Intravenous contrast enhancement in computed tomography of pelvic malignancies.

The value of intravenous contract enhancement in computed tomography (CT) of pelvic malignancies was analyzed in 124 cases. Contrast enhancement of pelvic organs depends on the smooth muscle component and the administered iodine dose. Contrary to parenchymal organs in the abdomen the iodine washout from smooth muscle organs is slow. A rapid (approximately 5 minutes) intravenous infusion of 300 ml diatrizoate 30% followed immediately by scanning of the area of interest was found optimum to improve the CT visualization of tumors in all pelvic organs with the exception of the prostate. Depending on the organ to be investigated the intravenous contrast administration has to be combined with retrograde gas insufflation into bladder and/or rectum with insertion of a tampon into the vagina. The described technique is particularly useful for the assessment of early tumor extension beyond the primary organ.

Contrast Media↗

[An animal experimental model for evaluating endothelial damage caused by various angiographic contrast media (author's transl)].

The endothelial damage caused by X-ray contrast media is tested on "en face preparations" of the rat aorta after silver staining. Hypertonic contrast media cause dehydration of the vessels so that solutions of silver nitrate penetrate into the media during the phase of rehydration whereby medial transverse lines are stained. These artifacts can be avoided by 3 minutes rehydration of the vessels by Ringer solution prior to silver staining. In addition it is recommended to add highly diluted silver nitrate to the fixing media in order to intensify the pattern of the endothelial silver lines. Modern contrast media may cause very little endothelial damage, however, significant differences can be detected when in addition to the evaluation of larger foci damages of single endothelial cells ("brown cells" with sparing of the nuclei and "pseudonuclei") are evaluated as well. No significant differences were found between the endothelial toxicity of meglumin diatrizoate and meglumin iothalamate.

[Intolerance reactions to ionic and nonionic contrast media in cardiac diagnosis. A randomized double-blind study].

Reactions to an ionic and a nonionic contrast medium were compared in a randomized double-blind trial involving 1153 patients (897 men, 256 women; mean age 56 [18-84] years) who underwent left-heart catheterization with coronary angiography. 584 patients (group 1) received the ionic contrast medium meglumine diatrizoate (Urografin 76%), while 569 (group 2) received the nonionic iopromide (Ultravist -370). Reactions of skin and mucosae, heart and circulation, lungs and respiratory tract, as well as of the central nervous system were noted. There was no significant difference between the two groups with respect to age, weight, height, allergic predisposition, left-ventricular ejection fraction, or serum electrolyte concentrations. Mild reactions, not requiring any treatment, occurred in 108 of the 1153 patients (9.4%), 85 (14.6%) in group 1 and 23 (4%) in group 2 (P less than 0.001). Severe reaction requiring treatment occurred in eight patients of group 1 (1.4%) and 14 of group 2 (2.5%)--difference not significant. Ventricular fibrillation was more frequent among group 2 patients (four compared with one in group 1), while two of three asystoles requiring treatment occurred in group 1 patients. The results show that, while mild reactions not requiring treatment were clearly less frequent after nonionic contrast medium injection, the risk of severe reactions was not reduced by their use.

[Risk of contrast medium reactions in cardiac diagnosis. Pretreatment with glucocorticoids and antihistaminics in known cases of intolerance to contrast media].

Among 4178 patients who between 1980 and 1986 had undergone left-heart catheterization with left-heart injections and coronary angiography there were 76 (1.8%) with previous reactions to contrast media. These latter patients were given, three days before the planned investigation, 6 alpha-methylprednisolone, 24 mg daily by mouth, and phenhydramine hydrogenmaleinate, 150 mg daily, and two hours before the investigation 80 mg 6 alpha-methylprednisolone hemisuccinate intravenously. The effect of this prophylactic regimen was tested prospectively. Diatrizoate 76% was the contrast medium used. Of 4102 patients without known contrast-medium intolerance 137 (3.34%) had a reaction, 27 of them (0.66%) severe enough to require treatment. Among the 76 patients with known previous reactions, nine (11.8%) had reactions, one very severe requiring treatment, the others mild. The described pre-injection regimen thus allows indicated left-heart contrast-medium injection to be undertaken at a justifiable risk.

Iodine concentrations in the rat kidney measured by X-ray microanalysis. Comparison of concentrations and viscosities in the proximal tubules and renal pelvis after intravenous injections of contrast media.

