Arterial hypotension and hypoxaemia during total hip replacement: the importance of thromboplastic products, fat embolism and acrylic monomers.

Thirteen patients submitted to total hip replacement surgery by the Charnley technique were studied. Operations were performed under epidural analgesia with the patients awake and breathing air. During the surgical procedure, the magnitude of tissue-thromboplastic activity, the amount of fat globules, the presence of bone marrow cells and the concentrations of acrylic monomers were determined in the pulmonary arterial blood. Simultaneously, arterial blood gases and blood pressure were monitored. Marked reductions of the arterial blood pressure and arterial oxygen tension occurred after impaction of the femoral prosthesis, and minor depressions appeared after insertion of the acetabular prosthesis. A significant correlation was found between the release of tissue-thromboplastic products into the pulmonary circulation, i.e., products that initiate intravascular coagulation and the circulatory and respiratory reactions. The pulmonary fat droplets, per se, seem to be of minor importance, and the release of acrylic monomers is probably of no importance for these reactions.

Formation of a ternary complex with serum albumin: an explanation for the effect of alpha-mercapto-beta-(2-furan) acrylic acid on rat plasma zinc clearance.

1. Four, six and eight hours after gavage of rats with alpha-mercapto-beta-(2-furan)acrylic acid (MFA) (25 mg/kg) serum zinc concentration was increased ten-fold over control levels and a mean molar ratio of 1 albumin:0-4 zinc:0-8 MFA was found for seventeen sera. 2. At pH 7-5 a maximum of 1 mole of MFA could be bound per mole of metal-free bovine serum albumin. 3. In the presence of zinc ion, albumin-zinc-MFA complexes formed, since for each mole of albumin-zinc complex an additional mole of MFA could be bound to albumin. Complexes up to a molar stoichiometry of 1 albumin:2 zinc:3 MFA were prepared. 4. MFA stabilized the albumin-zinc complex against dissociation. 5. Formation of similar complexes in vivo may account for the markedly delayed clearance of plasma zinc rats administered beta-aryl derivatives of alpha-mercapto-acrylic acid.

Effect of denture cleansers on the surface roughness and hardness of a microwave-cured acrylic resin and dental alloys.

PURPOSE: This study evaluated the effect of denture cleansers on the surface hardness of a denture base resin, and on the surface roughness of the resin and Co-Cr and Ti-6Al-4V alloys. MATERIALS AND METHODS: Forty-eight disc-shaped specimens were fabricated of a microwave-cured acrylic resin, each having one of the alloys attached to its surface. The specimens were randomly divided into 6 groups, each consisting of 8 samples. Specimens were exposed to one of the three cleansing treatments (polident, manipulation pharmacy cleanser, and water) as follows. Group I: Co-Cr + polident; Group II: Co-Cr + manipulation; Group III: Co-Cr + water; Group IV: Ti-6Al-4V + polident; Group V: Ti-6Al-4V + manipulation; and Group VI: Ti-6Al-4V + water. Three exposures lasting 5 minutes each were conducted daily, and repeated after storage periods of 1, 14, and 29 days in artificial saliva at 37 degrees C. Hardness and roughness measurements were undertaken immediately after specimen preparation (T0) and on the 1st (T1), 15th (T15), and 30th (T30) day following the beginning of storage. Three roughness and hardness evaluations were carried out for each sample and testing time, and mean values were calculated. Results were analyzed using ANOVA and linear regression. RESULTS: The Knoop hardness test demonstrated differences (p < 0.05) between Groups I and IV at T1 and T30 (14.30 +/- 2.78; 14.06 +/- 1.76) and between Groups II and V at T15 (16.99 +/- 2.24). Significant differences (p < 0.05) in resin roughness (in microm) were observed between Groups I and IV at T15 and T30 (0.14 +/- 0.06; 0.21 +/- 0.38). With regard to Co-Cr, roughness data showed differences (p < 0.05) for all groups at T30 (Group I: 0.15 +/- 0.07; Group II: 2.43 +/- 0.66; Group III: 4.05 +/- 1.03), for Group II at T1 (0.10 +/- 0.03), and for Group I at T15 (0.15 +/- 0.02). There were significant differences (p < 0.05) in titanium roughness for Group IV at T15 (0.12 +/- 0.01) and T30 (0.11 +/- 0.04). CONCLUSIONS: Manipulated cleanser containing sodium perborate increased surface roughness and hardness, probably due to its incapacity to remove the pellicle formed on the acrylic resin and dental alloys.

