The development and testing of a prototype on-line radioiodine monitor for nuclear power stations.

A prototype on-line monitor has been developed which is capable of detecting radioiodine in the presence of as much as 1 X 10(6) higher concentration of noble gases. The system contains two identical radiation monitoring chambers through which the monitored air and a purging gas alternately cycle. Each chamber contains a silver zeolite filter which has a high retention of the various forms of airborne radioiodine but low retention of noble gases. During the purging cycle the radioactive noble gases are quickly purged from the filter and chamber and the lower levels of radioiodine accumulated on the filter are detected. This system has been successfully tested using short-lived radionuclides simulating vented reactor gases resulting from an abnormal condition.

Hormetic effects in pharmacology: pharmacological inversions as prototypes for hormesis.

A biological, physiological or biochemical response to a drug at a low dose may be completely opposite to that response when a larger dose is administered. Also, a response to a single dose of an agent may shortly afterwards be completely reversed. These observations are as old as the use of medicinal preparations or alcoholic beverages. The term "pharmacological inversions" can be used to describe these phenomena. A subset of the dose-response relationship is that of "hormesis." This term, "hormesis," is often used to denote the beneficial response of an organism to a low dose of a physical or chemical agent and a detrimental response to a much larger dose. Many, but not all, of the pharmacological inversions can serve as prototypes of hormesis. Examples of the dose-time effect or the dose-response relationship (which may include the phenomenon of hormesis) can be found among these agents: alcoholic beverages, anesthetic gases, barbiturates, some tranquilizers, many vitamins, caffeine, nicotine, salicylates, enzyme inducers and some toxic metals.

A personal dosimeter prototype for static magnetic fields.

A personal dosimeter prototype, for static magnetic fields with three Hall probes, has been designed and built to provide a reliable instrument for long-term analysis. The probe output signals, proportional to the magnetic flux density components of the vector B, are filtered, sampled, and stored in a buffer memory sufficient to cover a whole worker shift. The data sampling rate is high enough to record the operator movements within the fringes of the magnetic field. The content of the buffer memory is then transferred on a personal computer and registered on individual dosimetric cards. The data can be used for personal exposure monitoring.

Initial experience with a prototype staple detector.

Stainless steel staples can be overgrown by granulation tissue or skin grafts and become buried in tissue. Although they are generally asymptomatic, they can on rare occasion erode to the surface or complicate the radiographic evaluation of pain not related to the staples. A device to facilitate detection of retained staples would be desirable, but it has been difficult to develop because stainless steel staples have poor magnetic and conductive properties. A prototype device, based on the presence of metal disturbing a low-power electromagnetic field, was developed. With human studies committee approval, this miniaturized detector was used in 13 burn patients to detect staples during planned removal of large numbers of staples. Staples were first removed using visual inspection and palpation of the wounds. This was followed by use of the staple detector. The age of the patients was 10.8 +/- 3.3 years, and burn size was 54.6 +/- 8.8% of the body surface. In 8 (62%) of the patients one or more additional staples were detected by the device that would otherwise have been overlooked. In 4 (31%) of the patients there was a false-positive signal, possibly related to topical silver in the wounds, that required additional focused physical examination. A portable staple detector has been developed. Initial experience with the device is favorable and warrants an expanded trial, which is in the planning stages.

Prototype breast coil for MR-guided needle localization.

OBJECTIVE: Evaluation of breast lesions detected by MR alone is a problem: Preoperative MR-guided localization is difficult, since the breast is not accessible by a needle within the breast coil. Magnetic resonance-guided core biopsy and needle aspiration are impossible. Together with Siemens Erlangen, a prototype localization breast coil has been developed and tested. MATERIALS AND METHODS: The device consists of a circular polarized coil, which contains two plates for mediolateral breast compression. They are perforated with numerous holes and thus allow access to the breast from both sides. By means of bushings, which fit into the holes, sterile needle insertion in a horizontal path is possible. RESULTS: So far, precise needle insertion has been possible in 10 of 11 lesions, allowing exact needle insertion into 5 invasive carcinomas (1 MR-detected), 1 MR-detected additional focus, 2 in situ carcinomas (1 MR-detected), and 3 benign lesions. CONCLUSION: Our studies show that MR-guided needle localization is possible.

Prototypicality ratings of DSM-III criteria for personality disorders.