PURPOSE: To measure the iodine concentrations in the proximal tubules and renal pelvis after i.v. injections of contrast media (CM) at 1600 mg I/kg b.w., using a micropuncture technique and X-ray microanalysis. MATERIAL AND METHODS: The correlation between the viscosity of each CM and its iodine concentration was evaluated and the viscosity of the fluid in the proximal tubule and renal pelvis was estimated in rats. RESULTS: After iotrolan injection, the iodine concentration in the proximal tubular fluid had increased to values about three times higher than those reached with diatrizoate, iohexol and ioxaglate. Similarly, iotrolan tended to produce a higher iodine concentration in the renal pelvis than did the other CM. The urine viscosity in the renal pelvis was much higher after the iotrolan injection than after the other CM injections. CONCLUSION: High urine viscosity after iotrolan injection can at least partly explain our previous findings of a prolonged increase in tubular hydrostatic pressure and a prolonged decrease in the single nephron glomerular filtration rate after administration of this CM.

Upper gastrointestinal contrast study in the management of small bowel obstruction--a prospective randomised study.

OBJECTIVE: To find out whether contrast radiography helps to resolve small bowel obstruction. DESIGN: Prospective randomised trial. SETTING: University hospital, Norway. SUBJECTS: 98 consecutive patients with symptoms of small bowel obstruction and a plain abdominal radiograph that confirmed the diagnosis. INTERVENTIONS: The patients were randomly allocated to receive a mixture of barium and sodium diatrizoate (Gastrografin) (n = 48) or not (n = 50). Both groups were followed up clinically and by repeated abdominal films. MAIN OUTCOME MEASURES: Non-operative resolution of small bowel obstruction; number of patients with strangulated bowel; bowel resections; mortality; complications; hospital stay; and time from admission to operation. RESULTS: No significant differences were observed between the groups in the incidence of non-operative resolution (31/48 in contrast group, 35/50 in control group, OR: 0.89), strangulation obstruction (1/48 in contrast group, 4/50 in control group, OR: 0.24), bowel resection (3/48 in contrast group, 4/50 in control group, OR: 0.76), complications (8/48 in contrast group, 5/50 in control group, OR: 1.80), mortality (3/48 in contrast group, 1/50 in control group, OR: 3.26), and hospital stay (0-7 days: 34/48 in contrast group, 38/50 in control group, p = 0.95). The contrast group had a shorter interval between admission and operation than the control group (0-24 hours: 12/48 in contrast group, 3/50 in control group, p = 0.005). CONCLUSION: The contrast examination did not contribute to the resolution of small bowel obstruction.

Identification of osmotically active solutes in urine.

Measurement of the fractional excretion of sodium ions (FENa) and the urinary specific gravity (SG) aids considerably in the clinical assessment of fluid and Na+ balance. However, these determinations may not provide meaningful information if the patient has been treated with a diuretic. In particular, osmotic agents can affect circulating volume by causing natriuresis with a relatively high urinary SG, even in the face of volume and Na+ depletion. Two methods used frequently to identify osmotic diuretics were examined for accuracy. A new equation using the last two digits of SG was derived to identify osmotic agents without the need for an independent measurement of osmolality. The new equation was clearly superior to the more traditional formulas in that it identified accurately the presence of sodium diatrizoate, an ionized molecule, in each of 12 samples, whereas this contrast material was detected in only one sample by determining the difference between measured osmolality and the sum of the usual urinary constituents. A simplified version of the new formula was statistically as accurate as the more complex equation.

Toxicity studies of coronary arteriographic media in dogs.

Sequential injections of different contrast material in the canine coronary artery were performed while ventricular pressure and EKG were recorded. Larger volumes of contrast material caused more fibrillation and more drop in pressure. Preliminary myocardial infarction increased pressure effects. Diatrizoate methylglucamine (Renografin-76) caused more fibrillation and lowering of ventricular pressure than iothalamate or metrizoate Coronar. Calcium lessened the ventricular pressure depression from iothalamate.

Activation of serum complement by contrast media.

Evidence is presented for the activation of serum complement by contrast media, in vitro and in vivo. Activation as a function of concentration was measured and the increasing order of effectiveness was found to be metrizamide, iothalamate, diatrizoate, acetrizoate, iodipamide and iopanoate. This order is the same as for protein binding and enzyme inhibition. The activation mechanism for iodipamide, and by inference for the other compounds, does not involve gamma-globulin aggregation. Serial daily injections in normal dogs resulted in substantial declines in serum complement over several days. Guinea pigs which were depleted of serum complement with cobra venom factor were found to be no less sensitive to lethal doses of iodipamide than those with normal complement. Implications of these findings are discussed.