Implantation of single-piece foldable acrylic IOLs in small children in the Ukraine.

PURPOSE: To evaluate the efficacy and outcome of implantation of a single-piece foldable acrylic intraocular lens (IOL) in children younger than 2 years in the Ukraine. METHODS: A total of 31 children from all over the Ukraine with a median age of 11 months (range 2-28 months) underwent cataract surgery with anterior and posterior capsulorhexis, implantation of a single-piece AcrySof SA30AL IOL and anterior dry vitrectomy. Of these, 22 patients attended the last follow-up. Ten patients had follow-up periods of 12 months; 12 had follow-up periods of 18 months. At the postoperative visits, we investigated visual acuity (VA), whether or not the subject had strabismus, lens epithelial cell migration and signs of secondary glaucoma. RESULTS: The single-piece IOL remained clinically centred in all eyes. No secondary glaucoma was detected. Eleven patients needed surgery for after-cataract. All but two patients achieved better VA after surgery. Many patients suffered from deprivation amblyopia as quite a few cataracts were probably congenital; however, the improvement in VA shows that cataract surgery was worth performing despite the late intervention. CONCLUSION: The foldable single-piece acrylic IOL seems to be well tolerated by children aged 0-2 years. No serious complications were seen and no patient developed secondary glaucoma within the follow-up time. However, lens epithelial cells are very lively in this age group.

Comparison of the immediate effects on the sub-gingival microflora of acrylic strips containing 40% chlorhexidine, metronidazole or tetracycline.

In the management of chronic periodontitis, there has been a renewed interest in the local delivery of antimicrobial drugs into periodontal pockets. This study assessed the effects of the acrylic strip delivery system containing chlorhexidine, metronidazole or tetracycline on subgingival microflora assessed by dark field microscopy. Strips containing 40% chlorhexidine, metronidazole or tetracycline were placed for 2 to 3 days into pockets greater than 6 mm which bled on probing. Plaque samples were obtained before and after treatment and counts of morphological and motile groups of organisms were made by dark field microscopy. Prior to treatment, the dark field microscopic counts were similar to those previously reported for diseased sites with motile bacteria, in particular spirochaetes, present in high numbers. Following treatment, all 3 antimicrobial drugs produced a significant increase in the proportion of cocci and significant decreases in all other types of organisms. Motile organisms, in particular, were markedly reduced and spirochaetes could not be recovered from some sites treated with metronidazole and tetracycline. Metronidazole was significantly more effective than tetracycline or chlorhexidine on spirochaetes. The results indicate that acrylic strips may be useful in the management of chronic periodontitis as an adjunct to routine mechanical methods.

Simple bacteriological methods to assess changes in subgingival microflora produced by metronidazole-containing acrylic strips placed into periodontal pockets.

Dark field microscopy is perhaps the simplest microbiological technique to monitor the effects of treatment methods on bacterial plaque. However, the method provides qualitative rather than true quantitative data. This study attempts to measure changes in flora by a number of methods following the placement of 40% metronidazole acrylic strips into periodontal pockets. 10 patients with periodontal pockets greater than 6 mm which bled on probing had acrylic strips placed for 2 to 3 days. Prior to insertion and after removal, subgingival plaque samples were collected into 1 ml of saline and processed as follows. (a) Dark field microscopy for qualitative shifts. (b) Gram stain for differential counts. (c) Counting chamber for total counts. (d) Serial dilution and culture for total cultivable counts. After treatment the significant changes were as follows. (a) % increase in cocci and decreases in other forms, particularly motile organisms. (b) Gram-positive cocci increased and Gram-negative bacilli decreased. (c) Total counts obtained by the sampling method decreased greater than 80%. (d) Total cultivable counts decreased greater than 75%. (e) The efficiency of culturing appeared low and the potential problems of sampling to quantification were identified. The consistency of the changes produced suggested that using simple bacteriological techniques it was possible to show marked changes in the numbers and types of organism present in subgingival plaque following the use of local antimicrobial therapy.