Although DSM-III personality disorder criteria have demonstrated acceptable reliability, the question of validity has not been adequately addressed. A first step in establishing the validity of diagnoses is to establish the validity of the criteria used to assess each diagnosis. The content validity of diagnostic criteria was investigated in relation to the larger set of potential criteria culled from the psychiatric literature. For each DSM-III axis II diagnosis, a panel of clinicians rated how prototypical each potential criterion was of the diagnosis in question. The results reveal problems with the organization and content of the criteria for most diagnoses. Many DSM-III criteria are composed of several statements linked by conjunctions or disjunctions. These component statements often received markedly different ratings, suggesting that criteria should be single statements. For most diagnoses, traits not included in DSM-III received higher ratings than did some DSM-III criteria. Suggestions are made to improve the distinctiveness and content validity of paranoid, schizoid, antisocial, borderline, avoidant, dependent, and compulsive personality disorders. The results for schizotypal personality disorder suggest that many clinicians are uncertain about this diagnosis. These findings provide a systematic way to modify definitions that contrasts with the more arbitrary ways in which diagnoses have previously been defined and redefined.

Mechanical strength, fatigue life, and failure analysis of two prototypes and five conventional tibial locking screws.

OBJECTIVE: To investigate the effects of the design and microstructure on the mechanical strength of tibial locking devices. DESIGN AND METHODS: The mechanical strength of two prototypes of specially developed locking devices (a both-ends-threaded screw and an unthreaded bolt) was tested and compared with that of five types of commercially available tibial locking screws (Synthes, Howmedica, Richards, Osteo AG, and Zimmer) with similar dimensions. The devices were inserted into a polyethylene tube and loaded at their midpoint by a materials testing machine to simulate a three-point bending test. Single-loading yielding strength and cyclic-loading fatigue life were then measured. Failure analysis of the fractured screws was performed to investigate the microstructure and potential causes of the fatigue fracture. RESULTS: Test results showed that both yielding strength and fatigue life were closely related to the section modulus of the inner diameter of screws. Among the threaded screws, the both-ends-threaded screws had a higher yielding strength and longer fatigue life than the Osteo AG, Howmedica, Richards, and Zimmer screws. The unthreaded bolts had a lower yielding strength than Synthes screws, but they demonstrated the longest fatigue life among all. In failure analysis of broken screws, no metallurgical or manufacturing defects were found except for surface microimperfections. CONCLUSIONS: The implants investigated in this study are manufactured with high-quality materials and manufacturing processes. The main cause of hardware failure was mechanical overloading. The five commercially used tibial locking screws had a relatively short fatigue life under high loading. Removing the screw threads might substantially increase the fatigue life of the locking devices. In unthreaded bolts, this increase might be tenfold to a hundredfold.

Clinical pharmacology of pinacidil, a prototype for drugs that affect potassium channels.

The clinical pharmacology of potassium channel openers has been reviewed using pinacidil as a prototype drug. When administered acutely or chronically, the hemodynamic and neuroendocrine profile is that of a peripheral arterial vasodilator. The drug produces decreases in peripheral vascular resistance, and subsequent blood pressure decreases are associated with reflex increments in heart rate. When studied, plasma catecholamines increased about twofold during chronic therapy. Plasma renin activity, however, was not increased during chronic therapy with pinacidil monotherapy. When patients were treated with pinacidil doses ranging from 12.5 to 75 mg b.i.d., 66.9% of patients had a decrease in supine diastolic blood pressure to below 91 mm Hg and 10 mm Hg less than baseline, whereas only 23.9% of patients had similar falls during placebo treatment. During maintenance therapy with pinacidil, the average blood pressure during the daytime dosing interval was 137.8 +/- 1.2/83.4 +/- 0.7 mm Hg (mean +/- SEM). Titration of pinacidil as monotherapy resulted in a characteristic adverse event profile dominated by the presence of dose-related edema. Other characteristic events included tachycardia, palpitations and headache. When pinacidil was given to patients unresponsive to hydrochlorothiazide (25 mg b.i.d.), similar efficacy relative to placebo was noted with a change of post-dose supine diastolic blood pressure in the pinacidil group of 13.5 +/- 0.8 mm Hg and 7.3 +/- 0.9 mm Hg in the placebo group.(ABSTRACT TRUNCATED AT 250 WORDS)

Fatigue strength of locking screws and prototypes used in small-diameter tibial nails: a biomechanical study.