Arthrography with Amipaque (metrizamide) and other contrast media. A roentgenographic and histologic evaluation in rabbits.

Arthrography of the knee joint was performed in 30 rabbits for comparison of Amipaque (metrizamide) with Urografin 60% (meglumine-sodium diatrizoate), Isopaque Amin (meglumine-calcium metrizoate), Conray Meglumin 282 (meglumine iothalamate) and Dimer-X (meglumine iocarmate). Films taken at regular intervals show longer contrast duration, and less hydrarthrosis after injection of Amipaque. Minimal focal inflammatory reaction in the synovial membrane is found in many cases when the animals are sacrificed 1 day or 7 days after the injections. No significant difference can be found between the contrast media, and similar changes are also seen after injection of physiologic saline.

Comparative efficacy of coronary vasodilatory methods.

The relative efficacy and sequelae of several coronary vasodilator methods were compared in dogs. Maximal coronary vasodilation with least apparent side effect was achieved with infusion of adenosine triphosphate (ATP) at the rate of approximately 600 microng/minute (approximately 20 microng/kg/minute). This method increased conductance to 7 to 8 times control levels and may have fully depleted vasodilator reserve. Bolus injections of ATP or papaverine alone increased conductances three to four fold and approximated the effects of 15 seconds of occlusion of the circumflex coronary artery. Duration of effect was variable. Diatrizoate injections were somewhat less effective vasodilators and produced greater electrocardiographic and systemic effects. Bolus injections of mixtures of ATP and papaverine demonstrated additive effects, and increased conductance 5 to 6 times control levels. In further exploring the diagnostic value of vasodilatory redistribution of myocardial blood flow or appearance of collateral circulation as a sign of a hemodynamically significant stenosis, selective intracoronary infusion of ATP is a promising vasodilatory method.

Osmotic effect and solubility of amipaque (metrizamide) in the gastrointestinal tract.

Loops of small intestine in seven rabbits were resected with intact pedicle and ligated at both ends after instillation of 0.5 ml of Gastrografin, Urografin 76%, Amipaque 370 mg I/ml, Amipaque 170 mg I/ml or physiologic saline. After half an hour, the amount of fluid in the loops containing Gastrografin and Urografin 76% increases about twice as much as in the loops containing Amipague with the same iodine concentration because of their greater osmolality. The differences between the loops with isotonic Amipaque (170 mg I/ml) and physiologic saline are not significant. Precipitation occurs when sodium and meglumine salts of diatrizoate, metrizoate, iothalamate, iocarmate and ioglycamate are mixed with 0.05 N HCl. No precipitation occurs with Amipaque, not even when the HCl concentration is as high as 1.2 N. Precipitation occurs when Gastrografin is added to gastric juices with low pH, but is not seen with Amipaque should be a suitable contrast medium for gastrointestinal examinations because of its low osmolality and toxicity and good solubility in gastric juice.

Uptake of contrast materials by experimental acute myocardial infarctions: a preliminary report.

The concentration of iodine within infarcted and normal myocardium after intravenous administration of contrast material was determined by fluorescence excitation analysis in seven dogs at 48 hours after coronary arterial ligation. The iodine concentration of infarcted myocardial tissue was several times greater than normal myocardium after administration of meglumine/sodium diatrizoate, iodipamide, and an experimental polymer of iothalamic acid.

The diagnostic potential of contrast enhancement pharmacokinetics.

Diastizoate concentrations in the blood and twelve tissues and organs of 20 rats were measured at 40 seconds, and 2, 5, 15 and 60 minutes after bolus intravenous administration of 125 I-labeled diatrizoate and 131 I-labeled albumin. Contrast enhancement (HU) and distribution volumes(%) were calculated from this data, and the results are presented graphically. The tissues can be separated into two groups according to the rapidity of extravascular contrast medium uptake. The liver appeared to concentrate the contrast medium. Curves of local contrast enhancement with time could be constructed by a CT scanner from the data of several consecutive scans and are of potential importance in the differential diagnosis of lesions detected on CT. Repeat postcontrast CT scans could be used to detect and characterize lesions which raise a diagnostic problem. The potential diagnostic advantages of calculating percent distribution volumes from CT scans are demonstrated and discussed.

