Accuracy of serum IgM and IgA monoclonal protein measurements by densitometry.
We previously reported that proper dilution of sera that contain IgG monoclonal proteins (M-proteins) is necessary for accurate quantitation of serum albumin and M-protein concentrations separated by serum protein electrophoresis (SPE) using the Beckman PARAGON agarose electrophoresis system. We now report the significance of pre-electrophoretic serum dilution for M-protein quantitation of sera from patients with IgA and IgM monoclonal gammopathy. We measured M-proteins by SPE in 82 serum samples from 29 patients with IgA and 72 samples from 23 patients with IgM monodonal gammopathy. The serum M-protein concentrations (mean +/- SD) at 1:5, 1:10, and 1:20 dilutions (v/v) for all samples of both types were 49.7 +/- 12.9, 49.1 +/- 13.1, and 47.8 +/- 13.0 g/L, respectively. Thirty-two (20.8%) of 154 sera showed varying degrees of increase in M-protein concentrations with serum dilutions higher than 1:5; only 8 (5.2%) showed an increase 3 SDs. By SPE, the M-protein concentration (mean +/- SD) of these 8 sera at 1:5, 1:10, and 1:20 dilutions were 52.6 +/- 7.8, 57.1 +/- 7.2, and 57.6 +/- 7.1 g/L, respectively; the albumin concentrations (mean +/- SD) were 41.4 +/- 4.4, 37.9 +/- 3.8, and 37.1 +/- 2.9 g/L, respectively. The corresponding albumin concentration (mean +/- SD) was 36.8 +/- 3.7 g/L, assayed by the bromcresol green dye-binding method. These 8 samples were obtained from 3 patients, 2 with IgM kappa and 1 with IgA lambda monoclonal gammopathy. On the electrophoresis membranes, the M-protein bands of these 8 samples were narrow, thin, and dense; upon scanning, they appeared taller and thinner than the corresponding albumin bands. The samples of this subset contained relatively high concentrations of M-protein and total serum protein. We conclude that a pre-electrophoretic dilution of 1:5 (v/v) is adequate for most sera with IgA or IgM M-proteins. However, 1:10 or 1:20 dilution is occasionally required for a subset of sera with IgA or IgM M-proteins that show an unusually thin, narrow, and dense M-protein band and have high total serum protein content.

Determination of sodium cromoglycate in pharmaceutical dosage forms using TLC-densitometry.
A TLC-UV densitometric method for the determination of sodium cromoglycate (SCG) in ophthalmic solutions, gels and capsules has been developed. After TLC separation of active substance on silica gel GF254 using methanol-water-ethylacetate (15:45:40 v/v/v) as the mobile phase, densitometric measurements were performed with HPTLC scanner at 254 nm. The proposed method is rapid and simple, free from interference by adjuvants and can be suggested for the routine analysis of sodium cromoglycate.

[The proteincomposition of human parotid saliva in acute and chronic parotitis. Quantitativ densitometry of single protein-fractions in comparison to normal secretions].
By means of acrylamidgelectrophoresis parotid secretions of patients with acute and chronic parotitis were examined. Thereby significant changes in protein patterns of electrophoretic separation were found. Especially two bands are described in detail. One in the cathodal near gel region, the isoamylases, and the otherone in the anodal part of the gel, identified as albumin. In rest and under stimulation these bands show signficant changes compared to normal secretions. Furthermore information about the capacity of the parotid gland parenchyma is won by observing the changes of amylase bands in rest and under stimulation in chronic parotitis without actute exacerbations. The amylase shows a remarked decreased secretion in chronic parotitis under stimulation corresponding to parenchymalteration. The same happens with the albuminexcretion as parameter for ductlesions. In chronic parotitis albmuninexcretion goes parallel to duct changes in sialography. Therefore by discelectrophoretic separation of native parotid saliva a helpful mean for diagnostic use is given, besides sialography and scintigraphy.

[Bone metabolism in dialysis patient assessed by biochemical markers and densitometry].
A decrease in bone mineral density is common in patients with chronic renal failure. It is also a risk factor for fractures in this population. The aim of the study was to evaluate bone mineral density-BMD and some biochemical markers of bone metabolism in regard to the method of renal replacement therapy: hemodialysis or peritoneal dialysis. The studies were performed in two groups of patients: 2 patients maintained on chronic hemodialyses (HD) and 21 patients treated with chronic ambulatory peritoneal dialysis (CAPD). Bone mineral density was measured using dual energy X-ray absorptiometry (DEXA) in L2-L4 segments of lumbar spine and femoral neck. Concentrations of parathormon, osteocalcin, bone-specific alkaline phosphatase, serum CrossLaps (degradation products of C-terminal telopeptides of type I collagen) vitamin D3 were studied using commercially available kits. In femoral neck bone mineral density was significantly higher in CAPD patients when compared to HD, without significant differences in bone mineral density in lumbar spine. There was statistically significant correlation between BMD of the lumbar spine and time of hemodialysis (r = 0.39, p < 0.05). In CAPD patients BMD of lumbar spine correlated negatively with vitamin D3 (r = -0.54, p < 0.05), osteocalcin (r = -0.54, p < 0.05), and positively with body mass index-BMI (r = 0.63, p < 0.01). BMD of femoral neck correlated positively with BMI (r = 0.59, p < 0.01), and negatively with osteocalcin (r = -0.63, p < 0.05) and time on CAPD (r = -0.52, p < 0.05). On the basis of our finding we conclude that BMD depends on time of renal replacement therapy. Biochemical markers of bone metabolism poorly correlate with bone mineral density in dialyzed patients.