BACKGROUND: One major problem with the use of small-diameter nails in the treatment of tibial fractures is the high rate of fatigue fractures of the locking screws. The objective of this study was to correlate such parameters as diameter of locking screws, influence of notch effect, and material with fatigue strength and fatigue limit. METHODS: In a biomechanical study, the mechanical properties of 11 different locking screws and screw designs to be used in solid tibial nails were tested under typical fatigue conditions. RESULTS: The results showed that an increase in the screw's diameter significantly increased the fatigue strength of the tested locking screws and prototypes (p < 0.0005). A 20% increase of diameter improved fatigue strength by 25 to 70%. Absence of thread at the location where the screw contacts the nail's aperture (where the main load is transmitted to the screw and where the screw, therefore, usually tends to break) may double the fatigue strength and fatigue limit by avoiding the negative notch effect of the screw's thread. It was further significant that titanium alloys had a higher fatigue strength compared with stainless steel alloys (p < 0.001). However, the fatigue strength of titanium implants is massively reduced by minimal damage to the surface. CONCLUSION: Optimizing the core diameter of the locking screws at the aperture of the solid tibial nail by using screws without a continuous thread generates a fatigue strength two to three times higher compared with screws with a continuous thread. Increasing fatigue strength of locking screws might decrease malunion in fractures stabilized by small-diameter nails.

Long-term care insurance. Premium estimates for prototype policies.

Private insurance has been suggested as a way to relieve the growing pressure on Medicaid budgets brought about by the long-term care needs of the elderly. This paper provides premium estimates for prototype long-term care insurance policies. Alternative specifications of services covered, amount of benefits paid per day, waiting period before benefits begin, years of coverage, age at time of purchase, administrative expenses, risk selection, and tax treatments are examined. The estimates are useful in understanding the general order of magnitude of premiums for various types of policies and assumptions. They may also be useful in deriving initial rates for new types of policies or in setting initial rates for an insurer with no access to actual experience data.

A proposed prototype for identifying and correcting sources of measurement error in classification systems.

Because many raters are generally involved in the implementation of a patient classification system, interrater reliability is always a concern in the development and use of such a system. In this article, a case example is used to demonstrate a prototype for identifying measurement error introduced at each step in the classification process (assessment, creating summary item responses, and use of these responses for categorization) and to illustrate how this identification may lead to error reduction strategies. The methods of analyses included percent agreement, Kappa, and visual inspection of contingency tables displaying interrater responses to assessment items, summary items, and the placement category. The extent to which raters followed instructions was analyzed by comparing their responses with computer-generated responses across the classification steps. In addition, raters were interviewed regarding their use of the system.

Most env and gag subtype A HIV-1 viruses circulating in West and West Central Africa are similar to the prototype AG recombinant virus IBNG.

The genetic subtype was identified in gag and env of 219 HIV-1-positive samples collected in different African countries, 44 from Senegal, 55 from Cameroon, 82 from Gabon, and 38 from Djibouti. In total, 20 (9.1%) samples had discordant subtypes between gag and env, 6 of 44 (13.9%) in Senegal, 4 of 55 (7.2%) in Cameroon, 1 of 38 (2.6%) in Djibouti, and 10 of 82 (12.1%) in Gabon. Subtypes A and G were predominantly involved in the recombination events. Phylogenetic tree analysis of gag showed that an important number of the A sequences form a distinct subcluster with the AG-IBNG prototype strain (a complex A/G mosaic virus): 27 of 32 (84.3%) in Senegal, 12 of 17 (70.6%) in Nigeria, 24 of 39 (61.5%) in Cameroon, and 38 of 70 (54.3%) in Gabon. Full-length genome analysis of 3 and additional sequences in pol for 10 such strains confirmed that they have a similar complex A/G mosaic genomic structure. These data suggest that in West Africa, most probably between 60% and 84% of the subtype A viruses are recombinant AG-IBNG viruses. This finding has potential implications on future vaccine, diagnostic, and treatment strategies. The actual and future role of these viruses in the global pandemic must be monitored in all new molecular epidemiologic studies, a discrimination between subtype A and AG-IBNG-like viruses is necessary.

Hemodynamic characteristics of a mixed flow pump prototype: progress report of in vitro and acute animal experiments.

