[Actinomycetomas in Senegal: study of 90 cases].

Mycetoma is a pathological process during which eumycetomic (fungal) or actinomycotic causative agents from exogenous source produce grains. Medical treatment is available for actinomycetomas and surgery is still the main treatment for eumycetoma. We report 90 cases of actinomycetoma occuring in male adult patients coming mainly from central Senegal. Patients living far from health structures consult after a long evolution period for tumors and pains which prevent them from carrying out their activities. The three etiological agents in our patients were Actinomadura pelletieri (60 cases), Actinomadura madurae (25 cases) and Streptomyces somaliensis (5 cases). The three clinical features are inflammatory forms (75 cases) mainly due to Actinomadura pelletieri, tumoral forms (13 cases) and cystic forms (2 cases). Lesions are localized on the foot in 50% of cases and in other part of the body for the other half. Bone damage was observed in 55% of cases. 83% of the patients were cured after a one-year treatment of sulfametoxazole adminstered orally. Two patients died of visceral involvement.

[Nasal polyposis masquerading epidermoid carcinoma of the maxillary sinus. Importance of the biopsy in FENS].

Sinonasal polyposis sometimes can masquerade malignant pathology of the paranasal sinuses and nose. We report the case of a 60 years old male diagnosed initially as solitary polyp and operated by endoscopic surgery with intrasurgical biopsy because a suspect of malignancy. The result was epidermoid carcinoma from left maxillary sinus that was confirmed after the definitive anatomopathologic exam. After Caldwell-Luc and complete removal of the tumoration, the patient evolutioned satisfactory being free of illness by clinical and radiological explorations after 1 year follow-up.

Sequential loss of suppressor genes for three specific functions during in vivo carcinogenesis.

As normal cells undergo neoplastic transformation, multiple suppressor gene functions are lost or inactivated. However, the relative contribution that individual suppressor gene defects play in the sequential evolution of solid tumors has not yet been evaluated in a readily analyzable experimental model of carcinogenesis. The present study was undertaken to: (a) document the loss of suppressor genes implicated in the control of angiogenic activity, anchorage and serum growth requirements, and proliferative life span, in populations of hamster buccal pouch (HBP) keratinocytes (Kr) initiated in vivo with the chemical carcinogen 7,12 dimethylbenz(a)anthracene and (b), to determine what combination of defective suppressor genes are necessary for tumorigenesis. Kr were isolated from HBPs at various times after treating the mucosal surfaces in vivo with 7,12 dimethylbenz(a)anthracene. Cells or their conditioned culture media were evaluated for: (a) angiogenic activity in vivo and in vitro, (b) anchorage independent growth, (c) growth in low serum, (d) immortality, and (e) tumorigenicity. Angiogenic activity and immortality were the first two phenotypes detected with anchorage independence and tumorigenesis emerging late in the carcinogenic process. Hybrids generated between Kr which were angiogenic, but otherwise negative for all other phenotypic traits, and a transformed and tumorigenic HBP carcinoma cell line, E1-1, were suppressed for all phenotypes except angiogenic activity and none of the hybrids were tumorigenic. In contrast, Kr positive for angiogenic activity, anchorage independence, immortality, and tumorigenicity and hybrids generated between these cells and E1-1 carcinoma, were tumorigenic. However, hybrids between a nontumorigenic, anchorage independent, immortal but nonangiogenic Kr, and the E1-1 line were not tumorigenic. These results suggest that (a) angiogenic activity is an early phenotypic trait that emerges as a result of the loss of a suppressor gene function and (b) loss of this function is essential but not sufficient for the development of HBP tumors.

[Paratesticular rhabdomyosarcoma with intracaval neoplastic involvement: presentation of a case and review of the literature].

One case of paratesticular embryonal rhabdomyosarcoma (RMS), affected during its evolution by a tumoral thrombus in the inferior vena cava. This unusual association forced the use of a cardiopulmonary by-pass, profound hypothermia and circulatory arrest, in order to carry out complete exeresis of the damage. Also, revision of the literature emphasizing that today's therapeutical approach for RMS should essentially be multidisciplinary.

[Prenatal diagnosis of sacro-coccygeal teratomas].

Seventeen sacrococcygeal teratomas with prenatal diagnosis are studied. The diagnosis was made at 27 weeks. Three fetuses died in utero, two of them from the evolution of the tumor. Two died post-natally from hemorrhage and one post natally because of his prematurity. From the eleven long term survivors, one developed a recurrence and five sequelae. This study confirms the poor prognosis of fetal sacrococcygeal teratomas. The 2 major prognosis factors are the growth of the tumor and the occurrence of fetal congestive heart failure. That congestive heart failure is diagnosed by Doppler echocardiography before hydrops and placentomegaly who are of very poor prognosis. The extraction of the fetus is mandatory if these features appeared after 30 weeks. Before this term fetal surgery is the only effective treatment. At a long term, the risk of recurrence is low but implies a long term surveillance of the level of alfa foeto protein.