Hepatic CT contrast enhancement: effect of dose, duration of infusion, and time elapsed following infusion.

Contrast enhancement of the liver of anesthetized, paralyzed, and artificially ventilated rabbits was measured during suspended respiration prior to, during, and after the intravenous infusion of meglumine diatrizoate at doses of 208, 416, and 624 mg I/kg. Infusion time was 30 seconds, and, at the highest dose, infusion times of 1, 2, and 4 minutes were also used. Highest contrast enhancement values were obtained at the conclusion of each infusion, with contrast enhancement increasing proportionately with increase in dose. Highest enhancement was obtained at all times studied from the most rapid infusion. Unlike cranial CT, optimum hepatic contrast enhancement in body CT requires rapid contrast medium injection with immediate CT scanning with a very fast scanner.

Intravenous angiography using scanned projection radiography: preliminary investigation of a new method.

The use of digital subtraction techniques combined with fluoroscopy has rekindled interest in arteriography using intranvenous injections of contrast media. A new method is proposed for intravenous angiography in which an x-ray source and xenon detector array from a computed tomographic (CT) scanner are used to scan a region of interest to produce projection image. In order to provide adequate visualization of small concentrations of iodine in blood vessels, a subtraction scheme is used to remove the contribution from overlapping soft tissue and bone. Initial experiments with a temporal subtraction algorithm on phantoms have demonstrated the ability to image simulated blood vessels of 1.7-mm diameter containing dilute diatrizoate with an iodine concentration of 3.7 mg/cc, at an exposure of less than 100 mR. Vascular structures 5-8 mm in diameter have been imaged in dogs with iodine concentrations of less than 37 mg/cc using temporal subtraction. Principal advantages of the method over other film or fluoroscopic subtraction techniques are: 1) wide dynamic range an low noise of the (CT) detectors, providing excellent iodine sensitivity; 2) high scatter rejection; and 3) efficient utilization of x-ray dose.

Particulate contrast media for computed tomographic scanning of the liver.

A contrast agent that is selectively accumulated in the liver should greatly improve the diagnostic value of contrast enhanced CT scanning. The advantages and disadvantages of different classes of hepatographic agents are briefly reviewed. Experimental results obtained with the particulate contrast agents, iothalamate ethyl ester and iodipamide ethyl ester, are presented in more detail. Following intravenous infusion of iodipamide ethyl ester, approximately 60% of the injected dose is accumulated in the rat liver. CT scanning experiments involving iothalamate ethyl ester infusions in New Zealand White rabbits demonstrate significantly higher liver contrast enhancement at 10-30 minutes postinfusion than is observed with diatrizoate at a sixfold greater iodine dose. The selective accumulation of a particulate contrast agent in hepatic reticuloendothelial cells compared to virtually no accumulation in implanted VX2 carcinoma demonstrates the important potential value of these agents in improving detection of liver metastases.

Cardiovascular radiology. The fibrillatory propensities of contrast agents.

Ventricular fibrillation threshold (VFT), the electrical stimulus strength required to fibrillate the heart during the vulnerable period of the cardiac cycle, has been used to evaluate the properties of many arrhythmogenic and antiarrhythmic drugs, and is easily adaptable to determining ventricular vulnerability following coronary arteriography. Using this method, the authors find that there is a large decrease in VFT within seconds following intracoronary arteriography with 1 ml of sodium meglumine diatrizoate. During the first minute, the threshold decreases by an additional 10%, then returns rapidly and completely to control levels. The change in threshold persists much longer than artery-to-sinus transit time, longer than changes in contractility, and somewhat longer than repolarization changes in the surface electrocardiogram. There is a strong dose relation. Injections through impacted catheters or slow injections cause disproportionate decreases in VFT. Ionic solutions of CaCl2 progressively lower VFT, but, when added to sodium meglumine iothalamate solutions, minimize the fibrillatory potential of ionic contrast agents in a complex way. A modified model of myocardial ischemia produced by intracoronary mercury injection is described. With myocardial ischemia, the fibrillation threshold falls, and during coronary arteriography still lower thresholds are seen. The use of VFT measurements is recommended for further evaluation of the fibrillatory potential of contrast media in clinically relevant situations.