A new dual-inlet mixed-flow blood pump was designed and tested in our laboratory. The objective of the present study was to analyze hemodynamic characteristics of the pump prototype in vitro and during acute in vivo experiments. The mixed-flow pump was first tested in vitro and then implanted in 11 pigs and 3 calves. The left ventricular apex was cannulated with the pump and an outflow graft was anastomosed to the descending thoracic aorta. Flow and pressure probes were also implanted. Animals were killed 3 to 12 hours after surgery. In 11 pigs, pump outflow averaged 3.8 +/- 0.4, 4.5 +/- 0.4, 5.2 +/- 0.8, 5.9 +/- 0.3, and 6.5 l/min at 8,000, 9,000, 10,000, 11,000, and 12,000 pump speed in rpm. Differential pressure at the pump averaged 45 +/- 6, 54 +/- 8, 68 +/- 16, 70 +/- 12, and 85 +/- 7 mm Hg at 8,000, 9,000, 10,000, 11,000 and 12,000 rpm. Mean aortic pressure averaged 64 +/- 15 mm Hg throughout the procedures. In 3 calves, mean aortic pressure and left ventricular pressure remained stable during 4, 6, and 9 hours of support at 9,500, 10,000, 10,500, 11,000, and 11,500 rpm. The hemodynamic performance of our mixed-flow pump appears satisfactory during short-term support in animals. It supports similarly to axial-flow blood pumps in clinical trials. Based on these findings, an ameliorated design of this mixed-flow pump running at smaller rotational speed against a similar pressure head is under way.

Volumetric cine imaging for cardiovascular circulation using prototype 256-detector row computed tomography scanner (4-dimensional computed tomography): a preliminary study with a porcine model.

This is a preliminary demonstration of volumetric cine imaging of cardiovascular circulation in domestic pigs using a prototype 256-detector row computed tomography (CT) scanner. The scan range is approximately 120 mm in the craniocaudal direction, with a 0.5-mm slice thickness. The thin sections can be used to create cine loops in multiple planes. Thus, the 256-detector row CT scanner overcomes some of the limitations of present helical CT methods.

The Leeuwenhoek lecture, 1983. A prototype vaccine to prevent Epstein-Barr virus-associated tumours.

EB virus is a herpesvirus that infects all human communities. The infection is life-long and usually asymptomatic. Excessive reaction to primary infection leads to infectious mononucleosis while immunological failures give fatal lymphoproliferative diseases. The virus is associated with endemic Burkitt's lymphoma and undifferentiated nasopharyngeal carcinoma. In world cancer terms Burkitt's lymphoma is insignificant, but nasopharyngeal carcinoma has a high incidence in certain important populations. By analogy with herpesvirus-induced animal cancers, prevention of infection should greatly reduce subsequent development of tumours. A prototype vaccine has therefore been produced based on the virus-determined antigen (MA gp340) that elicits virus-neutralizing antibodies. A sensitive assay has permitted the elaboration of an efficient antigen preparation method and the product has been rendered highly immunogenic, as tested in mice and rabbits, by incorporation in liposomes. The only animal suitable for experimental EB virus infection is the little-known cottontop tamarin; a breeding colony has been successfully established and protection against virus challenge assessed in immunized tamarins. The overall structure of the antigen has been determined in preparation for future production by synthesis or rDNA technology.

A model for vesicular exanthema virus, the prototype of the calicivirus group.

The structure of vesicular exanthema virus, the prototype member of the calicivirus group, has been studied in more detail. The RNA comprises 18% of mol. wt. of about 2.8 x 10(6), based on polyacrylamide gel electrophoresis experiments in the presence of formaldehyde. The virus contains one major polypeptide, mol. wt. 70 x 10(3) as determined by polyacrylamide gel electrophoresis and by chromatography on Sepharose 6B in the presence of 6 M-guanidine. Further evidence for the presence of a single major polypeptide was obtained by tryptic peptide analysis of 35S-methionine labelled virus. The mol. wt. of a protein oligomer produced by adjusting the pH of virus suspensions to 3.5 was c. 200 x 10(3). On the basis of these data we propose a T = 3 model for the virus capsid incorporating 180 copies of the virus protein.

Conservation of antigenic properties and sequences encoding the envelope proteins of prototype Hantaan virus and two virus isolates from Korean haemorrhagic fever patients.

Viruses isolated from the blood of two Korean haemorrhagic fever patients were propagated in cell culture and compared to prototype Hantaan virus which was isolated from Apodemus mice. The antigenic properties of the human isolates were found to be closely related to Hantaan virus by plaque reduction neutralization, haemagglutination inhibition and fluorescent antibody staining with both polyclonal and monoclonal antibodies. The medium genome segment of each human isolate was sequenced and compared to that of Hantaan virus. Nucleotides comprising the Hantaan virus G1 and G2 envelope protein-coding regions differed from those of the other viruses by only 5.4% and 5.7%. The human isolates differed from one another by 1.6%. The nucleotide differences resulted in predicted amino acid variations of 1.3% to 2.3% among the three viruses, with the majority occurring as conservative substitutions in G1.