Epicutaneous induction of tolerance with acrylates and related compounds.

Epicutaneous application of acrylates and related compounds 14 and 7 days before sensitization with either methyl acrylate or trimethylol propane triacrylate induced tolerance to the resultant contact reactions. This tolerance could not be correlated with either the degree of cross reactivity between these compounds or with their ability to react covalently with amino or sulphydryl groups of proteins. These results are discussed in relation to other epicutaneous tolerizers in the dinitrobenzene and poison ivy systems.

Syncrystallization: a technique for temporization of immediately loaded implants with metal-reinforced acrylic resin restorations.

BACKGROUND: Rigid temporization has been recognized to have a significant impact on the peri-implant tissue response in immediate implant loading since it reduces the mechanical stress exerted on each implant. PURPOSE: A successful protocol for immediate loading of multiple implants depends on an adequate fixation and immobility of the implants to prevent the risk of micromovements in relation to the surrounding bone. The objective of this article was to evaluate a prosthetic concept for an accelerated rigid splinting of multiple implants for same-day immediate loading with metal-reinforced provisional restorations using a technique of welding temporary implant abutments with a prefabricated titanium bar directly in the oral cavity (syncrystallization). MATERIALS AND METHODS: Between June 2004 and January 2005, immediate loading of threaded implants with a metal- reinforced acrylic resin provisional restoration at stage 1 surgery was evaluated in 40 consecutive patients. A total of 192 implants were placed in selected edentulous or partially edentulous patients using the syncrystallization technique. Once the titanium bar was welded intraorally to the abutments, opaque was applied and the provisional restoration was relined and screw-retained the same day. In addition, a comparison of deformations and stress distributions in implant-supported, metal-reinforced and nonmetal-reinforced resin provisional restorations was analyzed in the edentulous mandible by a three-dimensional finite element model (FEM). RESULTS: All of the 192 rigidly temporized immediately loaded implants osseointegrated. An implant success rate of 100% was achieved over a period of 6 months postplacement. No fracture or luting cement failure of the provisional restoration occurred during the observation time. Compared to mere acrylic superstructures, a significant reduction of deformation and strain within metal-reinforced provisional restorations was detected by FEM analysis. CONCLUSION: The results of this study indicate that the syncrystallization technique allows an expedite and adequate rigid splinting of multiple immediately loaded implants. The advantages of the technique are: (1) reduction of treatment time for immediate temporization at stage 1 surgery; (2) predictable fixation and immobility of implants in the early stages of bone healing; and (3) less time for repairing provisional restorations as a result of no or rare fracture.

A prospective, randomised comparison of single and three piece acrylic foldable intraocular lenses.

AIMS: To compare the postoperative performance of single and three piece acrylic foldable intraocular lenses (IOLs). METHODS: 20 patients underwent bilateral cataract surgery with a single piece SA30AL IOL in one eye and a three piece MA30BA IOL in the other eye. The eyes were randomly assigned to either a single or three piece lens. The amount of IOL decentration and tilt, area of anterior capsule opening, and degree of posterior capsule opacification were measured using the Scheimpflug anterior segment analysis system (Nidek EAS-1000). Visual acuity and contrast sensitivity were examined. Measurements were performed by masked examiners before and 1 day, 1 week, 1, 3, 6, and 18 months after surgery. RESULTS: There were no significant differences between the two groups (p>0.05, paired t test) in the amount of IOL decentration, IOL tilt, area of anterior capsule opening, degree of posterior capsule opacification, best corrected visual acuity, and contrast sensitivity throughout the 18 month follow up period. CONCLUSION: The single and three piece acrylic foldable IOLs are equally stable in the eye after surgery.

Influence of glistenings on the optical quality of acrylic foldable intraocular lens.