[Chemotherapy of cancer of the exocrine pancreas].

Most pancreatic cancers cannot be resected at the time of diagnosis. The efficiency of chemotherapy is poor and does not seem to modify the spontaneous evolution of the tumor. 5FU alone, whatever the regimen, is the most active drug leading to a response rate of 20-30%; polychemotherapy protocols--particularly FAM and SMF--are not any more active; survival of patients does not differ when using mono- or polychemotherapy. Patients with pancreatic cancer that cannot be resected should be treated by 5FU monochemotherapy when not entering a clinical trial.

[Adenoid cystic carcinoma in the head and neck. A retrospective study of 12 cases].

Basing themselves on a series of twelve cases of cervicofacial cylindromas, the authors review the different clinical data pertaining to the evolution of these tumors, which they find to be prevalent in females aged around sixty. Cylindromas predominantly occur in the submandibular and accessory salivary glands. Out of twelve cases, the report mentions three local recurrences and two pulmonary metastases. Management was surgical in all instances.

[Intrathoracic goiter. Apropos of 36 cases].

This article is concerned with 36 cases of plunging goiters responding to Eschapasse and Merlier definition. The frequency of plunging goiters is 5.2%, women are predominating with 96% and the average of age is 45 years. In 34%, these goiters gave rise to fow symptoms, while there was mediastinal complication in the others cases. Paraclinic investigations are limited to a face and profile pulmonary roentgenographies, and scintigraphy of the thyroid. In the treatment, cervicotomy proved to the completely satisfactory to removal of all the goiters and there were no need to sternotomy or thoracotomy. The pathology revealed two malignant tumors. Post operative evolution was uneventful except one death in the 10th post operative day by pulmonary emboli.

[Rhabdomyosarcomas discovered in newborn infants. Study of markers of muscle differentiation].

Rhabdomyosarcomas were observed at birth in two premature infants. The evolution of these tumors was both rapid and fatal. One case was a botryoid sarcoma of the bladder and the second was a cervical alveolar sarcoma of Riopelle and Thériault. A cytological differentiation of the tumor was observed in the first case spontaneously and in the second following chemotherapy. In the second case histology, immunocytochemistry and a novel biochemical analysis of the contractile proteins demonstrated this differentiation of the tumor. The cellular reactivity varies according to the degree of muscular differentiation that can be defined either by studying the type of intermediate filaments present or the expression of specific muscle contractile proteins.

[Non-Hodgkin's lymphoma with splenic granulomatous reaction].

A 44-year old female had centrofollicular stage IV non-Hodgkin lymphoma, with splenic and bone marrow infiltration. The histological study of the spleen showed abundant nodes, some of them predominantly lymphoid and others consisting of epithelioid granulomas. With double labelling immunohistochemical techniques three types of nodes were observed: neoplastic lymphoid ones, surrounded by histiocytic cells, others also lymphoid with histiocyte infiltration, and, finally, others constituted by epithelioid granulomas. This suggests an evolution from the tumoral lymphoid nodule to the epithelioid granuloma, which may be caused by a defensive phenomenon against lymphomatous proliferation. The major importance of the recognition of this histological pattern lies in that it may obscure the diagnosis of lymphoma.

[Unusual course of a case of urethral rhabdomyosarcoma in a 10-year-old boy].

The authors report a case of rhabdomyosarcoma of posterior urethra they observed in a 10 year-old boy who was referred for an acute urinary retention. The story of the patient included other urinary tract troubles: macroscopic haematuria and stranguria had been observed when the boy was four year old and at that time cystography and cystoscopy grave normal results. In the following years the patient presented repeatedly episodes of haematuria, stranguria, dysuria and urinary tract infections. The authors stress the slow evolution of the tumor and the difficulties for diagnosing the disease in its early phase.

[Paraneoplastic syndromes in cancer of the kidney (author's transl)].

Starting with the description of their own observations of paraneoplastic syndromes in adult patients with cancer of the kidneys, the authors discuss the possible pathogenetic mechanisms and the more characteristic clinical features of said syndrome. They conclude by advocating a systematic search for such paraneoplastic syndromes as an aid to early diagnosis of renal malignancy and also as a working parameter for assessing the evolution of the tumor and the effectiveness of treatment.

[Aspects of megakaryocytic viral production in relation to lymphocytic leukemia in the rat].

