of invasions and the nature of its course were studied in 71 patients with Echinococcus granulosus infection and in 18 with E. multilocularis infection. It has been shown that the effects of HLA antigens on the realization of invasion is heterodirectional; the factors predisposing to the occurrence of echinococcosis are HLA-B5, B18, and A28 antigens, whereas those preventing their realization are HLA-B14 antigen. The carriage of HLA-B5 and B18 antigens determines a high risk of E. granulosus infection of the lungs and liver. The realization of the latter is prevented by the carriage of HLA-B27 antigen. The development of E. multilocularis infection of the liver is promoted by that of HLA-A11, whereas the carriage of HLA-A9 antigen prevents the occurrence of this lesion. Solitary hepatic lesion in E. granulosus infection correlates with HLA-A10 and B18 antigens, while multiple lesion with HLA-B5 antigen, but the development of E. multilocularis infection metastases is associated with HLA-B8 antigen. The occurrence of spontaneous cystic ruptures of E. granulosus correlates with HLA-A32, A26, and B5 antigens, which is likely to associated with cystic formation defect due to the influence of these factors.