AIMS: To assess the influence of glistenings on the optical quality of acrylic foldable intraocular lens. METHODS: Several degrees of glistenings in the optic were experimentally created by immersing the lens in water at 37 degrees C for 48 hours and then at 25 degrees C for 24 hours. Optical bench tests were carried out in water including measurements of spectral transmittance with the spectrophotometer, intensity of forward scattering using the integrating sphere photometer, modulation transfer function, and resolving power at various contrasts with and without the veiling glare light source. RESULTS: Glistenings of 1+ to 4+ degrees were created, among which the 4+ glistenings seemed to be extremely intense and thought to be beyond the range of clinical settings. Clinically feasible level of glistenings, up to 3+, did not adversely influence spectral transmittance, scattering, modulation transfer function, and resolving power at various contrasts. The 4+ glistenings caused mild to moderate deteriorations of the optical quality of the lens. CONCLUSION: The optical quality of the acrylic foldable intraocular lens is not significantly affected by the level of glistenings usually seen in the clinical setting.

Contact dermatitis from penetration of rubber gloves by acrylic monomer.

An orthopaedic surgeon developed dermatitis from acrylic materials. The acrylic monomer was found to penetrate surgical rubber gloves readily. Cases of "rubber glove dermatitis" with negative patch tests may have a similar explanation. Laboratory tests suggest that monomer does not damage rubber sufficiently to allow bacteria to penetrate gloves, but it remains possible that this would happen under theatre conditions.

Cardiovascular effects of acrylic bone cement in rabbits and cats.

The cardiovascular responses to forcing acrylic bone cement, Plasticine, or soft paraffin wax into the medullary cavity of the femur have been studied in rabbits and cats. An acute fall in blood pressure, occurring within a few seconds of insertion, was demonstrated with each substance. In a few of the animals the blood pressure response had a second more protracted component and it is suggested that more than one mechanism is involved. The cardiovascular effects that have been observed in man when acrylic cement is used in prosthetic hip surgery also may be due to more than one mechanism.

Cardiovascular effects of implanted acrylic bone cement.

A pilot study has shown that there is usually but not invariably a fall in systemic arterial blood pressure within 90 seconds of implanting acrylic cement into the femoral shaft during hip arthroplasty. There is usually no change in arterial blood pressure on implanting acrylic cement into the acetabulum. The observed hypotension may be due to absorption of monomer or additives into the circulation, but the role of other factors needs investigation.

The role of capsulotomy and intraocular lens type on tilt and decentration of polymethylmethacrylate and foldable acrylic lenses.

The purpose of this study is to determine the effects of different anterior capsulotomy techniques and intraocular lens (IOL) types on IOL tilt and decentration. For this purpose the device using Purkinje reflections and photographic documentation were evaluated. A total of 107 cataractous eyes undergoing IOL implant surgery were divided into four groups based on the type of IOL and capsulotomy: group 1, single-piece polymethylmethacrylate (PMMA) IOL with a haptic distance of 13.50 mm; group 2, single-piece PMMA IOL with 12. 0 mm overall length; group 3, three-piece foldable acrylic IOL with 13.00 mm PMMA haptic distance; group 4, the same IOL as group 1. While continuous circular capsulorhexis was the type of capsulotomy in the first three groups, envelope capsulotomy was used in group 4. Mean tilt and decentration were significantly less when capsulorhexis technique was used in comparison with envelope capsulotomy. Mean tilt of group 1 (2.83+/-0.89 degrees) and mean decentration of group 2 (0.28+/-0.14 mm) were higher within the first three groups. The differences regarding both the IOL decentration and tilt between the first three groups were statistically significant. This study shows that if the IOLs were placed properly in the capsular bag after continuous circular capsulorhexis, foldable acrylic IOLs with PMMA haptics are superior in terms of tilt and decentration.

Immunogenicity of acrylate chemicals as assessed by antibody induction.

The immunogenicities of two acrylate chemicals, trimethylolpropane triacrylate (TMPTA) and methyl acrylate (MeAc), and one related vinyl compound, 4-vinyl pyridine (4VP), were investigated by determining the in vivo induction of IgG antibodies in guinea pigs. The injection of the chemicals emulsified in Freund's complete adjuvant resulted in the induction of serum antibody responses against MeAc and 4VP but not TMPTA. However, antibody with anti-TMPTA activity was produced following immunization of guinea pigs with TMPTA conjugated to protein, which allowed comparisons to be made of the immunogenic structural features of the compounds.