Two viral populations BL/F (EL) and BL/F (SL) were derived from RadLV-Rs by propagation in rats where they induced respectively a generalized lymphoma in 5-6 weeks or a thymic lymphoma killing the animals in 5-6 months. In both cases, 10 days after inoculation of viral extract, numerous viral particles are present in the megakaryocytes (MKC) of the bone marrow and the spleen. Our results suggested a production rather than a passive accumulation of those particles by the MKC. The kinetics of blood platelet level for both leukemias showed a thrombocytopenia corresponding with the macroscopic development of the tumor. Therefore the evolution of the blood platelet level is not related to the MKC viremia. This suggests a lack of direct effect of virus BL/F on the MKC metabolism.

[Xeroderma pigmentosum: development of photoformation of hydrogen peroxide and catalytic activity in skin cells; application to an early diagnosis and to a tentative treatment].

In Xeroderma pigmentosum (XP) a genetic-recessive ultraviolette-hypersensitive disease we found an accumulation of photoproduced H2O2 in skin cells accompanied by a breakdown of the catalatic activity. Both disorders intensified as the disease progressed. Concomitantly with these processes, one electrochemical peak of urine decreases. By the time epitheliomas appeared, this peak was no longer detectable. Several applications to the skin of a cream containing catalase seemed to prevent the formation and the evolution of neoplastic tumors during the treatment.

[Instrumental dilatation of esophageal and gastric stenoses using Gruntzig's catheter and Celestin's dilator. Preliminary results].

The aim of this study was to present the technique of endoscopic dilatation and the preliminary results obtained with 2 new instruments, the Grüntzig balloon catheter, and the Celestin dilator, in patients with esophageal or gastric strictures. The Grüntzig balloon catheter was used 11 times in 7 patients (6 adults and one child aged 1 1/2 year), for the dilatation of severe or irregular strictures of the esophagus (5 cases), the stomach (1 case) and the pylorus (1 case). Partial (4 cases) or complete relief (2) were observed and allowed subsequent use of the Celestin bougies (2 cases), or the introduction of an esophageal prosthesis (1 case). In the child with caustic stricture of the esophagus, dilatation with the Grüntzig balloon-catheter was complicated twice by perforation. In the second case, the perforation was related to the technique used in dilatation and was treated by conservation measures. Endoscopic dilatations with the Celestin bougies were performed in 27 patients with benign (including 9 peptic strictures) or malignant strictures (14 cases, 7 related to esophageal cancer, and 7 to cancer of the cardia). A total of 74 dilatations were performed, attaining a maximum diameter of 16 or 18 mm in one session, in 21 patients (78 p. 100 of the cases). In the case of peptic stricture, the anatomical result was excellent in 8 patients and the functional result good in 7 of 9 cases. In case of malignant stricture, the result was conditioned by the evolution of the tumor; recurrence of dysphagia called for either repeated dilatations (8 cases) or the introduction of an esophageal prosthesis (5 cases).(ABSTRACT TRUNCATED AT 250 WORDS)

[Evolution of a morphological variant and the prognosis in lymphogranulomatosis].

The authors present the results of a retrospective analysis of the clinical peculiarities of a course of Hodgkin's disease (HD) as compared to the results of histological examination of lymph node biopsies taken for diagnostic purposes and operative material from 235 patients after exploratory laparotomy with splenectomy. It was shown that rapid transformation (from the onset of disease) of the HD histological variant of the type of "lymphoid prevalence", "nodular sclerosis" or "mixed cellular" into the "lymphoid depletion" variant attests to high rates of the evolution of a tumor process and makes it possible to recognize the subacute nature of a course of disease before treatment. The above phenomenon points to poor prognosis and should be considered in the choice of therapeutic tactics.

[Hypothalamic hamartoma and gelastic crises. Apropos of 7 cases].

Seven cases of hypothalamic hamartomas with gelastic seizures are reported. A precocious puberty was found in 4 cases. The normal neurologic examination and lack of sign of intracranial hypertension were in contrast with the severity of the epileptic seizures, of the mental impairment and of the behavioral disorders. The fact that the presenting symptom may be gelastic seizures is stressed. CT scan is the best means to assess the diagnosis and to follow the evolution of these tumors. Except for the management of the precocious puberty, the treatment is disappointing and neurosurgical indications are quite exceptional.

[Retinal phacomas. Apropos of 10 cases of astrocytoma].

Clinical and angiographic findings in 10 cases of retinal astrocytomas seen at the Clinique Ophtalmologique de Créteil are described. Classical features concerning onset of the disease, clinical aspects, and evolution of these tumors are confirmed. Some atypical findings are reported, including the marked proportion of isolated astrocytomas unrelated to phacomatosis, and the high frequency of complications of these lesions, usually considered to be stable.
